Úvod: Hrudní pás umožňuje pořídit 1svodový EKG záznam. Získaná data byla validována pro měření srdeční frekvence a rovněž i pro detekci fibrilace síní díky srovnání s krátkými EKG záznamy z holterovského EKG měření u selektovaných pacientů. Zatím ale nebyla ověřena možnost vyhodnocení dlouhých EKG záznamů u neselektovaných kardiologických pacientů se širokým spektrem srdečních chorob. Metodologie a výsledky: Do studie bylo zařazeno 54 hospitalizovaných a 53 ambulantních pacientů a 54 zdravých kontrol (n = 161 celkově). U všech účastníků studie byl pomocí hrudního pásu Polar H10 pořízen 1-2hodinový EKG záznam (celkově 1 153 229 úderů srdce; průměrná srdeční frekvence 76,6/min; sinusový rytmus u 86,3 %, fibrilace síní zjištěna u 13,7 %; dokumentováno 0,46 % síňových extrasystol a 0,49 % komorových extrasystol). Z výše uvedeného počtu 1 153 229 srdečních tepů jich 1 128 319 bylo hodnoceno lékařem jako snadno interpretovatelných. Celkově tak bylo 2,16 % záznamu vyhodnoceno jako obtížně interpretovatelný nebo neinterpretovatelný šum (A: 2,31 %; B: 1,95 %; C: 2,20 %). Z EKG záznamu z hrudního pásu lékař při srovnání s 12svodovým EKG záznamem spolehlivě určil základní srdeční rytmus u většiny účastníků (u 51/54 [94,4 %] hospitalizovaných pacientů a u 100 % ambulantních pacientů a zdravých kontrol). U tří jedinců byl základní rytmus na EKG vyhodnocen jako nejasný. U všech tří byly všechny komplexy QRS stimulované. U hospitalizovaných pacientů byl EKG záznam z hrudního pásu zobrazený v reálném čase na mobilním telefonu srovnatelný s EKG záznamem z telemetrického monitorování (shoda v 53 z 54 případů; 98,1 %). Závěr: EKG záznam z hrudního pásu, pořízený u hospitalizovaných i ambulantních pacientů s různými typy poruch srdečního rytmu, stejně tak jako u zdravých kontrol, lze v každodenní praxi použít pro zhodnocení základního srdečního rytmu, záchyt fibrilace síní i extrasystol, a to při minimálním procentu obtížně hodnotitelných záznamů. Opatrnosti je třeba při interpretaci EKG záznamu u pacientů se stimulovaným rytmem a u pacientů s flutterem síní. Hrudní pás je tak možno použít pro kontinuální EKG monitorování, hodnocení srdečního rytmu i screening fibrilace síní.
Background: The chest-belt can be used to obtain a 1-lead ECG. Data from it have been validated for the determination of heart rate and for the possibility to detect atrial fibrillation (AF) compared to ECG-Holter on a short ECG recording in selected patients. However, validation of the possibility to evaluate long ECG recordings in patients with a wide range of heart diseases has not yet been performed. Methodology and results: 54 hospitalized patients, 53 outpatients and 54 healthy controls were enrolled in the study (n = 161 in total). Using a Polar H10 chest-belt, 1-2 hours of ECG were recorded in all patients (1 153 229 heartbeats, average heart rate 76.6/min, 86.3% in sinus rhythm, 13.7% with atrial fibrillation, 0.46% atrial premature beats, 0.49% ventricular premature beats). The presence of noise was 2.16% (A: 2.31%; B: 1.95%; C: 2.20%). 1 128 319 /1 153 229 were evaluated as easy to interpret. Using ECG from the belt, the basic rhythm was reliably determined by the physician in majority of patients (51/54, 94.4% in hospitalized patients; in 100% of outpatients and healthy controls) when compared to 12-lead ECG. 3 cases were evaluated as unclear; in all of these cases, all QRS complexes were stimulated by a pacemaker. In hospitalized patients, real-time ECG from the belt was comparable to telemetric ECG monitoring (match in 53/54, 98.1%). Conclusion: The ECG obtained from the chest-belt in hospitalized patients and outpatients with a wide range of cardiovascular diseases, as well as in healthy individuals, is usable in real practice for evaluation of baseline rhythm, atrial fibrillation and premature contractions with a minimal proportion of difficulties to interpret recordings due to artefacts. Caution should be exercised in interpretation of the ECG in patients with stimulated rhythm and in patients with atrial flutter. The chest belt can be used as a means for continuous monitoring of ECG, evaluation of rhythm and screening of atrial fibrillation.
