Cíl: Posoudit léčbu suprakondylických zlomenin distálního humeru u souboru dětí s tímto poraněním, léčených v KNTB Zlín. Zhodnotit výsledky léčby, včetně procentuálního zastoupení jednotlivých komplikací. Úvod: Shrnuje současný pohled na problematiku suprakondylické zlomeniny. Metodika: Retrospektivní studie souboru dětí, ošetřených na Traumatologickém oddělení KNTB Zlín v letech 2014-2018 s tímto poraněním. Výsledky: V daném období bylo ošetřeno celkem 228 pacientů, z toho čtyři repozicí a sádrovou fixací v celkové anestezii a 95 repozicí a osteosyntézou retrográdně zavedenými Ki dráty. Většina pacientů (94,7 %) byla operována urgentně (do šesti hodin od příchodu do ambulance). Preferovanou metodou na našem pracovišti je zkřížená perkutánní fixace Ki dráty (CRCPP). U 12 dětí (12,6 % operovaných) bylo nutno přistoupit k repozici otevřené, prováděné převážně z limitovaného zadního přístupu. Ve skupině operovaných dětí bylo zaznamenáno 20 případů (21,5 %) neurologických komplikací, z toho nejčastěji poranění n. ulnaris (12), případně n. ulnaris i n. medianus (4), nebo samostatně n. medianus (3), u dvou případů byla vzhledem k přetrvávající poruše prokrvení periferie indikována otevřená repozice s revizí NC svazku z předního přístupu. Žádné z ošetřených dětí nevykazovalo závažné trvalé následky. Nejčastějším následkem bylo lehké omezení hybnosti, vyjádřené lehkým omezením flexe, lehkou hyperextenzí lokte, nebo kombinací obojího (celkově 14 případů). V celkovém souboru pacientů jsme pozorovali dva případy poruchy růstu ve smyslu cubitus varus. Nebyl zaznamenán kompartment syndrom ani Volkmannova kontraktura končetiny. Závěr: Suprakondylická zlomenina humeru je častý úraz s relativně vysokým rizikem komplikací. Některé otázky ohledně optimálního managementu této zlomeniny stále nebyly přesvědčivě zodpovězeny a zaslouží si další zkoumání.
Objective: To assess the treatment of supracondylar fractures of the distal humerus in children, which were treated in Tomas Bata Hospital, Zlin. To evaluate the results in the context of contemporary studies. Introduction: Study presents the supracondylar fracture. Method: Retrospective study of a group of 228 patients with this type of fracture, treated between the years 2014-2018 in Traumatology department of Tomas Bata Hospital, Zlin. Results: In this group, 4 children underwent closed reduction and splinting in general anesthesia, 95 were operated on, from which 94,5 % urgently (less than 6 hours after first contact with the patient). The prefered method was closed reduction and percutaneous pinning by crossed Ki wires (CRCPP). In 12 cases (12,6 % of the operations) it was necessary to perform an open reduction, mostly from the limited posterior access. In the group of operatively treated children, there have been noticed 20 cases of some neural injury, mostly n. ulnaris (12), both n. ulnaris and medianus (4), n. medianus only (3). There were 2 cases of pale-pulsless hand after the reduction, which led to the revision of the neurovascular structures from the anterior access. None of the children displayed permanent consequences envolving their everyday lives. In minority of cases (14) at the end of the follow-up, persisted slight deficiency of the flexion, or slight hyperextension of the elbow, or the combination of both. We have recorded 2 cases of the cubitus varus deformity. In this cohort, there was no incidence of the compartment syndrome nor the Volkmann contracture of the extremity. Conclusion: Supracondylar fracture is a frequent injury with relatively high risk of complications. Some questions of the optimal treatment have still not been answered and need further research.
