Úvod: Pro cílené navýšení dávky v lůžku tumoru po operaci pro karcinom prsu je vhodné znát přesnou polohu původního tumoru. Proto na našem pracovišti provádíme u pacientek před operací pro karcinom prsu CT vyšetření v ozařovací poloze. Metody: Pacientky podstoupily v plánovací poloze pro radioterapii předoperačně nativní CT vyšetření, tato data byla využita k fúzi se standardním plánovacím CT při ozařování lůžka tumoru. Hodnotili jsme, zda je tumor na předoperačním CT identifikovatelný. Dále jsme provedli zakreslení jednoho ozařovacího objemu ve standardním plánovacím CT a druhého v CT s provedenou fúzí s předoperačním vyšetřením a tyto objemy jsme porovnali. Výsledky: Z celkového počtu 554 pacientek jsme byli schopni přesnou polohu tumoru prsu identifikovat ve 463 případech (83,6 %). V souboru 50 náhodně vybraných pacientek došlo k úpravě zakreslení cílového objemu pro plánované navýšení dávky ozáření do lůžka tumoru u 20 pacientek (40 %), ke zmenšení došlo v 9 případech (18 %) a ke zvětšení v 11 případech (22 %). Závěr: Výsledky naší studie ukázaly možnost využití předoperačního CT v plánovací poloze u pacientek s potvrzeným karcinomem prsu. Tato metoda nám umožňuje přesnější lokalizaci lůžka tumoru a tím přesnější zakreslení cílového objemu pro cílené navýšení dávky. Potvrdili jsme vliv předoperačního CT na velikost zakreslení cílového objemu k ozáření lůžka tumoru.
Introduction: The exact location of the original tumor should be known for a targeted increase in the dose to the tumor bed after breast cancer surgery. Therefore, at our site, we perform CT examinations of patients in the radiation position before breast cancer surgery. Methods: Preoperative native CT scans were performed in the patients in the planning position for radiotherapy; these data were fused with standard planning CT for boost irradiation. We evaluated whether the tumor was accurately identifiable in preoperative CT scans. We also contoured one irradiation volume in the standard planning CT scans and the other in the fusion CT scans with preoperative examination, and compared these volumes. Results: Out of the total number of 554 patients, we were able to identify the exact location of the breast tumor in 463 cases (83.6 %). In a group of 50 randomly selected patients, the clinical target volume for the boost dose to the postlumpectomy cavity was changed in 20 patients (40%) – decreased in 9 cases (18%) and increased in 11 cases (22%). Conclusion: As shown by the results of our study, preoperative CT in the planning position can be used in patients with confirmed breast cancer. This method allows us to more accurately locate the tumor bed and thus more accurately draw the target volume for boost irradiation. We confirmed that preoperative CT had an impact on the size of the target volume.
PURPOSE: This paper compares individual radiation therapy techniques used for prostate cancer and their benefits in clinical practice. METHODS: We retrospectively analyzed 921 patients with localized prostate tumors treated between 1997 and 2012. We divided the patients into four groups according to the selected treatment technique (conformal radiation therapy [3DCRT], intensity-modulated radiation therapy [IMRT], image-guided radiation therapy [IGRT], and volumetric-modulated arc therapy [VMAT]) and evaluated the incidence of acute and chronic gastrointestinal (GI) and genitourinary (GU) toxicity. RESULTS: The incidence of grade 2 or greater acute GU and GI toxicity was significantly higher among techniques other than IGRT (p˂0.001). We found the same results in the case of grade 3 or greater acute GU toxicity (p˂0.001). Grade 3 or higher acute GI toxicity occurred only in one patient treated by 3DCRT. Cumulative late GI toxicity of grade 2 or higher and grade 3 or higher was recorded over 3 years significantly more frequently among non-IGRT techniques as compared to IGRT (p˂0.001). As regards GU toxicity, we found significantly higher incidence only for grade 2 or higher (p˂0.001), not for grade 3 or higher. No occurrence of grade 4 toxicity was recorded. The greatest incidence of patients without acute and chronic GI/GU toxicity was recorded in connection with VMAT. CONCLUSION: IGRT demonstrated a pronounced reduction in acute and chronic GU and GI toxicity as compared to non-IGRT techniques in the treatment of localized prostate cancer.