Takotsubo syndrom (TS) je znám i pod označením stresová kardiomyopatie. Spouštěči TS jsou vnější nebo vnitřní, fyzický nebo psychický stres spojený s katecholaminovou bouří, nadměrnou sympatickou aktivitou nervového systému nebo s dysfunkcí mikrovaskulatury v přítomnosti systémového zánětu. Popisujeme případ pacienta s takotsubo kardiomyopatií spojenou s fibrilací síní a s fibromuskulární dysplazií levé přední sestupné větve.
Takotsubo syndrome (TS) is well-known to be a stress-induced cardiomyopathy. TS is triggered by external or internal, physical or emotional stress associated with a catecholamine storm, overactivity of sympathetic nerves, or microvascular dysfunction in the setting of systemic inflammation. We report the case of a patient with Takotsubo cardiomyopathy associated with atrial fibrillation and fibromuscular dysplasia of the left anterior descending artery.

INTRODUCTION: Thermally induced cardiac lesions result in necrosis, edema, and inflammation. This tissue change may be seen with ultrasound. In this study, we sought to use intracardiac echocardiography (ICE) to evaluate pulmonary vein tissue morphology and assess the acute tissue changes that occur following radiofrequency (RF) or laser ablation for atrial fibrillation (AF). METHODS AND RESULTS: Patients with AF underwent pulmonary vein isolation (PVI) using irrigated RF or laser balloon. Pre- and post-ablation ICE imaging was performed from within each pulmonary vein (PV). At least 10 transverse imaging planes per PV were evaluated and each plane was divided into eight segments. The PV/atrial wall thickness and the luminal area were measured at each segment. Twenty-seven patients underwent PVI (15 with laser, 12 with RF). Ninety-eight pulmonary veins were analyzed (58 PVs laser; 40 PVs RF). At baseline, there were no regional differences in PV wall thickness in the right-sided veins. The anterior regions of left superior pulmonary vein (LSPV) and left inferior pulmonary vein (LIPV) were significantly thicker compared with the posterior and inferior regions (p < .01). Post-ablation, PV wall thickness in RF group increased 24.1% interquartile range (IQR) (17.2%-36.7%) compared with 1.2% IQR (0.4%-8.9%) in laser group, p = .004. In all PVs, RF ablation resulted in significantly greater percent increase in wall thickness compared with laser. Additionally, RF resulted in more variable changes in regional PV wall thickness; with more increases in wall thickness in anterior versus posterior LSPV (75.4 ± 58.5% vs. 46.8 ± 55.6%, p < .01), anterior versus posterior right superior pulmonary vein (RSPV) (62.9 ± 63.9% vs. 44.6 ± 51.7%, p < .05), and superior versus inferior RSPV (69.1 ± 45.4% vs. 35.9 ± 45%, p < .05). There were no significant regional differences in PV wall thickness changes for the laser group. CONCLUSIONS: Rotational ICE can be used to measure acute tissue changes with ablation. Regional variability in baseline wall thickness was nonuniformly present in PVs. Acute tissue changes occurred immediately post-ablation. Compared with laser balloon, RF shows markedly more thickening post-ablation with significant regional variations.
BACKGROUND: The next-generation Watchman FLX left atrial appendage closure (LAAC) device has: (1) an atraumatic closed distal end, (2) reduced height, (3) a recessed screw hub to decrease device-related thrombus (DRT), (4) two rows of J-shape anchors so redeployment is possible after full recapture, and (5) ability to treat a greater size range of LAA ostia. OBJECTIVE: To report, for the first time, the feasibility and safety of intracardiac echocardiography (ICE)-guided Watchman FLX implantation. METHODS: A single-center prospective registry of atrial fibrillation patients planned for LAAC with the FLX device underwent ICE-guided implantation with conscious sedation. Transesophageal echocardiography (TEE) imaging was done preprocedure (to assess LAA size and exclude thrombus) and at clinical follow-up at 6-12 weeks. Clinical outcomes were LAA closure success, complications, leak, or DRT on follow-up TEE and major safety events. RESULTS: The study included 30 patients: age 75 ± 8 years, 53% men, CHA2 DS2 -VASc 4.6 ± 1.6, and HAS-BLED 3.4 ± 1.1. The primary indication was prior bleeding in 60% (72% GI bleeding). The LAA orifice width and length were 22.7 ± 3.1 and 25.7 ± 5.7 mm, respectively. Technical success was 100% (the first-choice device was used in 28 of 30). Procedure time was less than 30 min in 27 of 30 cases, with 36 ± 15 ml contrast used. The final device size was 29.2 ± 4.7 mm with 21.6 ± 4.5% compression. There were no procedure-related complications. Follow-up TEE at a median 47 days follow-up showed 100% device success with no DRT or peridevice leak ≥5 mm. Major safety events occurred in 6.6% (2/30). CONCLUSION: The Watchman FLX device can be safely implanted with intraprocedural ICE imaging instead of TEE.

