Úvod: Autoři prezentují ve své práci výsledky léčby zlomenin patních kostí novou metodou pomocí patního hřebu C-NAIL. Cíl: Cílem práce je vyhodnotit výsledky léčby zlomenin patních kostí pomocí patního hřebu C-NAIL a porovnat je s výsledky léčby zlomenin patních kostí pomocí jiných implantátů. Materiál a metoda: Na oddělení úrazové chirurgie v nemocnici Pardubice autoři odoperovali za obddobí tří roků celkem 107 zlomenin patních kostí pomocí nové miniinvazivní metody – patního hřebu C-NAIL. K vyhodnocení funkčních výsledků použili mezinárodně uznávané skórovací schéma AOFAS (American Orthopaedic Foot and Ankle Society). Výsledky: Autoři vyhodnotili AOFAS funkční vyhodnocovací skóre vždy po půl a po jednom roce od operace pacienta. Naměřené hodnoty dosahovaly rozsahu 65 - 100 bodů, s průměrnou hodnotou 93,0 bodů po šesti měsících od operace a 94,1 bodů po 12 měsících od operace. Dále autoři vyhodnotili obnovení sníženého Böhlerova úhlu z úrazových hodnot 6,2° na 31,8°. Po třech měsících zaznamenali mírný pokles Böhlerova úhlu na 29,6° a po 12 měsících na 28,3°. Zaznamenali dva případy okrajové nekrózy rány (1,9 %) a jeden případ hluboké infekce (0,93 %). Závěr: Patní hřeb C-NAIL umožňuje miniinvazivním způsobem dosáhnout dobrou repozici kloubní plochy a vysokou biomechanickou stabilitu fragmentů zlomeniny patní kosti při výrazně nižším výskytu komplikací.
Introduction: The authors present the results of calcaneal fracture treatment using a new method with the calcaneal nail (C-NAIL). Aim: The aim is to evaluate the results of calcaneal fracture treatment using a new method with the C-NAIL and compare them with the results of calcaneal fracture treatment using other implants. Material and methods: At the Department of Traumatology Surgery in Pardubice Hospital, the authors performed a total of 107 calcaneal fracture surgeries over a period of 3 years using a new minimally invasive method – C-NAIL. The internationally accepted AOFAS (American Orthopaedic Foot and Ankle Society) scoring scheme was used to evaluate the performance results. Results: The authors evaluated the AOFAS score six months and one year after the patient’s surgery. The measured values ranged from 65-100 points, with an average of 93.0 after 6 months and 94.1 at 12 months after the surgery. They evaluated the restoration of the crossing Böhler angle in the traumatic values of 6.2° to 31.8°. After 3 months, the Böhler angle slightly decreased to 29.6° and after 12 months, to 28.3°. They recorded 2 cases of wound edge necrosis (1.9%) and one case of deep infection (0.93%). Conclusion: C-NAIL enables mini-invasive reposition of the articular surface and high biomechanical stability of calcaneal fractures with a significantly low incidence of complications.

Zlomenina distálního radia je v současnosti považována za nejčastější jednoduchou zlomeninu. Ze statistik vyplývá, že tento úraz postihne 17 % populace starší 60 let! (13) a je známo její dvouvrcholové rozložení v populaci. Statistika vedená na naší klinice tato čísla potvrzuje. Každým rokem ošetříme okolo 150 pacientů se zlomeninou distálního radia a zhruba 1/3 z nich řešíme operačně. Od roku 2007 je dominantní metodou operační léčby na naší klinice otevřená repozice a dlahová osteosyntéza LCP dlahou, a to převážně z volárního FCR přístupu. Ve svém článku bych chtěl ukázat obecné principy a indikace k operační a konzervativní léčbě.
Fractures of distal radium are currently considered to be the most common simple fractures. According to recent statistics up to 17 % of population over 60 years suffer this type of injury while its age distribution has been well described. Our own records confirm these observations. Every year there are approximately 150 patients treated for distal radium fractures at our department; one third of them undergo surgery. Since 2007 the predominant method of surgery at our department is open reposition and osteosynthesis using LCP splint from volar FCR exposure. Our article will summarize general principles of both surgical and conservative treatment approaches, which, despite being well-known, are not frequently performed.

