BACKGROUND: Genetic focal segmental glomerulosclerosis (FSGS) is caused by pathogenic variants in a broad spectrum of genes that have a variable representation based on subjects' ethnicity and/or age. The most frequently mutated autosomal recessive gene in FSGS is NPHS2. In this study, we analyzed the spectrum of NPHS2 variants and their associated phenotype in Czech adult FSGS patients. METHODS: A representative cohort of 234 adult patients with FSGS, derived from 225 families originating from all regions of Czechia, was analyzed by massively parallel sequencing. In this study, we focused on the comprehensive analysis of the NPHS2 gene. The histological classification of FSGS followed the Columbia classification. RESULTS: We detected seven (3%) cases bearing homozygous or compound heterozygous pathogenic NPHS2 variants. A single pathogenic variant c.868G > A (p.Val290Met) was found in the majority of NPHS2-positive cases (86%; 6 out of 7) in histologically confirmed instances of FSGS. Its allele frequency among unrelated NPHS2-associated FSGS patients was 50% (6/12), and Haplotype analysis predicted its origin to be a result of a founder effect. There is an identical V290M-related haplotype on all V290M alleles spanning a 0,7 Mb region flanking NPHS2 in Central European FSGS populations. The phenotype of the p.Val290Met NPHS2-associated FSGS demonstrated a later onset and a much milder course of the disease compared to other NPHS2 pathogenic variants associated with FSGS. The mean age of the FSGS diagnosis based on kidney biopsy evaluation was 31.2 ± 7.46 years. In 50% of all cases, the initial disease manifestation of proteinuria occurred only in adulthood, with 83% of these cases not presenting with edemas. One-third (33%) of the studied subjects progressed to ESRD (2 out of 6) at the mean age of 35.0 ± 2.82 years. CONCLUSIONS: We identified the most prevalent pathogenic variant, p.Val290Met, in the NPHS2 gene among Czech adult FSGS patients, which has arisen due to a founder effect in Central Europe. The documented milder course of the disease associated with this variant leads to the underdiagnosis in childhood. We established the histopathological features of the NPHS2-associated adult FSGS cases based on the Columbia classification. This might improve patient stratification and optimize their treatment.

Genetika se v současné kardiologii uplatňuje zejména u kardiovaskulárních onemocnění s mendelovskou dědičností. Mezi tato onemocnění patří kardiomyopatie, dědičné formy arytmických syndromů, aortální syndromy a některé vrozené vývojové vady. Společným rizikem všech zmíněných skupin onemocnění je náhlá srdeční smrt, proto je důležitou součástí i posmrtné vyšetření a péče o příbuzné a pozůstalé. Cílem klinicko-genetického vyšetření a genetického poradenství v kardiologii je provést podrobný popis onemocnění - fenotypizaci, sestavení rodokmenu, kaskádový kardiologický screening v rodině a v indikovaných případech molekulárně-genetickou diagnostiku - genotypizaci. Znalost molekulární podstaty genetických onemocnění otvírá možnosti tzv. molekulárně genetické stratifikace onemocnění, jejímž cílem je personalizovaná péče o nemocné a jejich příbuzné. Kardiogenetické vyšetření je komplexní záležitost, kdy je třeba budovat mezioborové týmy. Komunikace mezi jednotlivými odborníky i týmy v národním měřítku je zcela zásadní pro zajištění péče o pacienty s často velmi vzácnou až raritní formou dědičného onemocnění.
In contemporary cardiology, genetics is mainly applied to cardiovascular diseases with Mendelian inheritance. These diseases include cardiomyopathy, hereditary forms of arrhythmic syndromes, aortic syndromes, and some congenital developmental defects. The common risk of all mentioned groups of diseases is sudden cardiac death, which is why post-mortem examination and care for relatives and survivors are an important part. The goal of clinical genetic examination and genetic counselling in cardiology is to carry out a detailed description of the disease - phenotyping, compiling a family tree, cascade cardiology screening in the family and, in indicated cases, molecular genetic diagnosis - genotyping.

