Terminal complement blockade by humanised monoclonal antibody eculizumab has been used to treat transplantation-associated thrombotic microangiopathy (TA-TMA) in recent years. This retrospective international study conducted by the Paediatric Diseases (PDWP) and Inborn Error Working Party (IEWP) of the European Society for Blood and Marrow Transplantation (EBMT) describes outcome and response of 82 paediatric patients from 29 centres who developed TA-TMA and were treated with eculizumab between January 2014 and May 2019. The median time from hematopoietic stem cell transplantation (HSCT) to TA-TMA manifestation was 92 days (range: 7-606) and from TA-TMA diagnosis to the start of eculizumab treatment 6 days (range: 0-135). Most patients received eculizumab weekly (72%, n = 55) with a standard weight (kg)-based dose (78%, n = 64). Six months from beginning of eculizumab therapy, the cumulative incidence of TA-TMA resolution was 36.6% (95% CI: 26.2-47) and the overall survival (OS) was 47.1% (95% CI: 35.9-57.5). All 43 patients with unresolved TA-TMA died. The cause of death was HSCT-related in 41 patients. This study also documents poor outcome of patients without aGvHD and their frequent concomitant viral infections. Considering recent publications, intensified eculizumab dosing and complement monitoring could potentially improve upon outcomes observed in this study.

Cíl studie: Zhodnocení výsledků transplantace kmenových buněk krvetvorby (HSCT, hematopoietic stem cell transplantation) u dětí s dědičnými metabolickými poruchami (DMP) a maligní infantilní osteopetrózou (MIOP) v České republice od začátku transplantačního programu v roce 1989 do současnosti. Metody: V období 1/1993 až 12/2021 bylo ve FN v Motole provedeno celkem 31 alogenních HSCT u 22 pacientů s DMP a 5 pacientů s MIOP. Dárcem kmenových buněk byl HLA identický sourozenec (MSD, matched sibling donor), nepříbuzný dárce (MUD, matched unrelated donor) nebo haploidentický rodinný dárce (MMFD, mismatched family donor). Zdrojem kmenových buněk byla kostní dřeň (KD), periferní kmenové buňky (PBSC, peripheral blood stem cells) nebo pupečníková krev (UCB, umbilical cord blood). Předtransplantační přípravný režim byl použit v souladu s doporučením IEWP-EBMT (Evropská pracovní skupina pro transplantace vrozených onemocnění). Výsledky: 1. alogenní HSCT podstoupilo 22 pacientů s DMP a 5 pacientů s MIOP. Medián věku v době HSCT byl 19,5 měsíce. Primární přihojení štěpu po 1. transplantaci bylo dokumentováno u 24 z 27 pacientů (89 %). U 3 pacientů byla z důvodu nepřihojení či odhojení štěpu nutná retransplantace. Akutní reakce štěpu proti hostiteli (GVHD) byla dokumentována u 11 pacientů (41 %), těžkou formu gr. III vyvinul pouze 1 pacient, chronickou GVHD 1 pacient. Zemřelo celkem 6 pacientů, 3 na komplikace spojené s transplantací, 2 na progresi základního onemocnění, 1 pacient po úspěšné transplantaci na komplikace neurochirurgického výkonu. Celkové přežití je 78 %, 21 z 27 pacientů žije s mediánem doby sledování 169 měsíců po transplantaci (12–347 měsíců), 20 pacientů se zlepšením/stabilizací projevů onemocnění. Závěry: HSCT je v současné době standardní léčebnou metodou u vybraných DMP a MIOP. Naděje na úspěch transplantace vysoce převyšuje její rizika, zásadní je časně stanovená diagnóza a indikace k HSCT. Léčba pacientů vyžaduje komplexní přístup a spolupráci lékařů transplantačního a metabolického centra a dalších specialistů.
