Východiska: Za standard intenzivní léčby dospělých s akutní lymfoblastovou leukemií (ALL) jsou považovány protokoly pediatrického typu s prospektivním sledováním minimální reziduální nemoci (MRN). V ČR jsou používány od roku 2007. Soubor pacientů a metody: Mezi lety 2007–2020 bylo v pěti hematologických centrech léčeno 297 dospělých ve věku 18–65 let podle protokolu GMALL 07/2003, resp. ALL CELL 2012 Junior. Tato práce představuje retrospektivní analýzu výsledků jejich léčby. Výsledky: V kohortě Ph-negativní ALL dosáhlo 189 (93,1 %) pacientů kompletní remise, refrakterních bylo 5 (2,4 %) pacientů, časná mortalita byla 3,0 %. Relaps byl zaznamenán u 70 (34,5 %) nemocných v mediánu 10,6 měsíce. Celkové přežití (overall survival – OS) ve 3 a 5 letech dosahovalo 63,5 % a 55,9 % a přežití bez nemoci (disease-free survival – DFS) ve 3 a 5 letech pak 54,5 % a 49,7 %. Významně lepší OS měli mladí dospělí do 35 let (p = 0,015), pacienti bez vstupní infiltrace CNS (p = 0,016), s negativitou MRN před zahájením konsolidační léčby (p < 0,001), transplantovaní v 1. kompletní remisi (p < 0,001) a nemocní léčení po roce 2012 (p = 0,05). V multivariantní analýze byla jediným nezávislým faktorem, který ovlivňoval OS, MRN v 11. týdnu léčby (HR 3,06; p = 0,006). Pro DFS byla významná vstupní infiltrace CNS (HR 2,08; p = 0,038) a MRN v 11. týdnu (HR 2,15; p = 0,020). Ve skupině Ph-pozitivní ALL dosáhlo kompletní remise 84 (89,4 %) pacientů, refrakterní byl 1 (1,0 %) pacient, časná mortalita byla 4,3 %. Relabovalo 26 (27,7 %) nemocných v mediánu 8,6 měsíce. Přežití ve 3 a 5 letech dosahovalo 57,2 % a 52,4 % pro OS a 50,2 % a 44,9 % pro DFS. Statisticky lepší OS měli transplantovaní pacienti (p < 0,001) a nemocní diagnostikovaní po roce 2012 (p < 0,001). Závěr: Zavedení protokolů pediatrického typu s intenzifikací léčby podle hladin MRN se odrazilo v signifikantním zlepšení výsledků přežití dospělých pacientů s ALL.
Background: Pediatric-inspired protocols with prospective monitoring of minimal residual disease (MRD) are considered the standard of intensive treatment for adults with acute lymphoblastic leukemia (ALL). They have been used in the Czech Republic since 2007. Patients and methods: Two hundred and ninety-seven patients aged 18–65 years were treated at five hematology centers between 2007–2020 according to the GMALL 07/2003 protocol. This is a retrospective analysis of their treatment outcomes. Results: In the Ph-negative cohort, 189 (93.1%) patients achieved complete remission, 5 (2.4%) patients were refractory, and early mortality was 3.0%. Seventy (34.5%) patients experienced relapse in a median of 10.6 months. Overall survival (OS) at 3 and 5 years was 63.5% and 55.9%, disease-free survival (DFS) at 3 and 5 years was 54.5% and 49.7%, respectively. Young adults under 35 years of age (P = 0.015), patients without initial CNS infiltration (P = 0.016), with MRD negativity before consolidation treatment (P < 0.001), transplanted in the 1st complete remission (P < 0.001), and subjects treated after 2012 (P = 0.05) had significantly better overall survival. In a multivariate analysis, MRD at week 11 was the only independent factor affecting OS (HR 3.06; P = 0.006). For DFS, baseline CNS infiltration (HR 2.08; P = 0.038) and MRD at week 11 (HR 2.15; P = 0.020) were significant. In the Ph-positive cohort, 84 (89.4%) patients achieved complete remission, 1 (1.0%) patient was refractory, early mortality was 4.3%. Twenty-six (27.7%) patients relapsed in a median of 8.6 months. Survival at 3 and 5 years was 57.2% and 52.4% for OS and 50.2% and 44.9% for DFS, respectively. Transplanted patients and patients diagnosed after 2012 had statistically better overall survival (P < 0.001). Conclusion: The introduction of pediatric-inspired protocols with treatment intensification according to MRD levels resulted in a significant improvement in the survival outcomes of adult patients with ALL.

