Pancreas is a vital gland of gastrointestinal system with exocrine and endocrine secretory functions, interweaved into essential metabolic circuitries of the human body. Pancreatic ductal adenocarcinoma (PDAC) represents one of the most lethal malignancies, with a 5-year survival rate of 11%. This poor prognosis is primarily attributed to the absence of early symptoms, rapid metastatic dissemination, and the limited efficacy of current therapeutic interventions. Despite recent advancements in understanding the etiopathogenesis and treatment of PDAC, there remains a pressing need for improved individualized models, identification of novel molecular targets, and development of unbiased predictors of disease progression. Here we aim to explore the concept of precision medicine utilizing 3-dimensional, patient-specific cellular models of pancreatic tumors and discuss their potential applications in uncovering novel druggable molecular targets and predicting clinical parameters for individual patients.

BACKGROUND: This study develops a deep learning-based automated lesion segmentation model for whole-body 3D18F-fluorodeoxyglucose (FDG)-Position emission tomography (PET) with computed tomography (CT) images agnostic to disease location and site. METHOD: A publicly available lesion-annotated dataset of 1014 whole-body FDG-PET/CT images was used to train, validate, and test (70:10:20) eight configurations with 3D U-Net as the backbone architecture. The best-performing model on the test set was further evaluated on 3 different unseen cohorts consisting of osteosarcoma or neuroblastoma (OS cohort) (n = 13), pediatric solid tumors (ST cohort) (n = 14), and adult Pheochromocytoma/Paraganglioma (PHEO cohort) (n = 40). Both lesion-level and patient-level statistical analyses were conducted to validate the performance of the model on different cohorts. RESULTS: The best performing 3D full resolution nnUNet model achieved a lesion-level sensitivity and DISC of 71.70 % and 0.40 for the test set, 97.83 % and 0.73 for ST, 40.15 % and 0.36 for OS, and 78.37 % and 0.50 for the PHEO cohort. For the test set and PHEO cohort, the model has missed small volume and lower uptake lesions (p < 0.01), whereas no statistically significant differences (p > 0.05) were found in the false positive (FP) and false negative lesions volume and uptake for the OS and ST cohort. The predicted total lesion glycolysis is slightly higher than the ground truth because of FP calls, which experts can easily check and reject. CONCLUSION: The developed deep learning-based automated lesion segmentation AI model which utilizes 3D_FullRes configuration of the nnUNet framework showed promising and reliable performance for the whole-body FDG-PET/CT images.

Cluster analyzes of facial models of autistic patients aim to clarify whether it is possible to diagnose autism on the basis of facial features and further to stratify the autism spectrum disorder. We performed a cluster analysis of sets of 3D scans of ASD patients (116) and controls (157) using Euclidean and geodesic distances in order to recapitulate the published results on the Czech population. In the presented work, we show that the major factor determining the clustering structure and consequently also the correlation of resulting clusters with autism severity degree is body mass index corrected for age (BMIFA). After removing the BMIFA effect from the data in two independent ways, both the cluster structure and autism severity correlations disappeared. Despite the fact that the influence of body mass index (BMI) on facial dimensions was studied many times, this is the first time to our knowledge when BMI was incorporated into the faces clustering study and it thereby casts doubt on previous results. We also performed correlation analysis which showed that the only correction used in the existing clustering studies-dividing the facial distance by the average value within the face-is not eliminating correlation between facial distances and BMIFA within the facial cohort.

This article presents an overview of existing approaches to perform vectorcardiographic (VCG) diagnostics of ischemic heart disease (IHD). Individual methodologies are divided into categories to create a comprehensive and clear overview of electrical cardiac activity measurement, signal pre-processing, features extraction and classification procedures. An emphasis is placed on methods describing the electrical heart space (EHS) by several features extraction techniques based on spatiotemporal characteristics or signal modelling and signal transformations. Performance of individual methodologies are compared depending on classification of extent of ischemia, acute forms - myocardial infarction (MI) and myocardial scars localization. Based on a comparison of imaging methods, the advantages of VCG over the standard 12-leads ECG such as providing a 3D orthogonal leads imaging, better performance, and appropriate computer processing are highlighted. The issues of electrical cardiac activity measurements on body surface, the lack of VKG databases supported by a more accurate imaging method, possibility of comparison with the physiology of individual cases are outlined as potential reserves for future research.

The consideration of human and environmental exposure to dendrimers, including cytotoxicity, acute toxicity, and cell and tissue accumulation, is essential due to their significant potential for various biomedical applications. This study aimed to evaluate the biodistribution and toxicity of a novel methoxyphenyl phosphonium carbosilane dendrimer, a potential mitochondria-targeting vector for cancer therapeutics, in 2D and 3D cancer cell cultures and zebrafish embryos. We assessed its cytotoxicity (via MTT, ATP, and Spheroid growth inhibition assays) and cellular biodistribution. The dendrimer cytotoxicity was higher in cancer cells, likely due to its specific targeting to the mitochondrial compartment. In vivo studies using zebrafish demonstrated dendrimer distribution within the vascular and gastrointestinal systems, indicating a biodistribution profile that may be beneficial for systemic therapeutic delivery strategies. The methoxyphenyl phosphonium carbosilane dendrimer shows promise for applications in cancer cell delivery, but additional studies are required to confirm these findings using alternative labelling methods and more physiologically relevant models. Our results contribute to the growing body of evidence supporting the potential of carbosilane dendrimers as vectors for cancer therapeutics.