Objective: Evaluation of the results of hematopoietic stem cell transplantation (HSCT) in children with inborn errors of metabolism (IEM) and malignant infantile osteopetrosis (MIOP) in the Czech Republic from the beginning of the transplant programme in 1989 to the present. Methods: In the period 1/1993 to 12/2021 a total of 31 allogeneic HSCTs were performed at the University Hospital Motol in 22 patients with IEM and 5 patients with MIOP. Patients were transplanted from HLA identical sibling (MSD), matched unrelated donor (MUD) or mismatched family donor (MMFD). The source of stem cells was bone marrow (BM), peripheral blood stem cells (PBSC) or umbilical cord blood (UCB). The conditioning regimen was used in accordance with the IEWP-EBMT (Inborn Errors Working party – European Bone Marrow Transplant). Results: 22 patients with IEM and 5 patients with MIOP underwent the first allogeneic HSCT at the median age of 19,5 months. Engraftment after the first transplant was documented in 24 of 27 patients (89 %). Three patients underwent retransplantation for primary or secondary graft failure. Acute graft-versus-host disease (GVHD) was documented in 11 patients (41 %), severe form of aGVHD gr. III in 1 patient and chronic GVHD in 1 patient, respectively. A total of 6 patients died, 3 patients due to transplant-related mortality, 2 patients from disease progression, respectively and 1 patient died after a successful transplant due to complications of neurosurgery. The overall survival is 78 %, 21 of 27 patients are alive with a median follow-up of 169 months after transplantation (12-347 months), 20 of them with improvement or stabilization of the disease. Conclusions: HSCT is currently the standard therapy for selected IEM and MIOP. This procedure has become much safer during recent decades, timely diagnosis and indication for HSCT are essential. The treatment requires multidisciplinary management and continuous collaboration with other specialists.
Daratumumab, an anti-CD38 antibody, is used experimentally in the treatment of relapsed acute lymphoblastic leukemia (ALL). We treated five patients suffering from relapsed ALL with daratumumab. Four patients had T ALL, three of whom achieved complete remission (CR) after treatment and underwent stem cell transplant (SCT). Two of them had a second relapse and died 6 and 8 months after SCT, respectively. One transplanted T ALL patient remained in CR2 15 months after relapse. In the remaining T-ALL patient, the disease progressed under daratumumab treatment, and the patient died early after the first relapse. The B-cell precursor ALL patient with a second CD19-negative relapse, whose disease turned out to be resistant to the combination of daratumumab with chemotherapy, later achieved CR3 with inotuzumab ozogamicin, underwent SCT and remained in CR3. Leukemia burden should be monitored after daratumumab, and care should be taken not to misclassify leukemic cells with false negativity of surface CD38; using an antibody reacting with nondaratumumab epitopes is advantageous.
Cíl studie: Použití léčebného ozáření a vysokodávkované chemoterapie je spojeno s rizikem časných i pozdních komplikací. Postižení štítné žlázy je známým problémem doprovázejícím alogenní transplantaci kmenových buněk krvetvorby (HSCT – hematopoietic stem cell transplantation). Cílem této práce je zhodnotit výskyt a projevy postižení štítné žlázy u pacientů, kteří v našem centru podstoupili alogenní transplantaci v dětském věku a adolescenci (Transplantační jednotka kostní dřeně, Klinika dětské hematologie a onkologie, Fakultní nemocnice Motol, Praha). Pacienti a metody: Jedná se o retrospektivní analýzu dat pacientů, kteří podstoupili HSCT v dětském věku a v adolescenci a následně u nich bylo diagnostikováno postižení štítné žlázy. Funkci štítné žlázy jsme hodnotili klinicky a laboratorně v době před transplantací, šest měsíců a každý rok po transplantaci. Sonografické vyšetření bylo prováděno u pacientů s prokázanou tyreopatií a preventivně pak v delším časovém odstupu po transplantaci (více než pět let, zejména u skupiny ozářených pacientů). Zhodnotili jsme data 463 pacientů (295 mužů, 168 žen), transplantovaných v mediánu věku 7,8 roku (rozmezí 0,1–20,5) v letech 1989 až 2018. V době hodnocení přežíval každý z nich nejméně jeden rok po transplantaci. Pro maligní onemocnění bylo léčeno 301 pacientů (65 %). Jedna třetina pacientů měla jako součást předtransplantační přípravy celotělové ozáření (CTO), přípravný režim zahrnující použití Busulfanu byl uplatněn u téměř 50 % pacientů. Nepříbuzného