The purpose of this study was to determine the different levels of copper and zinc in the serum, urine, and scalp hair of patients with Wilson's disease receiving different, currently accepted methods of treatment to reduce the copper load (penicillamine-group 1, n = 8; zinc-group 2, n = 8; penicillamine+zinc-group 3, n = 8). Blood, urine, and hair samples were collected from the patients. All three treatments resulted in a significant decrease of the serum copper levels. Significantly increased levels of zinc in the serum were detected in the patients in groups 2 and 3 (19.1 and 18.8 micromol/l, respectively; p < 0.05). Copper excretion in the urine significantly increased during its administration to groups 1 and 3 (11.5 and 7.94 micromol/24 h respectively; p < 0.001) due to the effect of penicillamine. The administration of zinc as monotherapy (group 2) or in combination with penicillamine (group 3) led to an increase of its excretion (25.3 and 22.4 micromol/24 h, respectively; p < 0.01). Only an insignificant rise of the copper content in the hair was found in all three groups of patients. The content of zinc in the hair did not differ significantly in any of the groups in comparison with the control group.

V rámci biologického monitoringu byly v roce 2005 sledovány hladiny kadmia, olova, rtuti, mìdi, selenu a zinku v krvi a moèi dospìlé populace. Koncentrace kadmia v krvi kuøákù (medián) byla vyšší než u nekuøákù (1,30 vs. 0,50 μg/l). U žen byly nalezeny vyšší koncentrace kadmia v moèi ve srovnání s muži, u obou pohlaví byl pozorován vzestup hladin kadmia v moèi s vìkem. Koncentrace olova v krvi byly vyšší u mužù (35,4 μg/l) než u žen (26,6 μg/l) a stoupaly s vìkem. Koncentrace rtuti v krvi u mužù a žen byly 0,91 a 1,16 μg/l. Hladina selenu v krvi dospìlých byla 111 μg/l, hodnoty mìdi a zinku v krvi byly v normì a stabilizované. Muži mìli významnì nižší koncentrace mìdi v krvi ve srovnání se ženami neužívajícími a užívajícími orální antikoncepci (870 vs. 950 vs. 1305 μg/l).
In 2005 the concentrations of trace elements (cadmium, lead, mercury, copper, selenium, zinc) in blood and urine of adults were measured in the framework of the Biological Monitoring Project in the Czech Republic. Blood cadmium levels in adults were significantly influenced by smoking (medians 1.30 and 0.50 μg/l in smokers and non-smokers, respectively). The levels of cadmium in urine were higher in women than in men. Higher blood lead levels were observed in men (35,4 μg/l) in comparison to women (26,6 μg/l), plumbemia correlated with age. Blood mercury levels were 0.91 and 1.16 μg/l in men and women, respectively. Blood selenium levels were 111 μg/l. Blood copper and zinc levels are stabilized. Significantly lower blood copper levels were found in men in comparison with female non-users and users of oral contraception (870, 950 and 1305 μg/l, respectively).

Východisko. Stopové prvky - zinok, meď a selén - sú súčasťou antioxidačných enzýmov superoxiddizmutázy (SOD) a glutationperoxidázy (GHPx). Pri obličkových chorobách môžu zmeny metabolizmu stopových prvkov ovplyvniť rovnováhu v antioxidačnom obrannom systéme a prispievať k uplatneniu toxického účinku reaktívnych kyslíkových molekúl. Metódy a výsledky. Autori vyšetrili u 53 detí s chronickými nefropatiami (34 s chronickou glomerulonefritídou, 11 s chronickou renálnou insuficienciou a 8 detí v chronickom dialyzačnom programe) zinok a selén v krvi aj v moči, a antioxidačné enzýmy SOD a GHPx v erytrocytoch. Najnižšiu aktivitu SOD (737 ± 219 U/g Hb, p < 0,01) a koncentrácie zinku v sére (12,9 ± 3,2, p < 0,05) našli u detí v terminálnej urémii. GHPx bola pri porovnaní so skupinou zdravých detí zvýšená vo všetkých skupinách chorých. U dialyzovaných detí bola aktivita GHPx najvyššia (p < 0,01) a korelovala s koncentráciami selénu v moči (r = -0,86, p < 0,05). Aktivita SOD bola závislá na sérových koncentráciách medi (r = 0,78, p < 0,05). Najvyššia renálna exkrécia zinku a selénu bola u chorých s glomerulonef- ritídou (p < 0,05). Závery. Výsledky podporujú hypotézu o kauzálnom vzťahu medzi stopovými prvkami a antioxidačnými enzýmami.
Background. Trace elements - zinc, copper and selenium are part of antioxidant enzymes, superoxide dismutase (SOD) and glutathione peroxidase (GHPx). In renal diseases changes in the trace element metabolism may influence the equilibrium in the antioxidant defence system and enhance the toxic effect of reactive oxygen molecules. Methods and Results. The authors examined in 53 children with chronic neprophathies (34 with chronic glomerulonephritis, 11 with chronic renal failure and 8 children included in a chronic dialyzation programme) zinc and selenium in blood and urine and antioxidant enzymes SOD and GHPx in red blood cells. The lowest SOD activity (737 ± 219 U/g Hb, p < 0.01) and serum zinc concentration (12.9 ± 3.2, p < 0.05) were found in children in the terminal stage of uraemia, GHPx was as compared with the group of healthy children elevated in all groups of sick children. In dialysed children GHPx was highest (p < 0.01) and correlated with urinary selenium concentrations (r = -0.86, p < 0.05). The SOD activity depended on the serum copper concentrations (r = 0.78, p < 0.05). The highest renal zinc and selenium excretion was recorded in patients with glomerulonephritis (p < 0.05). Conclusions. The results support the hypothesis on a causal relationship between trace elements and antioxidant enzymes.

Uvádíme výsledky vyšetifeni sedmi pracovníků exponovaných zinku při bodovém svářeni spodků autobusů, u menz bylo zjištěno zvýšené vylučování zinku močí, svědčící pro zvýšenou expozici zinku. Šest vyšetřovaných bylo léčeno pio aiteriální hypertenzi, v laboratorním nálezu byla zjištěna šestkrát lehce a přechodně zvýšená aktivita ALT a pětkrát zvýšená koncentrace cholesterolu v plazmě. Z imunologických nálezů je nejčastější lehké snížení koncentrace alfa1antitrypsinu, klinicky se tento deficit neprojevil. Všetřované budeme nadále sledovat.
The authors present the results of examinations of seven workers engaged in welding of bottom parts of buses. In these workers elevated urinary zinc excretion was found suggesting increased exposure to zinc. The objective finding comprised most frequently arterial hypertension (six times), the laboratory finding revealed a slihght and transient increase of tiie ALT activity (six times) and elevated cholesterol concentrations (five times). As to immunological findings, a slight reduction ofthe alpha1 antitrypsin (five times) was most frequent, while this defect was not clinically manifest. The subjects will be followed up.
