AIM: To non-invasively identify the hemodynamic changes in critically ill children during the first 48 h following initiation of mechanical ventilation by the ultrasound cardiac output monitor (USCOM) method and compare the data in children with pulmonary and non-pulmonary pathology. MATERIALS AND METHODS: This was a prospective observational study to evaluate the influence of mechanical ventilation on hemodynamic changes and to describe hemodynamic profiles of mechanically ventilated children. A total of 56 children with respiratory failure were included in the present study. Ventilated patients are divided into two groups. Group A (n=36) includes patients with pulmonary pathology. Group B (n=20) consists of patients with extra pulmonary etiology of respiratory failure. Hemodynamic parameters (cardiac index and systemic vascular resistance index) were evaluated using ultrasound cardiac output monitoring (USCOM 1A) immediately following initiation of mechanical ventilation and again at 6, 12, and 48 h. Pharmacological circulatory support (inotropes, vasopressors, levosimendan and phosphodiesterase III inhibitors) was individually and continuously modified based on real-time hemodynamic parameters and optimal fluid balance. RESULTS: No significant differences in hemodynamic profiles were found between Group A and Group B. CONCLUSION: The protective strategy of mechanical ventilation was not associated with significant differences in hemodynamic profiles between children ventilated for pulmonary and non-pulmonary pathologies. CLINICAL SIGNIFICANCE: Hemodynamically unstable children ventilated for pulmonary pathology with the protective strategy of mechanical ventilation had a greater requirement for inotropic and combined inotropic and vasoactive circulatory support than children ventilated for non-pulmonary causes of respiratory failure.

These children are born with female external genitalia, and females are transmitters. CASE REPORT: We illustrate an unexpected diagnosis of CAIS in two siblings during examination for short stature, and describe transmission/carriers in the family along with ethical aspects. CONCLUSION: A genetic examination could have earlier revealed the transmission of c.2495G>Tp.(Arg832Leu) mutation in exon 7. Our experience highlights the possibility of prenatal testing for the management of pregnancy in a family with a history of CAIS. The implications of prenatal testing in relation to CAIS with clearer explication of ethical and clinical issues warrant further investigation.

Cíl: Cílem retrospektivní studie bylo zmapování výskytu monogenního diabetu MODY (Maturity Onset Diabetes of the Young) u pacientů vyšetřených na Dětské klinice FN Plzeň v letech 2000–2017. Do studie bylo zařazeno 16 pacientů s monogenním diabetem, z toho 9 dívek a 7 chlapců průměrného věku 10,2 roku (rozmezí 3–17 let). Metody: U všech pacientů jsme retrospektivně hodnotili zastoupení pohlaví, věk v době diagnózy, familiární výskyt onemocnění, vstupní parametry sacharidového metabolismu (glykémie nalačno, glykovaný hemoglobin HbA1c), orální glukózový toleranční test (oGTT), intravenózní glukózový toleranční test (ivGTT) a specifické autoprotilátky proti β-buňkám pankreatu. Diagnóza MODY diabetu byla potvrzena molekulárně genetickým vyšetřením genů GCK, HNF1A, HNF4A a HNF1B. Výsledky: V hodnoceném souboru byl ve 13 případech potvrzen glukokinázový diabetes (GCK-MODY) a ve 3 případech diabetes transkripčních faktorů (HNF-diabetes) – HNF4A-MODY, HNF1A-MODY a HNF1B-MODY. Pacienti s GCK-MODY měli mírně zvýšenou lačnou glykémii (6,3–7,3 mmol/l) a HbA1c (medián 46 mmol/mol), při oGTT v 94 % poruchu glukózové tolerance, při ivGTT fyziologickou iniciální fázi sekrece inzulinu (FPIR) a negativní specifické autoprotilátky proti β-buňkám pankreatu. V jedné rodině byla potvrzena nejčastěji popisovaná mutace P.Glu40Lys v genu pro glukokinázu v ČR. Pacienti s HNF-diabetem měli diabetický průběh oGTT, sníženou FPIR při ivGTT, medián HbA1c 64 mmol/mol a negativní specifické autoprotilátky. Diagnóza GCK-MODY v rodinách umožnila ukončení léčby inzulinem a perorálními antidiabetiky (PAD) a v případě HNF-diabetu došlo k převedení léčby z inzulinu na PAD. Závěr: U pacientů s poruchou glukózové tolerance nebo s diabetes mellitus je třeba diferenciálně diagnosticky myslet na MODY diabetes. Potvrzení nebo vyloučení genové mutace predikuje vývoj onemocnění a volbu správné terapie. Znalost genové mutace umožňuje záchyt onemocnění v presymptomatické fázi v postižených rodinách.
