Terapie srdečního selhání je komplexní zahrnuje jak režimová opatření, tak farmakologické a nefarmakologické postupy. Moderní léčba vedla k prodloužení přežití nemocných a zlepšení kvality jejich života. I přesto zůstává prognóza nemocných se srdečním selháním závažná. Základem farmakologické léčby je inhibice patologicky zvýšené aktivity sympatoadrenergního systému (β-blokátory) a systému renin-angiotenzin-aldosteron (inhibitory angiotenzin konvertujícího enzymu/blokátory receptorů AT1 pro angiotenzin II a antagonisté mineralokortikoidních receptorů). Zcela novou lékovou skupinou v léčbě srdečního selhání s pozitivním přínosem na mortalitu jsou duální inhibitory angiotenzinových receptorů a neprilysinu (sakubitril/valsartan). Velkým příslibem v léčbě srdečního selhání jsou inhibitory SGLT2. Současná strategie léčby je založena na doporučeních Evropské kardiologické společnosti z roku 2016 a konsenzu expertů Asociace srdečního selhání Evropské kardiologické společnosti z roku 2019.
Therapy of heart failure is complex, it contains lifestyle changes, pharmacological and non-pharmacological approaches. Modern treatment reduces mortality and improves quality of life. Nevertheless, the prognosis of patients with heart failure remains severe. Pharmacological therapy is based on the inhibition of the pathologically enhanced activity of the sympatoadrenergic system (betablockers) and the renin-angiotensin-aldosterone system (angiotensin-converting enzyme inhibitors/AT1 receptor blockers for angiotensin II and mineralocorticoid receptor antagonists). Angiotensin receptor – neprilysin inhibitors (sacubitril/valsartan) are a new drug class in the treatment of heart failure with a positive mortality benefit. SGLT2 inhibitors are a great promise in the treatment of heart failure. The current treatment strategy is based on the recommendations of the 2016 European Society of Cardiology and the consensus of the European Heart Society Association of 2019 experts.

The causes of nonischemic dilated cardiomyopathy are classified as genetic or nongenetic, but environmental factors such as metal pollutants may interact with genetic susceptibility. The presence of metal particles has been detected in the myocardium, including in those patients with dilated cardiomyopathy. It is also known that hypersensitivity reactions can induce inflammation in tissue. The present study aimed to verify if metal-induced delayed-type hypersensitivity is present in patients with nonischemic dilated cardiomyopathy. The patient group consisted of 30 patients with newly diagnosed dilated cardiomyopathy; the control group comprised 41 healthy subjects. All patients and control subjects provided blood samples for lymphocyte transformation testing (MELISA®) to assess possible hypersensitivity to seven common metals. Specific exposure to metals was based on interview data. Results showed that exposure to cadmium and lead (p = 0.0002), aluminum (p = 0.0006), nickel (p = 0.0012), and chromium (p = 0.0065) was more often reported by patients than controls. The patients also had significantly more frequent hypersensitivity reactions to mercury (26.7% vs. 7.3%, p = 0.014624), nickel (40% vs. 12.2%, p = 0.02341), and silver (20% vs. 4.8%, p = 0.025468) than the control group. Patients with dilated cardiomyopathy had greater exposure to certain metals compared with healthy controls. Hypersensitivity to metals was more frequent in patients with dilated cardiomyopathy, suggesting a possible association that warrants further investigation.

