A fatal case of 67-year-old female with metastatic breast cancer on chemotherapy complicated with febrile neutropenia, colitis and sepsis due to Clostridium septicum is presented. Important clinical symptoms, laboratory and radiology findings together with therapy and outcome of neuropenic colitis are also discussed.

In our study we present an overview of the use of Oxford Nanopore Technologies (ONT) sequencing technology on the background of Enteric fever. Unlike traditional methods (e.g., qPCR, serological tests), the nanopore sequencing technology enables virtually real-time data generation and highly accurate pathogen identification and characterization. Blood cultures were obtained from a 48-year-old female patient suffering from a high fever, headache and diarrhea. Nevertheless, both the initial serological tests and stool culture appeared to be negative. Therefore, the bacterial isolate from blood culture was used for nanopore sequencing (ONT). This technique in combination with subsequent bioinformatic analyses allowed for prompt identification of the disease-causative agent as Salmonella enterica subsp. enterica serovar Paratyphi A. The National Reference Laboratory for Salmonella (NIPH) independently reported this isolate also as serovar Paratyphi A on the basis of results of biochemical and agglutination tests. Therefore, our results are in concordance with certified standards. Furthermore, the data enabled us to assess some basic questions concerning the comparative genomics, i.e., to describe whether the isolated strain differs from the formerly published ones or not. Quite surprisingly, these results indicate that we have detected a novel and so far, unknown variety of this bacteria.
Bakteriémie vyvolaná Staphylococcus aureus (SAB) je nejčastější infekcí krevního řečiště s vysokou incidencí a letalitou. Při zjištění SAB je velmi důležité dodržet správný vyšetřovací a léčebný postup. Záměrem studie bylo posoudit klinické a epidemiologické charakteristiky případů SAB zachycených v ÚVN Praha a zhodnotit dodržení doporučených postupů v diagnostice a léčbě. Celkem bylo do retrospektivní studie zařazeno 65 pacientů, u nichž byl zachycen S. aureus v hemokultuře. V 60 případech (92 %) byla SAB vyvolána kmeny MSSA a u 5 pacientů (8 %) byly detekovány kmeny MRSA. Zdroj infekce se podařilo prokázat v 83 % případů. Průměrná smrtnost v analyzovaném souboru byla 28 %. Výsledky analýzy indikátorů kvality péče naznačily nedostatky v diagnostickém i léčebném postupu u 65 % pacientů. Studie podpořila nutnost konziliárních vyšetření odborníkem na infekční choroby u lůžka.
Staphylococcus aureus bacteremia (SAB) is the most common cause of severe bloodstream infections with high incidence and lethality. The diagnosis of SAB must be followed by an appropriate diagnostic and therapeutic management. The aim of the study was to identify clinical and epidemiological characteristics of SAB cases treated in the Central Military Hospital Prague and to assess compliance with the guidelines for diagnosis and therapy. A total of 65 patients with S. aureus blood culture positivity were enrolled in the retrospective study. Sixty SAB cases (92%) were caused by MSSA strains, and MRSA strains were detected in five patients (8%). The source of the infection was confirmed in 83% of patients. The average case fatality rate in the study cohort was 28%. The analysis of the quality of care suggested both diagnostic and therapeutic insufficiencies in 65% of SAB patients. Overall, the result of the study supports the importance of the role of infectious disease specialists played in the bedside management of patients with SAB.

Spread of carbapenemase-producing bacteria has been described all over the world. This phenomenon may be accelerated by many factors, including wars and natural disasters. In this report, we described an NDM-1-producing Klebsiella pneumonia ST11 recovered from a patient injured during the Maidan revolution in Ukraine. To our knowledge, this is the first report of a carbapenemase-producing Enterobacteriaceae in Ukraine and one of several reports describing wound colonization/infection of humans injured during war.

Autory je předložen přehled aktuálně dostupných informací o syndromu toxického šoku (TSS – toxic shock syndrome) – život ohrožujícím onemocnění, v jehož patogenezi sehrávají klíčovou úlohu toxiny grampozitivních bakterií Staphylococcus aureus a Streptococcus pyogenes. Autoři se věnují epidemiologii a patogenezi onemocnění, upozorňují na důležitá anamnestická data i klinické známky, které by mohly na rozvoj TSS poukazovat. V neposlední řadě zmiňují současný pohled na adekvátní diagnostiku a terapii v případě rozvíjejícího se či již rozvinutého syndromu. Pro názornost je referát doplněn popisem vlastních klinických případů, které demonstrují dvě etiologicky odlišné formy této poměrně vzácné nozologické jednotky.
The authors present an up-to-date review of toxic shock syndrome (TSS) – a life-threatening condition where toxins of the Gram-positive bacteria Staphyloccocus aureus and Streptococcus pyogenes play a key role in the pathogenesis. The authors provide insight into the epidemiology and pathogenesis of the disease and point out the relevant patient history data and clinical signs and symptoms that may indicate progression of TSS. Last but not least, the state of the art diagnostic and therapeutic approaches to early and full blown TSS are summarized. Case reports are presented to illustrate two different etiological forms of this relatively rare nosological entity.
