Hlavným cieľom štúdie je analyzovať dokumenty z vybraných databáz zaoberajúce sa včasnou intervenciou u detí s detskou mozgovou obrnou v ranom veku a poskytnúť prehľad o rozličných metódach a postupoch používaných v rámci včasnej intervencie pre deti s DMO. TEORETICKÉ VÝCHODISKÁ: Poskytovanie sociálnej služby včasnej intervencie deťom v ranom veku je rozhodujúce pre ich ďalší vývoj a pre fungovanie celej rodiny. METÓDY: Obsahová analýza štúdií bola realizovaná v máji 2019 pomocou počítačového vyhľadávania. Vyhľadávanie bolo realizované v nasledovných databázach: SCOPUS (n = 37); Web of Sciences Current Content (n = 7); Web of Sciences Core Collection (n = 13) a Science Direct Databases (n = 1166). Analyzovaných bolo spolu 6 vybraných štúdií. VÝSLEDKY: Dôležitosť včasnej intervencie je najviac viditeľná v oblastiach: pohybové schopnosti, manuálne schopnosti, komunikácia, sociálny rozvoj. IMPLIKÁCIE PRE SOCIÁLNU PRÁCU: Včasná intervencia ako služba na podporu rodín s deťmi má v súčasnosti nezastupiteľné miesto v slovenskej legislatíve. Včasné poskytnutie tejto služby totiž spolu s prístupom orientovaným na rodinu v rámci systematického interdisciplinárneho prístupu je pre rodinu s dieťaťom s detskou mozgovou obrnou veľmi dôležité.
The main objective of this study is to analyse documents from selected databases of groups of children with cerebral palsy in regards to early childhood intervention and provide an overview of the different methods and procedures used. THEORETICAL BASE: Providing early childhood intervention in families with children with cerebral palsy is crucial for the further development of children and their families. METHODS: The content analysis of the studies was conducted on research done in May 2019. Documents were found through computer searches of SCOPUS (n = 37); Web of Sciences Current Content (n = 7); Web of Sciences Core Collection (n = 13) and Science Direct Databases (n = 1166). OUTCOMES: Finally, six studies were included. The importance of early childhood intervention is mostly visible in these areas: motor ability, manual ability, communication, and social development. The early start of intervention and a family-centred approach are important in the work with the family of children with cerebral palsy. SOCIAL WORK IMPLICATIONS: Early childhood intervention as a social service for supporting the families of children with disabilities are currently a crucial part of Slovak legislation. The timely provision of this service in the context of a systematic interdisciplinary approach, along with an approach that is oriented towards the family, is very important for the families of children with cerebral palsy.

Families are not only the main living area for such children with cerebral palsy but also their most reliable spiritual home. During the long-term care process, family members, especially those main caregivers are under diverse great pressure, the primary aim of this study was to examine family factors and support strategies used. Data regarding situation of children with cerebral palsy and their family factors were obtained from patient records. Results indicated that family coping styles associate with family factors and spiritual support was among the family coping strategies used most often. Not surprisingly social support was used least often, caregivers tended to seek out internal support (e. g. support from the extended family). Findings suggest the necessity to target policy makers, community supports, and attitude towards family rehabilitation. In the long run, to meet diversified needs of the child and their families, it’s necessary to provide support services for the aim of enhancing children and parents’ self-help ability.

