In a case record the authors present an account on a patient with an opaque media of the left eye on account of seclusion and occlusion of the pupil. The cause of the unilateral chronic uveitis was elucidated by ultrasonic examination with visualization type A. The cause was a calcified lens dislocated into the vitreous body and fixed to the posterior wall of the eye. The finding was confirmed by a negative X-ray picture and positive finding of a small foreign body with high density on CT. Dislocation of the lens occurred after a blow on the left eye during boxing 15 years before the patient sought medical assistance on account of a painful practically blind eyeball.

Bisphosphonates are commonly used in the treatment of osteoporosis. Long-term use without drug holiday causes the risk of atypical fractures. Subtrochanteric and femoral stress fractures are among the frequently described complications. In our case report; a stress fracture of the scapular spine, a previously undescribed adverse effect of bisphosphonates, is presented. PURPOSE OF THE STUDY Use of an all-polyethylene (all-poly, AP) tibial component in primary total knee arthroplasty is still an attractive option considering the durability of replacement, the elimination of backside wear and the lower cost compared to modular metalbacked tibia. The purpose of the study was to evaluate the long-term results of the total knee replacement using the P.F.C. Sigma system with a monoblock all-polyethylene tibial component implanted at the 1st Orthopedic Department of the St. Anne's University Hospital and Masaryk University Brno in the period 1999-2010. MATERIAL AND METHODS In the monitored period, 911 total knee replacements using P.F.C. Sigma with an all-polyethylene tibial component were performed. This cohort of patients was evaluated at least 10 years after the primary replacement surgery. Altogether 323 knee replacements in 289 patients were evaluated. The clinical outcomes were assessed according to the Knee Society Clinical Rating System (KSS) and the Total Knee Arthroplasty Roentgenographic Evaluation and Scoring System. The replacement survival was evaluated using the Kaplan-Meier survival analysis. RESULTS In the evaluated group, the average KS was 82.6 points and the average functional score was 74.7 points. Excellent results were recorded in 213 cases, good results in 78 and satisfactory in 23 replacements. Poor clinical results were reported in 9 patients. The average range of motion was 105.8°. In 5 knees (1.5%) revision surgery was performed for infection complications, 3 cases were complicated due to periprosthetic fractures, 4 replacements were revised due to anterior knee pain and 8 knees showed a limited range of motion. Only 1 replacement was revised for aseptic loosening, with no need for reimplantation of the component. The survivorship of the implant was 98.5 % at the follow-up of 12.8 years, including the reasons of reimplantation. DISCUSSION Total knee replacement with an all-polyethylene tibial component is not so popular as the metal-backed modular implants. The all-polyethylene tibial component has certain advantages: lower unit costs, no backside wear, no liner dislocation, possibility of conservative bone resection. On the other hand, this type of implant is not modular and augmentations or longer stems cannot be used. Even though this type of endoprosthesis is recommended exclusively for elderly patients with a lower level of activity, the current mid- and long-term results show that indications for all-poly implants are much broader. CONCLUSIONS The total knee replacement with an all-polyethylene tibial component shows very good long-term clinical outcomes with an excellent survival rate. The all-poly design is cost-effective and may be used not only in elderly patients. Key words: total knee replacement, all-polyethylene tibial component, long-term results.
PURPOSE: The purpose of this study was to evaluate the long-term clinical and radiographic outcomes of the Balgrist total hip replacement cup. METHODS: We report the results of a retrospective review of 195 total hip prostheses with Balgrist cup implanted in 178 patients. There were 59 men and 119 women with a mean age at surgery of 52.3 years (range, 19-72). Cox regression was used to determine the influence of independent variables on the survivorship of the prosthesis. RESULTS: There were 117 (66 %) of 178 patients, and 131 (67 %) of 195 hips available for assessment at a mean follow-up of 17.4 years. A total of 21 patients (22 hips) were lost from follow-up (12 %). A total of 25 patients (27 hips) required some type of re-operation during the follow-up period. Eight of 27 re-operations (30 %) were directly caused by the Balgrist cup failure due to fatigue fracture of the metallic shell (n = 5; 19 %), polyethylene wear (n = 2; 7 %) and aseptic loosening (n = 1; 4 %). The mean polyethylene wear rate was 0.068 mm/year (range, 0.008-0.230; SD = 0.043). The risk of re-operation for any reason was decreased with dysplastic hip as a primary diagnosis (hazard ratio; HR = 0.250; 95 % CI 0.086-0.725) and with greater age at the time of surgery (HR = 0.923; 95 % CI 0.880-0.969). The 19-year survivorship with re-operation for any reason was 75.6 % (95 % CI 67.0-84.2). The 19-year survivorship with re-operation for the Balgrist cup failure was 90.5 % (95 % CI 83.4-97.6). CONCLUSIONS: The Balgrist cup continues to provide excellent clinical and radiological outcomes. This is associated at least in part with a low polyethylene wear rate. The main reason for the Balgrist cup failure is fatigue fracture of the metallic shell.

