Idiopathic scoliosis (IS) is a common medical condition in children, characterized by three-dimensional spinal curve and strong evidence of genetic predisposition. The purpose of the present case-control study is to examine the association between the polymorphic variant rs11190870 (T/C), near the LBX1 gene, and IS predisposition in distinct subgroups based on age at onset, family history and gender. A total of 127 IS patients and 254 unrelated controls of Southeastern European descent were recruited. The genotyping was carried out by TaqMan real-time amplification technology. The results were analyzed by the Pearson's Chi-squared Test and the Fisher's Exact Test with a value of p less than 0.05 as statistically significant. The T allele and homozygous TT genotype were associated with a greater incidence of IS. Our results suggest that there is a genetic association with IS in adolescents, familial and non-familial cases, and in females. Larger case-control studies are necessary to examine the genetic factors of IS/AIS etiology in infants, juveniles and males. In conclusion, the molecular genetic identification of diagnostic and prognostic molecular markers would make an early treatment including minimally invasive procedures possible.
PURPOSE OF THE STUDY Neuromuscular deformities of the spine represent surgically uneasy to solve problems as well as serious handicaps causing sitting instability, pressure ulcers as well as pain. The aim of our study is to conduct a retrospective clinical analysis of the results of surgical correction of these deformities. This paper presents the use of a recent technique of sacral-alar-iliac (S2AI) screws and its comparison with other techniques of pelvic stabilisation. MATERIAL AND METHODS The group of 41 patients treated surgically with S2AI screws technique and transpedicular or hybrid instrumentation of the spine consisted of patients with the primary diagnosis of muscular dysthrophy, spinal muscular atrophy, cerebral palsy and some other neuromuscular diseases. The results of pelvic obliquity correction and scoliotic correction in combined neuromuscular deformities of the spine and pelvis were analysed. The technique of S2AI screws implantation and the possibility of their free-hand technique implementation were presented. RESULTS In the followed-up group of patients an average correction of pelvic obliquity by 81% (from 29.1 degrees before the operation to 5.6 degrees after the operation) was reported. On average, 74% correction of scoliotic spine deformity was achieved (from 83.3 degrees before the operation to 22.5 degrees after the operation). In both the cases neither a significant loss of correction at the minimum one-year follow-up nor any serious complications associated with grappling of pelvic fixation were observed. DISCUSSION The S2AI screws offer at least the same stability and ability of correction as iliac screws and at the same time they provide significantly better results compared with the older methods of pelvic fixation such as the Galvestone technique. With a good knowledge of the surgical technique and anatomical aspects this technique can be applied in the form of a free-hand technique. Navigation as well as robotic techniques can help with the accurate positioning of the S2AI screw. Transfixation of sacroiliacal syndesmosis in patients with a neuromuscular handicap does not lead to deterioration of their mobility. CONCLUSIONS Simultaneous stabilisation of spine and pelvis makes it possible to achieve a good quality correction of the deformity and good clinical results over a long period of time. It allows for stability of the sitting position of the patients and improves the quality of their lives. Nowadays, the S2AI screws are considered to be biomechanically the best quality pelvic fixation, eliminating subcutaneous prominence of the instrumentation and reducing the risk of skin decubitus. Je prezentována kazuistika dvou pacientek s Parkinsonovým onemocněním komplikovaným ohybovým syndromem páteře (kamptokormia) a osteoporotickými zlomeninami. Pacientky byly léčeny dorzální instrumentovanou fúzí s rekonstrukcí předního sloupce páteře. V pooperačním období došlo u obou pacientek k junkčnímu selhání instrumentace a vzniku patologických zlomenin v terénu osteoporózy s nutností dalšího revizního operačního výkonu s prodloužením dorzální instrumentace v rozsahu Th10-S1. U obou pacientek došlo pouze kčástečnému zlepšení. Pacientky jsou schopny chodit pouze krátké vzdálenosti s nutností pravidelného užívání analgetik.
A case report is presented, of two patients with Parkinson's disease complicated by Bending spine syndrome (camptocormia) and osteoporotic vertebral fracture. Patients were treated with posterior instrumented fusion with the reconstruction of the anterior column of the spine. In the postoperative period the junctional instrumentation failure with development of pathological osteoporotic fracture occurred in both of the patients with the necessity of further revision surgery with posterior instrumentation extension in the range of Th10-S1. Both patients experiened only partial improvement. Patients are able to walk only short distances with the necessity of using oral analgesics

Autoři ve svém příspěvku uvádějí první výsledky šetření, které bylo zaměřeno na zjištění hmotností školních tašek chlapců a dívek 1. a 2. tříd základních škol v olomouckém regionu. Celkem byla zjištěna hmotnost u 502 školních tašek, změřeno 502 dětí (263 dívek a 239 chlapců). Z výsledků měření je zřejmé, že hmotnost tašky se zvyšuje s postupem žáků do vyšší třídy. Největší rozdíl mezi minimální a maximální hodnotou hmotnosti tašky byl zjištěn u 8letých dívek, činil 8,9 kg. Výsledky ukázaly, že procentuální podíl hmotnosti tašky doporučeným 10 % tělesné hmotnosti dítěte odpovídal pouze u 17,5 % dětí. 82,5 % dětí pak nosí aktovky těžké (10–20 %), přičemž 18,3 % dětí nosí aktovky o hmotnosti nad 20 % své tělesné hmotnosti, což už lze považovat za extrémně těžké. Nepřiměřená hmotnost školní tašky pak může ovlivňovat držení těla a výskyt bolesti zad.
Effects of the weights of school bags carried by pupils from the first two classes of primary schools in the Olomouc region (502 schoolbags belonging to 263 girls and 239 boys) on the pupils' posture were examined. The weight of the schoolbags of pupils from the 2nd year class was found to be higher than of pupils in the first year. The greatest difference between minimum and maximum weights (8.9 kilograms) was detected in the schoolbags of 8 year old girls. Only 17.5% of children carried bags that were within the recommended 10% of the child's body weight. Thus, 82.5% of the children carry heavier bags and 18.3% of these pupils carry extremely heavy bags (over 20% of their body weight).We conclude that the excessive weight of school bags contributes to poor posture and backache amongst these children.