PURPOSE OF THE STUDY Evaluation of outcome after surgical treatment of humerus metastases with a focus on tumour and patient derived factors, timing and strategy of intervention, surgical outcome and complications. MATERIAL AND METHODS Sixty-fie patients with a mean age of 64.3 years (range 25-89) with 66 metastases of the humerus were surgically treated in a 7-year time-period and retrospectively reviewed. RESULTS Renal cell carcinoma and breast cancer were the most abundant types of primary tumour. The mean time from diagnosis of primary tumour to fist metastasis was 14.5 months (range 0-173). The mean time from diagnosis of metastasis to surgery was 21.4 months (range 0-173). 38/28 intramedullary nails/locking plates were used for 58/8 manifest/impending pathological fractures. Mean cumulative survival was 16.3 months and implant failure rate was 6.1% with a mean time from initial surgery to revision of 22.2-20.6 months. CONCLUSIONS Our data indicate that treatment with intramedullary fiation or cement augmented plate osteosynthesis is successful for the vast majority of patients, but thorough clinical evaluation and precise decision making adapted to the patient's estimated life expectancy must be applied to avoid overtreatment or risk of implant failure. Cíl studie: Naše retrospektivní multicentrická studie (3 centra daná pracovišti autorů) hodnotí 2leté výsledky léčby za použití PHILOS dlahy u pacientů s dislokovanými tří- nebo čtyřúlomkovými zlomeninami proximálního humeru. Materiál a metody: Do výsledků studie bylo zařazeno 120 pacientů (78 žen a 42 mužů ve věku 20 až 86 let). Pacienti s mnohočetnými poraněními a pacienti s patologickými zlomeninami byli ze studie vyřazeni. Celkem 81 pacientů bylo starších a celkem 39 bylo mladších 65 let. Operováno bylo z deltoidopektorálního přístupu v plážové poloze pacienta. Všichni pacienti byli kontrolováni ve 2 a 6 týdnech od operace a následně ve 3, 6, 9, 12 a 24 měsících. Funkční výsledek byl hodnocen za použití Constant skóre. Standardními rtg snímky na každé kontrole byla hodnocena pozice dlahy a postup hojení kosti. Funkční výsledek u pacientů starších 65 let byl porovnáván s výsledkem pacientů mladších 65 let. Výsledky: Průměrná doba sledování byla 24 měsíců (8 až 49). Celkem 117 pacientů bylo ke konci sledování primárně zhojeno klinicky a radiologicky. Průměrný čas do zhojení zlomeniny byl 11,5 týdne (7 až 20) od operace. Průměrné Constant skóre při poslední kontrole bylo 73,7. Průměrné Constant skóre u pacientů starších 65 let bylo 71,3 (36 až 80) u mladších 65 let 74,5 (42 až 96). Neprokázali jsme rozdíl ve funkčních výsledcích mezi mladšími (<65 let) a staršími (>65 let) pacienty. Použití PHILOS dlahy bylo v našem souboru spojeno s malým počtem komplikací (7,5 %) a minimální nutností následné reoperace. Diskuze: Na základě provedené retrospektivní studie se domníváme, že PHILOS dlaha přináší dobré výsledky v léčbě pacientů s dislokovanými tří- a čtyřúlomkovými zlomeninami proximálního humeru, zajišťuje adekvátní stabilitu úlomků s možností časné mobilizace bez ohrožení kostního hojení a garantuje dobrý funkční a klinický výsledek, při relativně nízkém riziku komplikací.
Purpose of the study: Our retrospective multicentric study evaluated 2-year results of using a PHILOS plate in treatment of displaced 3- and 4-part fractures of proximal humerus. Material and methods: 120 patients were included in the study (78 women, 42 men, aged 20 to 86). Patients with multiple injuries and patients with pathological fractures were excluded from the study. 81 patients were older than 65 years and 39 were younger. Deltoidpectoral approach was used in beach-chair position of the patient. All patients were examined 2 and 6 weeks following the surgery and then at 3, 6 ,9, 12 and 24 months. Functional outcome was evaluated using Constant score. Standard X-ray imaging was used to evaluate the position of the plate and progress of bone healing. Functional outcome of patients older than 65 years was compared to younger patients. Results: Average period of follow-up was 24 months (8 to 49). Total of 117 patients were primarily healed both clinically and radiologically speaking. Average healing time was 11,5 weeks (7 to 20) from the surgery. Average Constant score at final examination was 73,7. Average Constant score in patients older than 65 years was 71,3 (36 to 80), in younger patients it was 74,5 (42 to 96). We did not prove a statistically significant difference of functional outcome between younger (<65 years) and older patients (>65 years). Using a PHILOS plate was not associated with higher number of complications (7,5%). Discussion: Depending on our retrospective study we consider that using a PHILOS plate brings good results in treatment of patients with displaced 3-and 4-part fractures of proximal humerus, secures adequate stability with the possibility of early mobilisation without threatening the bone healing and guarantees a good functional and clinical outcome, with a relatively low risk of complications.

