Introduction: Thalassemia is a healthcare challenging disease all over the world. It imparts a great burden on patients' families and healthcare institutions. Scientists focus on new aspects to overcome these challenges and increase patient tolerance of disease complications. This study aims to quantify β-Hydroxybutyrate Dehydrogenase BHBDH activity in thalassemia patients compared to the control group and their correlation with the patient's demographic characteristics.Methods: To do so, serum was collected from patients and the control group and analyzed biochemically for targeted laboratory tests. We determined β-Hydroxybutyrate Dehydrogenase from normal human serum using biochemical molecular techniques.Results: The results showed that BHBDH activity is significantly higher in the patients compared to the control group regardless of age, sex, or marital status. The results confirmed that enzyme activity and the purification folds were (0.0214 U/ml) and (51.7) respectively for the partially purified enzyme. Furthermore, the proportional molecular weight of the incompletely isolated β-Hydroxybutyrate Dehydrogenase was (125.8±0.5 kDa) using gel filtration chromatography. The comparative molecular weight of the subunit of partially isolated β-Hydroxybutyrate Dehydrogenase was (32.1±0.5 kDa) using SDS-PAGE.Conclusion: we demonstrate that BHBDH enzymatic activity is higher than control and this could be a prognostic or diagnostic tool in thalassemia patients.

Prezentujeme kazuistiku 35leté primigravidy/nulipary, která je nosičkou recesivní formy β-talasémie. Před první návštěvou v naší ambulanci byla diagnóza známá od dětství, a to na základě vyšetření krevního obrazu a rodinné anamnézy, nicméně nebyla geneticky potvrzena. Partner měl v dětství anémii a taktéž u něj byla potvrzena heterozygotní forma β-talasémie. Oběma bylo pomocí DNA-sekvenování verifikováno nosičství β-talasémie s mutací IVS I-6. T>C až během probíhající gravidity. Prvotrimestrální screening plodu byl negativní, z biopsie choriových klků a následného genetického vyšetření se však potvrdila homozygotní varianta β-talasémie. β-thalassemia maior je závažné vrozené onemocnění s nejvyšším výskytem v subtropických geografických oblastech. I přes velký pokrok v terapii zůstává stále vysoká morbidita i mortalita postižených jedinců. Díky globálním společenským změnám a migraci se u nás mohou objevovat choroby, které se dříve ve střední Evropě nevyskytovaly. Zdravotnictví by mělo být připravené na nové výzvy a dokázat jim čelit účinnou primární i sekundární prevencí vč. prenatální diagnostiky.
We present a case of a 35-year-old primigravid nulliparous woman, which is a carrier of recessive form of beta thalassemia. Prior to the first visit at our clinic, the diagnosis was not genetically confirmed, it was based only on the blood-count in the childhood. Her partner also had childhood anaemia, and heterozygous form was suspected. They both underwent a genetic counselling; both were diagnosed with beta thalassemia minor by DNA sequencing. The first trimester foetal screening was negative, but the homozygous form of beta thalassemia was confirmed from a chorionic villus biopsy and subsequent genetic examination. Beta thalassemia maior is a serious congenital autosomal recessive disease with the highest incidence in subtropical geographical areas. Despite great progress in therapy, the morbidity and mortality of affected individuals remains high. Due to global social changes and migration, genetic rare diseases that did not occur in Central Europe may emerge. Healthcare should be well prepared and be able to cope with an effective primary and secondary prevention including the prenatal testing.

