Makroenzym aspartátaminotransferáza (dále makro‑AST) je jednou z příčin izolovaného zvýšení aspartátaminotransferázy (AST) v séru. Uvádíme kazuistiku normálně prospívající dívky s náhodně zachycenou přetrvávající izolovanou elevací AST. Podrobným vyšetřením bylo zjištěno, že se jedná o elevaci sérové AST na podkladě cirkulujících makro-AST bez přítomnosti dalšího organického onemocnění.
The macroenzyme of aspartate aminotransferase (macro-AST) is one of the causes of the isolated increase in serum aspartate aminotransferase (AST). We present a case report of a well-thriving girl with an accidentally detected persistent isolated AST elevation. A detailed laboratory analysis revealed this was due to an elevation of serum AST based on circulating macro-AST without the presence of another organic disease.

Nonalcoholic steatohepatitis (NASH) is characterized by hepatic steatosis with inflammation and fibrosis. Membrane endoglin (Eng) expression is shown to participate in fibrosis, and plasma concentrations of soluble endoglin (sEng) are increased in patients with hypercholesterolemia and type 2 diabetes mellitus. We hypothesize that NASH increases both hepatic Eng expression and sEng in blood and that high levels of sEng modulate cholesterol and bile acid (BA) metabolism and affect NASH progression. Three-month-old transgenic male mice overexpressing human sEng and their wild type littermates are fed for six months with either a high-saturated fat, high-fructose high-cholesterol (FFC) diet or a chow diet. Evaluation of NASH, Liquid chromatography-mass spectrometry (LC/MS) analysis of BA, hepatic expression of Eng, inflammation, fibrosis markers, enzymes and transporters involved in hepatic cholesterol and BA metabolism are assessed using Real-Time Quantitative Reverse Transcription Polymerase Chain reaction (qRT-PCR) and Western blot. The FFC diet significantly increases mouse sEng levels and increases hepatic expression of Eng. High levels of human sEng results in increased hepatic deposition of cholesterol due to reduced conversion into BA, as well as redirects the metabolism of triglycerides (TAG) to its accumulation in the liver, via reduced TAG elimination by β-oxidation combined with reduced hepatic efflux. We propose that sEng might be a biomarker of NASH development, and the presence of high levels of sEng might support NASH aggravation by impairing the essential defensive mechanism protecting NASH liver against excessive TAG and cholesterol accumulation, suggesting the importance of high sEng levels in patients prone to develop NASH.

Stanovení sérové aktivity aspartátaminotransferázy (AST) patří k základním biochemickým vyšetřením. AST se v ojedinělých případech váže na sérové imunoglobuliny a tvoří makroenzym zvaný makro AST, který se vyznačuje vysokou molekulovou hmotností a delším biologickým poločasem, což způsobuje jeho dlouhodobě zvýšenou sérovou aktivitu bez klinické příčiny. Pacienti s neobjasněnou elevací enzymu v séru bývají často v rámci diferenciální diagnostiky podrobně vyšetřováni včetně invazivních a finančně náročných vyšetření. Včasný záchyt makrokomplexových forem enzymů pomocí precipitace imunoglobulinů v séru či plazmě s polyethylenglykolem může těmto náročným vyšetřením zabránit.
Measurement of serum aspartate aminotransferase (AST, SGOT, serum glutamic-oxaloacetic transaminase) activity is one of the most common biochemical tests. In rare cases AST binds to immunoglobulins and forms macroenzyme called macro AST, which has high molecular weight and longer biological half-time. This causes persistent elevation of AST activity without clinical correlate. Patients with unexplained serum AST elevation usually undergo detailed investigation including invasive and costly procedures. Early identification of macroenzymes by serum or plasma precipitation with polyethylene glycol could reduce the number of costly and invasive investigations.