ÚVOD: Intraartikulární zlomeninu proximálního humeru považujeme za závažnou traumatologic - kou diagnózu. Operační řešení je stále oblastí roz - sáhlé diskuze a předmětem mnoha odborných člán - ků. CÍL: Cílem této práce je porovnání funkčního vý - sledku jednotlivých metod osteosyntéz i konzer - vativního řešení s ohledem na věk pacienta. V naší studii jsme si položili 3 otázky. 1. Jsou zlomeniny C 1.1, C 1.2, C 1.3, C 2.1 indiková - ny ke konzervativní terapii? 2. Jsou zlomeniny C 2.2, C 2.3, C 3.1, C 3.2, C 3.3 pod 65 let věku pacienta indikovány k osteosyntéze úh - lově stabilní dlahou? 3. Jsou zlomeniny C 2.2, C 2.3, C 3.1, C 3.2, C 3.3 nad 65 let věku pacienta indikovány k implantaci hemi - artroplastiky (CKP)? MATERIÁL A METODIKA: Do naší studie jsme zařadi - li celkem 159 pacientů s intraartikulárními zlomeni - nami proximálního humeru ošetřovaných na našem pracovišti v letech 2009 – 2013. Pacienti absolvovali RTG a CT vyšetření a byli klasifikováni dle AO (Ar - beitsgemeinschaft für osteosynthesefragen) klasi - fikace [ 13 ]. Dle klasifikace zlomeniny, biologického věku, přidružených onemocnění, lokálního stavu a nároků pacienta na pohyblivost a funkci, byl volen konzervativní či operační postup a daný typ osteo - syntézy. Pacienty jsme následně sledovali do 1 roku od úrazu, kdy jsme prováděli pravidelné RTG kon- troly (6 týdnů, 3 měsíce a 1 rok od úrazu) a zhodno - tili jsme CSS (Constant-Murley Shoulder Score) [ 5 ] jeden rok od úrazu. VÝSLEDKY: Dle naší studie u zlomenin C 1 a C 2.1 operační léčba nemá signifikantně lepší výsledky než léčba konzervativní. Pouze mladší pacienti měli signifikantně lepší funkční výsledky bez ohledu na terapeutický přístup. U C 2.2, 2.3 a C 3 zlomenin jsme signifikantně neprokázali lepší funkční výsledky PHILOS (Proximal Humeral Internal Locking System) dlahy pod 65 let proti hemiartroplastice. A taktéž jsme u C 2.2, 2.3 a C 3 zlomenin neprokázali lepší funkční výsledky he - miartroplastiky nad 65 let věku proti PHILOS dlaze. ZÁVĚR: Výsledky naší studie potvrzují složitost a nejednoznačnost postupu ošetřování intraartiku - lárních zlomenin proximálního humeru.
