Součástí psychiatrických onemocnění jsou některé logopedické diagnózy. K jejich pochopení je nezbytné odlišit řeč, jazyk a sociální komunikaci, jejichž deficity jsou zároveň ukazateli dalších duševních poruch. Narušením vývoje jazyka vznikne vývojová porucha jazyka, v zahraničí čím dál více známá pod zkratkou DLD, v ČR pod termínem vývojová dysfázie. Tato porucha náleží k neurovývojovým poruchám (NVP), které jsou charakteristické silným genetickým pozadím, strukturálními abnormitami v mozku a tendencí přetrvávat celoživotně. Protože mezi společné znaky NVP patří i vzájemné překrývání, DLD vytváří komorbidní spojení s ostatními NVP. Přehledový článek upozorňuje na perzistentně sníženou kvalitu života u jedinců s DLD, a tím na závažně vysokou prevalenci typických psychiatrických onemocnění i sociální patologie u této populace. Zároveň analyzuje hlavní příčinu tohoto komorbidního spojení a v diskusi hledá společné řešení. Z toho důvodu je příspěvek určen klinickým logopedům a pedopsychiatrům, aby nové poznatky uplatnili v klinické praxi a společně se zasadili o systémové řešení – změnou vlastního přístupu a osvětou.
Psychiatric diseases include some logopaedic diagnoses. To understand them, it is necessary to distinguish between speech, language and social communication, deficits of which are also indicators of other mental disorders. Disruption of language development results in developmental language disorder, increasingly known abroad under the abbreviation DLD, and in the Czech Republic under the term "developmental dysphasia". This disorder belongs among neurodevelopmental disorders (NDDs), which are characterised by a strong genetic background, structural abnormalities in the brain, and a tendency to persist throughout life. Because common features of NDDs have a mutual overlap, DLD creates a comorbid connection with other NDDs. This review article draws attention to the persistently reduced quality of life in individuals with DLD, and thus to the seriously high prevalence of typical psychiatric diseases and social pathology within this group of people. At the same time, it analyses the main cause of this comorbid connection and looks for a common solution within the discussion. For this reason, the paper is intended for clinical speech-language pathologists and paediatric psychiatrists, so that they can apply new findings in clinical practice and jointly advocate a systemic solution - by changing their own approach and raising public awareness.
Objectives. The Social Responsiveness Scale (SRS) and the Empathizing/Systemizing Quotient (EQ/SQ) scale are both used for the assessment of individuals with autism spectrum disorder (ASD). This study aimed to investigate the use of both scales to assess another neurodevelopmental disorder, namely developmental dysphasia (DD). Additionally, we examined differences in social, empathetic, and systemizing characteristics between ASD and DD groups. Sample and settings. The authors examined 103 children with (1) ASD (n = 30, mean age 8.4 ± 2.6 years), (2) DD (n = 35, mean age 8.9 ± 2.3 years), and (3) healthy control children (HC; n = 38, mean age 9.2 ± 1.6 years) using the SRS and EQ/SQ assessments. Subjects with additional psychiatric diagnoses, e.g., intellectual disabilities and/or genetic syndromes, were excluded from the study. Statistical analysis. Descriptive statistics, one-way ANOVA, chi-square test, and Kruskal-Wallis test with the Bonferroni correction were used. Hypotheses. The null hypothesis was that there are no significant differences between the ASD, DD and HC subgroups on the Social Responsiveness Scale and on the Empathizing/Systemizing Quotients. Results. There were statistically significant differences on the SRS between the ASD, DD, and HC groups (97.0 vs. 52.0 vs. 29.0, p<0.001). Differences in EQ scores were also significant between the groups in total (17.0 vs. 30.0 vs. 35.0, p<0.001); however, post-hoc tests did not confirm a significant difference between the DD and HC groups. There were no statistically significant differences on the SQ score between the groups. Limitations. The gender imbalance of our subgroups and the relatively small sample size of our study were the main limitations of our study.
Vývojová dysfázie je charakterizována narušením vývoje jazyka. Systémově je zařazena mezi novou kategorii jedenácté revize Mezinárodní klasifikace nemocí s názvem Neurovývojové poruchy. Jejich koncept vznikl na podkladě poznání obdobné etiopatogeneze: silné genetické pozadí vede ke strukturálním změnám mozku. Mezi další společné znaky patří komorbidní charakter se vzájemným překrýváním, tendence přetrvávat a důsledky v sociální, emocionální a akademické oblasti. Poruchy mají významně odlišné publikační indexy, přičemž vývojová dysfázie, přestože patří k poruchám s nejvyšší prevalencí, je stále méně známá. Navíc po dlouhá léta nebyla sjednocená její terminologie a symptomatologie. V letech 2016 a 2017 došlo ke konsenzu anglofonních zemí v terminologii a identifikaci poruchy, upřesňuje se diagnostika, nejméně studií s prokázanými výsledky pochází ze samotné léčby. V ČR dosud neexistovalo standardní opatření pro stanovení diagnózy a navržení intervence. Z toho důvodu předkládáme doporučení, které vychází jednak ze současných zahraničních poznatků a jednak ze současného stavu českého diagnostického materiálu i českých studií a obecně českého prostředí, neboť diagnostika poruchy je vázána na jazykové a kulturní prostředí. Hlavním oborem v procesu diagnostiky a léčby vývojové dysfázie je klinická logopedie. Nezbytným oborem je rovněž dětská neurologie. Příspěvek seznamuje s postupem diagnostiky, diferenciální diagnostiky, doplňujících vyšetření pro suspekce komorbidních chorob, specifikuje víceoborový tým a doporučuje průběh terapie.
