Chronické žilní onemocnění (CVD – chronic venous disease) představuje celosvětově závažný zdravotní problém, protože jeho prevalence je vysoká – až 84 % populace včetně pacientů s klinickým stadiem C0 (studie Vein Consult Program z roku 2012). Základní součástí konzervativní léčby jsou venoaktivní léky (VAD – venoactive drugs). V roce 2014 byla publikována nová doporučení pro léčbu CVD. Došlo ke změně klasifikace VAD s ohledem na jejich bezpečnost a účinnost. Mikronizovaná purifikovaná flavonoidní frakce (MPFF) je podle nových doporučení jediným venoaktivním lékem se stupněm doporučení 1B (doporučení silné, kvalita důkazů střední – nejvyšší stupeň doporučení). Rutosidy, extrakt z listů červené vinné révy, calcium dobesilát, extrakt ze semen koňského kaštanu a extrakt z listnatce mají stupeň doporučení 2B, nemikronizované diosminy a Gingko biloba pak 2C. MPFF je indikována pro všechna stadia CVD a jako jediný venoaktivní lék je doporučována u léčby žilních bércových vředů.
Chronic venous disease (CVD) represents a major health problem, as its prevalence is high – up to 84% of population including patients with clinical stage C0 (Vein Consult Programme, 2012). Venoactive drugs (VAD) are a basic conservative treatment of CVD. New guidelines for the treatment of CVD were published in 2014. There are changes in classification of VAD based on their safety and efficacy. Micronized purified flavonoid fraction (MPFF) is now the only VAD with grade 1B (a strong recommendation based on moderate evidence). Rutosides, red wine leaves extracts, calcium dobesilate, horse chestnut seed extract and Ruscus extract are all given grade 2B (weak recommendations based on moderate evidence). Nonmicronized diosmins and Gingko biloba are given grade 2C (weak recommendations based on low quality evidence). MPFF is indicated in all stages of CVD and it is the only VAD recommendated in the treatment of venous leg ulcers.
