AIMS: To analyse the hallmarks of ocular manifestations of and therapeutic modalities for syphilis in the last two decades. The clinical features of syphilitic uveitis, and association with the human immunodeficiency virus (HIV) coinfection are described. METHODS: Retrospective study of 16 patients diagnosed with ocular syphilis confirmed by serological tests in the General University Hospital in Prague between the years 2004 and 2021. General characteristics of ocular and systemic manifestations and visual functions were analysed. RESULTS: An increasing incidence of syphilitic uveitis correlates with a general rise in syphilis cases. In our study, the ocular manifestation of syphilis was panuveitis (44%), posterior uveitis (31%) and anterior uveitis (25%). Posterior uveitis was found in 3 patients (19%) associated with preretinal infiltrates, that are often present in syphilitic uveitis. The worst visual outcomes were among patients with human immunodeficiency virus (HIV) coinfection and/or neurosyphilis, however the data were not significant. Optic disc edema was present in 56%, macular involvement in 37% of patients. Overall, 31% of patients in our cohort had persistent visual field defects due to impairment of their optic nerve or macula despite the final median Snellen visual acuity of 1.0. Two out of sixteen patients were treated with corticosteroids in addition to antibiotics. CONCLUSION: Posterior uveitis with preretinal infiltrates and optic disc edema should arouse suspicion of ocular syphilis. Recent data show the advantages of adjacent systemic corticosteroid treatment for severe forms of syphilitic uveitis and/or neuritis. Our observation supports this finding.

Peripheral lesion of the facial nerve leads to ipsilateral palsy of facial muscles. Weakening of the orbicularis oculi muscle results in inability to close the eyelids, i.e., lagophtalmos. As a result of lagophtalmos, the lower parts of the eye surface dry and cause a formation of exposure keratopathy of variable severity. Chronic corneal changes lead to development of vascularized corneal scar, which permanently deteriorates the visual acuity. Immediately after the diagnosis of peripheral facial nerve palsy by the neurologist, frequent application of ophthalmic lubricants and an early ophthalmological examination are recommended. The treatment of eye complications depends on the character of corneal involvement and extent of lagophtalmos, considering the possibility of its spontaneous restitution. The therapy is surgical in serious cases, aimed at adjusting the position of the eyelids, and in case of a non-healing corneal ulcer, the amniotic membrane transplant is indicated.
Periferní léze nervus facialis vede ke stejnostranné paréze mimických svalů. Oslabení musculus orbicularis oculi má za následek nedovírání oční štěrbiny, tedy lagoftalmus. V důsledku lagoftalmu osychají dolní části povrchu oka, což se projeví rozvojem různě závažné expoziční keratopatie. Chronické změny rohovky způsobují její jizvení doprovázené vaskularizací a vedou k trvalému zhoršení zrakové ostrosti. Bezprostředně po stanovení diagnózy neurologem je doporučena frekventní aplikace lubrikancií do spojivkového vaku a časné oftalmologické vyšetření. Léčba očních komplikací závisí na charakteru postižení rohovky a rozsahu lagoftalmu s přihlédnutím k možnosti jeho spontánní restituce. Terapie závažnějších stavů je chirurgická, zaměřena na úpravu polohy a postavení víček a v případě nehojícího se vředu rohovky je řešením transplantace amniové membrány.

