IMPORTANCE: In treating atrial fibrillation, pulsed field ablation (PFA) is a novel energy modality with comparable efficacy to conventional thermal ablation, such as radiofrequency ablation (RFA), but with the benefit of some preferentiality to myocardial tissue ablation. Studies have demonstrated important safety advantages, including the absence of esophageal injury or pulmonary vein stenosis and only rare phrenic nerve injury. However, there is emerging evidence of coronary artery vasospasm provoked by PFA. OBJECTIVE: To compare the incidence and severity of left circumflex arterial vasospasm between PFA and RFA during adjacent ablation along the mitral isthmus. DESIGN, SETTING, AND PARTICIPANTS: This prospective cohort study enrolled consecutive adult patients receiving first-ever PFA or RFA of the mitral isthmus during catheter ablation of atrial fibrillation in 2022 with acute follow-up at a single referral European center. EXPOSURE: A posterolateral mitral isthmus line was created using either a multielectrode pentaspline PFA catheter (endocardial ablation) or a saline-irrigated RFA catheter. Simultaneous diagnostic coronary angiography was performed before, during, and after catheter ablation. Nitroglycerin was planned for spasm persisting beyond 20 minutes or for significant electrocardiographic changes. MAIN OUTCOMES AND MEASURES: The frequency and severity of left circumflex arterial vasospasm was assessed and monitored, as were time to remission and any need for nitroglycerin administration. RESULTS: Of 26 included patients, 19 (73%) were male, and the mean (SD) age was 65.5 (9.3) years. Patients underwent either PFA (n = 17) or RFA (n = 9) along the mitral isthmus. Coronary spasm was observed in 7 of 17 patients (41.2%) undergoing PFA: in 7 of 9 (77.8%) when the mitral isthmus ablation line was situated superiorly and in 0 of 8 when situated inferiorly. Conversely, coronary spasm did not occur in any of the 9 patients undergoing RFA. Of 5 patients in whom crossover PFA was performed after RFA failed to achieve conduction block, coronary spasm occurred in 3 (60%). Most instances of spasm (9 of 10 [90%]) were subclinical, with 2 (20%) requiring nitroglycerin administration. The median (range) time to resolution of spasm was 5 (5-25) minutes. CONCLUSION AND RELEVANCE: When creating a mitral isthmus ablation line during catheter ablation of atrial fibrillation, adjacent left circumflex arterial vasospasm frequently occurred with PFA and not RFA but was typically subclinical.

BACKGROUND: Pulsed field ablation (PFA) has a unique safety profile when used to treat atrial fibrillation, largely related to its preferentiality for myocardial tissue ablation, in particular, esophageal sparing. A pentaspline catheter was the first such PFA system studied clinically for atrial fibrillation ablation; in these initial regulatory trials, the catheter was used for pulmonary vein isolation and left atrial posterior wall ablation. Since its regulatory approval in Europe, in clinical practice, physicians have ablated beyond pulmonary vein isolation and left atrial posterior wall ablation to expanded lesion sets in closer proximity to coronary arteries. This is an unstudied important issue because preclinical and clinical data have raised the potential for coronary arterial spasm. Herein, we studied the vasospastic potential of PFA lesion sets, both remote from and adjacent to coronary arteries. METHODS: During routine atrial fibrillation ablation using the pentaspline PFA catheter, coronary angiography was performed before, during, and after pulsed field applications. The lesion sets studied included: (1) those remote from the coronary arteries such as pulmonary vein isolation (n=25 patients) and left atrial posterior wall ablation (n=5), and (2) ablation of the cavotricuspid isthmus (n=20) that is situated adjacent to the right coronary artery. RESULTS: During pulmonary vein isolation and left atrial posterior wall ablation, coronary spasm did not occur, but cavotricuspid isthmus ablation provoked severe subtotal vasospasm in 5 of 5 (100%) consecutive patients, and this was relieved by intracoronary nitroglycerin in 5.5±3.5 minutes. ST-segment elevation was not observed. However, no patient (0%, P=0.004) had severe spasm if first administered parenteral nitroglycerin, either intracoronary (n=5) or intravenous (n=10), before treatment. CONCLUSIONS: Coronary vasospasm was not provoked during PFA at locations remote from coronary arteries, but when the energy is delivered adjacent to a coronary artery, PFA routinely provokes subclinical vasospasm. This phenomenon is attenuated by nitroglycerin, administered either post hoc to treat spasm or as prophylaxis.

Možnosti použití vazodilatancií v této indikaci jsou předmětem tohoto článku.
In angiology, microcirculatory diseases are most commonly connected with peripheral vascular disease. A prototype of a well‑defined microcirculatory disorder, though present in many different diseases, treatable with vasoactive therapy is the Raynaud syndrome. The aim of this article is an overview of vasodilatation therapy in this indication.

