Autozómovo dominantná polycystická choroba obličiek (ADPKD) je genetické ochorenie s vysokou incidenciou, ktoré patrí medzi najčastejšie príčiny renálneho zlyhania. Napriek intenzívnemu vývoju nových liečebných možností zostáva dialýza a transplantácia obličky jedinou efektívnou liečbou konečného štádia ochorenia. V súčasnosti však nastal výrazný progres v pochopení molekulovej patogenézy vzniku ochorenia, a to odhalením úlohy primárneho cília. Zmena dĺžky primárneho cília je spojená s mnohými patologickými procesmi súvisiacimi so vznikom a progresiou ADPKD. Súčasné štúdie jednoznačne potvrdili, že obnovením dĺžky primárneho cília farmakologickou reguláciou možno dosiahnuť zastavenie cystického rastu, prevenciu vzniku fibrózy a zlepšenie funkcie obličiek. Otvára sa tak nová éra tzv. „cilioterapie“, ktorá predstavuje sľubný cieľ pre vývoj nových liečebných možností nielen pre ADPKD, ale aj pre celý rad iných ciliopatií.
Autosomal dominant polycystic kidney disease (ADPKD) is a genetic disease with high incidence, which is among the most common cause of renal failure. Despite the intensive development of new treatment options, dialysis and transplantation of kidney remain the only effective treatment of end-stage renal disease. Currently, however, there was significant progress in understanding the molecular pathogenesis of the disease and its discovery of the role of the primary cilium. The change of length of the primary cilium is associated with many pathological processes related to the development and progression of ADPKD. Recent studies have unequivocally confirmed that the resumption of the primary cilium length by the pharmacological regulation can be achieved the stopping the cystic growth, prevention of fibrosis and improved of the kidney function. Its open a new era so called „ciliotherapy“ , which represents a promising target for the development of new therapeutic option not only for ADPKD, but for a number of other ciliopathies.

Cieľ: Infekcia larvami pásomnice M. vogae spôsobuje fibrogenézu pečene hostiteľa v chronickej fáze ochorenia, ktorá sa nedá potlačiť používanými antihelmintikmi. Cieľom práce bolo overiť, či podávanie praziquantelu so silymarínom ovplyvní tento proces, pričom na hodnotenie sme použili biochemické markeryfibrogenézy. Metódy: Myši (samce, ICR)sa infikovali larvami M. vogae orálne (p.o.) a od 15. do 24. dňa po infekcii (p.i.) sa im podávalo (p.o.) buďantihelmintikum praziquntel (PZQ) samotné alebo v kombinácii s hepatopretektívom silymarínom (SIL). Neliečené infikované myši slúžili ako kontrola. Na biochemické stanovenia sa použilo sérum a pečeň, ktoré boli odobraté myšiam počas troch týždňov po liečbe (p.t.). Výsledky: U myší infikovaných larvami M. vogae sa po liečbe kombináciou PZQ a SIL zistil signifikantný (p < 0,05) vzostup antioxidačnej aktivity v pečeňovom parenchýme a regenerácia hepatocytov, pokles peroxidácie bunkových lipidov a fibrogenézy, ako aj zvýšenie efektívnosti liečby porovnaním s liečbou samotným PZQ. Závery: Zistilo sa, že silymarín výrazne znižuje množstvo reaktívnych foriem kyslíka produkovaných zápalovými bunkami počas infekcie M. vogae v pečeni myší, čím potláča peroxidáciu lipidov, prispieva k regenerácii hepatocytov a pôsobí antifibroticky. Získané výsledky poukazujú na potrebu súčasného a dlhodobého podávania antihelmintika s hepatoprotektívnou a antioxidačne pôsobiacou látkou, akou je napr. silymarín, počas parazitárnej infekcie spôsobujúcej fibrózu pečene.
