OBJECTIVE: Developmental and epileptic encephalopathies (DEEs) are a group of severe, early-onset epilepsies characterised by refractory seizures, developmental delay, or regression and generally poor prognosis. DEE are now known to have an identifiable molecular genetic basis and are usually examined using a gene panel. However, for many patients, the genetic cause has still not been identified. The aims of this study were to identify causal variants for DEE in patients for whom the previous examination with a gene panel did not determine their genetic diagnosis. It also aims for a detailed description and broadening of the phenotypic spectrum of several rare DEEs. METHODS: In the last five years (2015-2020), 141 patients from all over the Czech Republic were referred to our department for genetic testing in association with their diagnosis of epilepsy. All patients underwent custom-designed gene panel testing prior to enrolment into the study, and their results were inconclusive. We opted for whole exome sequencing (WES) to identify the cause of their disorder. If a causal or potentially causal variant was identified, we performed a detailed clinical evaluation and phenotype-genotype correlation study to better describe the specific rare subtypes. RESULTS: Explanatory causative variants were detected in 20 patients (14%), likely pathogenic variants that explain the epilepsy in 5 patients (3.5%) and likely pathogenic variants that do not fully explain the epilepsy in 11 patients (7.5%), and variants in candidate genes in 4 patients (3%). Variants were mostly de novo 29/40 (72.5%). SIGNIFICANCE: WES enables us to identify the cause of the disease in additional patients, even after gene panel testing. It is very important to perform a WES in DEE patients as soon as possible, since it will spare the patients and their families many years of a diagnostic odyssey. In particular, patients with rare epilepsies might significantly benefit from this approach, and we propose using WES as a new standard in the diagnosis of DEE instead of targeted gene panel testing.

Cílem článku je poskytnout čtenáři návod pro orientaci v problematice. Pro stanovení diagnózy nekonvulzivního status epilepticus (NCSE) je nutné EEG. Za tímto účelem vznikly v posledních letech souběžně dva kompatibilní diagnostické systémy, které jsou navzájem propojené. Jsou to jednak tzv. Salcburská kritéria pro diagnózu NCSE, která definují EEG nálezy jednoznačné - potvrzující diagnózu NCSE - a nálezy nejednoznačné, které této diagnóze odpovídat mohou, ale nemusí (tzv. "možný NCSE"). Salcburská kritéria využívají precizních definicí druhého systému - Standardizované terminologie EEG u kriticky nemocných (ACNS-SCCET), který se tím stává hlavním nástrojem pro popis EEG u všech pacientů s podezřením na NCSE. Poslední verze ACNS-SCCET z roku 2021 obsahuje i návrh definicí epileptických záchvatů a statů včetně NCSE (převážně akceptující Salcburská kritéria) a tzv. "iktálně-interiktálního kontinua", které představuje synonymum pro "možný NCSE".
EEG is essential for diagnosis of non-convulsive status epilepticus (NCSE). Two compatible diagnostic systems have been developed for this purpose during last 15 years. The Salzburg Consensus Criteria for Nonconvulsive Status Epilepticus define unequivocal EEG patterns that confirm NCSE but also patterns that may or may not represent NCSE (i. e. "possible NCSE"). The Salzburg Criteria adopt the rigorous definitions of the American Clinical Neurophysiology Society's Standardized Critical Care EEG Terminology (ACNS-SCCET), which, in turn, became the main tool for description of EEG in all patients suspected to suffer from NCSE. Its latest 2021 version contains new propositions for the definitions of epileptic seizures and status epilepticus that are compatible with Salzburg Criteria, as well as a proposition for the definition of "ictal-interictal continuum", which is synonymous with "possible NCSE".
BACKGROUND AND OBJECTIVES: Pathogenic STXBP1 variants cause a severe early-onset developmental and epileptic encephalopathy (STXBP1-DEE). We aimed to investigate the natural history of STXBP1-DEE in adults focusing on seizure evolution, the presence of movement disorders, and the level of functional (in)dependence. METHODS: In this observational study, patients with a minimum age of 18 years carrying a (likely) pathogenic STXBP1 variant were recruited through medical genetics departments and epilepsy centers. Treating clinicians completed clinical questionnaires and performed semistructured video examinations while performing tasks from the (modified) Unified Parkinson Disease Rating Scale when possible. RESULTS: Thirty adult patients were included for summary statistics, with video recordings available for 19 patients. The median age at last follow-up was 24 years (range 18-58 years). All patients had epilepsy, with a median onset age of 3.5 months. At last follow-up, 80% of adults had treatment-resistant seizures despite long periods of seizure freedom in 37%. Tonic-clonic, focal, and tonic seizures were most frequent in adults. Epileptic spasms, an unusual feature beyond infancy, were present in 3 adults. All individuals had developmental impairment. Periods of regression were present in 59% and did not always correlate with flare-ups in seizure activity. Eighty-seven percent had severe or profound intellectual disability, 42% had autistic features, and 65% had significant behavioral problems. Video examinations showed gait disorders in all 12 patients able to walk, including postural abnormalities with external rotation of the feet, broad-based gait, and asymmetric posture/dystonia. Tremor, present in 56%, was predominantly of the intention/action type. Stereotypies were seen in 63%. Functional outcome concerning mobility was variable ranging from independent walking (50%) to wheelchair dependence (39%). Seventy-one percent of adults were nonverbal, and all were dependent on caregivers for most activities of daily living. DISCUSSION: STXBP1-DEE warrants continuous monitoring for seizures in adult life. Periods of regression are more frequent than previously established and can occur into adulthood. Movement disorders are often present and involve multiple systems. Although functional mobility is variable in adulthood, STXBP1-DEE frequently leads to severe cognitive impairments and a high level of functional dependence. Understanding the natural history of STXBP1-DEE is important for prognostication and will inform future therapeutic trials.