AIMS: Catheter ablation (CA) for atrial fibrillation (AF) has a considerable risk of procedural complications. Major vascular complications (MVCs) appear to be the most frequent. This study investigated gender differences in MVCs in patients undergoing CA for AF in a high-volume tertiary center. METHODS: A total of 4734 CAs for AF (65% paroxysmal, 26% repeated procedures) were performed at our center between January 2006 and August 2018. Patients (71% males) aged 60 ± 10 years and had a body mass index of 29 ± 4 kg/m2 at the time of the procedure. Radiofrequency point-by-point ablation was employed in 96.3% of procedures with the use of three-dimensional navigation systems and facilitated by intracardiac echocardiography. Pulmonary vein isolation was mandatory; cavotricuspid isthmus and left atrial substrate ablation were performed in 22% and 38% procedures, respectively. MVCs were defined as those that resulted in permanent injury, required intervention, or prolonged hospitalization. Their rates and risk factors were compared between genders. RESULTS: A total of 112 (2.4%) MVCs were detected: 54/1512 (3.5%) in females and 58/3222 (1.8%) in males (p < .0001). On multivariate analysis, lower body height was the only risk factor for MVCs in females (p = .0005). On the contrary, advanced age was associated with MVCs in males (p = .006). CONCLUSION: Females have a higher risk of MVCs following CA for AF compared to males. This difference is driven by lower body size in females. Low body height in females and advanced age in males are independent predictors of MVCs. Ultrasound-guided venipuncture lowered the MVC rate in males.

Téma poruchy srdečního rytmu a sport zahrnuje jak závažné poruchy rytmu asociované s náhlou srdeční smrtí, tak i arytmie méně rizikové, které se vyskytují u sportovců ve vyšší míře. Mezi příčiny vzniku vážné poruchy rytmu u sportovců patří hypertrofická kardiomyopatie, arytmogenní kardiomyopatie, dilatační kardiomyopatie, syndrom dlouhého intervalu QT, syndrom časné repolarizace a Wolffův-Parkinsonův-Whiteův (WPW) syndrom. Příčinou maligní arytmie mohou být i anomálie věnčitých tepen nebo myokarditidy. U sportujících jedinců je přítomno i vyšší riziko výskytu fibrilace síní. Problematickým tématem je přítomnost komorových extrasystol, která může být zcela nevýznamná, ale může reprezentovat i závažné srdeční onemocnění.
Cardiac arrhythmias in competitive sports include serious ventricular tachycardias associated with structural heart diseases and arrhythmogenic syndromes, but also less risky supraventricular tachycardias. The most com- mon structural cause of sudden cardiac death is hypertrophic cardiomyopathy, followed by arrhythmogenic cardiomyopathy, dilated cardiomyopathy, long QT syndrome, early repolarization and WPW syndrome. Sudden cardiac death could occur in case of anomalous origins of coronary arteries and myocarditis. This review addresses also less risky arrhythmias which are more frequently observed in athletes, such as atrial fibrillation. Although premature ventricular beats in young athletes are usually benign, they may rarely mark underlying heart disease and risk of sudden cardiac death.

