BACKGROUND: Mesonephric-like adenocarcinoma (M-LAC) is a rare, recently described tumor occurring in the uterine corpus and ovary, which shares the same morphological and immunohistochemical features with the more common mesonephric adenocarcinoma (MAC), which mostly arises the uterine cervix. Despite the similarities between these tumors, the histogenesis of M-LAC is still disputable. CASE PRESENTATION: Sixty-one-year-old woman presented with an advanced tumor of the left ovary with intraabdominal spread and liver metastases. After receiving 5 cycles of neoadjuvant chemotherapy, she underwent a hysterectomy with bilateral salpingo-oophorectomy, and resection of the liver metastasis, omentum, and appendix. Histologically, the ovarian tumor consisted of two components, whose morphology and immunohistochemical results were typical of either a serous borderline tumor (immunohistochemical positivity for PAX8, WT1, ER and PR) or a mesonephric-like carcinoma (immunohistochemical positivity for PAX8, TTF1 and GATA3). Only the component of the mesonephric-like adenocarcinoma metastasized to the omentum and liver. A molecular analysis with a panel of 271 genes (size 1020 kbp) was performed separately on samples from the borderline tumor, primary ovarian mesonephric-like adenocarcinoma, and liver metastasis. The results showed the clonal origin of all samples, which shared the same KRAS (NM_004985.3:c.34G > T, p.(G12C)) and PIK3CA (NM_006218.2:c.1633G > A, p.(E545K)) somatic mutations. Moreover, in the sample from the primary mesonephric-like carcinoma and its liver metastasis a likely pathogenic somatic MYCN mutation (NM_005378.4:c.131C > T, p.(P44L) was found. In all samples, the deletion of exons 9-10 in the CHEK2 gene was present, which is in concordance with the previously performed genetic testing of the blood specimen which revealed the hereditary CHEK2 mutation in this patient. CONCLUSIONS: Our result support the theory that at least some mesonephric-like ovarian adenocarcinomas are of Müllerian origin. The serous borderline tumor seems to be a precursor of mesonephric-like adenocarcinoma, which has been proven in our case by both tumors sharing the same mutations, and the presence of cumulative molecular aberrations in the mesonephric-like adenocarcinoma.

Cíl: Analyzovat rizikové faktory pro rozvoj recidivy u pacientek s borderline ovariálními tumory. Typ studie: Retrospektivní analýza 10letého souboru pacientek. Název a sídlo pracoviště: Gynekologicko-porodnická klinika 3. LF UK a FNKV, Praha. Metodika: U 59 pacientek s borderline ovariálním tumorem (BOT) jsme sledovali věk, histopatologický typ, DNA ploiditu, stadium, přítomnost invazivních nebo neinvazivních peritoneálních implantů, typ operačního výkonu, pooperační reziduum, adjuvantní léčbu, výskyt recidivy a dlouhodobou prognózu pacientek. Výsledky: Medián sledování pacientek byl 47 měsíců (rozmezí 1–144). V celém souboru bylo diagnostikováno 5 (8,5 %) DNA aneuploidních nádorů, z nich ve 4 případech šlo o pacientky mladší 50 let, ve 4 případech o časná stadia serózního typu BOT a žádný z nich dosud nerecidivoval. Pouze u 2 pacientek (3,4 %) došlo k rozvoji recidivy onemocnění – v obou případech šlo o mladé pacientky po konzervativní operaci pro serózní diploidní BOT ve stadiu I nebo II. Jediným statisticky významným parametrem v našem souboru byl konzervativní operační výkon (p = 0,0159). Závěr: Ve vyšetřovaném souboru jsme neprokázali vliv DNA ploidity na vznik recidivy onemocnění nebo celkové přežití u pacientek s borderline nádory ovaria. Jediným statisticky významným parametrem pro vznik recidivy bylo provedení konzervativní operace, které však nemělo vliv na celkové přežití.
Objective: To evaluate risk factors for development of recurrent disease in borderline ovarian tumors. Design: Retrospective study of 10-years single institution population. Setting: Dept. of Gynecology and Obstetrics, 3rd Medical Faculty of Charles University in Prague. Method: 59 consecutive cases of borderline ovarian tumors (BOT) were analyzed for age, histopathological type, DNA ploidy, stage, presence of invasive and non-invasive peritoneal implants, type of surgical procedure, residual disease, adjuvant therapy, recurrence and long-time prognosis of the patients. Results: Median follow-up was 47 months (range 1-144). There were 5 (8.5%) patients with DNA aneuploid tumors in the study group; 4 of them were younger than 50 years, 4 of them were early stage serous BOT; no one recur so far. No death of disease was described in the whole study group; only 2 patients (3.4%) developed recurrent disease – both were young patients after conservative surgery for serous diploid stage I/II BOT. Conservative surgery was the only significant factor for recurrence in univariate analysis (p = 0.0159) in our setting. Conclusion: DNA ploidy was not proved to be prognostic factor in borderline ovarian tumors in our study group. The only significant risk factor for development of recurrent disease was conservative surgery, with no influence on overall survival.