Cieľ: Ochorenie detskej mozgovej obrny je možné označiť ako neurovývojové neprogresívne postihnutie mozgu, ktoré spôsobuje narušenie motorického vývoja dieťaťa. Veľmi často zasahuje do viacerých oblastí vývoja – kombinuje sa s mentálnym postihnutím, zmyslovými chybami a ovplyvňuje rozvoj komunikačných zručností. Cieľom štúdie bolo vytvoriť novú teóriu s použitím metaparadigmy ošetrovateľstva pomocou metódy „zakotvenej teórie“, ktorá by dokazovala opodstatnenosť a súvzťažnosť ošetrovateľskej starostlivosti so všetkými oblasťami života (fyzickej, psychickej, sociálnej, legislatívnej) u takto postihnutých detí. Metódy: V štúdii sme vypracovali komplexné kazuistiky troch prípadov. Na získavanie informácií sme zvolili metódu pološtandardizovaného rozhovoru a metódu štúdia zdravotnej dokumentácie oslovených osôb. Komplexné kazuistiky sme analyzovali metódou „zakotvenej teórie“. Výsledky: Informácie, ktoré sme získali z kazuistík nám poskytli cenné vyjadrenia, na základe ktorých sme vytvorili novú teóriu s použitím metaparadigmy ošetrovateľstva podľa metódy „zakotvenej teórie“, ktorá poukazuje na kauzalitu vzťahu ošetrovateľskej starostlivosti so všetkými oblasťami života (fyzickej, psychickej, sociálnej, legislatívnej) u detí so spastickou formou detskej mozgovej obrny. Zároveň poukazuje aj na potrebu multidisplinárnej spolupráce odborníkov z oblasti neurológie, fyzioterapie, psychológie, logopédie, špeciálnej pedagogiky i sociálnej oblasti v rámci interakcie s pacientom. Záver: Naša teória dokazuje, že kvalitná ošetrovateľská starostlivosť, komplexný multidisciplinárny prístup odborníkov z oblasti neurológie, fyzioterapie, psychológie, špeciálnej pedagogiky ako aj sociálnej starostlivosti môže zlepšiť podmienky života dieťaťa s mozgovou obrnou v celom holistickom rozsahu.
Aim: Disease of cerebral palsy can be described as neurodevelopmental disability non-progressive brain, which causes disruption of motor development children. Oft en spans multiple areas of development – combined with intellectual disabilities, sensory defects, and aff ects the development of communications kills. Th e aim of the study was to develop a new theory using metaparadigm nursing, using a “grounded theory”, which would prove the validity and correlation nursing care to every sphere of life (physical, psychological, social, legislative) for those aff ected children. Methods: Are working-out complex casuist of three case. On inquiries are elect method semistructured conversation and method study health documentation title person. Complex casuist are analyzing method “grounded theory”. Results: Information that are acquirement us off er precious expression following those are create new theory with metaparadigm nursing by methods “grounded theory”, which mention causality relation nursing care with region life (physically, psychic, social, legislative) child with spastic form infantile cerebral palsy and which also points to the need for multidisciplinary cooperation experts in neurology, physiotherapy, psychology, speechtherapy, special education and social interaction with in the area of the patients. Conclusion: Our theory shows that quality nursing care, comprehensive multidisciplinary approach experts in neurology, physiotherapy, psychology, special education and social care can improve the living conditions of chilren with cerebral palsy in its holistic range.
Práce je zaměřena na problematiku pacientů s poruchami pohybu v domácím prostředí. Jsou zde nastíněny nejčastější poruchy pohybu, se kterými se můžeme v domácí péči setkat. Dále jsou uvedeny nejčastější příčiny poruch pohybu. Je kladen důraz na komplexní péči o pacienty s poruchami pohybu. Tato část se věnuje pacientům s roztroušenou sklerózou mozkomíšní a dětskou mozkovou obrnou.

The most frequent movement disorders that may be encountered in home care are outlined here. Further, the most frequent causes of movement disorders are discussed. Particular emphasis is given to a complex care of patients suffering with movement disorders. This part deals with patients with multiple sclerosis and cerebral palsy.

Úvod: Rodina patří k nejdůležitějším článkům v léčbě dítěte s handicapem. Kvalitu interakcí rodiny s dítětem mohou zásadně ovlivnit nenaplněné potřeby rodičů. Znalost potřeb rodiny dítěte s postižením umožní odborníkům poskytovat komplexní péči zaměřenou nejen na dítě, ale také na rodinu, tak jak určuje aktuální trend péče v ošetřovatelství – Family Centered Care. Cílem práce bylo shrnout dostupné informace o potřebách rodičů dětí s dětskou mozkovou obrnou (DMO). Metodika: Realizované výzkumy, zaměřené na potřeby rodičů dětí s DMO, byly vyhledány pomocí Booleovských operátorů v odborných licencovaných databázích a volně přístupných databázích. Kritériem pro výběr výzkumů byly studie publikované v letech 2000–2012, studie publikované v anglickém jazyce, plný text publikovaného výzkumu, stáří dítěte s DMO mezi 1 a 7 lety a přístup kvantitativního nebo kombinovaného výzkumu. Analýze bylo podrobeno 40 odborných článků. Výsledky: Bylo identifikováno 9 oblastí potřeb rodičů: podpora zdraví, podpora okolí, rodinné vztahy, služby, informace a komunikace, management nemoci, finance, časový management a adaptace rodiny.
Introduction: Family is one of the most important elements in the treatment of a handicapped child. However, the quality of interaction between the family and the child may be significantly affected by unfulfilled needs of the parents. Knowledge of the needs of a family with a handicapped child enables experts to provide comprehensive care focused not only on the child but also on the family in accordance with the current trend in nursing care – Family Centered Care. The aim of the paper was to summarize available information about the needs of parents of children with infantile cerebral palsy (ICP). Methodology: Research focused on the needs of parents of children with ICP were searched by means of Boolean operators in professional licensed databases and in freely accessible databases. The criteria for the selection of papers were: studies published between 2000 and 2012, studies published in English, full text of the published research, age of children with ICP between 1 and 7 years, and the quantitative or combined research approach. A total of 40 research papers were analysed. Findings: Nine areas of parents’ needs were identified: health support, support from others, family relationships, services, information and communication, disease management, funds, time management and family adaptation.

