OBJECTIVES: The aim of the study was to obtain data on demands on the intensive care capacities to treat COVID-19 patients, and to identify predictors for in-hospital mortality. METHODS: The prospective observational multicentre study carried out from 1 March till 30 June 2020 included adult patients with confirmed SARS-CoV-2 infection with respiratory failure requiring ventilatory support or high-flow nasal oxygen therapy (HFNO). RESULTS: Seventy-four patients, 46 males and 28 females, median age 67.5 (Q1-Q3: 56-75) years, were included. Sixty-four patients (86.5%) had comorbidity. Sixty-six patients (89.2%) were mechanically ventilated, four of them received extracorporeal membrane oxygenation therapy. Eight patients (10.8%) were treated with non-invasive ventilation and HFNO only. The median of intensive care unit (ICU) stay was 22.5 days. Eighteen patients (24.3%) needed continuous renal replacement therapy. Thirty patients (40.5%) died. Age and acute kidney injury were identified as independent predictors of in-hospital death, and chronic kidney disease showed trend towards statistical significance for poor outcome. CONCLUSIONS: Sufficient number of intensive care beds, organ support equipment and well-trained staff is a decisive factor in managing the COVID-19 epidemic. The study focused on the needs of intensive care in the COVID-19 patients. Advanced age and acute kidney injury were identified as independent predictors for in-hospital mortality. When compared to clinical course and ICU management of patients with severe community-acquired pneumonia caused by other pathogens, we observed prolonged need for ventilatory support, high rate of progression to acute respiratory distress syndrome and significant mortality in studied population.

In this retrospective study we assessed the frequency of hypogammaglobulinemia in 708 patients with SIRS, severe sepsis and septic shock. We evaluated the relationship between hypogammaglobulinemia IgG, IgM and 28 day mortality. Total of 708 patients and 1,513 samples were analyzed. In the three subgroups we investigated, patients met the criteria of SIRS, severe sepsis and septic shock. IgG hypogammaglobulinemia was demonstrated in 114 patients with severe sepsis (25.2%), 11 septic shock patients (24.4%), and in 29 SIRS patients (13.9%). IgM hypogammaglobulinemia was documented in 55 patients with severe sepsis (12.2%), 6 septic shock patients (13.3%), and in 17 SIRS patients (8.1%). Mortality of patients with severe sepsis and normal IgG levels was significantly lower (111 patients; 32.8%) compared with those with IgG hypogammaglobulinemia (49 patients; 43.0%; p=0.001). Mortality of patients with septic shock and IgG hypogammaglobulinemia (n=5) was significantly higher compared with those with normal IgG levels (45.5% vs. 38.2%; p=0.001). Mortality of patients with severe sepsis and IgM hypogammaglobulinemia did not differ from that of patients with normal IgM levels (37.0 vs. 41.8%). Mortality of patients with septic shock and IgM hypogammaglobulinemia was significantly higher compared with those with normal IgM levels (50% vs. 38.5%; p=0.0001). This study documented relatively high incidence of hypogammaglobulinemia IgG and IgM in patients with severe sepsis, septic shock and SIRS respectively. The presence of IgG hypogammaglobulinemia in patients with severe sepsis is independent factor of mortality.

Cíl studie: Autoři popisují velice dobré zkušenosti s rutinním stanovením aktivního vitaminu B12 – holotranskobalaminu (HTC) u pacientů odeslaných do metabolické ambulance Nemocnice na Homolce za posledních 5 let. Pacienti a metody: Za rok navštíví ambulanci zhruba 1000 dospělých i dětských pacientů s metabolickými poruchami, u kterých je diferenciálnědiagnosticky indikováno vyšetření HTC a stanoveno metodikou MEIA AxSym Active B-12 Assay (Abbott). Výsledky: Deficit HTC nacházíme kromě makrocytární anémie také u pacientů s nepoznanými neuropsychiatrickými, gastrointestinálními, srdečními a perikoncepčními problémy. Zřetelný deficit HTC je také prokazován při mozkových příhodách, infarktech myokardu, emboliích plícnice, trombóz, tromboembolií, typických i atypických perniciózních anémií, u mentálního opoždění a kognitivních poruch, u žen trpících dysfertilitou aj. Vybrané kazuistiky jsou stručně dokumentovány a dokazují, že správná diagnóza byla možná jen díky zavedení tohoto vyšetření do rutinního provozu laboratoře. Závěr: Autoři doporučují vyšetření HTC u shora uvedených klinických symptomů, protože spolu se stanovením metylmalonátu a homocysteinu dokáže snížená hladina HTC překvapivě objasnit vyvolávající příčinu mnoha různorodých nálezů a určit tu správnou diagnózu či pochopit neočekávané komplikace.
Objective: Authors are commenting very good experience with routinely use of holotranscobalamin (HTC) in patients attending the metabolic surgery of Hospital Homolka within last 5 yrs. Patients and Method: Among 1000 adult or children patients attending yearly the metabolic surgery the typical cases of HTC deficiency were detected in patients with unexplained neurological, psychiatric, gastroenterological, cardiologic, obstetric, nutritional and metabolic symptoms. The MEIA AxSym Active B-12 Assay (Abbott) method was introduced in routine laboratory use instead of total vitamin B-12 estimation. Results: Typical clinical cases of patients suffering from heart attacks, pulmonary embolism, typical or atypical pernicious anemias as well as in patients with deep vein thrombosis, thromboembolism, in patients with neuronal and perception disorders, in females suffering from fertility disorders are presented in details. All these patients were detected only thanks to introducing of very efficient method for HTC estimation in the routine use in clinical chemical laboratory. Conclusion: In many typical or atypical clinical cases of known or unknown origin, the estimation of active form of vitamin B-12-HTC together with methylmalonic acid and total homocysteine could be very helpful to find the reason for corresponding diagnosis or complications.