Navzdory narůstajícím léčebným možnostem představuje relaps akutní myeloidní leukemie nadále terapeutickou výzvu. Zavedení venetoklaxu do klinické praxe vedlo k rozšíření armamentária účinných protinádorových léků. Článek přináší dvě kazuistiky ukazující použití kombinace venetoklaxu s azacitidinem u pacientů s opakovaně relabovaným onemocněním.
Despite increasing treatment options, acute myeloid leukemia relapse remains a therapeutic challenge. The introduction of venetoclax into clinical practice led to the expansion of the armamentarium of effective antitumor drugs. The article presents two case reports showing the use of the combination of venetoclax and azacitidine in patients with repeatedly relapsed disease.

Onemocnění COVID-19, způsobené virem SARS-CoV-2, vyvolává řadu změn v krevním obraze. Covid skóre (CS) je proměnná, vypočítaná z 10 parametrů krevního obrazu, umožňující predikci závažného průběhu onemocnění COVID-19. Hodnoty CS se mohou pohybovat od 0 do 28. V pilotní práci Linssen et al byla hodnota CS v rozmezí 0–3 asociována s klinicky nekomplikovaným průběhem onemocnění, naopak hodnoty 4 a více predikovaly závažný průběh s potřebou intenzivní péče nebo fatálním koncem. Cílem práce bylo ověřit prognostický potenciál CS v souboru 76 nemocných s prokázaným onemocněním COVID-19 vyšetřených ve FN Olomouc v průběhu prosince 2021 až února 2022. Celkem 44 (57,9 %) nemocných mělo hodnotu CS v rozmezí 0–3 a 32 (42,1 %) nemocných mělo CS 4 a více. Nemocní s CS ≥ 4 významně častěji vyžadovali hospitalizaci na lůžkách intenzivní péče (78,1 vs. 20,5 %; p < 0,0001) a artefi ciální ventilační podporu (75,0 vs. 20,5 %; p < 0,0001). Mortalita nemocných byla nesignifi kantně vyšší ve skupině s CS ≥ 4 ve srovnání s nemocnými s hodnotou CS 0–3 (25,0 vs. 11,4 %; p = 0,119). Práce dále prokázala asociaci mezi hodnotou CS a již známými rizikovými faktory komplikovaného průběhu COVID-19. Medián hodnoty D-dimerů a CRP byl významně vyšší při stanovení dia gnózy u nemocných s hodnotou CS ≥ 4 (2006,0 vs. 594,5 μg/ l; p < 0,0001; resp. 132,6 vs. 36,9 mg/ l; p < 0,0001) a signifi kantně častěji byla vstupně zjištěna zánětlivá plicní infi ltrace (96,9 vs. 47,7 %; p < 0,0001). Výsledky práce potvrdily prognostický potenciál CS v predikci komplikovaného průběhu onemocnění COVID-19, s potřebou hospitalizace a ventilační podpory. Při prognostické stratifi kaci nemocného je vždy nutné individuálně zvážit i rizikové faktory na straně pacienta, vakcinační status a podanou specifi ckou protivirovou terapii v kontextu aktuálně dominující varianty viru SARS-CoV-2.
COVID-19 disease, caused by the SARS-CoV-2 virus, induces a number of changes in the blood count. The Covid Score (CS) is a variable, calculated from 10 parameters of the blood count, enabling the prediction of COVID-19 disease severity. CS values can range from 0 to 28. In the pilot study by Linssen et al, a CS value in the range of 0–3 was associated with a clinically uncomplicated course of the disease, while values of 4 or more predicted a severe course with the need for intensive care or a fatal outcome. The aim of the work was to verify the prognostic potential of CS in a group of 76 patients with proven COVID-19 infection examined at the Olomouc University Hospital between December 2021 and February 2022. A total of 44 (57.9%) patients had a CS value in the range of 0–3 and 32 (42.1%) of patients had CS 4 or more. Patients with CS ≥ 4 were more likely to require hospitalization in intensive care units (78.1 vs. 20.5%; P < 0.0001) and artificial ventilation (75.0 vs. 20.5%; P < 0.0001). The mortality rate was non-significantly higher in the group with CS ≥ 4 compared to patients with CS 0–3 (25.0 vs. 11.4%; P = 0.119). The work also demonstrated the association between the CS value and already known risk factors for the complicated course of COVID-19. The median value of D-dimers and CRP was significantly higher at diagnosis in patients with CS value ≥ 4 (2006.0 vs. 594.5 μg/l; P < 0.0001; respectively 132.6 vs. 36.9 mg/L; P < 0.0001) and inflammatory lung infiltration was also detected significantly more often at admission (96.9 vs. 47.7%; P < 0.0001). The results of the work confirmed the prognostic potential of CS in predicting the course of COVID-19 disease, with the need for hospitalization and ventilation support. During the prognostic stratification of patients, it is always necessary to also consider the patient’s risk factors, vaccination status and specific antiviral therapy administered in the context of the currently dominant variants of the SARS-CoV-2 virus.