Idiopatická skolióza je trojrozměrná strukturální deformita páteře s nejasnou etiologií. V konzervativní terapii uplatňujeme fyzioterapeutické přístupy a korzetování. Efektivita korzetoterapie je kontroverzním tématem. Cílem sdělení je nabídnout východiska pro diskusi o konzervativní terapii a korzetování nemocných s juvenilní idiopatickou skoliózou.
Idiopathic scoliosis is a three-dimensional spine deformity of uncertain etiology. Our mode of conservative therapy employs physiotherapeutic approach and application of corsets. Effective corset therapy is a controversial area. The communication suggests terminus a quo for a discussion on the conservative treatment and applications of corsets in patients with juvenile idiopathic scoliosis.

Neuromuskulární deformity páteře jsou svým charakterem i léčebným postupem zásadně odlišné od deformit idiopatických. Nejčastěji se setkáváme s paralytickými nebo rigidními kyfoskoliózami. Přidružená deformita pánve může být vyjádřena nadměrným předním či zadním sklonem, obliquitou, rotací či komplexní deformitou. Dle typu postižení rozlišujeme deformity primárně ne uropatické a myopatické. Cílem této práce je seznámit s charakterem deformit, jejich klasifikací a léčebnými možnostmi, včetně zhodnocení efektivity a bezpečnosti operačního řešení. Soubor a metodika: Sledovali jsme frontální a sagitální rovinu páteře a pánve u celkového souboru 42 pacientů s různým neuromuskulárním postižením a skolioticko u deformitou. K objektivnímu zhodnocení operačních výsledků bylo použito RTG vyšetření před operací a po ní se základním statistickým vyhodnocením. Výsledky: Výsledky RTG analýzy operovaných pacientů prokázaly velmi dobrou korekci skoliotické deformity s nastolením výhodnějšího postavení pánve ve frontální i sagitální rovině. V souboru 42 operovaných pacientů bylo dosaženo korekce páteře ve frontální rovině v průměru o 54 %. V sagitální rovině došlo ke zlepšení všech sledovaných parametrů (sagitální profil, T9 tilt, sagitální tilt). V oblasti pánve studie prokázala významno u korekci obliquity pánve, jakož i snížení nadměrné horizontalizace sakra. Všechny dosažené korekce významně přispěly ke zlepšení stability sedu a případného stoje. Závěry: Operační terapie ne uromuskulárních deformit páteře kombinací technik Luque - Galveston koriguje skoliotické křivky se zajištěním stabilní kompenzované páteře a vyvážené pánve bez další ztráty motorických či senzitivních funkcí. Takto je zajištěna stabilita stoje a chůze se zlepšením funkčního stavu kardiopulmonálního aparátu a celkového životního komfortu hendikepovaného pacienta.
Objective: There is an essential difference between neuromuscular deformities of the spine and idiopathic deformities in terms of their nature and therapeutic approach. Most often, we have to do with cases of paralytic or rigid kyphoscoliosis. Associated deformity of the pelvis may be manifested by excessive frontal or posterior inclination, obliquity, rotation or complex deformity. Based on the type of disorder, we distinguish primarily neuropathic and myopathic deformities. The goal of the article is to inform about the character of deformities, their classification and therapeutic options, and to evaluate the effect and safety of surgical solution. Patient and method: We monitored the frontal and sagital plane of the spine and pelvis in a total of 42 patients with different neuromuscular disorders and scoliotic deformities. X-ray examination before and after surgery with basic statistical evaluation was used for objective assessment of results of surgery. Results: The results of the X-ray analysis of the patients who underwent surgery showed a very good correction of scoliotic deformity which resulted in an improved position of the pelvis both in frontal and sagital plane. The frontal plane spine correction was achieved in 54% of a total of 42 operated patients included in the set. An improvement in all the monitored parametres (i.e. sagittal profile, T9 tilt, sagittal tilt) was achieved in the sagittal plane. The study brought evidence of a significant correction in pelvic obliquity in the pelvic region, as well as of reduced excessive horizontalization of the sacrum. All the corrections achieved had a significant effect on the improvement of the quality of the patients? sitting and also standing position. Conclusion: Surgical therapy of neuromuscular deformities of the spine using a combination of Luque and Galvestone techniques corrects scoliotic curves securing stable, compensated spine and balanced pelvis without further loss of motor or sense functions. This way, stability of the standing position and walking is secured, with concomitant improvement of functional status of the cardio-pulmonary apparatus and overall well-being of the affected patient.
