Úvod: Náhlé příhody břišní představují naléhavé situace vyžadující od chirurga rychlou a správnou diagnostiku i léčbu. Zahrnují široké spektrum diagnóz a týkají se všech věkových skupin. U nemocných, u nichž náhlé příhody břišní souvisejí s onkologickou diagnózou, je situace často modifikována rozsahem a pokročilostí základního onemocnění. Metody: Metodou retrospektivní studie byl analyzován soubor nemocných přijatých do Masarykova onkologického ústavu s klinickým obrazem náhlé příhody břišní v letech 2011–2017. Výsledky: V sedmiletém období bylo operováno celkem 601 nemocných, ve 43 % případů pro obstrukci pasáže gastrointestinálního traktu, ve 33 % pro známky zánětlivého nitrobřišního postižení a v 17 % pro krvácení. Pouze necelá 4 % ze všech nemocných náhlou příhodou břišní bylo operováno pro akutní appendicitidu nebo cholecystitidu. Závěr: Problematika náhlých příhod břišních u onkologicky nemocných je diametrálně odlišná od běžné populace, a to především zastoupením příčin náhlých příhod břišních, kde dominují obstrukce trávicí trubice.
Introduction: In general, abdominal emergencies are urgent situations that require a prompt and correct diagnosis and treatment. They involve a broad spectrum of diagnoses and can occur in all age groups. The situation is often modified in oncologic patients according to the extent and level of progression of the primary oncological disease. Methods: A retrospective study was conducted to analyze the group of adult patients with abdominal emergencies treated in Masaryk Memorial Cancer Institute between 2011−2017. Results: In total, 601 patients underwent emergency surgery during the 7-year period. The causes included gastrointestinal obstruction (43%), intra-abdominal inflammatory complications (33%) and bleeding (17%). Acute appendicitis or cholecystitis was the cause in only less than 4% of all patients. Conclusion: The problems of acute abdominal emergencies in oncologic patients are substantially different from those in the general population, particularly in terms of the causes where gastrointestinal obstruction is the leading cause.

Úvod: Poškození kůže s hlubšími podkožními strukturami jsou častými nežádoucími účinky radioterapie. Limitují pak možnosti jejího pokračování a léčba následných komplikací je rutinní náplní praxe radiačního onkologa. Většinově se jedná o projevy akutní s perspektivou zhojení v řádech týdnů ad integrum. Méně často, avšak nezřídka s vážnými projevy, se objevuje toxicita chronická v horizontu měsíců, nebo dokonce mnoha let po ozařování, mající podobu postradiačního vředu s potenciálem vzniku sekundární malignity. Stran chirurgie se může jednat o jednu z nejobtížněji léčitelných chronických lézí, kdy je v extrémních případech k jejich vyléčení potřebná extenzivní resekce celé oblasti defektu a hlubších struktur s navazujícím komplexním, plasticko-chirurgickým, výkonem. Kazuistika: Tato kazuistika prezentuje rychle progredující zhoršení chronických postradiačních změn v oblasti levého prsu, k němuž došlo po 33 let od radioterapie, a to po zcela nezávažné traumatizaci kůže. Před tímto zvratem byly u pacientky v rámci dispenzarizace popsány pouze dlouhodobě stacionární změny běžného charakteru. Po vleklém ambulantním ošetřování defektu s neuspokojivými výsledky, kdy pacientka opakovaně odmítala radikální výkon ve smyslu sanační mastektomie, nakonec stav pro rychlou progresi nálezu vyústil v komplexní chirurgický výkon s nutnou spoluprací mammárního, plastického a hrudního chirurga. Závěr: Ačkoli se změny podobného charakteru vzácněji vyskytují i s odstupem mnoha let s podobnou závažností, nenacházíme v literatuře popsán podobně dramatický vývoj a následnou progresi po třech desetiletích od terapie, vyžadující takto rozsáhlou intervenci.