BACKGROUND: Unlike for paroxysmal atrial fibrillation (AF), pulmonary vein isolation (PVI) alone is considered insufficient for many patients with persistent AF. Adjunctive ablation of the left atrial posterior wall (LAPW) may improve outcomes, but is limited by both the difficulty of achieving lesion durability and concerns of damage to the esophagus-situated behind the LAPW. OBJECTIVES: This study sought to assess the safety and lesion durability of pulsed field ablation (PFA) for both PVI and LAPW ablation in persistent AF. METHODS: PersAFOne is a single-arm study evaluating biphasic, bipolar PFA using a multispline catheter for PVI and LAPW ablation under intracardiac echocardiographic guidance. A focal PFA catheter was used for cavotricuspid isthmus ablation. No esophageal protection strategy was used. Invasive remapping was mandated at 2 to 3 months to assess lesion durability. RESULTS: In 25 patients, acute PVI (96 of 96 pulmonary veins [PVs]; mean ablation time: 22 min; interquartile range [IQR]: 15 to 29 min) and LAPW ablation (24 of 24 patients; median ablation time: 10 min; IQR: 6 to 13 min) were 100% acutely successful with the multispline PFA catheter alone. Using the focal PFA catheter, acute cavotricuspid isthmus block was achieved in 13 of 13 patients (median: 9 min; IQR: 6 to 12 min). The median total procedure time was 125 min (IQR: 108 to 166 min) (including a median of 28 min [IQR: 25 to 33 min] for voltage mapping), with a median of 16 min (IQR: 12 to 23 min) fluoroscopy. Post-procedure esophagogastroduodenoscopy and repeat cardiac computed tomography revealed no mucosal lesions or PV narrowing, respectively. Invasive remapping demonstrated durable isolation (defined by entrance block) in 82 of 85 PVs (96%) and 21 of 21 LAPWs (100%) treated with the pentaspline catheter. In 3 patients, there was localized scar regression of the LAPW ablation, albeit without conduction breakthrough. CONCLUSIONS: The unique safety profile of PFA potentiated efficient, safe, and durable PVI and LAPW ablation. This extends the potential role of PFA beyond paroxysmal to persistent forms of AF. Pulmonary vein isolation (PVI) is the cornerstone in the treatment of patients with paroxysmal atrial fibrillation (PAF). Some research has suggested studies have shown that modification of ganglionated plexuses (GP), performed with PVI, could lead to even better outcomes. The aim of this study was to determine the effect of PVI on the autonomic system. Heart rate variability (HRV) was used as a marker of autonomic system activity. Twenty-six PAF patients underwent PVI (PVI group) and twenty patients underwent PVI plus a GP ablation (GP group). In each group, 5 min long ECG signals obtained before and after the electrophysiology EP study were analyzed. Time and frequency domain parameters were evaluated. Vagal responses during ablation were observed in 15 (58 %) patients in the PVI group and in 12 (60 %) patients in the GP group. The change in normalized power in the low frequency (LF) and in the LF/HF ratio, before and after ablation, was statistically significant in both groups (LF/HF 2.6+/-1.6 before vs. 1.4+/-1.7 after ablation in PVI group and LF/HF 3.3+/-2.6 before vs. 1.8+/-1.9 after ablation in the GP group). Relative to heart rate variability parameters, there were no differences between PVI and PVI + plus GP ablation.

Background Identification and elimination of nonpulmonary vein targets may improve clinical outcomes in patients with persistent atrial fibrillation (AF). We report on the use of a novel, noncontact imaging and mapping system that uses ultrasound to reconstruct atrial chamber anatomy and measures timing and density of dipolar, ionic activation (ie, charge density) across the myocardium to guide ablation of atrial arrhythmias. Methods The prospective, nonrandomized UNCOVER AF trial (Utilizing Novel Dipole Density Capabilities to Objectively Visualize the Etiology of Rhythms in Atrial Fibrillation) was conducted at 13 centers across Europe and Canada. Patients with persistent AF (>7 days, <1 year) aged 18 to 80 years, scheduled for de novo catheter ablation, were eligible. Before pulmonary vein isolation, AF was mapped and then iteratively remapped to guide each subsequent ablation of charge density-identified targets. AF recurrence was evaluated at 3, 6, 9, and 12 months using continuous 24-hour ECG monitors. The primary effectiveness outcome was freedom from AF >30 seconds at 12 months for a single procedure with a secondary outcome being acute procedural efficacy. The primary safety outcome was freedom from device/procedure-related major adverse events. Results Between October 2016 and April 2017, 129 patients were enrolled, and 127 underwent mapping and catheter ablation. Acute procedural efficacy was demonstrated in 125 patients (98%). At 12 months, single procedure freedom from AF on or off antiarrhythmic drugs was 72.5% (95% CI, 63.9%-80.3%). After 1 or 2 procedures, freedom from AF was 93.2% (95% CI, 87.1%-97.0%). A total of 29 (23%) retreatments because of arrhythmia recurrence were performed with average time from index procedure to first retreatment being 7 months. The primary safety outcome was 98% with no device-related major adverse events reported. Conclusions This novel ultrasound imaging and charge density mapping system safely guided ablation of nonpulmonary vein targets in persistent AF patients with 73% single procedure and 93% second procedure freedom from AF at 12 months.
