Aim of the study: Recently appearing diabetes mellitus can be the very first symptom of otherwise asymptomatic pancreatic cancer. Methodology: 1. Formation of study group of cca 100 patients with recently diagnosed atypical (labile) diabetes mellitus. 2.Complex diabetologic laboratory testing, including the determination of diabetogenic factors. 3. Electrogastrography to exclude diabetic gastroparesis as a etiological factor of the lability of diabetes. 4. Endosonography of the pancreas to detect asymptomatic pancreatic tumors. In case of presence of suspect focal changes fine needle aspiration biopsy to obtain cytologically verified diagnosis.
Předmět řešení: Nově se objevující diabetes mellitus může být prvním příznakem jinak asymptomatického karcinomu pankreatu. Metodika: 1. Vytvoření souboru cca 100 pacientů s recentně diagnostikovaným atypickým (labilním) diabetem. 2. Komplexní laboratornídiabetologické vyšetření, včetně stanovení diabetogenních faktorů v séru, k určení typu diabetu. 3. Elekrogastrografické vyšetření k vyloučení diabetické gastroparézy s následnou labilitou diabetu na podkladě dysmotility žaludku s nepravidelnou evakuacíobsahu do střeva. 4. Endosonografické vyšetření pankreatu k detekci asymptomatických tumorů pankreatu. V případě suspektního nálezu provedení tenkojehlové biopsie ke stanovení diagnózy.

Tissue ischemia in chronic pancreatitis is one of the factors influencing oxidative stress; this stress is an important factor of changes leading towards carcinogenesis of the pancreas, in this case. The goal of this study is to predict changes that canlead to cancer of the pancreas in patients with chronic pancreatitis.
Tkáňová ischemie v terénu chronického zánětu patří mezi faktory iniciující oxidativní stres. Ten je významným faktorem změn, které se podílí na karcinogenezi, v daném případě pankreatu. Cílem studie je predikce změn, které u osob s chronickou pankreatitidou mohou vést ke vzniku kacrinomu pankreatu.

OBJECTIVES: Chronic pancreatitis (CP) is a heterogeneous disease defined as chronic inflammatory changes of the pancreatic tissue caused by variety of aetiologies. Oxidative stress accompanying the inflammatory processes has been suggested as an important factor contributing to CP development. The aim of this study was to determine levels of lipid peroxidation products malondialdehyde (MDA) and 4-hydroxynonenal (4-HNE), together with nitrites and the total antioxidant capacity in the plasma of patients with CP and control subjects. DESIGN: One hundred and five patients with chronic pancreatitis and twenty seven healthy controls were included into this study. Levels of MDA and 4-HNE were analyzed using high-performance liquid chromatography. The total antioxidant capacity of plasma against peroxyl radicals was evaluated using chemiluminescent determination. Nitrites were determined using Griess reaction. Biochemical and haematological parameters were measured by standard methods. RESULTS: The plasma levels of both MDA and 4-HNE, together with the plasma levels of nitrites, were significantly higher in CP patients, compared to healthy controls. The total antioxidant capacity did not differ significantly. Biochemical parameters were in the normal range. The MDA and 4-HNE levels correlated positively with the levels of high-density lipoprotein cholesterol. Nitrite levels correlated positively with C-reactive protein, total white blood cells, and triglycerides. CONCLUSION: The significantly increased plasma levels of MDA, 4-HNE, and nitrites indicate that oxidative stress is present in patients with CP and that it may play a role in initiation and maintenance of inflammation within the pancreatic tissue in CP patients.