Monogenic diabetes MODY in childhood: a retrospective study of patients diagnosed at the Department of Pediatrics, University Hospital in Pilsen in 2000–2017 Objective: The aim of the retrospective study was to investigate the incidence of monogenic diabetes MODY (Maturity Onset Diabetes of the Young) in patients examined at the Department of Pediatrics, University Hospital in Pilsen in 2000–2017. 16 patients with monogenic diabetes were included in the study: 9 girls and 7 boys with an average age of 10.2 years (ranging from 3 to 17 years). Methods: The following parameters were retrospectively analysed in all patients: sex, age at the time of diagnosis, familial incidence of the disease, baseline parameters of carbohydrate metabolism (fasting blood glucose, glycated hemoglobin HbA1c), oral glucose tolerance test (oGTT), intravenous glucose tolerance test (ivGTT), and islet cell autoantibodies. The diagnosis of MODY diabetes was confirmed by molecular genetic testing of GCK, HNF1A, HNF4A a HNF1B genes. Results: In the analysed group, 13 cases of GCK-MODY and 3 cases of HNF-diabetes (HNF4A-MODY, HNF1A-MODY, HNF1B-MODY) were confirmed. Patients with GCK-MODY had a slightly elevated fasting blood glucose (6.3–7.3 mmol/l) and HbA1c (median of 46 mmol/mol), impaired glucose tolerance on the oGTT in 94%, physiological first-phase insulin response (FPIR) during ivGTT, and negative islet cell autoantibodies. In one family, the most frequently described mutation in the glucokinase gene p.Glu40Lys in the Czech Republic was confirmed. Patients with HNF-diabetes had a diabetic curve on the oGTT, decreased FPIR during ivGTT, median HbA1c of 64 mmol/mol, and negative islet cell autoantibodies. The diagnosis of GCK-MODY in the families allowed discontinuation of treatment with insulin and oral antidiabetic drugs (OADs) and, in the case of HNF-diabetes, switching from insulin to OADs. Conclusion: The differential diagnosis in patients with impaired glucose tolerance or diabetes mellitus should include MODY diabetes. Confirmation or ruling out of the gene mutation predicts disease progression and proper choice of therapy. Knowledge of the gene mutation enables disease detection in the presymptomatic phase in the affected families.

BACKGROUND: The prevalence of autoimmune thyroiditis (AIT), as the most common autoimmune disease (AD) and papillary thyroid cancer (PTC) is steadily rising in children. The aim of this study was to determine the coexistence of other AD and thyroid carcinoma (TC) in AIT. METHODS: The cross-sectional study conducted at a tertiary center comprised AIT children (< 19 years). Data on age/sex, thyroid function and ultrasound, autoantibodies, associated AD, familial occurence of AD and the occurence of TC for each child were collected. RESULTS: In total, 231 eligible patients (77% females) were included. The most common onset (66%) was during adolescence. At onset, hypothyroidism was detected in 59.3%; hashitoxicosis in 1.3%. The positivity of both autoantibodies was present in 60.6%, the negativity was in 3,5%. We confirmed a high frequency (44.6%) of AD with AIT predominance in parents and/or grandparents of patients and in siblings (7.4%). 15.2% had at least 1 comorbid AD, of which type 1 diabetes mellitus was the most common (8.5%). Over a period of 7 years TC was diagnosed in 16 patients (mean age 13.5 years) with predominance of PTC in 15 (94%) patients. AIT had concurrently 69% patients. 56% of patients had metastases (89% in AIT subjects). An invasive PTC was present in 44% (86% in AIT subjects). CONCLUSIONS: The prevalence rate of AD in AIT and first-degree relatives is high, and several new associations have been reported. Providers should be aware of comorbidities and TC in AIT as this would help in early diagnoses and timely interventions.