Introduction: At the present time, the skin tumors are among the most common cancers. Optimal therapy is based on the extent of the disease and the age of the patient. The need for radiotherapy occurs for inoperable locally advanced tumors and in the event of failure, salvage surgery is applied. Materials and methods: We provided a case report of an older patient with giant squamous cell skin carcinoma and a review of published articles. Results: We present a rare case of giant squamous cell skin carcinoma with metastatic satellite tumors that was primarily treated with curative radiotherapy. Five months after radiotherapy, a recurrent tumor was detected at the site of origin and the treatment was completed by salvage surgery. Full remission was achieved for four years. Conclusion: Despite the seemingly incurable finding it is always necessary to consider radical treatment regardless of the patient´s age. Curative treatment could achieve long term remission in the group of older patients.

The remediation of a soil contaminated with Zn, Pb and Cd was tested by using biochar (BC), nano zero-valent iron (nZVI) and a combination of these two (BC + nZVI). Each amendment was individually applied to the soil at 2 wt%. We tested the influence of (i) the used amendments, (ii) time, and (iii) soil moisture conditions on the metal availability and soil physico-chemical parameters using various extraction methods, as well as soil pore water samplings. We found that metal availability was mainly affected by pH under the influence of time and water content. Among the tested treatments, BC was the most successful, resulting in the lowest amounts of the target metals in the pore water and the smallest temporal changes in metal concentrations and pH in the soil. The use of nZVI efficiently decreased water-extractable Pb in the short- and long-term. The BC + nZVI treatment also yielded promising results regarding the immobilisation of the studied metals. Time provoked a general decrease in pH, which occasionally increased the available metal concentrations. Raising the soil water content increased the pH and subsequently lowered the available metal concentrations in the pore water. The mechanisms of metal stabilisation were further investigated by SEM/EDS. The results indicated that the used soil amendments enhanced the binding of Zn, Pb, and Cd on Fe/Mn/Al oxides/hydroxides, which in turn resulted in the stabilisation of the target metals.

Východiska: Kombinace radioterapie s modulovanou intenzitou svazku (intensity modulated radiation therapy – IMRT) a obrazem řízené radioterapie (image guided radiotherapy – IGRT) hraje díky šetření zdravých tkání v léčbě karcinomu prostaty významnou roli. Prezentujeme zde klinické výsledky a nežádoucí účinky u 260 mužů léčených eskalovanou dávkou záření pomocí techniky IGRT. Materiál a metody: Od září 2008 do února 2012 podstoupilo 260 pacientů s lokalizovaným karcinomem prostaty radikální radioterapii. Dvě stě nemocných bylo léčeno technikou IGRT na oblast prostaty a báze semenných váčků dávkou 78 Gy ve 39 frakcích adaptivním protokolem kombinujícím trojrozměrné zobrazení konickým svazkem (cone-beam computed tomography – CBCT) s kilovoltážními snímky a individuálním bezpečnostním lemem. Šedesát mužů podstoupilo léčbu identickou dávkou technikou RapidArc s izocentrickým lemem 6 mm při denní online verifikaci nastavení pacienta pomocí CBCT. Pozdní toxicita byla hodnocena prospektivně dle RTOG/FC-LENT škály. Výsledky: U 18 pacientů (6,9 %) se projevila akutní gastrointestinální toxicita 2. stupně. Toxicita 3. ani 4. stupně nebyla zaznamenána. Akutní genitourinární toxicita 2., resp. 3. stupně se projevila u 39 (15 %), resp. 6 nemocných (2,3 %).Genitourinární toxicita 4. stupně byla přítomna u 5 mužů (1,9 %), u všech pro nezbytnost přechodného zavedení permanentního močového katetru. Při mediánu sledování 84,2 měsíce byla kumulativní incidence chronické gastrointestinální a genitourinární toxicity 2. a vyššího stupně v 7 letech 4,4, resp. 7,1 %. Přežití bez relapsu prostatického specifického antigenu v 7 letech u pacientů s nízkým rizikem bylo 97,1 %, se středním rizikem 83,6 % a u nemocných s vysokým rizikem 75 %. Závěr: Kombinace IMRT a IGRT umožňuje dosahovat velice nízké toxicity léčby.