Mikroorganismy osídlující všechny povrchy sliznic a kůže tvoří s hostitelem komplexní ekosystém a nazýváme je mikrobiotou. Pro souhrnný genom mikrobioty se používá termín mikrobiom. Mikrobiota je důležitou součástí mechanizmů ovlivňujících řadu fyziologických i patologických pochodů, zejména imunitního systému hostitele. Na vznik a průběh autoimunitních onemocnění nejen v trávicím traktu, ale i ve vzdálených orgánech, včetně oka, mají významný vliv především mikroby osidlující střevo (střevní mikrobiota). Podíl mikrobioty a jejích změn (dysbiózy) na etiopatogenezi uveitid je zkoumán zatím především na experimentálních modelech. Snížení závažnosti neinfekčního nitroočního zánětu u bezmikrobních myší nebo u konvenčních myší léčených širokospektrými antibiotiky bylo pozorováno jak u indukovaného modelu experimentální autoimunitní uveitidy (EAU), tak u R161H myšího modelu spontánní uveitidy. Studie potvrdily, že aktivace autoreaktivních T lymfocytů se odehrává ve střevní stěně bez přítomnosti sítnicového antigenu. Řada experimentů se věnuje vlivu podávání probiotik na složení střevní mikrobioty a na průběh autoimunitní uveitidy. Naše pracovní skupina recentně prokázala signifikantní preventivní vliv podávání probiotika Escherichia coli Nissle 1917 na intenzitu zánětu u indukovaného modelu EAU. Dosud bylo publikováno jen několik studií zkoumajících střevní dysbiózu u pacientů s uveitidou (např. u Behçetovy nemoci či u Vogtova-Koyanagiho-Haradova syndromu). Předpokládá se, že výsledky preklinických studií najdou využití i v klinické praxi, a to převážně v linii profylaxe a terapie. Mohly by tak být uplatněny změny životního stylu, stravování, a především léčebné použití probiotik, případně přenos fekální mikrobioty
Microorganisms inhabiting all surfaces of mucous membranes and skin and forming a complex ecosystem with the host is called microbiota. The term microbiome is used for the aggregate genome of microbiota. The microbiota plays important role in the mechanisms of number of physiological and pathological processes, especially of the host's immune system. The origin and course of autoimmune diseases not only of the digestive tract, but also of the distant organs, including the eye, are significantly influenced by intestinal microbiota. The role of microbiota and its changes (dysbiosis) in the etiopathogenesis of uveitis has so far been studied mainly in experimental models. Reduction of severity of non-infectious intraocular inflammation in germ-free mice or in conventional mice treated with broad-spectrum antibiotics was observed in both the induced experimental autoimmune uveitis model (EAU) and the spontaneous R161H model. Studies have confirmed that autoreactive T cell activation occurs in the intestinal wall in the absence of retinal antigen. Recent experiments focused on the effect of probiotic administration on the composition of intestinal microbiota and on the course of autoimmune uveitis. Our study group demonstrated significant prophylactic effect of the administration of the probiotic Escherichia coli Nissle 1917 on the intensity of inflammation in EAU. To date, only a few studies have been published investigating intestinal dysbiosis in patients with uveitis (e.g., in Behcet's disease or Vogt-Koyanagi-Harada syndrome). The results of preclinical studies will be presumably used in clinical practice, mainly in the sense of prophylaxis and therapy, such as change in the lifestyle, diet and especially the therapeutic use of probiotics or the transfer of faecal microbiota.

Cíl: Cílem této studie bylo popsat klinické nálezy a molekulárně genetickou příčinu hereditární gelsolinové amyloidózy v rodině českého původu a u probandky slovenského původu. Soubor a metody: Jedinci účastnící se studie podstoupili oční, neurologické, nefrologické a kardiologické vyšetření. Pomocí Sangerova sekvenování byl proveden screening genu gelsolin (GSN). Výsledky: Identifikovány byly dvě mutace, které byly již dříve popsány v souvislosti s hereditární gelsolinovou amyloidózou. Konkrétně u sedmi jedinců českého původu byla v heterozygotním stavu zjištěna c.640G>T p. (Asp214Tyr) a u probandky slovenského původu c.640G>A p. (Asp214Asn). U většiny nositelů patogenních variant byla přítomna lineární průhledná depozita v rohovkách, tato nebyla patrná pouze u dvou nejmladších mužů ve věku 24 a 14 let. U jedinců ve čtvrté dekádě života dominovaly jako klinické známky onemocnění kromě rohovkových depozit povislá oční víčka a syndrom karpálního tunelu. Dva nejstarší pacienti ve věku 65 a 68 let měli i další typické znaky gelsolinové amyloidózy: syndrom suchého oka, cutis laxa a parézu n. facialis. U 68letého muže byly dále zjištěny těžká polyneuropatie, ataxie, dysartrie a arytmie s nutností implantace kardiostimulátoru. Závěr: Hereditární gelsolinovou amyloidózu je třeba zahrnovat do diferenciální diagnózy neuropatií a amyloidóz neznámé etiologie. Vzhledem k tomu, že ukládání amyloidu v rohovkách je možné snadno detekovat, má oční vyšetření nezastupitelnou úlohu v diagnostickém procesu.
Aim: The aim of this study was to describe the clinical findings and molecular genetic cause of hereditary gelsolin amyloidosis in a family of Czech origin and a proband of Slovak origin. Patients and methods: Study participants underwent ophthalmic, neurological, nephrological, and cardiological examination. Sanger sequencing was used to screen the gelsolin gene (GSN). Results: Two mutations previously reported to be associated with hereditary gelsolin amyloidosis were identified; c.640G>T p. (Asp214Tyr) in a heterozygous state was found in seven individuals of Czech origin and c.640G>A p. (Asp214Asn) in the proband of Slovak origin. Linear corneal deposits were observed in the majority of affected subjects with the exception of two men aged 24 and 14 years. In addition to corneal deposits, patients in their fourth decade of life had drooping eyelids and carpal tunnel syndrome. Two oldest patients, aged 65 and 68 years, had also other typical signs of gelsolin amyloidosis, including dry eye syndrome, cutis laxa, and facial nerve lesion. The 68-year-old subject also had severe polyneuropathy, ataxia, dysarthria, and arrhythmia necessitating pacemaker implantation. Conclusion: Hereditary gelsolin amyloidosis should be included in the differential diagnosis of neuropathies and amyloidosis of unknown etiology. Since amyloid deposition in the cornea is easily detectable, ophthalmic examination has a crucial role in the diagnosis of this disease.