PURPOSE: Focal cortical dysplasia (FCD) represents a common cause of refractory epilepsy. It is considered a sporadic disorder, but its occasional familial occurrence suggests the involvement of genetic mechanisms. METHODS: Siblings with intractable epilepsy were referred for epilepsy surgery evaluation. Both patients were examined using video-EEG monitoring, MRI examination and PET imaging. They underwent left anteromedial temporal lobe resection. RESULTS: Electroclinical features pointed to left temporal lobe epilepsy and MRI examination revealed typical signs of left-sided hippocampal sclerosis and increased white matter signal intensity in the left temporal pole. PET examination confirmed interictal hypometabolism in the left temporal lobe. Histopathological examination of resected tissue demonstrated the presence FCD type IIIa, i.e. hippocampal sclerosis and focal cortical dysplasia in the left temporal pole. CONCLUSION: We present a unique case of refractory mesial temporal lobe epilepsy in siblings, characterized by an identical clinical profile and histopathology of FCD type IIIa, who were successfully treated by epilepsy surgery. The presence of such a high concordance between the clinical and morphological data, together with the occurrence of epilepsy and febrile seizures in three generations of the family pedigree points towards a possible genetic nature of the observed FCD type IIIa.

Po prvním záchvatu je nutné určit, zda se jednalo o záchvat epileptický nebo neepileptický, a v případě záchvatu epileptického pátrat po možné akutní příčině. Odlišení záchvatů neepileptických a také identifikace záchvatů epileptických provokovaných (akutních symptomatických) je z hlediska stanovení dalšího postupu, ať již diagnostického či terapeutického, zcela zásadní. Pro odlišení jednotlivých typů záchvatů jsou rozhodující údaje v anamnéze se zaměřením na detailní popis průběhu záchvatu a okolností jeho vzniku, poté i výsledky pomocných vyšetření. Akutní symptomatické záchvaty představují až 40 % všech prvních epileptických záchvatů a vznikají v těsné časové souvislosti s probíhajícím postižením centrálního nervového systému (CNS) různé povahy. Jejich příčina je jasně identifikovatelná – nejčastěji se jedná o akutní cévní mozkové příhody, kraniocerebrální poranění, zánětlivá onemocnění CNS, významné jsou ale i vlivy primárně extracerebrální – toxické a metabolické poruchy, užívání léků nebo alkoholu. Pokud jde o první epileptický záchvat neprovokovaný nebo reflexní, je možné jej v případech přetrvávající predispozice s vysokým (> 60 %) rizikem opakování dle nově přijaté definice považovat za epilepsii, případně i zahájit dlouhodobou léčbu antiepileptiky. Po prvním neprovokovaném záchvatu by mělo být vždy doplněno vyšetření mozku magnetickou rezonancí v adekvátním protokolu. Z hlediska posouzení rizika opakování záchvatu má kromě anamnestických údajů význam i případná abnormita v EEG či odchylky v objektivním nálezu svědčící pro postižení CNS. Zahájení léčby je po prvním záchvatu indikováno při vysokém riziku opakování či při zvýšené pravděpodobnosti závažných komplikací dalšího záchvatu.
Following a first seizure, it is important to establish whether this was an epileptic or non-epileptic seizure, and to ascertain possible cause of an epileptic seizure. With respect to further management, diagnostic or therapeutic, it is crucial to differentiate non-epileptic from epileptic seizures and to identify provoked epileptic seizures (acute symptomatic). Patient history with a detailed description of the course of a seizure and circumstances under which it happened is instrumental in discriminating between the different types of seizures. Complementary investigations can further support the diagnostic process. Acute symptomatic seizures represent up to 40% of all single epileptic seizures and occur in close chronological proximity to an ongoing central nervous system injury of various origins. The source can easily be identified – most frequently this is an acute cerebrovascular event, craniocerebral injury, inflammatory CNS disorder, while primarily extracerebral effects are also important – toxic and metabolic disorders and drug or alcohol abuse. Unprovoked or reflex single epileptic seizures in patients with persisting predisposition and a high risk (> 60%) of relapse may, according to the new definition, be regarded as epilepsy and long-term antiepileptic treatment may be initiated. MRI under appropriate protocol should always be performed after the first unprovoked seizure. The risk of seizure recurrence can be estimated based on patient history as well as any EEG or physical examination abnormity that suggests an injury of the CNS. Antiepileptic treatment is indicated in patients with a single seizure if there is a high risk of recurrence or an increased likelihood of severe complications of further seizures. Význam provádění genetických testů v klinické praxi je však i přes pokroky v identifikaci genů ovlivňujících riziko vzniku různých epilepsií stále ve většině případů sporný. Tento II. díl poskytuje aktuální informace o genetice monogenně podmíněných idiopatických epileptických syndromů a epileptických encefalopatií s identifikovanou genetickou příčinou a recentní pohled na výtěžnost genetických testů u těchto syndromů.