PURPOSE OF THE STUDY To inform on our experience with the treatment of humeral shaft fractures by intramedullary osteosynthesis using Hackethal's bundle nailing and to evaluate indications for the current use of this technique. MATERIAL AND METHODS From January 2001 to December 2009, a total of 184 patients with humeral shaft fractures were treated surgically and, of these, 147 by Hackethal's nailing. From January 2006 60 patients treated by this technique were prospectively followed up and, of them, 50 were evaluated (83.3%) and included in this study. The average age of the study group was 51.2 years. Fifteen fractures (30%) were in the proximal third, 32 (64%) in the middle third and three (6%) in the distal third of the humeral diaphysis. According to the AO classification, the distal-third diaphyseal fractures were mostly type A2 (26.6%) and type B1 (20.1%), mid-third fractures were mostly type A3 (31.3%) and distal-third fractures were not evaluated because of their low number. Based on radiographic examination, patients with uncomplicated fractures of the humeral shaft were indicated for surgery. This was performed according to the original Hackethal's procedure with the patient in the prone position using Kirschner wires, 2 mm in diameter, as implants. The patients were followed up at 6 weeks and 3, 6, 9 and 12 months after surgery. On examination, healing of the operative wound, bony union achieved and radiographic findings of the fragment position were evaluated. The range of motion at the shoulder and elbow was assessed and subjective patients' feelings were recorded. RESULTS Of the 50 patients evaluated, 45 (90%) healed by first intention, four developed pseudoarthrosis (8%) and one required repeat surgery. No deep wound infection was recorded. Six patients (12%) had humeral angulation up to 10 degrees and one (2%) up to 13 degrees. This angulation was recorded in proximal-third diaphyseal fractures. A deviation in humeral internal rotation was found in two patients (4 %), in one of whom it was 40 degrees. Mild restriction of elbow extension (10 degrees) was found in two patients (4%), five (10%) reported slightly restricted motion of the shoulder and five, chiefly elderly patients with proximal-third diaphyseal fractures, had severe restriction of shoulder motion. Most of the patients (78%) reported full satisfaction, including return to normal daily activities, at 6 months after injury. When the therapy was finished at 12 months after surgery, 44 patients (88%) expressed their full satisfaction. DISCUSSION Our results give support to the previously reported good outcomes of Hackethal's osteosynthesis of the humeral shaft. This method is associated with a minimum of complications if the conditions of correct indication are fulfilled, i.e., uncomplicated fractures of the mid-third humeral diaphysis are considered. Filling the medullary cavity completely with K-wires or nails is an additional condition. Without these being met, good stability of the fracture is not achieved and non-union can develop, as shown by the analysis of cases with non-union in our patient group. CONCLUSIONS Hackethal's technique of osteosynthesis with elastic bundle nailing facilitates safe and easy treatment of uncomplicated diaphyseal fractures of the humerus. Its outcomes are comparable with the other currently used and more expensive methods. It is the method of first choice in the treatment of transverse and short oblique fractures of the mid-third of the humeral shaft.

PURPOSE OF THE STUDY Presentation of technical experience and the clinical and functional results of intramedullary fixation of forearm shaft fractures. MATERIAL AND METHODS Between January 1994 and December 2009, a total of 96 patients with 144 radial and/or ulnar fractures (ulna, 33; radius,15; both, 48) were treated by nailing (True/Flex®). According to the AO classification there were 22-A, 22-B and 22-C type fractures in 39 (41%), 44 (46%) and 13 (13%).cases, respectively. Of these, 82 (85%) were closed (types: 0, 48; I, 33; II, 1) and 14 (15%) were open (types: I, 13; II, 1; III, 0) fractures. Seventy-eight patients (81%) were followed up and their functional outcomes were evaluated according to the criteria of Anderson et al. The average interval between the operation and final follow-up was 28 months (15 to 96 months). RESULTS The average time to surgery was 2.2 days (0 to 25 days). Early complications were recorded in 4% of the patients (1x bursitis olecrani; 1x end cup replacement; 1x bending of nails) and late complications in 15% (5x non-union; 2x delay union; 4x bursitis olecrani; 1x ruptured tendon). Bone healing was achieved in 95% of the cases and took on average 16 weeks (7 to 34 weeks). No infection, refracture or synostosis occurred. Primary loss of reduction was recorded in four cases due to distraction in one, bent nails in two and a wrong size of the implant in one; secondary loss of reduction was found in three cases, with two cases of radius shortening and one 10°malrotation. No primary malrotation was recorded, but secondary loss of alignment was seen in the distal part of the radius and the proximal part of the ulna. Functional results according to the Anderson criteria were excellent and good in 87% of the cases. DISCUSSION Intramedullary mailing provides good stability to mid- and distal-third shaft fractures of the ulna and mid- and proximal-third shaft fractures of the radius, particularly in AO type A and type B fractures. The technical aspects of the method are analysed in detail in this paper. CONCLUSIONS True/Flex® intramedullary nailing, because of its minimally invasive approach and close reduction, allows us to minimise the risk of wound and neurovascular complications, especially in proximal shaft fractures of the radius.

