Úvod: Pooperačne ponechaný gázový materiál v paraspinálnej oblasti predstavuje vzácnu a väčšinou asymptomatickú komplikáciu, v chronických prípadoch však môže predstavovať diferenciálno-diagnostický problém. Kazuistika: Pacient, 69 rokov, so spinocelulárnym karcinómom laryngu a anamnézou lumbálnej diskektómie v segmentoch L4–L5 a L5–S1 vpravo vykonanej pred 20 rokmi. Počítačová pozitrónová emisná tomografia potvrdila zvýšený metabolizmus fluorodeoxyglukózy v sakrálnej oblasti. T2-vážené snímky magnetickej rezonancie zobrazili hypointenznú mäkkotkanivovú masu s hyperintenzným periférnym lemom. Peroperačný nález a biopsia potvrdili fibrotickú formáciu so zabudovanými vláknami gázového materiálu. Prezentovaná kazuistika sa zaraďuje k publikovaným prípadom s najdlhším časovým obdobím medzi spinálnym operačným výkonom a diagnózou gossypibómu. Závery: Ponechaný chirurgický gázový materiál nevykazuje špecifické klinické a rádiologické znaky. Mal by byť zahrnutý do diferenciálnej diagnostiky mäkkotkanivových más v paraspinálnej oblasti u pacientov po operácii chrbtice. Magnetická rezonancia a peroperačné nálezy predstavujú najlepšie modality pre diagnostiku gossypibómu.
Introduction: Surgical sponge retained after a surgery in the paraspinal area is a rare and mostly asymptomatic complication that, however, may represent a problem for differential diagnostics. Case report: Our patient was a 69-year-old man with squamous cell carcinoma of the larynx and after right-sided lumbar discectomy at the L4–L5 and L5–S1 levels performed 20 years ago. Computed positrone emission tomography confirmed increased metabolism of fluorine-deoxyglucose in the sacral region. T2-weighted MRI showed a hypointense soft tissue mass with hyperintense peripheral rim. Intraoperative finding and biopsy confirmed fibrous formation with imbedded fiber gauze material. Our case is among case reports with the longest time periods between the primary spine surgery and the diagnosis of gossypiboma. Conclusions: Retained surgical sponges do not present with any specific clinical or radiological signs. They should be included in differential diagnosis of soft tissue masses in the paraspinal region in patients after spinal surgery. Magnetic resonance imaging and intraoperative findings are the best modality for the diagnosis of gossypibomas.

The case of a 17-year-old patient with a pseudotumour of the forefoot caused by a retained toothpick fragment is reported. The patient had several examinations in a two-year period and was treated for synovialitis of the first and third metatarsophalangeal joints. However, radiography of the plantar surface was the only examination done during these two years. Therapy was unsuccessful. After admission to our department, ultrasonography was performed and a foreign body in granulation tissue was detected. Computed tomography and MRI confirmed the finding. The foreign body granuloma was removed by surgery and the patient healed successfully. Options for visualising wooden foreign bodies not detected on X-ray images are discussed. Penetrating heart injuries are immediate life-threatening situations. We present a case report of a 44-year-old man with a foreign body in his right heart. The injury happened while the man was working with a circular saw 15 months prior to the actual diagnosis. With respect to the size of the foreign body, its close proximity to the right coronary artery, the potential risk of bleeding, embolization, endocarditis or pericarditis, surgical therapy was indicated in spite of the fact that the patient was asymptomatic. The foreign body (a spring-segment of a roller blind) was successfully removed.

Reimplantation of a total hip arthroplasty (THA) with excessive heterotopic polyethylene granuloma is reported. An 84year- old woman, who had undergone THA reimplantation on the right side 4.5 years previously, was indicated for revision surgery due to aseptic failure of the acetabular component. Pre-operative examination revealed a pulsating mass below the right inguinal ligament that raised the suspicion of a pseudoaneurysm of the external iliac artery. Because of this suspected late complication of the first revision arthroplasty, surgical exploration was indicated by the vascular surgeon prior the second reimplantation. A CT-scan showed a large mass attached to a preserved lumen of the external iliac artery, and this was diagnosed by the vascular surgeon as a pseudoaneurysm. However, no pseudoaneurysm was found by a vascular surgery procedure, and the large mass ventrally extending the external iliac artery appeared to be a large polyethylene granuloma well enveloped in a fibrous pseudocapsula. The granuloma was removed and the wound sutured. Twelve days later, revision total hip arthroplasty and acetabular reconstruction were successfully carried out.