dárce mělo 327 pacientů, u 33,4 % byl dárcem shodný sourozenec nebo jiný rodinný dárce. Výsledky: Postižení štítné žlázy bylo diagnostikováno u 169 pacientů (36,5 % z celkového počtu) v mediánu dva roky (rozmezí: 0,4–21,3 roku) po HSCT. Primární hypotyreóza byla nejčastějším postižením (110 pacientů, 65,1 %), klinické příznaky mělo pouze devět pacientů. Zvýšené hladiny autoprotilátek (aTPO, aTG) byly detekovány u 95 pacientů. Uzly štítné žlázy byly v rámci ultrazvukového vyšetření nalezeny u 28 pacientů, v mediánu 11,5 roku (5,1–23,4) po transplantaci, 14 se již léčilo pro poruchu funkce štítné žlázy (hypotyreóza = 8, autoimunitní onemocnění štítné žlázy – AITD = 6). U osmi pacientů (1,7 % z celkového počtu, 28,5 % pacientů s uzlem) byl diagnostikován karcinom štítné žlázy v mediánu 10,8 roku (rozmezí 5,4–21,5 let) po transplantaci, sedm pacientů z osmi mělo přípravný režim s CTO. Závěry: Alogenní transplantace kmenových buněk krvetvorby je léčebnou modalitou pro řadu maligních i nemaligních onemocnění. Se stále se zlepšujícími výsledky léčby je nyní pozornost věnována také oblasti pozdních následků léčby u dlouhodobě přežívajících pacientů. Postižení štítné žlázy je jednou z nejčastějších komplikací. Je proto nezbytné pravidelně (po 6–12 měsících) kontrolovat a vyšetřovat funkci štítné žlázy klinicky a zejména laboratorními parametry. Riziko sekundárních malignit těchto pacientů roste v čase. Pečlivé sledování zahrnující ultrazvukové vyšetření (za 1–3 roky) je velmi důležité pro časnou detekci nádorů, zejména u pacientů po celotělovém záření.
Radiotherapy and high-dose chemotherapy can cause a lot of early and late effects. Thyroid dysfunction is a known late complication following HSCT (hematopoietic stem cell transplantation). The objective of this study was to analyze the occurrence and characteristics of thyroid dysfunction in patients who have undergone HSCT in childhood and adolescence at our center (HSCT Unit, Department of Pediatric Hematology and Oncology, University Hospital Motol, Prague). Patients and methods: A retrospective review of clinical records was conducted for patients who had undergone allogeneic HSCT in childhood and adolescence, and subsequently were diagnosed with thyroid involvement. Thyroid gland function was evaluated in regular terms (pre-HSCT, six months, one year and then annually after HSCT). Ultrasound examination was performed in patients with thyroid gland involvement and as a screening. We have evaluated data in 463 patients (295 male, 168 female) who underwent allogeneic transplantation at a median age of 7.8 years (range: 0.1–20.5) from 1989 till 2018 and were alive more than one year after transplantation at the time of evaluation. Three hundred one patient (65%) were treated for malignant disease. In one‑third of patients who underwent HSCT, total body irradiation (TBI) conditioning regimen was used, almost 50% of patients underwent Busulphan based condition regimen. Three hundred twenty-seven transplantations (66.6%) were performed from an unrelated donor, 33.4% from a matched sibling or other family donor. Results: One hundred sixty-nine patients (36.5%) were diagnosed with thyroid gland involvement at a median of 2.0 (range: 0.4–21.3) years after HSCT. Primary hypothyroidism was the most common type of thyroid dysfunction (110 patients – 65.1%), only nine of them were with clinical signs. Thyroid anti-bodies (aTPO, aTG) were detected in 95 patients. Thyroid nodules were found during ultrasound examination in 29 patients at a median of 11.5 (range 5.1–23.4) years after HSCT, fifteen patients were treated due to thyroid dysfunction (hypo = 8, AITD = 7). Thyroid carcinoma was diagnosed in 8 patients (in 1.7% of all patients, 27.6% of patients with nodules) at a median of 10.8 (range 5.4–21.5) years after HSCT. All but one had received TBI based conditioning regimen. Conclusions: Allogeneic hematopoietic stem cell transplantation is a potentially curative therapy for a variety of malignant and non-malignant disorders. With the improved outcome, increasing attention has been drawn to late complications in long-term survivors. Thyroid dysfunction is one of the most frequent complications. Regular evaluation of thyroid gland function (every 6–12 months) including laboratory parameters is highly recommended. The risk of secondary malignancies after HSCT is increasing within time. Careful follow-up of thyroid status including annual ultrasound examination every 1–3 years, is very important for early detection of tumor, namely in all patients exposed to TBI.