Léčení zlomenin horního konce pažní kosti je často obtížné, protože postihuje pacienty vyšších věkových skupin s výrazným stupněm osteoporózy. Běžné metody stabilní osteosyntézy pak často ztroskotávají na nízké pevnosti skeletu a obtížné fixaci malých úlomků. Dlaha Philos (Stratec Medical, Synthes - Oberdorf) využívající principu zamykatel-ných (LCP) šroubů je implantátem, který je dostatečně stabilní i u těchto problematických zlomenin. Autoři hodnotí soubor 33 pacientů operovaných od srpna 2002 do listopadu 2004 v Traumacentru Krajské nemocnice Liberec. V uvedeném souboru bylo 23 žen a 10 mužů, průměrný věk byl 57,8 let (64,7 u žen a 42,9 u mužů). U 24 pacientů s minimální dobou sledování 3 měsíce byly v prospektivní studii hodnoceny funkční výsledky podle Constant-Murley skóre. Průměrné Constant-Murley skóre bylo 79,3 (rozmezí 61-100), u zlomenin Neer 4 skóre 82,9 (64-96), Neer 5 skóre 72,3 (19-97) a u luxačních zlomenin Neer 6 bylo skóre 61,9 (36-79). Použití úhlově stabilní dlahy Philos představuje metodu volby u dislokovaných zlomenin proximálního humeru s výrazným stupněm osteoporózy, u tří a čtyřúlomkových zlomenin a u stavů spojených s rozlomením hlavice pažní kosti.
Treatment of fractures of the upper end of the humerus is often difficult because they frequently occur in older age groups with advanced osteoporosis. Common methods of stable osteosynthesis may fail owing to the low density of the skeleton and due to problemmatic fixation of small fragments. The plate Philos (Synthes) which uses the principle of locking screws (LCP) is an implant that is stable enough for these problematic fractures. The authors evaluated a group of 33 patients operated on at the Trauma Centre of the District Hospital of Liberec from August 2002 to November 2004. The group included 23 female and 10 male patients, average age 57,8 years (64,7 fe-male, 42,9 male). In 24 patients, monitored minimally over three months, functional results were evaluated according to the Constant-Murley Score in a prospective study. The average value of the Costant-Murley Score was 79,3 (range 61-100), in fractures Neer 4 score 82,9 (64-96 ), Neer 5 score 72,3 (19-97) and in luxation fractures Neer 6 score was 61,9 (36-79). Application of the axially stable plate Philos represents the method of choice for dislocated fractures of the proximal humerus in cases of marked osteoporosis, in three and four fragment fractures and in cases of broken head of humerus.

OBJECTIVE: This case report describes the treatment of a patient with arthralgia of the temporomandibular joint (TMJ) caused by disc displacement. BACKGROUND DATA: The goal of the treatment of TMJ arthralgia is to decrease pain by promotion of the musculoskeletal system's natural healing ability. METHODS: This report describes the complex treatment of TMJ arthralgia. Low-level laser therapy (LLLT) was chosen for its antiinflammatory and analgesic effects. Laser therapy was carried out using the GaAlAs diode laser with an output power of 400 mW, emitting radiation with a wavelength of 830 nm, and having energy density of 15 J/cm2; the laser radiation was applied by contact mode on four targeted spots in 10 sessions. Physiotherapy was recommended to this patient to prevent the injury of intraarticular tissue caused by incorrect movement during opening of the mouth. Splint stabilization and prosthetic treatment were used to reduce overloading of the TMJ, resulting from unstable occlusion and to help repositioning of the dislocated disc. RESULTS: Five applications of LLLT led to decrease of pain in the area of the TMJ on the Visual Analog Scale, from 20 to 5 mm. The anti-inflammatory effect of the laser was confirmed by thermographic examination. Before treatment, the temperature differences between the areas of the normal TMJ and TMJ with arthralgia was higher than 0.5 degrees C. However, at the conclusion of LLLT, temperatures in the areas surrounding the TMJ were equalized. CONCLUSION: This study showed the effectiveness of complex non-invasive treatment in patients with arthralgia of the TMJ. The analgesic and anti-inflammatory effects of LLLT were confirmed by infrared thermography.