Both physicians and common population have become more aware of possible genetic reasons for epilepsy and demands for genetic testing of patients with epilepsy have increased too. But despite the progress in identification of genes contributing to different epileptic syndromes the utility of genetic tests in clinical epileptology is often controversial. This second part of our article provides recent genetic information on idiopathic epileptic syndromes and epileptic encephalopathies with identified genetic aetiology, possibilities of genetic testing and current opinion on the clinical utility of genetic testing in respective syndromes.

Tento článek poskytuje aktuální přehled idiopatických epileptických syndromů a epileptických encefalopatií s identifikovanou genetickou příčinou a recentní pohled komise ILAE na výtěžnost genetických testů.

The overview of idiopathic epileptic syndromes and epileptic encephalopathies with identified genetic aetiology, possibilities of genetic testing and current opinion on the clinical utility of genetic testing in specific syndromes is provided.
Akutní symptomatické záchvaty jsou epileptické záchvaty vznikající v těsné časové souvislosti s probíhajícím postižením CNS různé povahy. Od záchvatů neprovokovaných se liší prognózou – mají vyšší časnou mortalitu, která souvisí s etiologií, a nižší riziko rozvoje epilepsie. Kromě odlišné patogeneze se předpokládá různý patofyziologický mechanizmus vzniku záchvatu. Akutní symptomatické záchvaty představují až 40 % všech prvních záchvatů. Jejich příčina je jasně identifikovatelná. Nejčastěji se jedná o akutní cévní příhody různé povahy, kraniocerebrální poranění, zánětlivá onemocnění CNS, toxické a metabolické poruchy, užívání alkoholu. Nejzávažnější formou akutního symptomatického záchvatu je akutní symptomatický status epilepticus. Z hlediska prognózy je důležité identifikovat příčinu záchvatu či statu, neboť léčba se zaměřuje na ovlivnění vyvolávající příčiny a na zastavení záchvatu.
The term acute symptomatic seizure describes a seizure occurring in close temporal relationship with various CNS insults. In contrast to unprovoked seizures, early mortality of acute symptomatic seizures is higher and largely aetiology-dependent. On the other hand, the risk of developing epilepsy following acute symptomatic seizure is lower than in unprovoked seizure. Apart from different pathogenesis, different mechanisms of ictogenesis are implied as well. Acute symptomatic seizures represent approximately 40% of all first seizures. Their aetiology is clearly identifiable – most common causes include cerebrovascular events, CNS injuries, inflammatory disorders of the CNS, metabolic derangements and drug-related and toxic causes, especially alcohol abuse. The most severe form of acute symptomatic seizure is acute symptomatic status epilepticus. Therapy is focused not only on seizure termination, but, more importantly, on treating the underlying aetiology. Therefore, timely ascertainment of the cause is a crucial factor determining the patients´ outcome.
