Heteropágy alebo parazitické zrastené dvojčatá predstavujú extrémne zriedkavú anomáliu, ktorá sa vyskytuje približne v 1 z 1 milióna prípadov. Sú charakterizované prítomnosťou parazitického dvojčaťa s významnou vrodenou abnormalitou, pripojeného k zvyčajne inak zdravému plodu. Dobre vyvinuté dvojča je známe ako „autosite“ alebo „hostiteľ“, zatiaľ čo ťažko postihnutý plod sa nazýva „parazit“. Prežitie defektného dvojčaťa závisí od kardiovaskulárneho systému druhého, relatívne normálneho plodu. Predstavujeme prípad 27-ročnej primigravidy v 14. týždni gravidity s ultrazvukovým nálezom poukazujúcim na parazitické zrastené dvojčatá, omfalopágy.
Heteropagus or parasitic conjoined twins represent an extremely rare anomaly, occurring in approximately 1 in 1 million cases. This condition is characterized by the presence of a parasitic twin with significant congenital abnormalities attached to an otherwise typically healthy fetus. The well-developed twin is known as the “autosite” or “host,” while the severely affected fetus is termed the “parasite.” Survival of the defective twin depends on the cardiovascular system of the second, relatively normal fetus. We present the case of a 27-year-old primigravida in her 14th week of pregnancy with ultrasound findings indicating parasitic conjoined twins, specifically omphalopagus.

Cieľ: Prezentácia raritného prenatálneho nálezu spojených dvojčiat. Kazuistika: Secundigravida nullipara, 28-ročná, bola prijatá pre profúzne krvácanie v 13. týždni gravidity. Na ultrazvukovom vyšetrení bola verifikovaná vitálna gravidita – gemini thoracopagus. Pacientka sa po konzultácii prenatálnej diagnostiky rozhodla pre ukončenie gravidity. Molekulová analýza DNA preukázala plod ženského pohlavia bez chromozómových anomálií. Záver: Incidencia zrastených dvojčiat je veľmi raritná. Dôležitú úlohu zohráva včasná prenatálna ultrazvuková diagnostika. Kazuistika prezentuje dvojičky, ktorých prognóza je veľmi nepriaznivá, nakoľko chirurgická separácia by bola nemožná z dôvodu zdieľania spoločného srdca.
Objective: We present a case report of conjoined twins. Case report: Secundigravida nullipara, 28-years old, admitted for profuse bleeding at 13 weeks of gestation. Ultrasound confirmed vital pregnancy of conjoined twins – thoracopagus. After prenatal diagnostic consultation the patient decided for termination of pregnancy. Molecular analysis confirmed a female fetus without any chromosomal anomalies. Conclusion: The occurrancce of conjoined twins is very rare. Early prenatal ultrasound diagnosis plays an important role. Presented case report describes conjoined twins with poor prognosis because of one shared malformed heart.

Cíl: Popis případu a autoptický nález u srostlice diagnostikované ve 24. týdnu gravidity. Výsledky: Prezentujeme případ 31leté ženy, primigravidy, odeslané na gynekologicko-porodnické oddělení naší nemocnice ke konziliárnímu vyšetření pro podezření na spojená dvojčata. Ultrasonografické vyšetření s následnou třídimenzionální (3D) rekonstrukcí obrazu potvrdilo diagnózu srostlice spojené v oblasti hlavy, hrudníku, horní části břicha a pupečníku (cefalothorakoomfalopagus). Pro vícečetné vady a morfologické abnormality neslučitelné se životem bylo těhotenství ukončeno a srostlice byla následně podrobena autoptickému vyšetření na patologicko-anatomickém oddělení. Pitva potvrdila původní ultrasonografickou diagnózu. Závěr: Cefalothorakoomfalopagus je jedna ze vzácně se vyskytujících forem spojených dvojčat, jejíž přesná incidence není pro malý počet zdokumentovaných případů známa.
Objective: Case description and autopsy fi nding in conjoined twins diagnosed in the 24th week of pregnancy. Results: We report a case of a 31-year-old primigravida who was referred to the Department of Gynecology and Obstetrics at our hospital with a presumptive dia gnosis of conjoined twins. The ultrasound examination with subsequent three-dimensional (3D) image reconstruction demonstrated twin gestation complicated by cephalothoracoomphalopagus. Observations demonstrated that the twins were joined over an area that extended from the head to the thoraces down to the central abdomen. In view of multiple congenital malformations incompatible with postnatal life, the pregnancy was terminated. The twins then underwent an autopsy at the Department of Pathology and the autopsy confi rmed previous diagnosis. Conclusion: Cephalothoracoomphalopagus is one of the rarest forms of conjoined twins with unknown incidence due to a very small number of documented cases.