Thalassemias are a heterogeneous group of red blood cell disorders, considered a major cause of morbidity and mortality among genetic diseases. However, there is still no universally available cure for thalassemias. The underlying basis of thalassemia pathology is the premature apoptotic destruction of erythroblasts causing ineffective erythropoiesis. In β-thalassemia, β-globin synthesis is reduced causing α-globin accumulation. Unpaired globin chains, with heme attached to them, accumulate in thalassemic erythroblasts causing oxidative stress and the premature cell death. We hypothesize that in β-thalassemia heme oxygenase (HO) 1 could play a pathogenic role in the development of anemia and ineffective erythropoiesis. To test this hypothesis, we exploited a mouse model of β-thalassemia intermedia, Th3/+ We observed that HO inhibition using tin protoporphyrin IX (SnPP) decreased heme-iron recycling in the liver and ameliorated anemia in the Th3/+ mice. SnPP administration led to a decrease in erythropoietin and increase in hepcidin serum levels, changes that were accompanied by an alleviation of ineffective erythropoiesis in Th3/+ mice. Additionally, the bone marrow from Th3/+ mice treated with SnPP exhibited decreased heme catabolism and diminished iron release as well as reduced apoptosis. Our results indicate that the iron released from heme because of HO activity contributes to the pathophysiology of thalassemia. Therefore, new therapies that suppress heme catabolism may be beneficial in ameliorating the anemia and ineffective erythropoiesis in thalassemias.

Přehledný článek přibližuje molekulárně-genetickou podstatu hemoglobinopatií, jejich klasifikaci a fenotypické projevy. Popis jednotlivých podskupin je doplněn o kazuistické příklady nemocných z české populace. Je uveden přehled 14 typů α-talasemických mutací, 34 β-talasemických alel, 4 4 δβ-talasemické alely a 22 hemoglobinových variant identifikovaných v české populaci u 876 osob z 579 rodin. Podrobněji jsou popsány hemoglobinopatie, které zde byly diagnostikovány prioritně: β-talasemická alela CD 38/39 (-C); Hb Olomouc; Hb Haná; Hb Hradec Králové a α-tal.-2 delece DNA o délce 18,3 kb, jejíž identifikace umožnila odhalit nový mechanizmus vzniku α-talasemie-2. Skutečnost, že hemoglobinopatie byly v ČR do konce roku 2017 diagnostikovány u téměř 900 nemocných, ukazuje, že u nás nejsou vzácné. To klade zvýšené nároky na jejich diagnostiku, včetně diagnostiky prenatální.
This article summarize molecular-genetic basis of hemoglobinopathies, their classification and phenotypic manifestations. The description of individual subgroups is supplemented with a case reports of patients diagnosed in the Czech population. This paper provides an overview of 14 types of α-thalassemic mutations, 34 β-thalassemic alleles, 4 4 δβ-thalassemic alleles and 22 hemoglobin variants identified in the Czech population in 876 persons from 579 families. In more detail are described hemoglobinopathies, that have been diagnosed and described as novel: β-thalassemic mutation CD 38/39 (-C); Hb Olomouc; Hb Hana; Hb Hradec Kralove and 18.3 kb deletion downstream of α-globin cluster leading to a new mechanism of α-thalassemia-2. The fact that until the end of 2017 hemoglobinopathies were diagnosed in nearly 900 patients shows that they are not rare in the Czech Republic. This brings increased demands for their diagnostics, including prenatal diagnosis.

Anémie, definovaná jako pokles koncentrace hemoglobinu pod dolní hranici normálních hodnot, je projevem mnoha chorobných stavů a pro adekvátní terapii pacienta je vždy nutné diferenciálně diagnosticky upřesnit její příčinu. Článek využívá morfologického dělení anemií na makrocytární, normocytární a mikrocytární a jako zvláštní skupinu vyčleňuje anémie se zvýšeným počtem retikulocytů v periferní krvi. Popsány jsou jednak klinické jednotky známé a obecně rozšířené, jako anémie sideropenická či anémie chronických chorob, ale také jednotky raritní, na které je však třeba v diagnostice anemického pacienta vždy pomýšlet. Incidence těchto vzácných onemocnění se navíc zvyšuje v důsledku zavedení molekulárně genetických metod do diagnostiky, prodlužující se délky přežití nemocných a také rapidně se zvyšující migrace z oblastí s endemickým výskytem těchto onemocnění. U každé klinické jednotky je popsána etiologie, základní patofyziologické mechanismy a klinické příznaky spolu s diagnostickými vyšetřeními. Terapie je, z důvodu diferenciálně diagnostického zaměření článku, zmíněna jen okrajově. Autoři jsou členy Centra pro vzácné choroby krvetvorby ÚHKT, které se zaměřuje zejména na vrozené a získané choroby červené krevní řady.