Determination of mTREM-1 expression on monocytes has been investigated as a perspective diagnostic method to distinguish infectious from non-infectious etiology of the inflammation. The aims of our study were: i) to investigate the expression of TREM-1 on monocytes in septic patients and in those after elective spinal surgery without infection; ii) to assess the dynamics of mTREM-1 expression on monocytes and its association with the outcome in patients with severe sepsis. Fifty two patients with severe sepsis, 20 healthy volunteers, and 20 patients after elective spinal surgery were involved in our study. TREM-1 expression on monocytes was evaluated by flow cytometry. Compared with the group of healthy adults (median 42.0, interquartile range (IQR) 30.3–76 MFI), mTREM-1 expression was increased in the group of septic patients both at entry (median 138.4, IQR 78.4–187.5 MFI) and the last examination (median 136.5, IQR 69.0–170.0 MFI) as well as in patients 24 hours after spinal surgery (median 138.5, IQR 45.3–165.5 MFI). The increase was statistically significant. mTREM-1 expression in patients undergoing spinal surgery and those with severe sepsis did not differ. TREM-1 expression on the monocytes in survivors was higher than in non-survivors (p=0.007). TREM-1 levels in septic non-surviving patients correlated weakly with TNF-α levels (r=0.38; p=0.003) and with HLA-DR/CD14 levels (r=0.38; p=0.003). Increased TREM-1 expression on monocytes is not associated exclusively with the presence of systemic infection.

Autoři sledovali možnost ovlivnění nitrolebntch poměrů inhalací oxidu dusnaxého (NO) u kraniocerebrálních poraněni (KCP) komplikovaných ARDS. HodnotíICP, CPP a vztah ICP/CPP za ustálených podmínek (MAP, PaOz, PaCOz„ tělesná teplota) v závislosti na koncentraci a době inhalace NO. Dospívají k závěru, že stabilita sledovaných parametrů připouští oprávněnou domněnku, že inhalace NO za uvedených podmínek neovlivňuje negativně nitrolebnt poměry.
The authors discuss the possibility to affect inxracranial milieu by nitric oxide (NO) inhalation in brain injury complicated by ARDS. Following parameters esere evaluated: ICP, CPP, ICP/CPP index at stable conditions (MAP, PaOz, PaCOz, body temperature) according to the concenxration and duration of inhalaxion of NO. Based opon the stability of the parameters, we can conclude Chat NO inhalation in the aforementioned setting does not reegatively affect intracranial milieu.
Autoři se zabývají významem monitorována vzájemného vztahu ICP a CPP, ktery' vyjadřuje formou indexu. Předpokládají, že tento vztah lépe charakterizuje poměry v intrakraniu u kraniocerebrálních poraněni (KCP), umožňuje hodnotit míru dysfunkce a zároveň optimalizovat terapii nitrolebnt hypertenze za simultánního monitorována cerebrální hemodynamiky, tkáňové oxygenace a vybraných biochemických parametrů.
The authors discuss the importance of relationship between ICP and CPP which is subsequenxly formulaxed as indu The relationship between those two criteria seems to provide better characteristics of the situation in the cranium during the head-brain injury. The index allows for evaluation of the severity of dysfunction and for optimalization of intracranial hypertension therapy, in accordance with the results obtaireed from simultaneous monitoring of cerebral haemodynamics and tissue oxygenation as well as selected biochemical parameters.