INTRODUCTION: Intraarticular fracture of the proxi - mal humerus is considered a severe trauma diagno - sis. Surgical treatment is still subject to extensive discussions and numerous articles . GOAL: The aim of this study is to compare the func - tional results of operative treatment methods and conservative treatment with respect to the age of the patient. In our study, we asked three questions. 1. Are fractures C 1.1, C 1.2, C 1.3, C 2.1 indicated for conservative therapy? 2. Are fractures C 2.2, C 2.3, C 3.1 C 3.2 C 3.3 in patients under the age of 65 in - dicated for osteosynthesis by locking compression plate? 3. Are fractures C 2.2, C 2.3, C 3.1 C 3.2 C 3.3 in patients over 65 years of age indicated for hemi - arthroplasty implantation (CKP)? METHODS: In our study we included 159 patients with intra-articular fractures of the proximal hume - rus treated in our department from 2009 to 2013. Majority of patients underwent CT examination and were classified according to the AO (Arbeitsgemein - schaft für osteosynthesefragen) classification [ 13 ]. Based on the classification of fractures, biological age, comorbidities, local state and requirements for patient mobility and function, patients were indica - ted for conservative treatment or surgical proce - dure and the respective type of osteosynthesis. Pa- tients were then followed up for 1 year after injury, underwent X-ray controls (6 weeks, 3 months and 1 year after injury), and CSS (Constant-Murley Shoul - der Score) [ 5 ] was evaluated 1 year after injury. RESULTS: According to our study, surgical treatment in C 1 and C 2.1 fractures has significantly better re - sults than conservative treatment. Only younger patients had significantly better functional results regardless of the treatment procedure. In C 2.2, 2.3 and C 3 fractures we did not prove sig - nificantly better functional outcome in surgical treatment by PHILOS (Proximal Humeral Internal Locking System) plate against hemiartroplasty in patients under 65 years of age. And also we did not prove in C 2.2, 2.3 and C 3 fractures better func - tional outcome in surgical treatment by hemiar - throplasty against PHILOS plate in patients over 65 years of age. CONCLUSION: The results of our study confirm the complexity and ambiguity of procedures for tre - atment of intra-articular fractures of the proximal humerus.

ÚVOD: Intraartikulární zlomeniny mají vysoké rizi - ko osteonekrózy hlavice humeru. S věkem dochází i k redukci kostní hmoty v oblasti hlavice humeru a stupeň osteoporózy limituje možnosti spolehlivé fixace osteosyntetickým materiálem. METODIKA: Do naší studie jsme zařadili celkem 29 pacientů s intraartikulárními zlomeninami proxi - málního humeru chirurgicky ošetřovaných LCP dla - hou (PHILOS®) na našem pracovišti v letech 2009 -2013. Pacienti absolvovali RTG a CT vyšetření a byli klasifikováni dle AO klasifikace. Prediktory ische - mie hlavice humeru dle studie Hertela z roku 2004 (délka metafyzárního fragmentu (calcar) menší než 8 mm, poranění mediálního závěsu, zlomeniny v ob - lasti anatomického krčku, 4úlomková zlomenina, angulace hlavice nad 45 stupňů, dislokace hrbolků nad 10 mm, glenohumerální luxace a fragmentace hlavice) byly porovnávány u pacientů s následnou osteonekrózou (parciální nebo kompletní) hlavice. VÝSLEDKY: Osteonekróza hlavice humeru se vy - skytla v 10 případech (34.5 %). Kompletní osteo - nekróza hlavice s jejím kolapsem a resorbcí jsme zaznamenali v 1 případu (3.4 %). Osteonekróza hlavice se vyskytovala u C3 zlomenin v 55.6 % pří - padů, u C2 zlomenin v 27.8 % případů. U C1 zlome - niny jsme nezaznamenali ostenekrózu hlavice ani u jednoho pacienta. Prediktory ischemie hlavice, tj. délka metafyzárního fragmentu menší než 8 mm, poranění mediálního závěsu, zlomenina v oblasti anatomického krčku a 4úlomková zlomenina byly přítomny u kompletní osteonekrózy hlavice. Vět - šina těchto prediktorů (70-100 %) byla přítomna u parciální osteonekrózy hlavice. ZÁVĚR: Riziko vzniku osteonekrózy je podstatně vyšší u C zlomenin. Pacienti s osteonekrózou hlavi - ce humeru měli podstatně horší funkční výsledky a vyžadovali revizní operaci v daleko větším počtu. Po prognózu hojení zlomeniny a riziko vzniku osteo- nekrózy hlavice je žádoucí zhodnocení prediktorů ischemie hlavice. U zlomenin s vysokým rizikem vzniku osteonekrózy hlavice by měla být zvážena primární implantace hemiartroplastiky.
