Východiska: Tato praktická studie se zabývá vyhodnocením významu používání vybavení a pomůcek usnadňujících péči o pacienty v léčebnách dlouhodobě nemocných. Vybavením nebo pomůckami se rozumí ošetřovatelské pomůcky, vybavení pokojů a prostředky usnadňující koupání pacientů.Cíl: Cílem bylo pomocí dotazníkového šetření s uzavřenými otázkami a využitím statistických metod zjistit, zda vnímání dostatečnosti tohoto vybavení závisí na některém z ukazatelů, či samo může mít vliv na spokojenost zdravotnického personálu. Jedná se o dílčí cíle rozsáhlejší studie, mapující spokojenost ošetřovatelského personálu i pacientů v léčebnách dlouhodobě nemocných.Metody: Kvantitativní dotazníková metoda (nestandardizovaný dotazník).Výsledky: Nejhorší výsledek byl zaznamenán v obou zařízeních u vybavení pokojů, lepší u pomůcek ke koupání a nejlépe pak dopadla dostupnost ošetřovatelských pomůcek potřebných k vykonávání ošetřovatelské péče. Celkové hodnocení personálu v LDN A dosáhlo skóre 4,18 a v LDN B skóre 4,29 z maxima 5. Mezi spokojeností personálu a dostatkem pomůcek byla vypočtena kladná hodnota Spearmanova korelačního koeficientu: rs=0,676. To představuje signifikantně významný vliv dostatku pomůcek na spokojenost personálu s p < 0,01. Ani jednou z použitých metod nebyl zjištěn signifikantní rozdíl ve vybavení pomůckami mezi oběma zdravotnickými zařízeními nebo zastoupenými pohlavími zdravotnických pracovníků.Závěr: Z našeho i jiných průzkumů vyplývá, že pracoviště jsou obvykle dobře materiálně vybavena. Navíc tento spíše technický či materiální faktor je obvykle v praxi řešitelný získáním finančních prostředků pro opatření případných chybějících pomůcek personálem v rámci svých kompetencí.
Background: This practice-based study evaluates the importance of using the equipment and aids to facilitate patient care in a long-term care facility. Equipment or aids are defined as nursing aids, room equipment and aids to facilitate patient bathing.Aim: The aim was to use a questionnaire survey with closed questions and statistical methods to find out whether the perception of the sufficiency of this equipment depends on any of the indicators, or whether it can itself have an impact on the satisfaction of nursing staff. These are partial aids of a larger study, mapping the satisfaction of nursing staff and patients in long-term care hospitals.Methods: quantitative questionnaire method (non-standardized questionnaire). Results: The worst result was recorded in both facilities for room equipment, better for bathing aids and the best result was for the aids needed for nursing care. The overall score of staff in long-term care hospital A was 4.18 and in long-term care hospital B it was 4.29. A positive Spearman's correlation coefficient calculated between the staff satisfaction and the availability of aids was rs=0.676. This represents a significant effect of aids sufficiency on staff satisfaction with p < 0.01. There was no significant difference in the provision of aids between the two health facilities or the gender of the nursing staff.Conclusion: Our and other surveys show, that workplaces are well equipped. Moreover, this rather technical or material factor is usually solvable in practice by obtaining funding by providing any missing equipment by staff within their com-petences.
Práce se zabývá retrospektivním vyhodnocením kvality života u ambulantních pacientů s chronickou fibrilací síní v jednom léčebném centru. Cílem bylo pomocí dotazníkového šetření s využitím standardních metod zjistit, zda různé aspekty kvality života závisejí na nějakém z konkrétních ukazatelů jako je pohlaví, věk nebo doba, která uplynula od stanovení diagnózy. Ukázalo se, že existují jisté signifikantní závislosti ukazatelů kvality života na věku, neprokázala se však závislost na pohlaví.
The work deals with retrospective evaluation of quality of life in outpatients with chronic atrial fibrillation in one medical center. The aim was to use a questionnaire survey using standard methods to determine whether various aspects of quality of life depend on any of the specific indicators such as gender, age or time elapsed since the diagnosis. There have been some significant dependencies of quality of life indicators on age, but no gender dependence has been demonstrated.

