AIM: The aim of the present study was to investigate myocardial perfusion in relation to disease history and laboratory parameters of atherosclerosis risk in asymptomatic patients with breast carcinoma. PATIENTS AND METHODS: One-hundred and eighty-one patients with breast carcinoma were studied. Myocardial perfusion was assessed using single-photon emission computed tomography (SPECT) with 99mtechnetium sestamibi. RESULTS: Perfusion defects were detected in 12 patients (7%). Higher body-mass index, increased concentrations of D-dimers, C-reactive protein, fibrinogen, glucose, triglycerides, and urinary albumin, a history of hypertension and of radiotherapy to the left chest wall were all associated with increased risk of perfusion defects. In a multivariate stepwise selection logistic regression model, body mass index, albuminuria and radiotherapy to the left hemithorax were significantly associated with the presence of perfusion defects. CONCLUSION: In addition to other factors, treatment history may be associated with the presence of perfusion defects in patients with breast cancer.

INTRODUCTION: Breast cancer treatment currently represents one of the biggest challenges in clinical oncology. The gold standard for axillary lymph node management is to perform sentinel node biopsy to avoid axillary dissection and its sequelae. The detection of radiocolloid flow outside the axillary nodes is a diagnostic and therapeutic challenge. METHODS: A database search at the Department of Oncology of Palacky University, Olomouc, Czech Republic, identified 127 patients who underwent breast cancer resection with a sentinel node procedure and had radiocolloid flow into the internal mammary nodes. Sentinel node lymphoscintigraphy was performed after intraparenchymal injection. Clinical and pathological data were collected to identify possible risk factors. RESULTS: Ten clinical and pathological parameters including age, tumor histology, axillary lymph node status, estrogen receptor expression, progesterone receptor expression, tumor grade, Ki-67 expression, Her-2 status, tumor size and tumor location were analyzed with regard to internal mammary node drainage. A cohort of 127 patients with detected drainage into the internal mammary nodes was compared with 135 patients without such drainage. Six significant risk factors, including age <50 years ( P <0.0313), tumor location in central and inner quadrants (P <0.012), larger tumor size (P <0.017), positive Her-2 status (P <0.025), progesterone receptor expression (P <10-4) and axillary lymph node involvement (P <0.01) were found to predict radiocolloid flow into the internal mammary nodes. CONCLUSION: Six parameters (patient age, tumor location, hormone receptor status, tumor size, Her-2 status and axillary lymph node status) should be considered in the management of breast cancer patients and help in the selection of patients for locoregional procedures encompassing the internal mammary nodes.

Východiska: Analyzovali jsme dostupná data o vykázané péči za účelem návrhu nového modelu úhrady radioterapie postaveného na prospektivním principu. Cílem bylo otestovat vysvětlující faktory (prediktory), které by dokázaly nejlépe roztřídit hospitalizační případy do nákladově homogenních skupin. Testovány byly čtyři potenciální prediktory – počet frakcí radioterapie, skupina techniky radioterapie, diagnostické skupiny a stadium onemocnění. Cílem naopak nebyl samotný návrh úhradového modelu jako takového. Metody: Analyzováno bylo celkem 7 440 hospitalizačních případů onkologicky nemocných pacientů z let 2007–2011 ve třech vybraných zdravotnických zařízeních. Zdrojem dat byla především databáze I‑COP vyvinutá Institutem biostatistiky a analýz Masarykovy univerzity ve spolupráci s komplexními onkologickými centry ČR, která na úrovni jednotlivých zdravotnických zařízení propojuje data z Národního onkologického registru s daty o vykázané péči zdravotnických zařízení zdravotním pojišťovnám. Výsledky: Při srovnání čtyř potenciálních prediktorů jsme vyhodnotili jako nejsilnější z nich počet aplikovaných frakcí radioterapie a použitou techniku radioterapie. Stadium onemocnění neprokázalo v tomto srovnání dostatečnou rozlišovací schopnost. Mezi diagnostickými skupinami byly identifikovány významné rozdíly v průměrných nákladech, ty však souvisely převážně s rozdílným spektrem použitých technik radioterapie a rozdílným počtem frakcí v jednotlivých skupinách. Uvnitř diagnostických skupin tak byly z hlediska vykázaných nákladů příliš velké rozdíly, které nejsou v případném úhradovém modelu žádoucí. Závěr: Pro potřeby prospektivního úhradového systému pro oblast radioterapie může být využita kombinace dvou prediktorů – počet aplikovaných frakcí radioterapie a technika radioterapie. Pro návrh úhradového modelu by byly vhodné další analýzy testující prediktivní schopnost záměru aplikované radioterapie. V současnosti však v ČR neexistují potřebné datové zdroje pro takovéto analýzy.