INTRODUCTION: Intra-articular humeral head fractures face a high risk of humeral head osteo - necrosis development. Reduced bone quality in the area of humeral head and osteoporosis in older pati - ents limit the options of successful osteosynthesis. MATERIAL AND METHODS: A total of twenty-ni - ne patients with intra-articular proximal humerus fractures treated surgically by LCP plate (PHILOS®) at our department from 2009 to 2013 were inclu - ded in our study. All patients underwent X-ray and CT examination and the fractures were classified according to the AO classifiication. Predictors of the humeral head ischemia defined by Hertel in 2004 (length of the metaphyseal calcar segment less than 8 mm, disruption of the medial hinge, fractu - res of the anatomical neck, four-fragments fractu - res, head angulation over 45 degrees, displacement of tuberosities over 10 mm, glenohumeral disloca - tion and head-split component) were used for fur - ther assessment of patients with humeral head os - teonecrosis (partial or complete). RESULTS: Humeral head osteonecrosis occurred in 10 out of 29 cases (34.5 %). Complete head osteo - necrosis with the collapse and resorption was no - ted only in 1 case (3.4 %). C3 fractures were affected by osteonecrosis in 55.6 % of cases, C2 fractures in 27.8 %. No avascular necrosis was noted in the C1 fracture group. Predictors of humeral head ische - mia, i.e. length of the metaphyseal calcar segment less than 8mm, disruption of the medial hinge, fractures of the anatomical neck and four-frag - ments fracture were found in the case of complete humeral head osteonecrosis. Most of the predictors (70-100%) have occurred in fractures with develo - ped partial head osteonecrosis. CONCLUSION: The risk of osteonecrosis is considerably higher in C type fractures. Patients with hu - meral head osteonecrosis reached a significantly worse functional outcome and required more re - vision operations. Evaluation of humeral head is - chemia predictors is important for the prognosis of fracture healing and for assessing the risk of head osteonecrosis. Fractures with highly predictable humeral head necrosis should be considered for primary hemiarthroplasty.

Fractures of the proximal humerus commonly affect elderly patients. The vast majority of proximal humeral fractures result from low-energy trauma in presence of osteoporosis. Incidence of proximal humeral fractures dramatically increased over the last decades. Recent epidemiological studies expect a rather stagnant incidence. Diversity of fracture types attenuates reliability of available classification systems. Even though, predictive morphologic criteria have been detected enabling a prognostic assessment. A short or absent metaphyseal head extension and disruption of the medial periosteal hinge reliably predict ischemia of the humeral head fragment. Still, humeral head necrosis may be prevented in early reduction and fixation. The range of treatment options consists of non-operative therapy, minimal-invasive osteosynthesis, open reduction and plate fixation, intramedullary nailing and primary arthroplasty. Most proximal humeral fractures in the elderly are stable injuries and can be successfully treated by non-operative means. Operative treatment of displaced, unstable fractures should resort to the least invasive procedure providing adequate reduction and fixation stability. To date, open reduction and locking plate osteosynthesis represents the standard operative procedure in displaced three- and four-part fractures. However, a number of risk factors may promote fixation failure or impair functional outcome, most important low local bone mineral density, residual varus displacement of the humeral head, insufficient restoration of medial calcar support, humeral head ischemia and insufficient fracture reduction. Innovation of fixation techniques (e. g. angular stable locking systems and bone augmentation) will further expand indications for operative fracture treatment. Outcome of hemi-arthroplasty is closely related to anatomical tuberosity healing and restoration of rotator cuff function. Reverse shoulder arthroplasty may provide satisfactory shoulder function in geriatric patients, rotator cuff dysfunction or failure of first-line treatment. Choice of treatment should be individualized and base on careful evaluation of patient-specific, fracture-specific and surgeon-specific aspects.