Nejčastější formou kongenitální adrenální hyperplazie (CAH) je defekt enzymu 21-hydroxylázy. Mezi vzácné formy vrozené adrenální insuficience patří defekty lokalizované na začátku steroidogeneze v kůře nadledvin, které vedou ke kongenitální lipoidní adrenální hyperplazii (lipoidní CAH) způsobenou defektem steroidogenního akutního regulačního proteinu (StAR protein) nebo P450scc adrenální insuficienci způsobenou defektem enzymu štěpícího postranní řetězec cholesterolu (P450scc). Klasické formy lipoidní CAH a P450scc adrenální insuficience jsou charakterizovány těžkou poruchou steroidogeneze v nadledvinách a gonádách. Všichni novorozenci se rodí s fenotypicky ženským genitálem. Život ohrožující akutní adrenální insuficience se manifestuje časně po narození. Neklasické formy lipoidní CAH a P450scc adrenální insuficience jsou způsobeny částečnou poruchou funkce StAR proteinu a enzymu P450scc. Pacienti se manifestují pozdním nástupem primární adrenální insuficience a neúplnou virilizací zevního genitálu u mužského pohlaví (mikropenis, retence testes, hypospadie). Popisujeme případ dítěte s vrozenou adrenální insuficiencí způsobenou defektem enzymu P450scc. Pacientka se manifestovala po narození rozvojem kardiopulmonální nestability, hyponatremií, hyperkalemií, hypoglykemií a metabolickou acidózou. Zevní genitál byl ženský, bez známek virilizace. Substituční terapie glukokortikoidy a mineralokortikoidy vedla k normalizaci klinického stavu a parametrů vnitřního prostředí. Ve 12 letech věku jsme odhalili vývoj hypergonadotropního hypogonadismu svědčící pro poruchu steroidogeneze v nadledvinách i v gonádách. Genetické vyšetření potvrdilo mutaci v CYP11A1 genu vedoucí k poruše funkce enzymu štěpícího postranní řetězec cholesterolu. Pacientka je složený heterozygot pro novou CYP11A1 missense mutaci c.412G>A (p.Gly138Arg) v exonu 2 a frameshift mutaci c.508_509delCT (p.Leu170Valfs*30) v exonu 3. P450scc adrenální insuficience způsobená mutací v CYP11A1 genu byla dosud popsána pouze u 29 pacientů. V České republice se jedná o první případ tohoto onemocnění.
The most frequent form of congenital adrenal hyperplasia (CAH) is 21-hydroxylase deficiency. The uncommon forms of CAH are defects at the beginning of steroidogenesis in the adrenal glands leading to either congenital lipoid adrenal hyperplasia (CLAH) caused by defect of steroidogenic acute regulatory protein (StAR protein) or P450scc deficiency caused by defect of cholesterol side-chain cleavage enzyme (P450scc). A classic form of both CLAH and P450scc deficiency is characterized by a severe disorder of steroidogenesis in the adrenal glands and in the gonads. All newborns have female external genitalia. The life-threatening adrenal insufficiency is manifested early after birth. In the nonclassic forms of CLAH and P450scc deficiency is a partial loss of function of StAR protein or P450scc. The patients present with late onset of primary adrenal insufficiency and undervirilised external genitalia in male patients (micropenis, cryptorchidism, hypospadias). We characterized a case of congenital adrenal insufficiency caused by P450scc deficiency. The patient presented after birth with cardiopulmonary instability, hyponatremia, hyperkalemia, hypoglycemia and metabolic acidosis. There were no signs of the external genitalia virilism. The replacement therapy with glucocorticoids and mineralocorticoids led to normal laboratory. At the age of 12 years, we confirmed hypergonadotropic hypogonadism, which revealed disorder of steroidogenesis in the adrenal glands and in the gonads. The genetic testing confirmed mutation in the CYP11A1 gene leading to deficiency of cholesterol side-chain cleavage enzyme. The patient is compound heterozygote for the novel CYP11A1 missense mutation c.412G>A (p.Gly138Arg) in exon 2 and frameshift mutation c.508_509delCT (p.Leu170Valfs*30) in exon 3. So far, only 29 patients with CYP11A1 mutations causing P450scc deficiency have been reported worldwide. There are no related reports in the Czech Republic.