Canisterapie se nejčastěji uplatňuje jako doplňková metoda u osob s dlouhodobým průběhem léčby s nepříznivou perspektivou. Jedna z forem canisterapie – polohování – je vhodná zejména pro děti s těžkými formami dětské mozkové obrny a pro děti s kombinovanými vadami. Cílem předkládané práce bylo zjistit, jak je sedmiletý chlapec s dystonicko-dyskinetickou formou dětské mozkové obrny a těžkou retardací psychomotorického vývoje schopen relaxovat v průběhu canisterapeutické lekce, zda a popř. v jakém časovém horizontu dojde k uvolnění jeho svalového napětí a zlepšení emočního ladění a zda pravidelné opakování canisterapeutických lekcí trvale zlepší stav dítěte. Práce je založena na technice zúčastněného pozorování. Sledovanými parametry byly dyskinetické pohyby hlavy a končetin dítěte, jejichž četnost v rámci canisterapeutického polohování byla ukazatelem jeho fyzického a psychického stavu. Pozorování bylo provedeno v průběhu deseti po sobě následujících lekcí s odstupem jednoho týdne kromě jedné výjimky, kdy bylo dítě nemocné. Díky polohování došlo ke snížení počtu dyskinéz přibližně na polovinu počáteční hodnoty a několikrát došlo dokonce k dočasnému úplnému vymizení dyskinéz. Dlouhodobé snížení jejich počtu vlivem canisterapie se však nepodařilo prokázat. Jednotlivé canisterapeutické lekce přispěly k celkové relaxaci dítěte a uvolnění svalového napětí dlaní a chodidel. Po lekcích bylo dítě spokojené a vždy s větší chutí komunikovalo se svým okolím. Přínos canisterapie, resp. polohování v rámci rehabilitačního programu chlapce, se ukázal být nesporný, je ovšem nutné zdůraznit, že získaná data a vyvozené závěry nelze zobecňovat na celou populaci dětí s touto formou dětské mozkové obrny, neboť i u jedinců se shodnou diagnózou se objevují značné rozdíly v efektu působení psa.
Canistherapy finds most frequently its applications as a supplementary method in persons with longterm course of the treatment with non-beneficial prognosis. One of forms of canistherapy – positioning – is particularly suitable for children with severe forms of poliomyelitis and those with combined defects. The target of the work presented here was to determine the ability to relax in a boy of 7 years of age with dystonic-dyskinetic form of poliomyelitis and severe retardation of the psychomotor development in the course of a canistherapeutic lesson; whether or possibly after what time period the relaxation of his muscle tone will be accomplished and emotional harmony will be improved and whether regular repeating of canistherapeutic lessons can permanently improve the child condition. The work is based on the technique of participating observation. The parameters investigated were dyskinetic motions of the head and limbs of the child, the frequency of which was a parameter of the physical and mental condition within the framework of canistherapeutic positioning. The observation was carried out in the course of ten subsequent lessons held at one-week intervals except one case when the child was ill. Thanks to positioning, a reduction was achieved in the rate of dyskinetic motions to about half the baseline value, and several times, even complete vanishing of dyskinesia was temporarily achieved. Long-term reduction in the rate to dyskinetic motions thanks to canistherapy was, however, not demonstrated. Particular canistherapeutic lessons contributed to the general relaxation of the child and relief of the muscle tension in palms and soles of the foot. After the lessons, the child was satisfied and always more interested in communication with the surroundings. The contribution of canistherapy or positioning within the framework of the rehabilitation programme of the boy was definitely demonstrated, but it is necessary to emphasize that the data acquired and conclusions derived cannot be generalized for the whole population of children with this form of poliomyelitis, since in individuals with the same diagnosis, there are differences in effects of the contact with the dog.
