Cíl: Cílem práce je upozornit na význam úponu šlachy m. tibailis posterior na funkci nohy. Šlacha m. tibialis posterior je důležitým stabilizátorem přednoží. Při její dysfunkci dochází k nestabilitě nohy v kloubu talonavikulárním a kalkaneokuboidním. Dochází k poklesu podélné klenby nožní, pata se stáčí do valgozity. Soubor a metodika Na naší klinice jsme od roku 2002–2009 léčili 11 pacientů s příznaky ruptury šlachy m. tibialis posterior. U těchto pacientů jsme před operací prováděli MRI vyšetření nohy se zaměřením na šlachu m. tibialis posterior. Při operaci jsme revidovali šlachu m. tibialis posterior. V 9 případech jsme nalezli kompletní rupturu šlachy m. tibialis posterior, ve 2 případech byla šlacha výrazně tenká a destruovaná revmatickým procesem. V těchto případech jsme volili izolovanou talonavikulární artrodézu. U jedné pacientky byla zjištěna ruptura šlachy m. tibialis posterior včas a byla možná její transosální sutura do os naviculare pedis. Výsledky: Na naší klinice jsme od roku 2002 do roku 2008 odoperovali 11 pacientů (10 žen a 1 muže). Průměrný věk v době operace byl 46,9 let. Hodnotili jsme subjektivní obtíže pacienta. Téměř všichni pacienti hodnotí operaci dobře. 10 pacientů je prakticky bez obtíží, u jednoho pacienta přetrvávají lehké bolesti v oblasti dorza nohy. Kitaoka Hindfoot score bylo 49,6 před operací a 89,6 po operaci. Z komplikací došlo u jedné pacientky k povrchovému zánětu v oblasti operační rány. U všech pacientů došlo ke zhojení artrodézy, pouze u jedné pacientky došlo k prodlouženému hojení artrodézy. Závěr: Včasné rozpoznání a včasná operační léčba zajistí stabilitu nohy a zabrání větší deformitě nohy, která by si již vyžádala větší operační výkon.
Aim: The aim of this work is to point out the significance of tibialis posterior tendon insertion on the function of the foot. Tibialis posterior tendon is a significant forefoot stabilizer. Dysfunction of this tendon leads to foot instability in the talonavicular and calcaneocuboid joints, lowering of the longitudinal arch, and causing valgus heel deformity. Patients and methods: From 2000 to 2009, we treated 11 patients with the signs of tibialis posterior tendon rupture. Prior to surgery, the patients were examined by MRI of the foot focusing on tibialis posterior tendon. During the surgery, the tibialis posterior tendon was repaired. In 9 cases, we found a complete tibialis posterior tendon rupture, and in 2 cases, the tendon was markedly thin and destructed by rheumatologic process. In these cases, we chose to perform anisolated talonavicular arthrodesis. In one female patient, rupture of tibialis posterior tendon was detected soon enough to enable its transosseal suture into naviculare bone. Results: From 2002 to 2008, we performed surgery in 11 patients (10 females and 1 male). The average age at the time of surgery was 46.9 years. Subjective ailment of each patient was evaluated. The surgery was perceived positively by almost all patients. Ten patients reported virtually no symptoms, whereas one patient still suffers from mild pain in the dorsum of the foot. Kitaoka Hindfoot score was 49.6 before and 89.6 after the operation. As to the complications of this procedure, one female patient suffered from superficial inflammation in the surgical wound. The arthrodesis healed uneventfully in all patients except one female patient, who experienced a prolonged healing of arthrodesis. Conclusion: Early recognition and surgical treatment ensures stability of the foot and prevents its deformity, which would require a major surgical intervention.