BACKGROUND: Infectious complications during induction chemotherapy of acute myeloid leukaemia are very common. Prophylactic use of antibiotics however is an ongoing challenge in this situation due to bacterial multi-drug resistance. The aim of this study was to provide a comprehensive overview of the incidence of infectious complications in patients with AML undergoing induction therapy using the "7+3" protocol without routine antibiotic prophylaxis at one clinical site providing specialised haematological care in the Czech Republic, over a period of 15 years. The study also evaluates the aetiological spectrum of causative agents and the development of antibiotic resistance in the context of the use of the various classes of antibiotics. The analysis includes evaluation of the importance of risk factors for infectious complications and their impact on treatment of the underlying disease. The data are compared with published figures for similar cohorts of patients. PATIENTS AND METHODS: This study presents a retrospective analysis of infectious complications in 242 patients with acute myeloid leukaemia undergoing the first cycle of induction therapy without routine antibiotic prophylaxis in one clinical site in Czech Republic during years 2006-2020. RESULTS: A total of 363 febrile episodes (FE) were recorded. At least 1 FE during the induction was detected in 229 (94.6%) patients. Clinically defined infection was the cause in 96 (26.4%) FEs and blood stream infection in 69 (19.0%) FEs. Both complications occurred simultaneously in 29 (8.0%) FEs. 169 (46.6%) FEs were evaluated as fever of unknown origin (FUO). The achievement of complete remission had a significant effect on the duration of the FE (6 vs. 9 days, P=0.0005) and on the overall survival duration (79.3 vs. 6.5 months, P<0.0001). Patients diagnosed with infection or FUO at diagnosis were significantly more likely to suffer from colonisation by multi-drug resistant bacterial strains at discharge (29.2% vs. 16.3%, P=0.022). This group of patients used antibiotic therapy for a significantly longer time (35 vs. 23 days, P<0.0001). Infection was a contributing cause of death in 18 (7.4%) patients. Mortality was significantly related to the failure to achieve complete remission (P<0.0001). CONCLUSION: Infectious mortality during induction treatment without routine antibiotic prophylaxis was comparable to the published cohorts with prophylaxis. Regular microbiology surveillance with adequate initial antibiotic treatment can compensate routine antibiotic prophylaxis with slower development of antibiotic resistance.

Správné řízení genové exprese hraje zásadní roli v diferenciaci hematopoetických kmenových buněk a dalších procesech hematopoézy. Zásah do epigenetické regulace genové exprese charakterizuje patogenezi celé řady nejen hematologických malignit. V genomech pacientů s akutní myeloidní leukemií, zejména ve skupině s normálním karyotypem, byly opakovaně prokázány somatické mutace v epigenetických regulátorech vč. DNMT3A, TET2, IDH1, IDH2, ASXL1, MLL nebo EZH2. Jedná se o časné preleukemické zásahy, které bývají stabilním ukazatelem klinického vývoje onemocnění a v kombinaci s „driver“ mutacemi (NPM1, FLT3, MLL, RUNX1 aj.) představují kandidátní markery pro monitorování minimální reziduální nemoci. Specifický mutační profil každého pacienta získaný s využitím moderních molekulárních metod, jako je sekvenování nové generace, může přinést zásadní informaci o případném rozvoji relapsu či dosažení remise onemocnění. Cílem této práce je shrnout poznatky o klinickém, biologickém a terapeutickém významu mutací vybraných epigenetických regulátorů rekurentních u akutní myeloidní leukemie.
Regulation of gene expression, especially in hematopoietic stem cells, plays an essential role in differentiation and other important processes of haematopoiesis. Pathogenesis of different malignant diseases is characterised by disruption of epigenetic regulation. Certain somatic mutations, including genes such as DNMT3A, TET2, IDH1, IDH2, ASXL1, MLL or EZH2, have been found in acute myeloid leukaemia, especially in patients with normal karyotype AML. These mutations are considered to be pre-leukemic events and are stable indicators of clinical course. In combination with driver mutations, they are suitable markers for monitoring minimal residual disease. Specific mutational profiles of individual patients could provide essential information about disease progression, relapse or achievement of complete remission. The aim of this article is to review current knowledge regarding the clinical and therapeutical value of mutations in selected genes involved in epigenetic regulation of gene expression in acute myeloid leukaemia.
