Various therapeutic methods of endocrine and metabolic diseases are allied with different therapeutic approach but also with different costs. On three groups of patients we would like to discover cost-effectiveness of different types of therapy. For these purposes a database application will be developed which provide data collecting and cost-effectiveness analysis in different therapeutic methods (benign thyroid diseases, screening for thyroid dysfunction and therapeutic methods of metabolic diseases with related co-morbidities). It will be possible in common medical outpatients department to evaluate cost/effect of therapeutic methods in endocrine and metabolic diseases, simulate short/long terms cost/benefit of various groups of patients.
Různé metody léčby endokrinních a metabolických onemocnění jsou spojeny nejen s různými klinickými výsledky, ale i s různými ekonomickými náklady. Na třech skupinách pacientů bychom chtěli zjistit nákladovou efektivitu různých metod léčby. K tomuto účelubychom chtěli vyvinout databázovou aplikaci, která umožní sběr dat pro analýzu nákladové efektivity terapeutických přístupů při léčbě benigních tyreoidálních onemocnění, screeningu funkčních tyreopatií a léčbě obezity a k ní přidruženými onemocněními. Lékaři tak budou mít možnost v podmínkách běžné ambulantní péče porovnávat náklady a přínosy při léčbě endokrinních a metabolických pacientů nebo modelovat krátkodobé i dlouhodobé (celoživotní) náklady na jednotlivé skupiny pacientů.

Úvod: Radiojod 131I je v léčbě hypertyreózy ve světě široce využíván. V České republice je však tato metoda stále indikována poměrně málo a dosud se realizovala pouze za hospitalizace. Naše práce překládá první systematickou analýzu zkušeností s ambulantním podáním radiojodu v České republice. Metody: 39 pacientům (32 žen a 8 mužů, 21 s imunogenní hypertyreózou Gravesova-Basedowova typu a 18 s hyperfunkčními uzly, průměrný věk 66,8 roku) byla po předchozím vyšetření ambulantně podána fixní aktivita 550 MBq 131I ve formě kapsle. U 32 z nich jsme po 12–42 měsících hodnotili efekt léčby a komplikace a porovnávali jsme finanční náklady léčby radiojodem v ambulantním režimu a za hospitalizace ve srovnání s operačním výkonem. Výsledky: Z 32 pacientů 9 (28 %) dosáhlo eutyreózy bez nutnosti tyreostatické nebo substituční léčby, u 18 (60 %) pacientů se rozvinula hypotyreóza s nutností substituce levotyroxinem, u 2 (6 %) bylo možné významně redukovat dávky tyreostatik a u 2 (6 %) pacientů byla léčba bez efektu. Účinnost léčby se nelišila v závislosti na etiologii či klinické závažnosti hypertyreózy, ale klesala s rostoucím objemem štítné žlázy. Pacienti, u kterých byl nedostatečný nebo jen částečný efekt léčby, měli medián objemu štítné žlázy větší než 40 ml. U jednoho pacienta došlo k mírnému přechodnému zhoršení endokrinní orbitopatie, jiné komplikace jsme nezaznamenali. Porovnáním finančních nákladů na 1 pacienta jsme zjistili, že náklady na ambulantní léčbu radiojodem (2 849 Kč) představují pouhých 16 % nákladů na stejnou léčbu za hospitalizace (17 472 Kč) a pouhých 25 % nákladů na případnou alternativní operaci (11 414 Kč). Závěr: Léčba hypertyreózy radiojodem 131I představuje efektivní a šetrnou alternativu operace a v ambulantním režimu je i mnohonásobně ekonomičtější. Ambulantní léčba 131I není vhodná pro pacienty s objemem štítné žlázy nad 40 ml.
Introduction: Radioiodine 131I therapy of hyperthyroidism on an outpatient basis is widely accepted over the world. In Czech Republic, however, radioiodine therapy is still not enough used, and has been realized on an inpatient basis to date. Our work is the first analysis of the experiences with radioiodine therapy of hyperthyroidism on an outpatient basis in Czech Republic. Methods: Capsule with 550 MBq of 131I was administered orally in 39 hyperthyroid patients (32 women and 8 men, 21 with autoimmune Graves’ hyperthyroidism and 18 with toxic thyroid nodules, mean age 66.8 years). In 32 of them we evaluated effectiveness and complications of therapy after 12–42 months. We also compared financial costs of the radioiodine treatment on an outpatient basis with the treatment in hospitalization and with surgery. Results: After the treatment, 9/32 (28 %) patients were euthyroid without thyrostatic/thyroxine treatment, 18/32 (60 %) patients were hypothyroid with thyroxine therapy, 2/32 (6 %) patients significantly decreased doses of thyrostatic drugs. In 2/32 (6 %) patients the treatment was ineffective. The effect of the treatment did not depend on the etiology and severity of hyperthyroidism, but decreased with thyroid volume. Patients with ineffective or only partially effective treatment had median of thyroid volume more than 40 ml. In 1 patient thyroid associated ophthalmopathy was moderately worsened. Other complications were not observed. If we compared financial costs in model with 1 patient, we found that the costs of radioiodine therapy on an outpatient basis (118.7 €) comprise only 16 % of the costs of radioiodine therapy in hospitalization (728 €) and only 25 % of the costs of surgery (475.6 €). Conclusion: Radioiodine 131I is effective and safe in the treatment of hyperthyroidism and the therapy on an outpatient basis is much cheaper choice. The therapy with 131I on an outpatient basis is not suitable in patients with thyroid volume more than 40 ml.