Cystické nádory pankreatu jsou definovány jako léze s převažující cystickou složkou. Jejich záchyt se v posledních letech zvyšuje, což může být dáno stoupající frekvencí zobrazovacích vyšetření. Hlavním klinickým problémem je jejich diferenciální diagnostika, která může být obtížná. V rámci diferenciální diagnostiky cystických procesů pankreatu je nutné odlišit – zánětlivé léze, – vrozené cystické útvary, a – solidní maligní tumory s cystickou degenerací. Ve skupině cystických nádorů je nutno rozlišit benigní, premaligní a maligní varianty. Mezi nejčastější cystické nádory pankreatu se řadí ¦ intraduktální papilární mucinózní nádor (IPMN), ¦ serózní cystadenom (SCA), a ¦ mucinózní cystická neoplazie (MCN). V tomto sdělení autoři popisují typické charakteristiky jednotlivých cystických tumorů pankreatu, zejména se soustřeďují na diagnostické metody, z nichž velký význam má endoskopická ultrasonografie.
Cystic tumours of the pancreas are lesions with a dominant cystic portion. Its incidence has been increasing in recent years, which may be due to the increasing frequency of imaging examinations. The main clinical problem is its differenatial diagnosis, which may be difficult. In the differential diagnosis of cystic lesions of the pancreas, it is important to distinguish between – inflammatory lesions, – inborn cystic lesions, and – solid malignant tumours with cystic degeneration. The most frequent cystic tumours of the pancreas are ¦ intraductal papillary mucinous neoplasia (IPMN), ¦ serous cystadenoma (SCA), and ¦ mucinous cystic neoplasia (MCN). In this paper, the authors describe the typical characteristics of cystic tumours, and particularly focus on diagnostic methods, of which endoscopic ultrasonography is of great importance.

BACKGROUND/AIMS: Pancreatic cystic neoplasms represent a heterogeneous group of tumors with varied malignant potential. The aim of this prospective study was to evaluate EUS, EUS-FNA and cyst fluid analysis in distinguishing serous cystadenomas (SCA) and mucinous cystic neoplasms (MCN). METHODOLOGY: Twelve patients with SCA (4 men, 8 women, mean age 58), 16 with MCN (3 men, 13 women, mean age 53) and 10 pancreatic non-tumorous cysts as controls (1 man, 9 women, mean age 43) were investigated by EUS-FNA from January 2003 to February 2006. Cyst fluid evaluation was done for cytology, amylase, CEA, CA 19-9, CA 72-4 and CA 15-3 (RIA). The final diagnosis was based on surgery & histology (14 patients) and/or follow-up after EUS-FNA (mean 15 months). RESULTS: In the MCN-group 13 mucinous cystadenomas, 2 cystadenocarcinomas and 1 malignant IPMT were found. EUS-FNA results: cytology (including staining for mucin) was diagnostic in 2/12 SCA (17%), 10/16 MCN (63%) and negative in all controls. Fluid CEA in MCN-group (mean 9487, 95%CI 0-23637) was significantly higher compared both with SCA-group (mean 22, 95%CI 0-54, p<0.001) and controls (mean 4, 95%CI 0.5-8, p<0.001). Similar results were found in fluid CA72-4 and fluid CA19-9. Accuracy of EUS-FNA with final diagnosis was 93%. CONCLUSIONS: EUS-FNA with cyst fluid CEA, CA72-4, CA19-9 and cytology are useful tools in differentiating SCA, MCN and non-tumorous cysts.

We report a 54-year-old female with a CT image of a 6x4 cm multiloculated pancreatic mass, invading the posterior wall of the stomach. At radiofluoroscopy the mid portion of the stomach was hypokinetic and narrowed, and at gastroscopy the mucosal folds in the corresponding area were rigid. Therefore, radiological methods strongly suggested the diagnosis of locally advanced cystadenocarcinoma. After surgery, pathomorphological examination showed marked peritumoural inflammation, and the cysts were lined by cuboidal, normotypic epithelium, not producing mucus. This is an unusual case of oligocystic variant of serous cystadenoma imitating invasive cystadenocarcinoma.