Enhanced permeability and retention (EPR) effect-based nanomedicine is a promising strategy for successful anticancer therapy. The EPR effect is based on tumor blood flow. Because advanced large tumors, as frequently seen in clinical settings, are heterogeneous, with regions of defective vasculature and blood flow, achieving the desired tumor drug delivery is difficult. Here, we utilized the EPR effect to increase drug delivery. To augment the EPR effect for improved therapeutic effects of nanomedicine, we exploited vascular mediators-the nitric oxide (NO) generators nitroglycerin (NG), hydroxyurea, and l-arginine. These compounds generate NO in tumors with relatively high selectivity. Using different nanosized drugs in our protocol significantly increased (1.5-2 times) delivery of nanomedicines to different solid tumor models, along with markedly improving (2-3-fold) the antitumor effects of these drugs. Also, in 7,12-dimethylbenz[a]anthracene-induced advanced end-stage breast cancer, often seen in clinical settings, 2 mg/kg polymer-conjugated pirarubicin (P-THP) with NG (0.2 mg/mouse) showed better effects than did 5 mg/kg P-THP, and 5 mg/kg P-THP used with NG resulted in cures or stable tumors (no tumor growth) for up to 120 days. Moreover, in a murine autochthonous azoxymethane/dextran sulfate sodium-induced colon cancer model, NO donors markedly improved the therapeutic effects of P-THP even after just one injection, results that were comparable with those achieved with three weekly P-THP treatments. These findings strongly suggest the potential usefulness of NO donors as EPR effect enhancers to improve the therapeutic efficacy of nanomedicines.

U pacientov s achaláziou je efekt nitroglycerínu využívaný na krátkodobé zlepšenie ťažkostí. Ako je známe, nitroglycerín znižuje tlak dolného pažerákového zvierača prostredníctvom produkcie oxidu dusnatého. Manometria s vysokým rozlíšením (HRM – high resolution manometria)poskytuje vysoko senzitívne topografické zobrazenie pažerákovej motility, čo umožňuje jej precíznu analýzu. Doposiaľ nie sú k dispozícií údaje o vplyve nitroglycerínu na pažerákovú motilitu s použitím HRM. Taktiež nie je známy ani jeho efekt na horný pažerákový zvierač u pacientov s achaláziou. Ciel a metodika: Cieľom štúdie bolo kvantifikovať vybrané parametre pažerákovej motility s použitím HRM u pacientov s achaláziou a sledovať efekt nitroglycerínu na zmenu týchto parametrov. Do štúdie bolo zahrnutých 15 pacientov. Zistilo sa, že sublinguálne podaný nitroglycerín signifikantne znižuje všetky HRM parametre popisujúce tlak dolného pažerákového zvierača. Navyše, nitroglycerín pravdepodobne ovplyvňuje aj funkciu horného pažerákového zvierača. Podľa našich informácií ide o prvú štúdiu, ktorá s použitím HRM kvantifikuje vplyv nitroglycerínu na pažerákovú motilitu u pacientov s achaláziou. Predpokladáme, že tzv. nitroglycerínový test by mohol byť v budúcnosti praktický nástroj štúdia motility pažeráka u pacientov s achaláziou.
Nitroglycerin has long been used to relieve symptoms in patients with achalasia. It is well known that nitroglycerin decreases the pressure of the lower esophageal sphincter via mechanisms involving the production of nitric oxide. High resolution manometry (HRM) with esophageal pressure topography provides a more robust and sensitive evaluation of esophageal function, which allows better quantification. However, the effects of nitroglycerin on esophageal function have not yet been quantified using HRM. For example, the effects of nitroglycerin on upper esophageal sphincter in patients with achalasia have not been defined. Aims and Methods: The aim of the study was to quantify the effect of nitroglycerin on selected HRM parameters in patients with achalasia. We performed HRM in 15 consecutive patients with achalasia. We quantified the effects of nitroglycerine on esophageal function in patients with achalasia by using HRM measurements according to Chicago criteria. HRM analysis revealed that in patients with achalasia nitroglycerin affects not only the lower esophageal sphincter, but also influences the function of the upper esophageal sphincter. Nitroglycerin challenge may be a useful tool for future motility studies in patients with achalasia.
Přehledné sdělení týkající se chronické stabilní anginy pectoris, její definice, praktického postupu vyšetření, volby jednotlivých druhů zátěžových testů a stratifikace rizika nemocných. Jsou uvedeny principy farmakologické léčby, která sleduje 2 hlavní cíle: odstranění symptomů (antiischemická léčba) a zlepšení prognózy (prevence kardiovaskulárních příhod) a jsou přehledně vyjmenovány základní indikace ke koronarografii a revaskularizaci myokardu.
Review article dealing with chronic stable angina pectoris, its definition, practical examinations, selection of different types of stress tests and patient risk stratification. Principles of pharmacological treatment, which pursues two main objectives: the elimination of symptoms (ischemic treatment) and improved prognosis (prevention of cardiovascular events), are clearly enumerated as well as basic indications for coronary angiography and revascularization.