Imunitně podmíněná zánětlivá onemocnění se dělí na autoimunitní a autoinflamatorní. Autoimunitní onemocnění jsou charakterizována přítomností autoreaktivních lymfocytů či autoprotilátek, např. primární Sjögrenův syndrom či revmatoidní artritida. Izolovaným očním onemocněním s předpokládanou autoimunitní etiologií je sympatická oftalmie či birdshot chorioretinopatie. Autoinflamatorní onemocnění jsou způsobena genetickými mutacemi v oblastech zapojených do regulace mechanismů vrozené imunity. Mezi monogenně podmíněná autoinflamatorní onemocnění patří hereditární periodické horečky, z očních onemocnění je to Blauův syndrom neboli sarkoidóza s časným začátkem. Článek shrnuje přehled současných znalostí patogeneze imunitně podmíněných uveitid, jejich imunologických mechanismů a možného iniciačního podílu infekce. Imunologické privilegium zajišťuje ochranu oka před silnou imunitní reakcí na cizí antigen, a to na úrovni fyzikální, imunitní, buněčné a molekulární. Narušení hematoretinální bariéry infekcí či mechanickým vlivem odkryje antigeny skryté v oku, vůči kterým nebyla během vývoje vytvořena tolerance, a tak dochází ke vzniku imunitního zánětu. Podle současných znalostí je významným zdrojem uveogenních podnětů i vlastní mikrobiom, zejména při jeho dysregulaci, tzn. dysbióze. U lidí je známá asociace nespecifických střevních zánětů s ankylózující spondylitidou a přední uveitidou. Vazbou mezi mikrobiomem a imunitně podmíněnými záněty se zabývá mnoho výzkumných záměrů s cílem terapeuticky ovlivnit střevní mikrobiom. Vzhledem k relativně vzácnému výskytu imunitně podmíněných uveitid u lidí jsou využívány experimentální modely ke studiu imunopatologických mechanismů a k testování nových terapeutických možností.
Immune mediated inflammatory diseases are categorized into autoimmune and autoinflammatory. Autoimmune etiology is represented by autoreactive lymphocytes or autoantibodies, e.g. primary Sjögren‘s syndrome or rheumatoid arthritis. Ocular specific diseases with presumed autoimmune origin are sympathetic ophthalmia or birdshot chorioretinopathy. Autoinflammatory diseases are caused by mutations in regulatory genes for specific immunity. Hereditary periodic fevers represent monogenic autoinflammatory diseases; eye specific is Blau syndrome also named sarcoidosis with early onset. This article reviews the actual knowledge about immune mediated uveitides, their immunological mechanisms and the possible trigger role of infection in autoimmune inflammation. Immune privilege provides a protection of the eye against any strong immune reaction to foreign antigen, based on physical, immune, humoral and molecular mechanisms. Antigens hidden within the eye are revealed in case of damage of hematoretinal barrier caused by infection or mechanical insult. These ocular antigens have not been set as tolerable during the development and immune reaction is initiated subsequently. Current studies demonstrate that uveogenic trigger might be generated by own microbiome, particularly when dysregulated, so called dysbiosis. There is a known association between idiopathic inflammatory bowel disease with ankylosing spondylitis and anterior uveitis in humans. Intensive research is focused on microbiome and immune mediated inflammatory disease to influence therapeutically the intestinal microbiome. The animal models are used to study the immunopathological mechanisms of uveitis and the new therapeutic strategies, because of relatively low incidence of immune mediated uveitis in humans.