BACKGROUND: It is controversial whether screening program for thyroid disorders in pregnancy should be universal or targeted case-finding. To evaluate the relationship between history, laboratory parameters and thyroid ultrasound (TUS) in pregnant women positive in universal screening. SUBJECTS AND METHODS: The screening included investigation of serum TSH (thyroid stimulating hormone) and TPOAb (antithyroperoxidase antibodies) in 5,520 unselected pregnant women in the 9-11th gestational week. In 822 the screening was positive: abnormal TSH (> 3.67 or < 0.06 mIU/l) and/or positive TPOAb (> 143 kIU/l). 200 consecutive women with positive screening were included it the study. RESULTS: 41 women (21%) had transient gestational hyperthyroidism (TGH) and 159 (79%) had a thyroid pathology: 10 (5%) overt hypothyroidism; 76 (38%) subclinical hypothyroidism, 7 (3.5%) overt hyperthyroidism and 66 (33%) euthyroid TPOAb positivity. After exclusion of TGH, only 74/159 (47%) women were classified as high-risk for thyroid disease according to their history. There were no significant clinical and laboratory differences between the high- vs. low-risk women, except for higher proportion of FT4<75th percentile (P=0.008) and larger thyroid volume in the high-risk group (P=0.04). Finally, only 66/126 (52%) of TPOAb-positive pregnant women had autoimmune pattern in TUS in comparison with 41/49 (84%) TPOAb-positive non-pregnant control women of comparable age (P<0.001). CONCLUSIONS: Less than half of the positively screened pregnant women can be classified as high-risk and almost half of them had not autoimmune pattern in TUS. High- and low-risk pregnant women have similar clinical and laboratory characteristics.

Úvod: Nemocnicní informacní systémy jsou ve fakultních nemocnicích vysoce rozšírené, ale nejsou dostacující jak pro využití behem lécby pacienta, tak i pro dlouhodobé lékarské výzkumy. Lékari si casto vedou své vlastní tabulky nebo databáze bez spolupráce s oddelením informatiky a casto také dochází k porušování zákonu Ceské republiky. Cíl: Tento clánek popisuje príklad využití vlastní lékarské aplikace, která umí spolupracovat s nemocnicním informacním systémem a je zároven vhodná pro použití v dlouhodobých lékarských výzkumech. Metody: Aplikace je vyvinula v jazyce Visual Basic. Je popsán farmakoekonomický výzkum lécby Radiojódem I135 u pacientu trpících onemocnením štítné žlázy. Výsledky: Financní náklady na lécbu jednoho pacienta ambulantne jsou 2850 Kc. Což je 16% ve srovnání s lécbou s hospitalizací a 25% ve srovnání s chirurgickou lécbou. Diskuze: Ambulantní lécba muže být pro zdravotnický systém vysoce motivující po financní stránce a zvýšit tak její podíl v metodách ve svuj prospech.
Background: Hospital Information Systems widely used in departments of university hospitals are not sufficient for both storing data about patients treatment and long-term research. Clinicians often use custom-developed applications which are maintained without any cooperation with the management of the hospitals and mostly break law in the Czech Republic. Objectives: This article describes using such an application in cooperation with the Hospital Information System. It also describes an example of a research of cost effectiveness thyroid gland diseases treatment using Radioiodine 131I in outpatient regime compared to hospitalization or an alternative surgery. Methods: The database application is developed in Visual Basic. The research studies the treatment by the Radioiodine 131I in 45 patients. We evaluated the financial cost of radioiodine therapy in the outpatient regime and hospitalization compared with a surgery. Results: The financial costs for 1 patient is 114 EUR, it means 16% if compared with the same treatment in a hospital and only 25% of the possible alternative operation. Conclusion: This study describes that the treatment by outpatient regime can be a motivating alternative compared to the treatment by 131I at a hospital or even the surgery.