Cíl studie: Chronická granulomatózní nemoc (CGD) je vzácná primární imunodeficience s X-vázanou nebo autosomálně recesivní dědičností způsobená defekty v genech kódujících podjednotky fagocytárních oxidáz z komplexu nikotinamid adenin dinukleotid fosfát (NADPH) oxidázy. Tento komplex je v buňce zodpovědný za respirační vzplanutí, což vede k usmrcení mikrobů požitých fagocytovými buňkami. NADPH oxidáza také hraje klíčovou roli v regulaci imunity, a proto je CGD též charakterizována autoimunitními a autoinflamačními projevy. Transplantace hematopoetických kmenových buněk (HSCT) je jedinou definitivní kurativní léčebnou metodou. Uvádíme zde výsledky pacientů s CGD, kteří podstoupili v letech 2007–2019 alogenní HSCT v dětských transplantačních centrech v Praze a Bratislavě. Metodika: Čtrnáct pacientů s chronickou granulomatózou podstoupilo první transplantaci hematopoetických kmenových buněk v mediánu věku 6,4 let (1,0–17,8). Genová mutace genu CYBB (X-vázaná dědičnost) byla identifikována u třinácti z nich. Zdrojem kmenových buněk byla kostní dřeň (n = 10) nebo periferní kmenové buňky (n = 4) HLA identického nepříbuzného dárce (n = 13) nebo zdravého HLA identického sourozence (n = 1). Výsledky: U všech 14 pacientů po transplantaci došlo k primárnímu přihojení štěpu. Pět pacientů, kteří později odhojili primární štěp, úspěšně podstoupili druhou transplantaci. Jeden pacient zemřel šest měsíců po HSCT v důsledku cerebrálního krvácení po opakovaném chirurgickém zákroku pro aspergilom mozku, u jiného pacienta byla diagnostikována sekundární akutní myeloidní leukémie osm let po HSCT a před 12 měsíci podstoupil druhou transplantaci. 13/14 pacientů žije s mediánem sledování 6,1 (0,1–12,2) let po poslední HSCT. Závěry: Alogenní HSCT byly provedeny bez závažnějších komplikací souvisejících s transplantací. Klíčovým bodem této terapie zůstává včasná indikace, stupeň postižení orgánů před transplantací a výběr vhodných dárců a přípravných režimů s cílem snížit riziko pozdních odhojení štěpů a potřebu re-transplantací.
Objective: Chronic Granulomatous Disease (CGD) is a rare primary immunodeficiency with X-linked or autosomal recessive inheritance caused by defects in the genes encoding phagocyte oxidase subunits of the nicotinamide adenine dinucleotide phosphate (NADPH) oxidase complex. This complex is responsible for the respiratory burst inside the cell, which leads to the killing of microbes ingested by phagocytic cells. NADPH oxidase also plays key role in regulating immunity and therefore CGD is also characterized by autoimmune and autoinflammatory manifestations. Hematopoietic stem cell transplant (HSCT) is the only definitive curative treatment. Here we present the outcome of patients with CGD who from 2007 till 2018 underwent allogeneic HSCT at pediatric transplant centers in Prague and Bratislava. Methods: Fourteen patients with chronic granulomatous disease received first hematopoietic stem cell transplant at a medium age of 6.4 years (1.0-17.8). CYBB gene mutation (X-linked) was identified in thirteen of them. The stem cell source was bone marrow (n=10) or peripheral stem cells (n=4) from a human leukocyte antigen identical unrelated (n=13) or healthy sibling (n=1) donor. Results: Primary engraftment was obtained in all 14 patients. Five patients, who later lost the primary graft, were successfully re-grafted following second HSCT. One patient died six months after HSCT due to cerebral bleeding following repeated surgery for cerebral aspergilloma, one developed secondary acute myeloid leukemia eight years after HSCT and 12 months ago underwent second transplant. 13/14 patients are alive with median follow-up 6.1 (range 0.1-12.2) years after the last HSCT. Conclusions: Allogeneic HSCT were safely performed without serious transplant-related complications. Early indication, pre-transplant organ involvement, donor selection and conditioning regimens remain the key points with the aim to decrease the risk of late graft failure and need for re-HSCT.