Introduction: Viral infections are considered the most frequent cause of myocarditis and dilated cardiomyopathy (DCM). Material and methods: We investigated the changes in viral presence and the impact of viral genome persistence in the myocardium on echocardiographic parameters, functional status and some laboratory parameters in a 6-month follow-up. Fifty-four patients with recent onset DCM, left ventricular ejection fraction < 40% and biopsy-proven myocarditis (> 14 mononuclear leukocytes/mm2 and/or > 7 T-lymphocytes/mm2) were enrolled. Polymerase chain reaction (PCR) was performed to detect pathogens in the myocardium. Patients were divided according to the administered therapy: standard heart failure medication (46 patients) and immunosuppressive therapy (8 patients). Results: In the standard heart failure medication group viral clearance was observed in 13 patients and viral persistence in 24 patients in the follow-up period. Comparing both groups, there was no statistically significant difference - LVEF improvement of 12.0 ±11.4% vs. 18.3 ±12.6%, decrease in NYHA class of 0.7 ±0.7 vs. 1.0 ±0.7, decline in NT-proBNP of 1335 ±1933 ng/l vs. 1942 ±3242 ng/l and decrease in infiltrating leukocytes of 11.1 ±15.8 vs. 6.7 ±23.0 cells/mm2 and T-lymphocytes of 5.8 ±15.1 vs. 1.8 ±10.9 cells/mm2 (all p = NS). A decrease in PCR positive patients from 37 to 29 was observed. The number of PVB19 positive PCR findings decreased from 5 to 4 in patients with immunosuppressive therapy. Conclusions: A decrease in the number of positive PCR findings in control endomyocardial biopsy was observed. Viral genome persistence was not associated with worse outcome in short-term follow-up.

Myokarditida je zánětlivé postižení srdečního svalu. Nejčastější příčinou myokarditidy jsou v průmyslově vyspělých zemích virové infekce. Myokarditida s dysfunkcí levé komory srdeční se nazývá zánětlivá kardiomyopatie (ZKMP) a představuje hlavní příčinu vzniku dilatační kardiomyopatie. Klinický obraz je velmi různorodý, nejčastěji se onemocnění prezentuje známkami srdečního selhání, arytmickými příznaky a bolestmi na hrudi. Přes významný pokrok v neinvazní diagnostice, zejména ve využití magnetické rezonance srdce, zůstává endomyokardiální biopsie nadále zlatým standardem diagnostiky myokarditidy. Terapie myokarditidy a ZKMP je založena především na restrikci fyzické zátěže a farmakoterapii srdečního selhání. Specifická léčba – imunosupresivní či antimikrobiální – může být v určitých situacích zvážena dle výsledku endomyokardiální biopsie. Implantace mechanické srdeční podpory či transplantace srdce je indikována pouze v nejzávažnějších případech.
Myocarditis is an inflammation of the heart muscle. The most common cause of myocarditis is viral infection in industrialized countries. Myocarditis with left ventricular dysfunction is called inflammatory cardiomyopathy and is the major cause of dilated cardiomyopathy. The clinical picture is very diverse, most often the patient present with signs of heart failure, arrhythmic symptoms and chest pains. Despite significant advances in non-invasive diagnostics, particularly magnetic resonance imaging, endomyocardial biopsy remains the gold standard of myocarditis diagnosis. Myocarditis and inflammatory cardiomyopathy therapy is primarily based on the restriction of physical activity and the pharmacotherapy of heart failure. Specific treatment – immunosuppressive or antimicrobial – may be considered in some cases according to endomyocardial biopsy results. Implantation of mechanical cardiac support or heart transplantation is indicated only in the most serious cases.

