PURPOSE: Crohn's disease is a chronic gastrointestinal inflammatory disease with possible extraintestinal symptoms. There are predisposing genetic factors and even monogenic variants of the disorder. One of the possible genetic factors are variants of the DUOX2 gene. The protein product of the DUOX2 gene is a dual oxidase enzyme producing H2O2 in the bowel. Reduced H2O2 levels impact mucosal homeostasis and contribute to the development of inflammatory bowel disease. Thus far, only 19 patients with IBD with the DUOX2 variants have been described. METHODS: Here we present a case report of an adolescent female diagnosed at eleven years of age with IBD that was subsequently reclassified as Crohn's disease. She was treated with immunosuppressants and biological therapy but experienced additional complications. Her peripheral blood lymphocyte DNA was studied using massive parallel sequencing. Detected variants were functionally studied. RESULTS: Whole exome sequencing found two novel DUOX2 gene variants: a de novo variant c.3646C>T; p.R1216W and a maternally inherited variant c.3391G>A; p.A1131T which were initially classified as variants of unknown significance. However, follow-up functional studies demonstrated that both DUOX2 variants led to impaired H2O2 generation, which led to their reclassification to the likely pathogenic class according to the ACMG.net. Therefore, we conclude that these variants are causative for the disease. CONCLUSIONS: Identifying novel variants in patients with Crohn's disease and their families is important for precision medicine approaches and understanding of the pathogenesis of likely "monogenic" rare forms of inflammatory bowel disease.

From the genetics perspective intellectual disability and autism spectrum disorders are a large heterogeneous group of rare diseases. Exomes will be sequenced in about 100 families with sporadic or familial occurrence of affected children. Variants will be evaluated using all inheritance models and criteria for pathogenicity. Variants in validated causal genes will be found and used clinically in the families. For variants in candidate genes, other patients with variants in the genes will be searched in other cohorts worldwide. Their existence, phenotypic similarity and functional studies can lead to validation of new causal genes. New patients with variants in rarely mutated genes contribute to the genotype-phenotype correlation. A database of common Czech benign population variants will be assembled. Analyses of gene panels, exomes and genomes will be compared. Optimisation of extant pipelines and other experience gained in the research setting (informed consent, attitudes towards uncertainty, incidentalome etc.) will help in the implementation of these methods in routine practi...
Mentální retardace a poruchy autistického spektra jsou z hlediska genetiky velkou heterogenní skupinou mnoha vzácných onemocnění. Budou sekvenovány exomy u asi 100 rodin s postiženými dětmi se sporadickým i familiálním výskytem. Varianty budou hodnoceny podle různých modelů dědičnosti a kritérií pro posouzení patogenity. Budou odhaleny varianty v již validovaných kauzálních genech, které budou v rodinách klinicky využity, a varianty v kandidátních genech, u kterých budou hledáni v jiných kohortách ve světě pacienti s variantami v týchž genech. Jejich existence, podobnost fenotypů a funkční studie mohou vést k validaci nových kauzálních genů. Popis nových pacientů s variantami ve vzácně mutovaných genech upřesní korelaci genotyp-fenotyp. Bude sestavena databáze častých českých benigních populačních variant. Bude porovnána analýza panelů genů, exomů a genomů. Rychlý rozvoj klinické genetiky je umožněn především rozvojem molekulárně genetické diagnostiky. Genetická laboratorní vyšetření mají specifika, která jsou dána zejména kontextem vyšetření germinálního genomu a významem výsledku vyšetření nejen pro testovaného pacienta, ale i pro další rodinné příslušníky. Z tohoto důvodu zákon č. 373/2011 Sb., o specifických zdravotních službách, který provádění genetických laboratorních vyšetření v České republice upravuje, výslovně vyžaduje získání písemné formy informovaného souhlasu před zahájením testování. Tento článek podrobně seznamuje čtenáře s procesem získávání informovaného souhlasu v širším rámci genetického poradenství. Informovaný souhlas v případě klinické genetiky slouží nejen pro informaci pacienta (což je jeho primární účel), ale také k informování lékařů jiných specializací, kteří chtějí genetická laboratorní vyšetření indikovat.
Rapid development of clinical genetics was enabled by the advances of molecular genetic laboratory diagnostics. Genetic laboratory testing has unique characteristics, and results of germinal genome testing has consequences not only for the patient but also for his relatives. Genetic laboratory testing in the Czech Republic is governed by the act no. 373/2011, which explicitly states that the testing requires the completion of a written informed consent. This article explains in detail the process of obtaining an informed consent within a broader framework of genetic counselling. An informed consent with genetic laboratory testing not only informs the patient (this being its primary purpose), but can also serve as a lead for physicians of other clinical specialties intending to order genetic laboratory tests.
Vzácná onemocnění v orofaciální oblasti jsou charakteristická populační prevalencí méně než 1 : 2000 obyvatel. Terapie těchto onemocnění je většinou dlouhodobá a je při ní nutná dobrá spolupráce jak rodičů, tak i odborníků napříč různými obory. Neznamená to však, že takový dětský pacient nemůže žít v dospělosti plnohodnotný život s bezproblémovým začleněním do společnosti. Při stanovování komplexního léčebného plánu u pacientů se zvláštními potřebami je třeba brát zřetel na leckdy omezenou možnost spolupráce pacienta, a proto se s výhodou využívá interdisciplinární spolupráce se specializovaným pracovištěm, které může provést ošetření v celkové anestezii aj. Na ošetření se podílí mnoho specialistů, od genetika přes ortodontistu až po maxilofaciálního chirurga. Kazuistické sdělení popisuje průběh ošetření pacienta s Kabuki syndromem na Stomatologické klinice dětí a dospělých 2. LF UK a FN Motol. Ortodontická léčba byla ukončena po 1 roce a 3 měsících s doporučením dalších kontrol a udržení zvýšené hygieny pomocí pravidelných návštěv dentální hygieny. Tento vzácný syndrom má prevalenci 1 : 32 000 a projevuje se souborem různých charakteristických znaků.
Rare diseases in orofacial region are characteristic of population prevalence less than 1 : 2000 inhabitants. Therapy of these diseases is mostly long-term and quality cooperation between parents and medical specialists across many different disciplines is necessary during the therapy. It does not mean that this child patient can not live full-fledged life with smooth integration into society in adulthood. It is necessary to take into account limited possibility of medically compromised patient’s cooperation during devising global therapeutic plan. It is made use of interdisciplinary cooperation with specialized department, which can give medical treatment in general anaesthesia etc. Many specialists take a part in the treatment from geneticist trough orthodontist to maxillofacial surgeon. The case report describes progress of the treatment of medically compromised patient with Kabuki syndrome on the Department of Paediatric Dentistry, Charles University, 2nd Faculty of Medicine and Motol University Hospital, Prague. Orthodontic treatment was finished after 1 year and 3 months with recommendation to keeping proper hygiene by periodic check-up of dental hygiene. This rare syndrome has prevalence 1 : 32 000 and it is manifested by many different characteristics.