Anemia, defined as hemoglobin level under lower normal limit, is a symptom of different pathologic conditions and the accuratedifferential diagnosis is necessary to determine the cause of anemia. The article uses the morphological classification of anemiato distinguish macrocytic, normocytic and microcytic types of anemias and divides anemias with increased amount of peripheralblood reticulocytes as a special group. It describes commonly known clinical units as iron deficiency anemia or anemia ofchronic disease, as so as rare clinical units, which are always need to think about in a differential diagnosis of an anemic patient.There is an increasing incidence of rare blood disorders due to introduction of molecular genetics methods into diagnostics,prolonged overall survival of patients and increasing migration from areas with endemic occurrence of these diseases. Etiology,basic pathophysiological mechanisms, main clinical features together with important diagnostic examinations are described byeach clinical unit. Due to the differential diagnostic focus of the article only basic knowledge about therapy is mentioned. Theauthors are members of the IHBT Center for Rare Disorders of Hematopoiesis, which is focused mainly on congenital and acquireddisorders of red blood cell.

Talasémie tvoří heterogenní skupinu vrozených poruch červené krevní řady. Příčinou je nerovnováha globinových řetězců v molekule hemoglobinu, která vede k neefektivní erytropoéze v kostní dřeni a ke zkrácení životního cyklu erytrocytů. Klinická a laboratorní manifestace je značně rozdílná a závisí na počtu postižených globinových genů. Jedinci s nosičstvím talasemické alely jsou asymptomatičtí a nacházíme u nich jen mikrocytózu erytrocytů a u některých snížení hladiny hemoglobinu. Těžší formy talasémií jsou provázeny závažnou mikrocytární anémií s vystupňovanou hemolýzou a s tím souvisejícími komplikacemi. Diagnostika talasémií spočívá v analýze hemoglobinového spektra a v detekci kauzální mutace molekulárně genetickými metodami. Léčba nositelů talasemické alely většinou není nutná, u těžších forem talasémie spočívá v pravidelné substituci erytrocytárními koncentráty a léčbě přidružených komplikací. Kauzální terapií je transplantace kmenových buněk krvetvorby a genová terapie. U vybraných párů je dostupná prenatální diagnostika.
Thalassemias represent heterogenic group of inhereted red blood cells disorders. It is caused by imbalance of globin chains in hemoglobin molecule, which leads to ineffective erythropoiesis in a bone marrow and shortening of erythrocyte´s life span. Clinical and laboratory presentation is heterogenic and depends on number of affected genes. Thalassemia carriers are asymptomatic with microcytic erythrocytes and hemoglobin level can be decreased in some cases. Severe forms of thalassemia are associated with severe microcytic hypochromic anemia, an increased hemolysis and with a related complications. Diagnosis of thalassemia consists of analysis of hemoglobin spectrum and detection of the causal mutations by molecular genetic methods.. Carriers of thlassemia allele require no therapy, patients with severe form need a regular transfusion regime. As a curative procedures, hematopoetic stem cell transplantation and gene therapy can be used. Prenatal testing is also available.