Background: As a part of the development of a new prospective payment model for radiotherapy we analyzed data on costs of care provided by three comprehensive cancer centers in the Czech Republic. Our aim was to find a combination of variables (predictors) which could be used to sort hospitalization cases into groups according to their costs, with each group having the same reimbursement rate. We tested four variables as possible predictors – number of fractions, stage of disease, radiotherapy technique and diagnostic group. Methods: We analyzed 7,440 hospitalization cases treated in three comprehensive cancer centers from 2007 to 2011. We acquired data from the I‑COP database developed by Institute of Biostatistics and Analyses of Masaryk University in cooperation with oncology centers that contains records from the National Oncological Registry along with data supplied by healthcare providers to insurance companies for the purpose of retrospective reimbursement. Results: When comparing the four variables mentioned above we found that number of fractions and radiotherapy technique were much stronger predictors than the other two variables. Stage of disease did not prove to be a relevant indicator of cost distinction. There were significant differences in costs among diagnostic groups but these were mostly driven by the technique of radiotherapy and the number of fractions. Within the diagnostic groups, the distribution of costs was too heterogeneous for the purpose of the new payment model. Conclusion: The combination of number of fractions and radiotherapy technique appears to be the most appropriate cost predictors to be involved in the prospective payment model proposal. Further analysis is planned to test the predictive value of intention of radiotherapy in order to determine differences in costs between palliative and curative treatment. Axilární disekce představuje již řadu let standardní přístup k axilárnímu stagingu v případě pozitivní sentinelové uzliny či v případě klinicky pozitivní axilární uzliny. V posledních letech se stále více zdůrazňuje morbidita tohoto výkonu s ohledem na dlouhodobé přežívání pacientek s časným karcinomem prsu. Ze stejného důvodu byly iniciovány dvě velké studie, a to studie AMAROS a Z0011, které měly zodpovědět otázku, zda v případě pozitivní sentinelové uzliny je nutné doplnění axilární disekce s ohledem na skutečnost, že více než 70 % těchto pacientek bude mít v axilární disekci negativní uzliny a vlastní výkon pouze zvyšuje morbiditu bez jakéhokoliv přínosu pro pacientky. Dle výsledků těchto studií se zdá, že v případě dodržení inkluzních kritérií je možné axilární disekci vynechat bez zásadního efektu na přežívání pacientek. Podobný výsledek přinesla studie IBCSG 23-01 v případě mikrometastáz v axilární sentinelové uzlině.
For a long period of time, axillary dissection represented a standard approach for axillary node management in the case of sentinel node biopsy positivity during early stage breast cancer treatment. In recent years, there has been a trend to highlight the morbidity of such an axillary procedure considering long‑term survival of early stage breast cancer patients. Two big trials, AMAROS and Z0011, were initiated to answer the question whether axillary dissection should be performed in the case of positivity of axillary sentinel node considering the fact that more than 70% of these patients will have no metastasis found during the axillary dissection and such a procedure only increases the morbidity of the surgery. Considering the results of the above mentioned trials, axillary dissection may be avoided in the case of fulfilling of inclusion criteria of these trials without any impact on the patient survival. IBCSG 23–01 study brought similar conclusion in the case of micrometastasis in axillary sentinel node.