AIMS: Cases of infectious uveitis in immunodeficient patients may present with atypical clinical features because the clinical course of disease is usually affected by the compromised immune system. Therefore, it is sometimes difficult to determine the correct diagnosis. The aim of this study was to evaluate a prevalence of immunodeficient HIV-negative individuals among patients with infectious uveitis and to assess diagnostic and therapeutic approaches. METHODS: A retrospective study. RESULTS: In years 2003-2017, we diagnosed 594 patients with infectious uveitis. In 35 of them, infectious uveitis occurred on the basis of immunodeficiency (malignancy, immunosuppressive therapy etc.). The most common infectious uveitis was cytomegalovirus retinitis, followed by acute retinal necrosis, herpetic anterior uveitis, endogenous fungal endophthalmitis, toxoplasmic retinochoroiditis, progressive outer retinal necrosis and syphilis. In indicated cases, intraocular fluid examination was a valuable diagnostic tool. After initiation of treatment, improvement of BCVA was observed in 13 eyes, stabilization in 13 eyes and deterioration in 11 eyes. In some patients who experienced adverse effects of medication, the recommended doses were decreased. CONCLUSION: Our experience shows that patients with diagnoses of acute retinal necrosis or progressive outer retinal necrosis have a poor visual prognosis. The occurrence of cytomegalovirus retinitis signifies a very unfavorable survival prognosis in patients who underwent hematopoietic stem cell transplantation; the patients in our study died within 1 year from cytomegalovirus retinitis diagnosis.

Autoimmune uveitis is a serious sightthreatening disease that in many cases fails to respond to conventional immunosuppressive or biological therapy. Experimental models used in research allow more detailed study of pathogenesis of the autoimmune process and testing new therapeutic strategies. Recent results show that infection can trigger autoimmune diseases, and some commensal microorganisms are essential in causing disease activity. The aim of this work was to assess the effect of broadspectrum antibiotics - combination of metronidazole and ciprofloxacin or metronidazole alone - on the intensity of intraocular inflammation in experimental autoimmune uveitis (EAU). EAU was induced in mouse strain C57BL/6J by interphotoreceptor retinoid- binding protein in complete Freund's adjuvant and pertussis toxin. The grade of uveitis was assessed clinically and histologically in haematoxylin and eosin- stained tissues. Lymphocytes and macrophages were detected in cryosections using the immunoperoxidase method with antibodies. The therapy was commenced one week before EAU induction and continued throughout the experiment. In addition, metronidazole treatment was also started two weeks before EAU induction. Antibiotics significantly reduced the intensity of uveitis compared to the control group (P < 0.05). The effects of combination of ciprofloxacin and metronidazole and of metronidazole alone were similar when the therapy started one week before EAU induction (P < 0.05). Metronidazole commenced two weeks before EAU induction and throughout the experiment suppressed the intensity of EAU with even higher statistical significance (P < 0.0001). It can be assumed that the high protective effect of metronidazole on EAU intensity may be due not only to its antimicrobial effect, but also to its immunomodulatory activity.

Imunitně podmíněná zánětlivá onemocnění nejčastěji postihují klouby, kůži, ledviny, plíce, mohou se manifestovat neuro‑ logickými a očními příznaky. Očním projevem imunitně podmíněných zánětlivých onemocnění může být postižení spojivky (konjunktivitida), skléry a episkléry (skleritida a episkleritida), rohovky (keratitida), sítnice (retinitida), uveální tkáně, tedy du‑ hovky, řasnatého tělesa a cévnatky (uveitida). Postiženo může být nejen oko, ale také zrakový nerv a periokulární tkáň – slzná žláza a měkké tkáně orbity. Oční projevy mohou být mírné i zrak ohrožující, někdy způsobují dyskomfort až bolest. Léčba lokální kapkami, periokulární injekcí či intravitreální aplikací léčiva do sklivce závisí na charakteru očního projevu a etiologii systémového onemocnění. V případě těžkého postižení spojeného s rizikem zhoršení vízu je indikována systémová terapie, která je vedena ve spolupráci se specialistou v daném oboru.
Immune mediated inflammatory diseases cause impairment of joints, skin, kidneys, lungs or cause neurological and ocular symp‑ toms. Ocular manifestation of immune mediated inflammatory diseases involves conjunctiva (conjunctivitis), sclera and episclera (scleritis and episcleritis), cornea (keratitis), retina (retinitis), uvea, which comprises iris, ciliary body and choroid (uveitis). Beside the eye, surrounding tissues may be affected – optic nerve, lacrimal gland and soft tissues of the orbit. The severity of ocular im‑ pairment may be variable, even sight threatening, sometimes cause discomfort or pain. Local therapy with eye drops, periocular injection or intravitreal application of therapeutics depends on type of ocular manifestation and etiology of systemic disease. In case of severe impairment with high risk of vision loss, the systemic therapy is indicated in cooperation with related specialist.