Introduction: Damage of the skin and its underlying structures is a common side effect of radiotherapy. These conditions limit further treatment and dealing with these complications is a routine practice of clinical oncologist. The majority of the complications are immediate, with a perspective of healing ad integrum within a few weeks. Less frequently, but sometimes with severe manifestations, chronic toxicity occurs belatedly after months, or even many years after irradiation, in form of post-radiation ulcer, for instance with potential of secondary malignant transformation. Regarding surgery, it might be one of the most challenging chronic wounds to treat. In extreme cases, extensive resection of the entire affected area is needed, inevitably ending with demanding reconstruction of the resulting defect. Case report: This case report presents a female patient with rapidly progressive post-radiation chest wall defect 33 years after the irradiation, when relatively insignificant skin injury occured. Prior to this sudden deterioration, only long-term, non-progressive changes, without a cutaneous defect, were described during the dispensarisation. After a protracted outpatient treatment with unsatisfactory results, when the patient repeatedly refused mastectomy, the condition inevitably led to the complex surgical procedure with necessary cooperation of breast, plastic and thoracic surgeons. Conclusion: Although changes of the similar severity rarely occur even after many years following the treatment, we haven‘t found such a dramatic change of the patient’s condition three decades after the therapy with urgency of this type of complex, surgical intervention in current literature.

Chirurgická resekce je hlavní účinnou léčebnou modalitou v časných stadiích nemalobuněčného karcinomu plic. Díky pokrokům v chirurgické technice se snížila rizika operačních výkonů, nicméně optimální postup u lokálně pokročilých stavů není vyjasněn. Naděje jsou vkládány do neoadjuvantní imunoterapie.
Surgical resection is a principal treatment modality in the early stages of non-small cell lung cancer. The risks of surgical procedures are decreasing due to advancements in surgical techniques. However, optimal treatment strategy in locally advanced stages is unclear. Neoadjuvant immunotherapy could be a future treatment alternative.
Background: Perioperative chemotherapy is a recommended treatment approach for localised oesophago-gastric junction adenocarcinoma, but not all patients respond to neoadjuvant chemotherapy. Early identification of non-responders and treatment adaptation in the preoperative period could improve outcomes. GastroPET is a national, multicentre phase II trial evaluating a 18FDG-PET/CT-guided preoperative treatment strategy with the R0 resection rate as a primary endpoint. Here, we report on the accuracy of the methodology, the feasibility of the study design and patient safety data after enrolment of the first 63 patients. Methods: Patients with locally advanced oesophago-gastric junction adenocarcinoma (Siewert I - III) stage Ib-IIIc underwent baseline 18FDG-PET/CT scanning and re-evaluation after 14 days of oxaliplatinum-5FU-(docetaxel) chemotherapy. Responders were defined by a ⩾ 35% decrease in tumour FDG standardised uptake value (SUV)average from baseline. Responders continued with the same chemotherapy for 2 to 3 months prior to surgery. PET-non-responders switched to preoperative chemoradiotherapy [weekly carboplatin and paclitaxel with concurrent radiotherapy (45 Gy in 25 fractions)]. Here, we aim to confirm the feasibility of FDG-PET-based response assessment in a multicenter setting and to compare local versus central reading. In addition, we report on the feasibility of the study conduct and patient safety data. Results: A total of 64 patients received baseline and sequential 14-day 18FDG-PET/CT scanning. And, 63 were allocated to the respective treatment arm according to PET-response [35 (56%) responders and 28 (44%) non-responders]. The concordance of local versus central reading of SUV changes was 100%. Until the date of this analysis, 47 patients (28 responders and 19 non-responders) completed surgery. Postoperative complications of grade ⩾ 3 (Common Terminology Criteria for Adverse Events, CTCAE Version 5.0) were reported in five responders (18%; 95% CI: 7.9-36%) and two non-responders (11%; 95% CI: 2.9-31%), with no statistical difference (p = 0.685). One patient in each arm died after surgery, leading to a postoperative in-hospital mortality rate of 4.3% (2/47 patients; 95% CI: 1.2-14%). Conclusion: Local and central FDG-SUV quantification and PET-response assessment showed high concordance. This confirms the accuracy of a PET-response-guided treatment algorithm for locally advanced oesophago-gastric junction cancer in a multicenter setting. Preoperative treatment adaptation revealed feasible and safe for patients.