Náhlá srdeční smrt (SCD) je u jedinců mladších 40 let velmi často způsobena dědičným kardiovaskulárním onemocněním. Identifikace těchto případů, provedení post mortem genetického vyšetření a kardiologického screeningového vyšetření přímých příbuzných jsou předpokladem k primární prevenci srdeční zástavy u pozůstalých. Cílem práce je analýza výsledků takového programu, který byl zaveden v České republice. Soubor a metodika: V období od 2016 do 2021 jsme v rámci grantového projektu řešili celkově 133 případů náhlého úmrtí, z nichž bylo podrobné kardiogenetické diagnostice podrobeno 115 úmrtí jedinců ve věku 0-59 let, celkem ze 110 rodin. Mrtví byli na základě pitevní diagnózy rozřazeni do kategorií dle typu kardiomyopatie (hypertrofická, dilatační nebo arytmogenní), kategorie bez nálezu strukturálního onemocnění srdce (SADS) nebo bez jasného pitevního nálezu (SUD/SUDI), případně náhlé úmrtí novorozence (SIDS) a akutní disekce velké cévy. Celkem 328 příbuzných bylo geneticky konzultováno a kardiologicky vyšetřeno. Po informovaném souhlasu následovala molekulární analýza metodou sekvenování nové generace (Sophia Genetics) celkem u 106 případů, kde byl k dispozici vhodný materiál pro analýzu. Výsledky: Anamnéza náhlého úmrtí či blízký příbuzný se srdečním selháním a/nebo komorovou arytmií byla zaznamenána v 33/115 případů (28,6 %) rodin. Velmi pravděpodobná nebo jistá molekulární příčina onemocnění (klasifikace 4 a 5) byla nalezena u 21/106 (19,8 %) v genech RYR2, KCNH2, KCNQ1, SCN5A, FLNC, GLA, TTN, TNNT2, RBM 20, MYBPC3, MYPN, FHL1, TGFBR1 a COL3A1. Záchyt příčinných variant kyseliny desoxyribonukleové (DNA) byl ve skupině náhlých úmrtí s předchozí rodinnou anamnézou 16/49 (32,6 %). Klinickým a genetickým vyšetřením bylo identifikováno 86/328 (26,2 %) příbuzných v riziku. Závěr: Multidisciplinární spolupráce spolu s centralizovanou molekulárně-genetickou diagnostikou umož- nila odhalit jistou molekulární příčinu SCD v 19,8 % případů. Významná část rodin měla pozitivní rodinnou anamnézu (28,6 %). Je možné tedy usoudit, že náhlé smrti by bylo možné zabránit provedením kaskádového rodinného screeningu až u jedné třetiny zemřelých.

Identification of SCD, postmortem genetic analysis along with the cardiological screening in first degree relatives represents an important tool for the primary prevention of cardiac arrest in victim's relatives and requires multicentric and multidisciplinary collaboration. Between 2016 and 2021 we dealt with totally 133 cases of sudden death, the complex cardiogenetic analysis was performed in 115 cases at the age of 0-59 years with post mortem diagnosis of cardiomyopathy, acute aortic dissection and cases without morphological finding explaining the cause of death (sudden arrhythmic death or sudden unexplained death). DNA was isolated from post mortem collected tissue samples or relative's blood and subjected to massively parallel sequencing (SophiaGenetics, Switzerland). Genetic counselling and cardiological examinations were carried out in 328 family members. Highly likely or certain molecular aetiology (i.e. based on presence of ACMG.net classification 4 to 5 variants) was disclosed in 19.8% of analysed cases in RYR2, KCNH2, KCNQ1, SCN5A, FLNC, GLA, TTN, TNNT2, RBM 20, MYBPC3, MYPN, FHL1, TGFBR1, and COL3A1 genes. With cardiogenetic screening we identified 83/328 (26.2%) relatives at risk of life threatening arrhythmias, who were offered individualised care. Conclusion: Our study with postmortem analysis could reveal a pathogenic DNA variant in 19.8% of cases and in 32.6% in cases with positive family history. A relevant number of studied victims had a positive family history, which indicates the importance of family cascade screening in patients with heart failure or malignant arrhythmias of unclear aetiology for primary prevention of sudden cardiac death.

PURPOSE: Within the Solve-RD project (https://solve-rd.eu/), the European Reference Network for Intellectual disability, TeleHealth, Autism and Congenital Anomalies aimed to investigate whether a reanalysis of exomes from unsolved cases based on ClinVar annotations could establish additional diagnoses. We present the results of the "ClinVar low-hanging fruit" reanalysis, reasons for the failure of previous analyses, and lessons learned. METHODS: Data from the first 3576 exomes (1522 probands and 2054 relatives) collected from European Reference Network for Intellectual disability, TeleHealth, Autism and Congenital Anomalies was reanalyzed by the Solve-RD consortium by evaluating for the presence of single-nucleotide variant, and small insertions and deletions already reported as (likely) pathogenic in ClinVar. Variants were filtered according to frequency, genotype, and mode of inheritance and reinterpreted. RESULTS: We identified causal variants in 59 cases (3.9%), 50 of them also raised by other approaches and 9 leading to new diagnoses, highlighting interpretation challenges: variants in genes not known to be involved in human disease at the time of the first analysis, misleading genotypes, or variants undetected by local pipelines (variants in off-target regions, low quality filters, low allelic balance, or high frequency). CONCLUSION: The "ClinVar low-hanging fruit" analysis represents an effective, fast, and easy approach to recover causal variants from exome sequencing data, herewith contributing to the reduction of the diagnostic deadlock.