AIMS: To compare the differences between patients with inflammatory cardiomyopathy (ICM) with and without improvement in left ventricular (LV) systolic function and to identify the relevant predictors of LV improvement. PATIENTS AND METHODS: The study included 63 patients with biopsy-proven ICM and heart failure symptoms of at least NYHA II, symptom duration ≤ 6 months, LV ejection fraction (LVEF) ≤ 40% assessed by echocardiography and presence of >14 mononuclear leukocytes (LCA+ cells)/mm(2) in biopsy samples. Patients were evaluated at baseline and after 6 months. RESULTS: In the group with LVEF improvement of ≥ 10% (I+ group, n = 41), LVEF increased from 24 ± 7% to 47 ± 8% (P < 0.001). In 22 patients (group I-), there was no or minimal LVEF increase (< 10%). In the I+ group, there were more LCA+ cells/mm(2) at baseline (25.1 ± 16.5 vs. 18.5 ± 4.4 cells/mm(2); P = 0.032) and a more significant decrease in LCA+ cells in the follow-up (reduction of 13.6 ± 14.3 cells/mm(2) vs. 5.0 ± 7.7 cells/mm(2) in the I- group; P = 0.009). The univariate logistic regression showed a possible association of number of LCA+ cells, LV end-diastolic diameter and N-terminal fragment of pro-brain natriuretic peptide (NTproBNP) value with LVEF improvement. In the multivariate analysis, only NTproBNP at diagnosis was confirmed as an independent predictor of LVEF improvement (OR=1.2; 1.003 to 1.394; P = 0.046). CONCLUSION: The LV systolic function improvement was observed in 65% of the patients. In these patients, the number of inflammatory cells at baseline was higher and decreased more but the higher baseline NTproBNP value was the only independent predictor of LVEF improvement.

Inflammatory cardiomyopathy is defined as inflammation of the heart muscle associated with impaired function of the myocardium. In our region, its etiology is most often viral. Viral infection is a possible trigger of immune and autoimmune mechanisms which contributed to the damage of myocardial function. Myocarditis is considered the most common cause of dilated cardiomyopathy. Typical manifestation of this disease is heart failure, chest pain, or arrhythmias. The most important noninvasive diagnostic method is magnetic resonance imaging, but the gold standard of diagnostics is invasive examination, endomyocardial biopsy. In a significant proportion of cases with impaired left ventricular systolic function, recovery occurs spontaneously in several weeks and therefore it is appropriate to postpone critical therapeutic decisions about 3-6 months after start of the treatment. Therapy is based on standard heart failure treatment; immunosuppressive or antimicrobial treatment may be considered in some cases depending on the results of endomyocardial biopsy. If severe dysfunction of the left ventricle persists, device therapy may be needed.

The study is focused on the detection of myocardial inflammation, to assess the relationship of myocarditis to the development of systolic left ventricular (LV) dysfunction, and also to evaluate viral presence in the myocardium. Clinical, echocardiographic and laboratory parameters will be evaluated in patients with newly diagnosed DCM of inflammatory and noninflammatory etiology, with or without the presence of viruses in the myocardium. The changes in viral load of PVB19 in ICM patients treated with immunosuppressive (IS) therapy and in the group without this therapy will be compared. A number of "healthy" donor hearts used for HTx should contain PVB19. (3,4) Viral presence and PVB19 viral load will be monitored in patients after HTx who are treated with aggressive IS therapy. It will show us the behavior of PVB19 in immunocompromised individuals which will be crucial to determine whether IS treatment can be administered to patients with ICM and the current presence of low load PVB19.
Studie je zaměřena na detekci zánětu v myokardu, na zhodnocení vztahu myokarditidy k rozvoji systolické dysfunkce levé komory (LK) srdeční, a také na problematiku virové přítomnosti v myokardu. Budou hodnoceny klinické, echokardiografické a laboratorní parametry u nemocných s nově diagnostikovanou DKMP zánětlivé i nezánětlivé etiologie, s nebo bez přítomnosti virů v myokardu. Budou porovnány změny virové nálože PVB19 u nemocných se ZKMP ve skupině léčené imunosupresivní terapií a ve skupině léčené pouze standardní léčbou srdečního selhání. PVB19 je přítomen v 50-80% zdravých srdcí (3,4), je tedy nepochybné, že řada dárcovských srdcí použitých k HTx jej obsahuje. U nemocných po HTx, kteří jsou současně léčeni agresivní imunosupresivní léčbou, bude sledována virová přítomnost a dynamika virové nálože PVB19. To nám ukáže chování PVB19 u těchto jedinců, což bude zásadní pro rozhodnutí, zda lze podávat imunosupresivní léčbu i nemocným se ZKMP a současnou přítomností nízké nálože PVB19 ve vzorcích EMB.
