When cardiopulmonary bypass (CPB) is used, the blood comes into contact with foreign surfaces. To diminish this impact, various types of biocompatibly coated surfaces have been developed. The study assessed the effects of heparin-coated CPB systems on the level and function of fibrinogen as measured by thromboelastography (TEG), as compared with non-coated systems. No statistically significant differences between both groups were revealed by comparing paired data. In our study, heparin-coated CPB circuits had no significant effect on either fibrinogen level or its function.

Heparin is commonly used to prevent obstruction of indwelling arterial catheters with blood clots. It is known to affect the outcomes of analysis of coagulation parameters with thromboelastography (TEG); therefore, it has been recommended to neutralize its effect with heparinase. However, heparinase may also neutralize the effect of low molecular weight heparin and endogenous heparinoids present in critically ill patients and thus yields unreliable results. The aim of this study was to evaluate the minimal discard blood volume needed to eliminate the effect of heparin flush on TEG parameters without the use of heparinase. Ten patients with indwelling arterial catheter were included in the study. Coagulation parameters were evaluated with kaolin-activated TEG. Blood samples were obtained after discarding 1, 2, 3, 4, 5 or 10 ml of blood to eliminate the effect of heparin. We investigated the influence of the discard volume on time until the first detectable clot (R), speed of clot development (alpha angle), maximal amplitude of the measured clot and time to maximal amplitude of the measured clot. We found an increase in coagulation (reflecting the heparin elimination) with the increasing discard volume between 1 and 4 ml. This was obvious from an increase in alpha angle and maximal amplitude of the measured clot and a decrease in R and time to maximal amplitude of the measured clot (P < 0.001). However, values obtained after discarding 4, 5 and 10 ml of blood did not differ markedly. To obtain valid information about TEG parameters, it is necessary to discard volume of at least 4 ml of blood (i.e., five times the volume of catheter dead space).

Various techniques for coating synthetic surfaces with fibrin structures were tested for seeding bovine pulmonary artery endothelial cells (EC). Two-dimensional fibrin (Fb) structures (2D Fb) were obtained by successively repeating adsorption of fibrinogen (Fbg), incubating the surface with thrombin (Thr) solution, and inhibiting surface-attached Thr. Three-dimensional fibrin networks immobilized at the surface (3D Fb) were formed by catalytic action of surface-attached thrombin on an ambient Fbg solution. Ultra-thin 3D Fbs were obtained if thrombin inhibitors antithrombin III and heparin were added into an Fbg solution. The formation of surface fibrin structures was observed in situ using surface plasmon resonance. The morphology of the structures was studied by transmission and scanning electron microscopy. A polylactide fibrous scaffold was modified with a surface fibrin film without filling the inner pores with a bulk gel. The growth of EC seeded on a polystyrene surface coated with the Fb films was evaluated by the number and morphology of the adhering ECs and the concentration of beta-actin, vinculin, alpha(v)-intergrin, and von Willebrand factor (vWF). The best initial cell spreading after 1 day was observed on 2D Fb and ultra-thin 3D Fb. The highest concentration of vWF, a marker of EC differentiation, was observed after 3 days on thick 3D Fbs. The highest EC population densities after 7 days were observed on 2D Fb and thick 3D Fb.

Enterococci from different free-living rodents were isolated and selected. The strains were genotyped and their antibiotic sensitivity and /or resistance, production of lactic acid, urease activity, the presence of enterocin structural genes, plasmid detection, and their binding ability to proteins were determined. Among 24 enterococcal strains, 17 strains were allotted to the species Enterococcus faecalis by tDNA-PCR, 7 strains being not yet identified. Only Enterococcus sp. ES66 possessed the structural genes for the production of enterocins (Ent) A and P. E. faecalis EE61 had the gene for EntL50B. The other isolates were Ent-gene-free. Enterococci were mostly sensitive to antibiotic treatment . The plasmid DNA was detected only in the strain EE97. The average value of lactic acid production reached 896 +/- 90 micromol/L. Most of the strains possessed low ureolytic activity. Enterococci bound very well sub-epithelial proteins, reaching the value of 3 by the particle agglutination assay, especially concerning the heparin, bovine lactoferrin and porcine fibronectin.

Using the layer-by-layer technique, ELISA polystyrene plates were coated with multilayer assemblies of albumin with various heparins or with multilayer assemblies of albumin. The coatings containing heparin were tested for their ability to potentiate thrombin inhibition by antithrombin and its dependence on the layer arrangement. The order of activities of surface bound heparins matched their order in solution; however their activity was reduced to less than 10% due to binding. The increasing number of layers increased the activity of the coatings suggesting that heparin inside the assemblies is available for the interaction. The albumin-heparin assemblies overcoated with albumin layers preserved about half of heparin activity. Platelets adhered in similar amounts to albumin-heparin and albumin coatings; however, in both cases platelets adhered more to single layer than to multilayer coatings. The adhesion of platelets to single layer coatings was also affected by the crosslinking of the coatings; more platelets adhered to less crosslinked single layer coatings while multilayer coatings remained essentially unaffected by crosslinking. If the coatings were dried and reswollen, a substantial number of platelets adhered to the reconditioned single layer coatings but the two layer coatings were affected much less and the adhesion of platelets to the coatings with three layers was close to normal. A minimum of three albumin-heparin or albumin layers is apparently required to shield the underlying surface and to achieve proper functioning of the coatings.