Background: The combination of intensity modulated radiation therapy (IMRT) and image guided radiotherapy (IGRT) plays a significant role in sparing normal tissue during prostate cancer treatment. We report the clinical outcomes of 260 patients treated with high-dose IGRT as well as the toxicity of high-dose IGRT in these patients. Materials and methods: From September 2008 to February 2012, 260 men with clinically localized prostate cancer underwent radical radiotherapy. Two hundred patients were treated with IMRT (78 Gy in 39 fractions) to the prostate and base of seminal vesicles using an adaptive protocol combining cone-beam computed tomography (CBCT) and kilovoltage image matching with individualized safety margin calculation. Sixty patients underwent treatment with the same prescribed dose using RapidArc with a reduced safety margin of 6 mm and daily online matching using CBCT. Late toxicity was scored prospectively according to the RTOG/FC-LENT scale. Results: Eighteen patients (6.9%) experienced acute grade 2 gastrointestinal toxicity. There was no acute grade 3 or 4 gastrointestinal toxicity. Thirty-nine patients (15%) experienced acute grade 2 genitourinary toxicity and 6 patients (2.3%) had grade 3 gerourinary toxicity. Genitourinary toxicity grade 4 was observed in 5 (1.9%) patients, due to installation of a urinary catheter. At a median follow up of 84.2 months, the estimated 7-year cumulative incidences of grade 2 gastrointestinal and genitourinary toxicity were 4.4 and 7.1% respectively. The estimated 7-year prostate specific antigen relapse free survival was 97.1% for low-risk disease, 83.6% for intermediate-risk disease and 75% for high-risk patients. Conclusion: The use of IMRT in combination with IGRT results in a low rate of late toxicity.

In this study, samples of soil and particulate matter obtained from the highly industrialized region of Ostrava, Czech Republic, are used for the toxicity evaluation of the selected metal(loid)s (Cd, Cr, Cu, Ni, Pb, Zn, As). We investigated the samples from sites supposedly affected the most by the local pollution sources using mineralogical techniques (XRD, SEM/EDS) to understand the solid speciation of the contaminants as the crucial factor affecting their release. Although the bulk composition was defined by common silicates and oxides that are rather resistant to leaching, the presence of tiny Ni, Pb, and/or Zn sulfate-like droplets indicated a potential increase of the solubility of these metals. In vitro tests simulating gastric and lung fluids were used to assess the exposure risk for humans, as well as metal(loid) bioaccessibility. Based on the results, the potential risk for the observed age group (3-year-old children) could be recognized, particularly in the cases of As, Pb and Cd for both oral and inhalation exposure. Arsenic exhibits high bioaccessibility (7.13-79.7%, with the median values of 10.6 and 15.6 for SGL and SLF, respectively), high daily intake (1.4- to 8.5-fold higher than the tolerable daily intake) and high concentrations in atmospheric PM10 (2.5 times the tolerable concentration in air). In contrast, Ni exceeded tolerable concentrations in the atmosphere up to 20-fold, but its bioaccessibility remained relatively low (0.1-22%), and Ni did not pose a major threat to human health. Cadmium, Pb and As originating from industrial activities and domestic heating have been suggested to be the most important pollutants (tolerable daily intake was exceeded by up to 74-, 34- and 8-fold for Cd, Pb and As, respectively).