Úvod: Těžká paréza až plegie peroneálního svalstva se vyskytuje jak v dětském věku (dětská mozková obrna), tak i v dospělosti (léze n. peroneus, radikulopatie L5, následky traumat CNS, mozkových příhod, u neuromuskulárních chorob). Soubor pacientů a metoda: Autoři prezentují vlastní soubor 18 nemocných, 14 mužů a čtyři ženy, kterým byla provedena transpozice šlachy m. tibialis posterior skrze interosseální membránu v průměru 9,2 měsíců po vzniku parézy. Výsledky: Při kontrole po téměř třech letech udávalo 64 % pacientů výborný a 29 % uspokojivý výsledek. Diskuze: Indikační kritéria, načasování operace, spektrum nemocných i neurologické a EMG nálezy jsou předmětem diskuze. Delší trvání peroneální plegie vede k rozvoji těžko odstranitelné kontraktury zadní skupiny svalů bérce, zkrácení Achillovy šlachy, kloubních pouzder i k ekvinózní deformitě nohy. Efektivním řešením perzistující peroneální plegie je transpozice šlachy m. tibialis posterior skrze interosseální membránu a ukotvení šlachy na bázi III. metatarzu.
Purpose of the study: A severe paresis or plegia of muscles innervated by the common peroneal nerve occurs in children (cerebral palsy) and in adults (peroneal nerve lesions, radiculopathies L5, sequellae after central nervous system injuries, stroke, in neuromuscular diseases). Material and methods: The authors present their own group of 18 patients, 14 men and 4 women. These patients underwent surgery (transfer of m. tibialis posterior tendon through the interosseal membrane) on average 9.2 months after developing plegia. Results: In a follow-up assessment almost 3 years after surgery, 64% of patients stated that their results were excellent and 29% of patients found their results satisfactory. Discussion: Indication criteria, timing of surgery, spectrum of patients, neurological and EMG findings are discussed. A longer duration of peroneal plegia leads to the development of hard to remove contractures of the posterior calf muscle group, shortening of the Achille?s tendon, joint capsules and equinnous deformity of the foot. Transfer of the m. tibialis posterior tendon through the interosseal membrane and anchoring it at the base of the third metatarsal bone is an effective solution to persistent peroneal plegia.

Úvod: Článek poskytuje náhled na léčbu získaného plochonoží dospělých s použitím medializační osteotomie patní kosti s prezentací vlastního souboru pacientů. Metoda a materiál: Hodnocený soubor čítá 19 pacientů operovaných v letech 2012-2016 ve věkovém rozmezí od 32 do 67 let. Podle Raikinovy klasifikace získaného plochonoží byli pacienti zařazeni do subtypu IIa. Základním prvkem operační korekce planovalgózní deformity nohy je korekce valgozity patní kosti pomocí medializační osteotomie s následným přídatným výkonem na mediálním pilíři či na Achillově šlaše. Soubor byl retrospektivně zhodnocen na základě porovnání tří rentgenologických parametrů před operací a za 6-8 měsíců pooperačně. Hodnocenými parametry byly Mearyho úhel, osa patní kosti v Saltzmannově projekci a procentuální krytí hlavice talu navikulární kostí. Výsledky: Počet úspěšných korekcí byl v tomto hodnocení u 17 z 19 pacientů celkem. Dále jsme zaznamenali u tří pacientů komplikaci s hojením rány a u jednoho byla nutná reoperace. Diskuse: Nejčastějšími způsoby, jak zafixovat medializační osteotomii patní kosti, jsou schodová úhlově stabilní dlaha nebo kompresní šrouby. Oba způsoby mají své výhody a nevýhody. Ovšem valgózní deformita zadonoží může být korigována i dalšími způsoby - subtalární artroereizou, prodloužením laterálního pilíře, osteotomií patní kosti nebo tripledézou jakožto finálním řešením pro postavení zadonoží. V rámci managementu získaného plochonoží je korekce postavení patní kosti vždy následována výkonem na mediálním pilíři, který dotváří optimální korekci daného případu.
Introduction: The article is intended to provide an overview of adult acquired flatfoot deformity through its basic pathomorphological signs. Furthermore, the authors asses their group of patients, who have undergone a surgical treatment of flatfoot deformity. Material and Methods: There were 19 patients involved in the study aged from 32 to 67 years. According to the Raikin's classification of acquired flatfoot, they fit into the IIa subtype. Medial displacement osteotomy of the calcaneus to correct its hypervalgosity represented an essential part of the surgical treatment. This procedure was followed by an additional surgery of the medial pillar or the Achilles tendon complex. Three radiographic signs were used for retrospective evaluation in the study. The Meary angle deviation, valgus deviation in Saltzman view for axial calcaneal alignment and percental talo-navicular coverage in frontal plane. These parameters were measured for assessment preoperatively and 6-8 months postoperatively. Results: Evaluating correction ability for calcaneus malalignment was our primary outcome for a clinical practice. There were 17 out of 19 patients with successful correction. In addition, we recorded healing complications in 3 patients. A revision procedure was necessary in one case. Discussion: Locking step plate or compressing screws are 2 most common way how to fix medial displacement osteotomy. Both procedures have its own advantages and disadvantages. Hindfoot malalignment could also be corrected by another procedure - sinus tarsi implant, lateral column lengthening, Z type osteotomy or a triple arthrodesis, which is a final procedure for hindfoot correction. Though, calcaneal osteotomy is barely used as a single surgery, as it needs to be followed by a variol medial column corrections in surgical flatfoot management.