Cíl studie: Cílem práce bylo zhodnotit současný přístup k dětským pacientům s Fanconiho anémií a selháním krvetvorby z pohledu indikace k transplantaci kmenových buněk krvetvorby. Fanconiho anémie (FA) je vzácný geneticky a klinicky heterogenní syndrom s významně zvýšenou predispozicí k rozvoji selhání kostní dřeně nebo malignity (leukémie, solidní nádory). FA je charakterizována přítomností somatických anomálií, které se vyskytují u přibližně 75–80 % postižených jedinců a zahrnují jeden nebo více z následujících stavů: malý vzrůst, abnormální pigmentace kůže, malformace skeletu horních či dolních končetin, mikrocefalie a anomálie zrakového a močopohlavního ústrojí. Kumulativní riziko rozvoje selhání krvetvorby je 50 %. Progresivní selhání krvetvorby se typicky projevuje v první dekádě věku, často zpočátku s trombocytopenií nebo leukopenií. Leukémie a karcinomy se objevují již ve věku cca 16 let. Diagnóza FA je stanovena na základě funkčního testu zvýšené fragility chromosomů bez stimulace či po stimulaci lymfocytů diepoxybutanem (DEB) nebo mitomycinem C (MMC). Diagnóza je potvrzena průkazem mutace některého z genů Fanconiho komplexu. Metody: Náš soubor zahrnuje 13 dětí (7 chlapců a 6 dívek) diagnostikovaných s FA ve věku 0–11,4 let (medián 5,8) v období 2005–2016. Prvními příznaky byla přítomnost nápadných vrozených vývojových anomálií (n = 4) nebo cytopenie (n = 9). Genetické vyšetření potvrdilo mutace genů ve skupině FANCA u osmi, FANCG u tří, FANCD2 u jednoho, u jednoho retrospektivní vyšetření probíhá. Indikací k transplantaci kmenových buněk krvetvorby (HSCT) bylo selhání krvetvorby diagnostikované v mediánu věku 6,4 roku (3,2–11,4). HSCT proběhla ve věku 4,6–15,1 let (medián 8,3). Zdrojem kmenových buněk byla kostní dřeň (n = 8), periferní kmenové buňky (n = 3) nebo pupečníková krev (n = 2) HLA identického nepříbuzného dárce (n = 11) nebo zdravého HLA identického sourozence (n = 2). Výsledky: U všech pacientů došlo po HSCT ke stabilnímu přihojení krvetvorby. Nedostatečná rekonstituce imunity byla příčinou úmrtí dvou pacientů, kteří zemřeli na CMV pneumonii 2,5 a 13 měsíců po transplantaci. U dalších dvou pacientů jsme diagnostikovali karcinom 7 a 10 let po transplantaci (u obou ve věku 15 let). Věk jedenácti žijících pacientů v době posledního sledování je 12,1–24,4 let (medián 19,4). V mediánu sledování 10 let žije celkem 11/13 pacientů (OS 84,6 %) 4,8–15,2 let po transplantaci. Závěry: HSCT je u pacientů s FA náročný léčebný zákrok. Při zohlednění biologie nemoci v přípravném režimu a soustředění pacientů do center s velkou transplantační zkušeností ale dosahuje velmi dobrých výsledků. Léčí selhání kostní dřeně a je prevencí vzniku krevních malignit. Indikaci je potřeba pečlivě vážit, protože HSCT je významná zátěž pro organismus a spekuluje se o tom, zda její provedení později nevede k dalšímu zvýšení rizika rozvoje nehematologických malignit. U indikovaných pacientů ale HSCT vede k významnému prodloužení života. Celoživotní multidisciplinární dispenzarizace všech pacientů s FA je nezbytná pro včasný záchyt selhání krvetvorby či maligního onemocnění. Preventivní opatření zahrnují minimalizaci radiační zátěže a styku se škodlivými látkami (včetně kouření). Za účelem snížení rizika gynekologického karcinomu u žen a rakoviny ústní dutiny u všech jedinců se doporučuje očkování proti lidskému papilomaviru (HPV).