OBJECTIVE: To develop response criteria for adult dermatomyositis (DM) and polymyositis (PM). METHODS: Expert surveys, logistic regression, and conjoint analysis were used to develop 287 definitions using core set measures. Myositis experts rated greater improvement among multiple pairwise scenarios in conjoint analysis surveys, where different levels of improvement in 2 core set measures were presented. The PAPRIKA (Potentially All Pairwise Rankings of All Possible Alternatives) method determined the relative weights of core set measures and conjoint analysis definitions. The performance characteristics of the definitions were evaluated on patient profiles using expert consensus (gold standard) and were validated using data from a clinical trial. The nominal group technique was used to reach consensus. RESULTS: Consensus was reached for a conjoint analysis-based continuous model using absolute percent change in core set measures (physician, patient, and extramuscular global activity, muscle strength, Health Assessment Questionnaire, and muscle enzyme levels). A total improvement score (range 0-100), determined by summing scores for each core set measure, was based on improvement in and relative weight of each core set measure. Thresholds for minimal, moderate, and major improvement were ≥20, ≥40, and ≥60 points in the total improvement score. The same criteria were chosen for juvenile DM, with different improvement thresholds. Sensitivity and specificity in DM/PM patient cohorts were 85% and 92%, 90% and 96%, and 92% and 98% for minimal, moderate, and major improvement, respectively. Definitions were validated in the clinical trial analysis for differentiating the physician rating of improvement (P < 0.001). CONCLUSION: The response criteria for adult DM/PM consisted of the conjoint analysis model based on absolute percent change in 6 core set measures, with thresholds for minimal, moderate, and major improvement.

OBJECTIVE: To develop response criteria for juvenile dermatomyositis (DM). METHODS: We analyzed the performance of 312 definitions that used core set measures from either the International Myositis Assessment and Clinical Studies Group (IMACS) or the Paediatric Rheumatology International Trials Organisation (PRINTO) and were derived from natural history data and a conjoint analysis survey. They were further validated using data from the PRINTO trial of prednisone alone compared to prednisone with methotrexate or cyclosporine and the Rituximab in Myositis (RIM) trial. At a consensus conference, experts considered 14 top candidate criteria based on their performance characteristics and clinical face validity, using nominal group technique. RESULTS: Consensus was reached for a conjoint analysis-based continuous model with a total improvement score of 0-100, using absolute percent change in core set measures of minimal (≥30), moderate (≥45), and major (≥70) improvement. The same criteria were chosen for adult DM/polymyositis, with differing thresholds for improvement. The sensitivity and specificity were 89% and 91-98% for minimal improvement, 92-94% and 94-99% for moderate improvement, and 91-98% and 85-86% for major improvement, respectively, in juvenile DM patient cohorts using the IMACS and PRINTO core set measures. These criteria were validated in the PRINTO trial for differentiating between treatment arms for minimal and moderate improvement (P = 0.009-0.057) and in the RIM trial for significantly differentiating the physician's rating for improvement (P < 0.006). CONCLUSION: The response criteria for juvenile DM consisted of a conjoint analysis-based model using a continuous improvement score based on absolute percent change in core set measures, with thresholds for minimal, moderate, and major improvement.

Vyšetrenie plazmatickej aktivity aspartát aminotransferázy, alanín aminotransferázy, γ-glutamyltransferázy a alkalickej fosfatázy patrí k základným testom používaným v diagnostike ochorení pečene. Hodnotenie abnormálnych výsledkov si vyžaduje istú mieru skúsenosti. Najvhodnejšie je riadiť sa určitým štandardizovaným postupom, aby nedošlo k prehliadnutiu závažnejšej patológie, ale ani k zbytočnému indikovaniu vyšetrení. Prístup k pacientovi závisí od naliehavosti stavu, načasovania elevácie a sprievodných okolností. Tento článok ponúka prehľadný návrh, ako postupovať pri vyšetrení elevácie pečeňových enzýmov aj z pohľadu evidence-based medicine.
The evaluation of increased plasmatic aspartate transaminase, alanine transaminase, γ-glutamyl transpeptidase, and alkaline phosphatase activities is fundamental to a differential diagnosis of liver disease. The correct interpretation of the results of tests for these activities demands much experience. The best option is to use the standardized guidelines to avoid overlooking important pathologies and performing unnecessary tests. Individual patients should be evaluated depending on the urgency of the situation, the timing of the elevation in liver enzymes, and other circumstances. This paper presents a simple recommendation about how to proceed when evaluating elevated liver enzyme levels with respect to evidence-based medicine.