Among explored nanomaterials, nanosponge-based systems have exhibited inhibitory effects for the biological neutralization of, and antiviral delivery against, severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2). More studies could pave the path for clarification of their biological neutralization mechanisms as well as the assessment of their long-term biocompatibility and biosafety issues before clinical translational studies. In this review, we discuss recent advances pertaining to antiviral delivery and inhibitory effects of nanosponges against SARS-CoV-2, focusing on important challenges and opportunities. Finally, as promising approaches for recapitulating the complex structure of different organs/tissues of the body, we discuss the use of 3D in vitro models to investigate the mechanism of SARS-CoV-2 infection and to find therapeutic targets to better manage and eradicate coronavirus 2019 (COVID-19).

Three-dimensional facial images are becoming more and more widespread. As such images provide more information about facial morphology than 2D imagery, they show great promise for use in future forensic applications, including age estimation and verification. This paper proposes an approach using random forests, a machine learning method, to develop and test models for classification of legal age thresholds (15 years and 18 years) using 3D facial landmarks. Our approach was developed on a set of 3D facial scans from 394 Czech individuals (194 males and 200 females) aged between 10 and 25 years. The dataset was retrieved from a sizable database of Central European faces - The FIDENTIS 3D Face Database. Three main types of input variables were processed using random forests: I) shape (size-invariant) coordinates of 3D landmarks, II) size and shape coordinates of 3D landmarks, and III) inter-landmark distances, angles and indices. The performance rates for the combinations of variables and age threshold were expressed in terms of sensitivity and specificity. The overall accuracy rates varied from 71.4%-91.5% (when the male and female samples were pooled). In general, higher accuracy was achieved for the age limit of 18 years than for 15 years. Whereas size-variant variables showed a better performance rate for the age limit of 15 years, the size-invariant variables (i.e., shape variables) were better for classifying individuals under 18 years. The verification models grounded on traditional variables (distances, angles, indices) yielded consistently higher performance rates on females than on males, whereas the inverse trend was observed for the models built on 3D coordinates. The results indicate that age verification based on 3D facial data with processing by the random forests method has high potential for further forensic or biometric applications.

OBJECTIVES: Stereotactic radiosurgery (SRS) is an established treatment option of small/medium-sized vestibular schwannomas (VSs). Concerning management of the large VSs, primary SRS remains a controversial option. Our retrospective study analyzes long-term radiological and clinical outcomes of SRS in large VSs. MATERIAL AND METHODS: We retrospectively analyzed 73 patients with single large VS, treated with SRS. Inclusion criteria were: tumor volume >4 cm3, follow-up >2 years, radiological (3D-volumetric studies) and clinical follow-up. SRS was either primary (94.5%) or secondary (5.5%) treatment. The median marginal dose (50%-isodose line) was 12Gy (11.5-12Gy). Fisher exact test, t-test, ANOVA, Kaplan-Meier and Cox regression models were performed when appropriate RESULTS: The median follow-up was 5.5 years. The median VS volume at SRS was 6.5 cm3 (range 4-14.2 cm3). The tumor control rates assessed from Kaplan-Meier curve were 88.3%, 82.4% and 74.7% 5.8 and 10 years after SRS, respectively. Tumor shrinkage was observed in 83.6% of patients (n=61), unchanged volume in 4.1% patients (n=3) and progression in 12.3% (n=9). The median tumor volume significantly decreased to 4.0 cm3, measured at 5-year follow-up (p<0.0001). Large cystic VSs responded better to SRS then homogeneous. Pre-SRS serviceable hearing was present in 37% of patients; 55% of these had hearing preserved after treatment. After SRS, new facial palsy (House-Brackmann gr. III-VI) appeared in 4.1% of patients; 9.6% of patients had transient brainstem/cranial nerves edema. For tumor progression, 8.2% of patients underwent resection, 2.8% of patients repeated SRS. CONCLUSION: Our results are showing that SRS might be safe and effective primary treatment even in large VSs. However, long-term tumor control rates are lower in comparison with small/medium-sized VSs. Thus, closer follow-up should be applied.
The RoScan is a novel, high-accuracy multispectral surface scanning system producing colored 3D models that include a thermal layer. (1) Background: at present, medicine still exhibits a lack of objective diagnostic methods. As many diseases involve thermal changes, thermography may appear to be a convenient technique for the given purpose; however, there are three limiting problems: exact localization, resolution vs. range, and impossibility of quantification. (2) Methods: the basic principles and benefits of the system are described. The procedures rely on a robotic manipulator with multiple sensors to create a multispectral 3D model. Importantly, the structure is robust, scene-independent, and features quantifiable measurement uncertainty; thus, all of the above problems of medical thermography are resolved. (3) Results: the benefits were demonstrated by several pilot case studies: medicament efficacy assessment in dermatology, objective recovery progress assessment in traumatology, applied force quantification in forensic sciences, exact localization of the cause of pain in physiotherapy, objective assessment of atopic dermatitis, and soft tissue volumetric measurements. (4) Conclusion: the RoScan addresses medical quantification, which embodies a frequent problem in several medical sectors, and can deliver new, objective information to improve the quality of healthcare and to eliminate false diagnoses.

The increasing risk of radiation exposure underlines the need for novel radioprotective agents. Hence, a series of novel 1-(2-hydroxyethyl)piperazine derivatives were designed and synthesized. Some of the compounds protected human cells against radiation-induced apoptosis and exhibited low cytotoxicity. Compared to the previous series of piperazine derivatives, compound 8 exhibited a radioprotective effect on cell survival in vitro and low toxicity in vivo. It also enhanced the survival of mice 30 days after whole-body irradiation (although this increase was not statistically significant). Taken together, our in vitro and in vivo data indicate that some of our compounds are valuable for further research as potential radioprotectors.