In this work, the analysis of data on atrial fibrillation (AF) burden from dual chamber pacemakers is used for supporting the anticoagulation treatment management. The aim is to evaluate the benefit of basic diagnostic functions to support oral anticoagulation therapy in patients with atrial fibrillation. These patients have increased risk of thromboembolism. If patients have an implanted pacemaker, the device’s diagnostic features monitor the frequency and duration of atrial fibrillation episodes. This data can then be used for further decisions. Statistical data processing was performed on a group of 117 patients with an implanted dual chamber pacemaker. From these results, we evaluated the benefits of the algorithms. In the whole group, a trend was observed in increase of the AF burden between the two monitored periods. The increase of AF burden occurred in 17 patients, while the decrease occurred in 6 patients only. Using simple logic functions, the numbers of patients with different binary values of the presence of AF, the presence of oral anticoagulation therapy, the risk CHA2DS2-VASc score and the values of AF burden were determined. Thus, in the whole group of patients, the diagnostic functions of the implanted devices contributed to the change in oral anticoagulation therapy for 24% of patients.

Subcutaneous implantable cardioverter defibrillator (S-ICD) protects the patients at risk for sudden cardiac death while leaving the heart and vasculature untouched. It provides life-saving therapy, but may also deliver inappropriate therapy. Presented case demonstrates a possibility of S-ICD therapy induction due to double-counting. It was original caused by lack of suitable sensing vectors and the solution was possible just particularly. As the inter-individual variability of subcutaneous cardiac signal is considerable, the patient screening should be necessary for identification of such patients, which have an unsuitable subcutaneous sensing signals.

Práce se zabývá retrospektivním vyhodnocením změny indexu tělesné hmotnosti (BMI) při radioterapii zhoubných nádorů hlavy a krku na krajském onkologickém pracovišti. V teoretické části jsou uvedeny informace o zhoubných nádorech hlavy a krku, možné nutriční podpoře a zajištění výživy těchto pacientů. Empirická část za pomoci kvantitativního průzkumného retrospektivního šetření porovnává index tělesné hmotnosti před a po ukončení radioterapeutické léčby a závislosti jeho změny na pohlaví a věku pacientů.
The work deals with a retrospective evaluation of the change in body mass index (BMI) during radiotherapy of head and neck cancer at the regional oncology department. The theoretical part provides information about head and neck cancer, possible nutritional support and nutrition of these patients. The empirical part, with the help of a quantitative exploratory retrospective survey, compares the body mass index before and after the end of radiotherapy treatment, and its dependence on patient's sex and age.

Implantable cardioverter-defibrillators (ICDs) are active cardiac implants for immediate treatment of ventricular arrhythmias. They provide life-saving therapy, but may also deliver inappropriate therapy. These two cases demonstrate a possibility of ICD therapy detection or induction encouraged by physiological patient breathing. The cases of 64-year-old and 30-year-old women patients are reported. The first one has received some therapy, the second one has not. The devices records did not show any abnormalities in trends or electrical parameters. As a solution, the detection parameters of the devices were optimized, with reference to individual physiologically reachable heart rates of the patients.

Pulse wave analysis permits non-invasive assessment of arterial elasticity indices. The contour varies in different parts of the circulation. It depends on physiological or pathophysiological conditions of the organism. The pathological events like arteriosclerosis or diabetes have a primary effect to the artery elasticity. Hypertension or some heart diseases also influence the pulse wave velocity and resulted in earlier wave reflections. There are several methods of pulse wave measurements based on different principles and depending on the type of measured pulse wave. The evaluation parameters can be assessed from the time domain, derivations, velocity or frequency domain. The main aim of this review article is to offer a recent overview of pulse wave measurement parameters and main results obtained. The principles of pulse wave measurement and current experience in clinical practice are shortly discussed too.