Developmental dysphasia is characterized by a disruption in language development. It is systematically classified as a new category of the 11th revision of the International Classification of Diseases called Neurodevelopmental Disorders. Their concept was developed on the basis of the recognition of a similar etiopathogenesis: a strong genetic background leads to structural changes in the brain. Other common features include a comorbid nature with mutual overlap, a tendency to persist and consequences in the social, emotional and academic domains. The disorders have significantly different publication indices, and developmental dysphasia, although one of the disorders with the highest prevalence, is still less well known. Moreover, for many years, its terminology and symptomatology have not been unified. In 2016 and 2017, there has been a consensus among English-speaking countries in terminology and identification of the disorder, the diagnosis is being refined, and the fewest studies with proven results come from the treatment itself. In the Czech Republic, there has not yet been a standard measure for making a diagnosis and proposing an intervention. For this reason, we present recommendations that are based both on current international knowledge and on the current state of Czech diagnostic material and Czech studies and the Czech environment in general, as the diagnosis of the disorder is linked to the linguistic and cultural environment. The main field in the process of diagnosis and treatment of developmental dysphasia is clinical speech therapy. Pediatric neurology is also an indispensable field. This manuscript introduces the process of diagnosis, differential diagnosis, and additional examinations for suspected comorbid conditions, specifies the multidisciplinary team and recommends a course of therapy.
Přehledný referát prezentuje současné poznatky o vývojové dysfázii, méně známé neurovývojové poruše, jejíž prevalenci a závažnost lze srovnat s poruchou s deficitem pozornosti a hyperaktivitou. Seznamuje s důsledky v akademickém, sociálním a emocionálním vývoji, se změnami i zavádějícími chybami v terminologii. Zmiňuje silné genetické pozadí i další etiologické činitele, které vedou ke strukturálním změnám mozku. Na popis klinického obrazu navazuje výčet komorbidních poruch, se kterými se překrývá. Porucha autistického spektra je zmiňována jak v této souvislosti, tak v části o diferenciální diagnostice. Příspěvek přináší komplexnější pohled na možnosti současné diagnostiky a léčby. Část je zpracována podle aktuálního konsenzu mezinárodního fóra v anglofonních zemích.
The review article presents current knowledge of developmental dysphasia, a lesser-known neurodevelopmental disorder, which prevalence and seriousness can be compared to attention deficit / hyperactivity disorder. It acquaints the consequences of the disorder in academic, social, and emotional development, with changes and misleading errors in the terminology. It refers to a strong genetic background and the other etiological factors that lead to structural changes in the brain. The description of the clinical picture is followed by the enumeration of overlapping co-morbid disorders. Autism spectrum disorder is mentioned both in this connection and in the chapter on differential diagnosis. The contribution brings more complex view of diagnostics and treatment. The part is compiled in accordance with a recent consensus from a significant international forum in English speaking countries.

Přehledový článek prezentuje vývojovou dysfázii na konci druhé dekády 21. století. Autorka se tak se čtenáři dělí o své poznatky díky přípravě disertační práce s názvem Vývojová dysfázie v konceptu neurovývojových poruch. Seznamuje je se závažnými důsledky poruchy, vysvětluje současné pojetí terminologie (včetně zavádějící chyby českého překladu), etiopatogeneze, symptomatologie, komorbidit, diagnostiky a zčásti i na ni navazujících intervenčních strategií. Většina článku se opírá o nedávné závěry významného mezinárodního fóra.
This review article presents findings on developmental dysphasia at the end of the second decade of the 21st century. The author shares her knowledge with the reader thanks to the preparation of doctoral thesis called Developmental dysphasia in the concept of neurodevelopmental disorders. This work explains the serious consequences of the disorder, explains the current concept of terminology (including the misleading error of the Czech translation), etiopathogenesis, symptomatology, comorbidities, diagnostics, and in part the related intervention strategies. Most of the article is based on recent conclusions from a major international forum.

Článek se zamýšlí nad vývojovými poruchami řeči, jazyka a komunikace z pohledu konce meziobdobí původních pravidel a nového konceptu. Vysvětluje rozdílnost pojetí dosavadní MKN-10 Světové zdravotnické organizace a před pěti lety vydané DSM-V Americké psychiatrické společnosti; nechává též nahlédnout do připravované verze ICD - 11. Zdůrazňuje, že neurovývojové poruchy, mezi které všechny vývojové poruchy komunikace patří, vyžadují konceptuální přístup, jehož bází jsou současné poznatky z neurobiologie a genetiky. Nové poznatky přinášejí zásadní změny v nazírání, z toho důvodu jsou nezbytnou výzvou pro moderní pojetí našeho oboru. Na závěr se autor článku zamýšlí nad potřebou změny péče o děti s neurovývojovými poruchami v koordinovaný systém s mezirezortním přesahem.
The article discusses developmental speech, language and communication disorders from the point of view of the end of the period between the original rules and the new concept. It explains the difference of concepts of the current ICD-10 World Health Organization and DSM-V American Psychiatric Society issued five years ago and leaves a look into the upcoming version of ICD-11. It emphasizes that neurodevelopmental disorders, among which all developmental communication disorders belong, require a conceptual approach based on current knowledge of neurobiology and genetics. New insights bring fundamental changes in the preview, that is why they are a must for the modern concept of our field. Finally, the author of the article considers the need to change the care of children with neurodevelopmental disorders in a coordinated system with an interdepartmental overlap.