Thiamine-responsive megaloblastic anemia (TRMA) is a rare autosomal recessive disorder caused by mutations in the SLC19A2 gene. To date at least 43 mutations have been reported for the gene encoding a plasma membrane thiamine transporter protein (THTR-1). TRMA has been reported in less than 80 cases worldwide. Here, we illustrate 2 female patients with TRMA first diagnosed in the Czech Republic and in central Europe being confirmed by sequencing of the THTR-1 gene SLC19A2. Both subjects are compound heterozygotes with 3 different mutations in the SLC19A2 gene. In case 2, the SLC19A2 intron 1 mutation c.204+2T>G has never been reported before. TRMA subjects are at risk of diabetic ketoacidosis during intercurrent disease and arrythmias. Thiamine supplementation has prevented hematological disorders over a few years in both pediatric subjects, and improved glycaemic control of diabetes mellitus. Patient 1 was suffering from hearing loss and rod-cone dystrophy at the time of diagnosis, however, she was unresponsive to thiamine substitution. Our patient 2 developed the hearing loss despite the early thiamine substitution, however no visual disorder had developed. The novel mutation described here extends the list of SLC19A2 mutations causing TRMA.

Úvod: Kardiopulmonální interakce ovlivňují srdeční výdej v průběhu umělé plicní ventilace. Není publikován dostatek dat týkajících se studia těchto interakcí u kriticky nemocných dětí. Metoda: Provedli jsme prospektivní observační studii hodnotící vliv umělé plicní ventilace na hemodynamické změny pomocí metody sonografického monitoringu srdečního výdeje (USCOM). Do studie bylo zařazeno celkem 56 dětí ventilovaných protektivní umělou plicní ventilací: 36 dětí bylo ventilováno pro plicní a 20 dětí pro mimoplicní patologii. Oběhové parametry (srdeční index, index systémové vaskulární rezistence) byly změřeny po zahájení umělé plicní ventilace a dále v intervalech 6, 12 a 48 hodin od zahájení umělé plicní ventilace. Medikamentózní podpora oběhu byla indikována na základě výsledků získaných metodou USCOM. Byla monitorována tekutinová bilance. Výsledky: Mezi skupinami nebyly nalezeny významné rozdíly v hemodynamických profilech. Děti ventilované pro plicní patologii měly tendenci vyžadovat častěji inotropní a kombinovanou inotropně/vazopresorickou podporu ve srovnání s dětmi ventilovanými pro mimoplicní patologii. Děti s mimoplicní patologií častěji vyžadovaly vazopresorickou podporu po zahájení umělé plicní ventilace. Rozdíl mezi skupinami nebyl statisticky významný. Obě studované skupiny vyžadovaly srovnatelné množství tekutin. Závěr: Protektivní strategie umělé plicní ventilace nebyla spojena se signifikantními rozdíly v hemodynamických profilech mezi pacienty ventilovanými pro plicní a mimoplicní patologii. U pacientů ventilovaných pro plicní patologii byl pozorován trend k častější potřebě inotropní a kombinované inotropně/vazopresorické podpory ve srovnání s pacienty ventilovanými pro mimoplicní patologii.
Introduction: Cardiopulmonary interactions influence cardiac output during mechanical ventilation. There is limited prospective data investigating these interactions in critically ill pediatric patients. Methods: We performed a prospective observational study to evaluate the influence of mechanical ventilation strategy on hemodynamic changes using ultrasound cardiac output monitoring (USCOM). This study was conducted with 56 children on protective mechanical ventilation for pulmonary (36 children) and non-pulmonary (20 children) pathology. Circulatory parameters (e.g., cardiac index and systemic vascular resistance index) were evaluated after initiation of mechanical ventilation and at 6, 12 and 48-hour intervals. The circulatory support therapy was indicated based on USCOM measurement results. Fluid balance was monitored. Results: No significant differences between the groups’ hemodynamic profiles were found. Children ventilated for pulmonary pathology tended to require more frequent inotropic and combined inotropic/vasopressor circulatory support than did the children ventilated for non-pulmonary pathology. Children ventilated for non-pulmonary pathology tended to require more frequent vasopressor circulatory support after the initiation of mechanical ventilation. Both groups required a similar amount of fluid during the study. Conclusion: The protective strategy of mechanical ventilation was not associated with significant differences in hemodynamic profiles between patients with pulmonary and non-pulmonary pathologies. Although not statistically significant, a trend was observed toward a more frequent requirement for inotropic and combined inotropic/vasopressor therapy in children ventilated for pulmonary pathology, compared to those ventilated for non-pulmonary pathology.