Effects of a high terbuthylazine concentration (3.3 mg/1) on Cyprinus carpio were studied using a commercial herbicide formulation Click 500 SC (terbuthylazine 500 g/l). The fish were exposed to the pesticide for 24 h and allowed to recover for 6 days. Biometric parameters, plasma biochemical parameters and biomarkers of oxidative stress as well as histopathological changes in selected tissues were assessed on day 1 and 7. After a 24-h exposure, there were significant alterations found in the activities of alanine aminotransferase (ALT) and aspartate aminotransferase (AST) as well as in the plasma concentrations of glucose, natrium, chlorides, calcium and phosphorus. Hepatosomatic index, plasma albumin and lactate reflected the treatment with a delay. Ion levels and ALT were found to be restored after a 6-day recovery period, which was too short for AST activity and glucose to diminish to the control levels. The histopathological examination revealed disorders in the gills of the exposed fish, however, the changes were not detected after a 6-day recovery period. The study shows high regeneration potential of the fish.

Srdeční selhání patří mezi závažné zdravotní problémy 21. století vedoucí zejména k častým rehospitalizacím, snižuje též kvalitu života a zhoršuje prognózu nemocných. Při progresi chronického srdečního selhání dochází často k narůstající neschopnosti srdce splňovat metabolické nároky většiny orgánů. Zatímco důsledky akutního srdečního selhání na renální funkce byly v literatuře mnohokrát popsány, komplexní popis dopadu akutního srdečního selhání na jaterní funkce v literatuře stále chybí. V léčbě akutního srdečního selhání můžeme využít hepatoprotektivní účinek levosimendanu, který spočívá pravděpodobně v jeho příznivém hemodynamickém působení podmíněném jeho specificky pozitivně inotropním a vazodilatačním efektem společně s přímým hepatocytárním působením se zvýšením metabolických a adaptačních rezerv hepatocytu.
Heart failure is the major public health problem of the 21st century, with frequent re-hospitalizations, impaired quality of life and prognosis. The progression of heart failure is often characterized by an increasing inability of the heart to meet the metabolic demands of most organs. While the consequences of acute heart failure on renal function have been described many times, a comprehensive description of the impact of acute heart failure on the liver function is still lacking. In the treatment of acute heart failure, we can exploit the hepatoprotective effect of levosimendan, which is probably due to its favorable hemodynamic effects, specifically because of its positive inotropic and vasodilatory effects, together with a direct hepatocytic effect, accompanied by an increase in metabolic and adaptation reserves of hepatocytes.

Graft-versus-host disease (GVHD) is the most frequent complication after allogeneic hematopoietic cell transplantation. We analyzed the kinetics of bilirubin and liver enzymes in 47 cases with liver GVHD and in 47 cases without GVHD after allogeneic transplantation for various hematological malignancies. The duration of an liver GVHD episode (LGVHD) was defined as the interval from the point when the criteria of LGVHD were met to the decrease to < 2 upper normal limit (UNL) for aminotransferases or bilirubin < 34 μmol/l for bilirubin. The imminent LGVHD episode was defined as the interval from the start of continuous increase (≥ 3 consecutive rising values) of bilirubin and liver enzymes above UNL to the point of LGVHD diagnosis.The number of imminent LGVHD episodes, and median length in days were as follows: bilirubin (39;5), ALT(28;12), AST(9;12), GGTP(34;9), and ALP(13;14). Statisticallly significant associations between asymptomatic continuous increase of bilirubin, ALT, and GGTP and later liver GVHD manifestation were found (p=0.004, p=0.008, p=0.005, respectively). The asymptomatic continuous increase in bilirubin, ALT, and GGTP occurred at a median of 5, 12, and 9 days before liver GVHD episode, respectively. In the control group without GVHD, median levels of bilirubin and liver enzymes were within normal limits and no continuous increase was observed.Kinetics of bilirubin and liver enzymes is useful for predicting of liver GVHD. A continuous increase of bilirubin and/or ALT, GGTP before the standard liver GVHD criteria are met can be a sign of coming liver GVHD.