Hemoglobinopatie (talasémie a hemoglobinové varianty) reprezentují skupinu vrozených mikrocytárních anémií, které jsou raritně diagnostikované i na našem území. Ve většině případů se jedná o jednotlivé pacienty nebo rodiny, často s odlišným etnickým původem. Klinické projevy onemocnění jsou rozdílné – od asymptomatického nosičství mutace až po těžké anémie s nutností pravidelné aplikace erytrocytárních koncentrátů. Cílem této práce je představit soubor pacientů, u kterých byla diagnostikovaná některá z forem hemoglobinopatie spolu s charakteristikou jednotlivých mutací. Metody: V letech 2007–2017 bylo vyšetřeno 32 dětí pro podezření na hemoglobinopatii. U pacientů byl vyšetřen kompletní krevní obraz, parametry metabolismu železa a speciální hematologická vyšetření. Jednotlivé mutace v HBA a HBB genech byly prokázané pomocí molekulárně genetických analýz. Výsledky: V našem souboru jsme detekovali u osmi pacientů tiché nosičství α-talasémie, u jedné pacientky nemoc hemoglobinu H, osmnáct pacientů s mutací β-globinového genu vedoucí k β-talasémii minor, jednu pacientku s β-talasémií major a čtyři pacienty s hemoglobinovými variantami – HbE, Hb Monroe, Hb Sydney a srpkovitá anémie (HbS). Závěr: Hemoglobinopatie lze v dnešní době velmi přesně diagnostikovat pomocí molekulárně genetických vyšetření – MLPA a Sangerova sekvenování. V důsledku celosvětové migrace lze očekávat importování dalších mutovaných alel globinových řetězců do genofondu české populace.
Hemoglobinopathies (thalassemias and structural hemoglobin variants) represent a group of inhereted microcytic anemias that are rarely diagnosed in our country. In most cases, these are individual patients or families, often with a different ethnic origin. Clinical manifestation can be heterogenic - from mild microcytic hypochromic anemia to severe, lifelong, transfusion-dependent anemia with multiorgan involvement. The aim of this work is to present a group of patients diagnosed with some forms of hemoglobinopathy together with characteristics of individual mutation. Methods: 32 children were examed for suspected hemoglobinopathy between 2007–2017. Complete blood count, iron metabolism parameters and special hematological tests were evaluated. Mutations in HBA and HBB genes were confirmed by molecular genetic testing. Results: We diagnosed eight α-thalassemia carriers, one patient with hemoglobin H disease, eighteen patients with β-thalassemia minor, one patient with β-thalassemia major and four patients with hemoglobin variants (HbE, Hb Monroe, Hb Sydney and sickle cell disease). Conclusion: Hemoglobinopathies can be exactly diagnosed by molecular genetic testing – MLPA and Sanger sequencing. As a result of global migration, new mutations of globin genes can be imported in Czech population genofond.
β-Thalassemia (β-thal) is considered rare in Central Europe. As in other malaria-free regions, the presence of β-thal in Central Europe reflects historical and recent immigration, and demographic changes that have influenced the genetic variability of the current populations living in this area. This study assesses the frequency and spectrum of mutations on the β-globin gene in Czech and Slovak subjects with clinical symptoms of thalassemia. The results of the initial part of this research were published more than two decades ago; the aim of this study was to update these original reports. During the period from 2002 to 2015, 400 cases from Czech and Slovak hematological centers were analyzed. Twenty-nine β-thal mutations, identified in 356 heterozygotes from 218 unrelated families, involve five unique mutations including a recently described insertion of a transposable L1 element into the β-globin gene. One mutation described here is reported for the first time. Most of the mutations were of Mediterranean origin and accounted for 82.0% of cases. All but one case studied were heterozygous carriers, manifesting β-thal minor, with rare exceptions represented by the rare (β(0)) codons 46/47 (+G) (HBB: c.142_142dupG) mutation associated with an α-globin gene quadruplication and by dominantly inherited β-thal with a more severe phenotype. One double heterozygous β-thal patient was a recent immigrant from Moldavia. The list of δβ-thal alleles (26 carriers, 16 families) contains Hb Lepore and two types of δβ(0)-thal deletions. In the past, genetic drift and migration as well as recent immigrations were responsible for the introduction of Mediterranean alleles, while several mutations described in single families were of local origin.