Objective: The aim of the study was to evaluate the current approach to pediatric patients with Fanconi anemia and bone marrow failure from the perspective of the indication for hematopoietic stem cell transplantation. Fanconi anemia (FA) is a rare genetically and clinically heterogeneous syndrome with a significantly increased predisposition to developing bone marrow failure or malignancy (leukemia, solid tumors). FA is characterized by the presence of somatic anomalies occurring in approximately 75–80% of affected individuals which include one or more of the following conditions: small stature, abnormal skin pigmentation, upper or lower limb skeletal malformation, microcephaly and visual and urogenital anomaly. The cumulative risk of developing bone marrow failure is 50%. Progressive bone marrow failure typically occurs in the first decade of age, often initially with thrombocytopenia or leukopenia. Leukemia and carcinomas occur as early as in 16 years of age. Diagnosis of FA is based on functional test of increased chromosome fragility without or with stimulation of lymphocytes with diepoxybutane (DEB) or mitomycin C (MMC). The diagnosis is confirmed by detection of a mutation in one of the genes of the Fanconi complex. Methods: Our cohort includes 13 children (7 boys and 6 girls) diagnosed with FA aged 0–11.4 years (median 5.8) between 2005 and 2016. The first symptoms were the presence of obvious congenital anomalies (n=4) or cytopenia (n=9). Genetic examination confirmed gene mutations in FANCA group in eight, FANCG in three, FANCD2 in one, in one retrospective examination is pending. Indication for hematopoietic stem cell transplantation (HSCT) was bone marrow failure diagnosed at a median age of 6.4 years (3.2–11.4). HSCT was performed at the age of 4.6–15.1 years (median 8.3). The stem cell source was bone marrow (n=8), peripheral stem cells (n=3) or cord blood (n=2) of an HLA identical unrelated donor (n=11) or a healthy HLA identical sibling (n=2). Results: All patients achieved stable hematopoietic engraftment after HSCT. Inadequate immune reconstitution was a contributing cause of death in two patients who died of CMV pneumonia 2.5 and 13 months after transplantation. In the other two patients we have diagnosed cancer 7 and 10 years after transplantation (both aged 15 years). The age of eleven living patients at the time of the last follow-up is 12.1–24.4 years (median 19.4). A total of 11/13 patients (OS 84.6%) are alive in the median follow-up 10 years (now 4.8–15.2 years) after transplantation. Conclusions: HSCT is a demanding medical procedure in patients with FA. Taking into account the biology of the disease for choice of the conditioning regimen and concentration of the patients in centers with a great transplantation experience, it achieves very good results. It is an effective therapy of bone marrow failure and prevents further development of hematological malignancies. However, the indication needs to be considered very carefully, because HSCT is a significant burden on the body and it is speculated that its implementation may impact on increased risk of developing non-haematological malignancies. However, HSCT results in significant prolongation of life in well-indicated patients. A lifelong multidis ciplinary follow-up of all patients with FA is essential for early detection of bone marrow failure or any malignant disease. Preventive measures include minimizing radiation exposure and contact with harmful substances (including smoking). Vaccination against human papillomavirus ((HPV) is recommended to reduce the risk of gynecological cancer in women and oral cancer in all subjects.

Mutations in the Sterile alpha motif domain containing 9 (SAMD9) gene have been described in patients with severe multisystem disorder, MIRAGE syndrome, but also in patients with bone marrow (BM) failure in the absence of other systemic symptoms. The role of hematopoietic stem cell transplantation (HSCT) in the management of the disease is still unclear. Here, we present a patient with a novel mutation in SAMD9 (c.2471 G>A, p.R824Q), manifesting with prominent gastrointestinal tract involvement and immunodeficiency, but without any sign of adrenal insufficiency typical for MIRAGE syndrome. He suffered from severe CMV (cytomegalovirus) infection at 3 months of age, with a delayed development of T lymphocyte functional response against CMV, profound T cell activation, significantly reduced B lymphocyte counts and impaired lymphocyte proliferative response. Cultured T cells displayed slightly lower calcium flux and decreased survival. At the age of 6 months, he developed severe neutropenia requiring G-CSF administration, and despite only mild morphological and immunophenotypical disturbances in the BM, 78% of the BM cells showed monosomy 7 at the age of 18 months. Surprisingly, T cell proliferation after CD3 stimulation and apoptosis of the cells normalized during the follow-up, possibly reflecting the gradual development of monosomy 7. Among other prominent symptoms, he had difficulty swallowing, requiring percutaneous endoscopic gastrostomy (PEG), frequent gastrointestinal infections, and perianal erosions. He suffered from repeated infections and periodic recurring fevers with the elevation of inflammatory markers. At 26 months of age, he underwent HSCT that significantly improved hematological and immunological laboratory parameters. Nevertheless, he continued to suffer from other conditions, and subsequently, he died at day 440 post-transplant due to sepsis. Pathogenicity of this novel SAMD9 mutation was confirmed experimentally. Expression of mutant SAMD9 caused a significant decrease in proliferation and increase in cell death of the transfected cells. Conclusion: We describe a novel SAMD9 mutation in a patient with prominent gastrointestinal and immunological symptoms but without adrenal hypoplasia. Thus, SAMD9 mutations should be considered as cause of enteropathy in pediatric patients. The insufficient therapeutic outcome of transplantation further questions the role of HSCT in the management of patients with SAMD9 mutations and multisystem involvement.