Náhlá srdeční smrt u jedinců mladších 40 let je ve významném procentu způsobena dědičným kardiovaskulárním onemocněním. Identifikace těchto případů, provedení post mortem genetického vyšetření a kardiologické screeningové vyšetření přímých příbuzných je prvním krokem k primární prevenci srdeční zástavy u pozůstalých. Tato péče vyžaduje multidisciplinární a multicentrickou spolupráci. Jistá až velmi pravděpodobná molekulární příčina onemocnění byla nalezena u 19,8 % analyzovaných případů, u 25 % příbuzných bylo zjištěno riziko významných arytmií.

Sequencing results were analysed using the SOPHiA GENETICS DDM bioinformatics platform (Switzerland). Genetic counselling and cardiological examinations were carried out in 328 family members. Highly likely or certain molecular aetiology (i.e. based on presence of ACMG.net Class 4 to 5 variants) was disclosed in 19,8 % of analysed cases in RYR2, KCNH2, KCNQ1, SCN5A, FLNC (stop), GLA, TTN, TNNT2, RBM 20, MYBPC3, MYPN, FHL1, TGFBR1, and COL3A1 genes. With cardiogenetic screening we identified 25 % relatives at risk of life threating arrhythmias and offered them an individualised care.

Pathogenic variants in JAG1 are known to cause Alagille syndrome (ALGS), a disorder that primarily affects the liver, lung, kidney, and skeleton. Whereas cardiac symptoms are also frequently observed in ALGS, thoracic aortic aneurysms have only been reported sporadically in postmortem autopsies. We here report two families with segregating JAG1 variants that present with isolated aneurysmal disease, as well as the first histological evaluation of aortic aneurysm tissue of a JAG1 variant carrier. Our observations shed more light on the pathomechanisms behind aneurysm formation in JAG1 variant harboring individuals and underline the importance of cardiovascular imaging in the clinical follow-up of such individuals.

Prezentujeme klinický případ laminopatie, která byla zjištěna u šestnáctileté pacientky referované k zařazení do transplantačního programu. Kromě pokročilého srdečního selhání na podkladě dilatační kardiomyopatie jsme u pacientky zjistili diskrétní flekční kontraktury v loketních kloubech. Genetické vyšetření prokázalo patogenní variantu laminu A/C LMNA:c.241T>G, p.Tyr81Asp. Tyto nálezy zapadaly do klinického obrazu Emeryho-Dreifussovy myopatie s převažujícím postižením srdce. Na základě kazuistiky diskutujeme klinické známky laminopatií, prognostickou stratifikaci nosičů patogenních variant LMNA a specifika kardiologického managementu těchto pacientů.
We present a case of laminopathy, which was diagnosed in a sixteen-year-old female patient referred for heart transplant enlistment. In addition to advanced heart failure due to dilated cardiomyopathy, we detected mild contractures of elbow flexors. Molecular genetic analysis revealed a pathogenic variant of lamin A/C LMNA:c.241T>G, p.Tyr81Asp. These findings were compatible with Emery-Dreifuss myopathy with predominant cardiac involvement. Furthermore, we discuss clinical presentation of laminopathy, prognostic stratification of carriers of pathogenic LMNA variants and clinical management of patients with laminopathy.

BACKGROUND AND AIMS: Vitamin D 24-hydroxylase is an enzyme encoded by the CYP24A1 gene, which inhibits the activation of vitamin D to form inactive metabolites. More than 20 currently described pathogenic variants (usually biallelic) of this gene are responsible for idiopathic infantile hypercalcemia manifested typically in childhood (often in newborns) with hypercalcemia, hypercalciuria, and nephrocalcinosis. However, a few patients (mostly with monoallelic heterozygous pathogenic variants) can develop mild symptoms in adulthood. CASE DESCRIPTION: We present the case of a 43-year-old male patient with hypertension and heterozygous Leiden mutation, with mural thrombi in the common iliac artery, who was sent by a nephrologist to endocrinological examination due to hypoparathyroidism, progressive hypercalcemia, hypercalciuria, and CKDG2A1. Complete laboratory and imaging methods (including PET-CT) excluded PTH-related peptide-mediated hypercalcemia and granulomatosis. Finally, the genetic analysis of the CYP24A1 gene revealed the presence of a novel combination of two heterozygous pathogenic variants: CYP24A1: c. 443T>C p.(Leu148Pro) and c.1186C>T p.(Arg396Trp). CONCLUSION: Differential diagnosis of patients with hypercalciuria, nephrocalcinosis, and hypercalcemia related to vitamin D exposure should include the CYP24A1 gene mutation. To the best of our knowledge, this is the first case of the novel combination of two heterozygous pathogenic variants of CYP24A1.