Design: Tapeworm infection with larvae of Mesocestoides vogae causes hepatic fibrogenesis of the host during the chronic phase of disease, which cannot be eliminated using anthelmintic drugs. We investigated the effect of the anthelmintic drug praziquantel and the hepatoprotective agent silymarin on the efficacy of treatment and biochemical markers of fibrosis and liver damage/regeneration. Methods: Mice (males, ICR) were infected with M. vogae larvae orally (p.o). During days 15-24 post infection (p.i.) they were treated with anthelmintic praziquantel (PZQ) only or together with the hepatoprotective agent silymarin (SIL). Infected-untreated mice served as the control. Biochemical markers were analysed in the serum and livers of the mice collected within three weeks post therapy (p.t.). Results: It was found that in mice infected with M. vogae larvae during the follow-up of therapy with PZQ and SIL, the antioxidative capacity of the liver parenchyma and regeneration of hepatocytes was elevated significantly (p < 0.05), cell lipids peroxidation and fibrogenesis was suppressed and efficacy of the treatment increased in comparison with PZQ treatment alone. Conclusion: It was found that silymarin co-administration markedly reduces the amount of reactive oxygen species produced by inflammatory cells during M. vogae infection in the liver of mice, resulting in suppressed lipid peroxidation. Moreover, it contributes to the regeneration of hepatocytes, acts antifibrotically, and all these effects of SIL could contribute to the significantly enhanced larvicidal effect of PZQ. The obtained results showthat simultaneous and long-term administration of the anthelmintic drug and a hepatoprotective/antioxidant agent such as silymarin are necessary for more effective treatment of parasitic infections causing hepatic fibrosis.

Úvod: V prírodných podmienkach Európy sa životný cyklus pásomnice Echinococcus multilocularis odohráva medzi liškami hrdzavými. Zrelé články pásomnice obsahujúce vajíčka parazita sa v čreve líšok oddelia a sú diseminované do prostredia prostredníctvom trusu zvierat. Týmto spósobom vajíčka móžu kontaminovat' rózne typy potravy ludí, najma lesného ovocia a zeleniny v záhradách v blízkosti lesa a tiež i pitnú vodu zo studní. Eudia sa následné móžu nakazit' požitím kontaminovanej potravy a vody. Popis prípadov: V tejto práci autoři opisuj ú dva zvláštně případy ludskej alveolárnej echinokokózy vzhladom na vzájomne odlišné klinické prejavy a tiež i protilátkovú imunitnú odpověď. Prvý případ je 14-ročné dievča, s expanzívnym parazitárnym ložiskom v pravom laloku pečené. ELISA testy, western blotová analýza séra a histologické vyšetřeme rezov z biopsie po PAS farbení potvrdili přítomnost' alveolárnej echinokokózy Druhý případ je 57-ročná pacientka s primárnou anamnézou ulcus ventriculis perforatum bez akýchkolvek príznakov parazitárneho ochorenia. Peroperačne sa zistila přítomnost' cystických útvarov na pečeni, biopsiou boli odobrane vzorky. Histologické vyšetrenia biopsií a následná molekulárna analýza DNA specifických genových úsekov potvrdili infekciu pásomnicou E. multilocularis. Serologické vyšetrenia (Western blot a Em2+ELISA) séra pacientky před a po operách boli negativné. Závěr: V súšasnosti sa v serologickej diagnosti-ke alveolárnej echinokokózy u ludí využívajú velmi citlivé komerčně dostupné testy ELISA a Western blot. Histologické vyšetřeme biopsií zvyšuje citlivost diagnostiky. Spectrum diagnostických metod na potvrdenie ochorenia móže byť obohatené o molekulárnu analýzu úsekov DNA kódujúcich specifické geny E. multilocularis z biopsií odobratých pri chirurgickom zákroku.
Introduction: The natural life cycle of E. multi-locularis in Europe involves predominantly red foxes as the definitive hosts. The mature segments of parasite containing eggs are dispersed in the environment via the faeces of carnivores. The eggs may contaminate various types of food, including fruits and vegetables collected from gardens or infected meadows and drinking water. Humans become infected by ingestion of contaminated food and water. Case report: In this study we report two unusual human cases of alveolar echinococcosis (AE) with regard to different clinical manifestation. The first case is a 14-year-old female patient having expansive parasitic foci in the right liver lobe. ELISA assay, western blot and histological examinations of the biopsy after PAS staining confirmed the presence of alveolar echinococcosis. The second case is a 57-year-old woman, operated on primarily for ulcus ventriculis perforatum with no symptoms for parasitic infection. The biopsies of the foci from the right liver lobe were taken incidentally, while performing the revision of the abdomen. In Case 2, AE was confirmed only after molecular characterisation of specific DNA fragment along with histological examinations. Western blot analysis and Em2 + ELISA assay were negative before and after operation. Conclusion: Today a highly sensitive enzyme-linked immunosorbent assays as well as a highly specific western blot system are commercially available for the serological diagnosis of human alveolar echinococcosis cases. Histological examination of biopsies can increase sensitivity of diagnosis. In addition, molecular biological tools which allow to detect DNA fragments of specific E. multilocularis genes from surgically resected and bioptic materials complete our diagnostic spectrum of methods.