BACKGROUND: The foot is often affected in patients with rheumatoid arthritis. Subtalar joints are involved more frequently than ankle joints. Deformities of subtalar joints often lead to painful flatfoot and valgus deformity of the heel. Major contributors to the early development of foot deformities include talonavicular joint destruction and tibialis posterior tendon dysfunction, mainly due to its rupture. METHODS: Between 2002 and 2005 we performed isolated talonavicular arthrodesis in 26 patients; twenty women and six men. Tibialis posterior tendon dysfunction was diagnosed preoperatively by physical examination and by MRI. Talonavicular fusion was achieved via screws in eight patients, memory staples in twelve patients and a combination of screws and memory staples in six cases. The average duration of immobilization after the surgery was four weeks, followed by rehabilitation. Full weight bearing was allowed two to three months after surgery. RESULTS: The mean age of the group at the time of the surgery was 43.6 years. MRI examination revealed a torn tendon in nine cases with no significant destruction of the talonavicular joint seen on X-rays. Mean of postoperative followup was 4.5 years (3 to 7 years). The mean of AOFAS Hindfoot score improved from 48.2 preoperatively to 88.6 points at the last postoperative followup. Eighteen patients had excellent results (none, mild occasional pain), six patients had moderate pain of the foot and two patients had severe pain in evaluation with the score. Complications included superficial wound infections in two patients and a nonunion developed in one case. CONCLUSIONS: Early isolated talonavicular arthrodesis provides excellent pain relief and prevents further progression of the foot deformities in patients with rheumatoid arthritis and tibialis posterior tendon dysfunction.

Získané rigidné plochonožie v dospelom veku (štádium III a IV podľa Johnsonovej klasifikácie) je charakterizované tým, že nie je možné deformitu pasívne korigovať do normálneho postavenia a operácia zachovávajúca kĺb už zvyčajne nie je možná. V štádiu III je nutná trojitá déza, respektíve jej modifikácia, pri ktorej sa vykonáva déza iba talonavikulárne a subtalárne a kalkaneokuboidný kĺb zostáva intaktný. Pri kontraktúre mäkkých tkanív laterálne je bezpečným postupom vykonanie takejto dézy z jedného mediálneho prístupu s elongáciou peroneálnych šliach za fibulou. V štádiu IV, charakterizovaným subluxáciou talu alebo artrózou členka, je zvyčajne nutné pristúpiť k pantalárnej déze, vo fixácii sa uplatňuje retrográdny klinec, dlahy a eventuálne iba osteosyntéza izolovanými skrutkami. Pri správnej operačnej technike a indikácii je možné dosiahnutie plantigrádnej nohy, ktorá, i keď je rigidná, je veľkým benefitom pre pacienta.
Adult-acquired rigid flatfoot /stage III and IV according to Johnson classification/ is defined as a deformity that can no longer be reduced into a normal position and the joint-sparing approach is usually not possible. The preferred approach in stage III is a triple arthrodesis or its variation , in which only subtalar and talonavicular joints are fused and calcaneocuboid jont stays intact. With soft tissue contracture on the lateral side the hindfoot arthrodesis from the single medial approach is usually indicated with tenotomy of the peroneal tendons behind fibula. In stage IV which is defined as a talar subluxation or ankle arthrosis , most predictable results gives a pantalar arthrodesis with various methods of fixation - with retrograde nails, plates or isolated screws. With propper indication and surgical technique the plantigrade foot can be achieved, which is of considerable benefit fot this difficult patient group.