The article describes results of treatement using blocking intramedullary osteosynthesis in 76 patients with diaphyseal fractures of the femur and tibia. Our experience application of blocking intramedullary osteosynthesis showed the following advantages causing early joints activity: no opening of fracture's line; decreased bleeding and risk of development of septic complication; blocking of above and below place of fracture excludes rotational angular mixture of bone fragments and shortening them in length. Needless of additional external immobilization in application of the method made possible the early activation of joints and shortening of hospital treatment. The received results testify the high efficiency of this method and further development the concept of early osteosynthesis in treatment of diaphyseal fractures of long bones in patients with a multiple trauma.

Osteogenesis imperfecta je vrozené systémové onemocnění, které je způsobeno u více než 90 % pacientů kvantitativním nebo kvalitativním defektem tvorby kolagenu typu I, který je základním proteinem kosti a pojivové tkáně. Molekulární genetika prokazuje nejčastější mutace genů COLI A1 a COLI A2 s lokalizací na dlouhém raménku chromozomu 7q a 17q. Onemocnění vede k různému stupni kostní lomivosti, ligamentózní laxicitě a změnám zabarvení sklér. Deformity skeletu po frakturách a asymetrický fyzární růst odpovídají stupni kostní fragility. Extrémní lomivost kostí může být až neslučitelná se životem. U nás se nejčastěji používá dělení OI dle Sillence, který rozlišuje 4 základní typy osteogenesis imperfecta na základě klinických a rentgenových příznaků, dědičnosti a vlastního průběhu onemocnění. Terapie pacientů představuje komplexní péči od rehabilitace přes medikamentózní léčbu s pomocí bisfosfonátů až po léčbu ortopedickou operační nejčastěji s užitím metody segmentární osteotomie a nitrodřeňové fixace (Kirschnerovy dráty, Prevotovy pruty, Küntscherův hřeb, teleskopický hřeb). Operační léčení pacientů s osteogenesis imperfecta je spojeno s vysokým procentem komplikací, jako je ohnutí hřebu, fraktura na hrotu hřebu, protruze hřebu nebo pseudoartróza. Umožnění stoje s pomocí ortéz a snížení četnosti fraktur je smyslem operační terapie. Cílem sdělení je uvedení komplexního přehledu o osteogenesis imperfecta od patogeneze, dělení, klinického obrazu po terapii.
Osteogenesis imperfecta is an inheritable connective tissue disorder with variable phenotypic expression. Quantitative and qualitative of the genetic error in type I collagen formation are typical in the 90 percent of patients. The major structural collagen of the skeletal system, including bone, ligament, and tendon, is type I collagen. Molecular genetic shows formation of mutant two separate genes – COLIA1 and COLIA2 located on the long arm of chromozome 7q and 17q. This disorder results in varying degrees of skeletal vulnerability, ligamentous laxity and scleral discoloration. The clinical picture varies according to the variety of the disease. Bone fragility and long bone deformities are of variable severity. Extreme bone fragility can lead to death in the perinatal period. Sillence classification delineated four distinct types of osteogenesis imperfecta, based on clinical, genetic and X ray characteristics. Treatment includes rehabilitation program, medical treatment (bisphosphonate therapy) and orthopaedic treatment – typicle multiple diaphyseal osteotomies with intramedullary fixation (Kirschner wire, Prevot rod, Küntscher nail, telescoping medular rod). There are many complications of a surgical technique for correction - migration of the rod, fracture of the bone, fracture of the wire, rod extrusion. The aim of orthopaedic surgery is to provide for ability to walk with bracing and decrease the number of fractures in patients with osteogenesis imperfecta.
