Vyhodnocení lékových interakcí s navržením správného řešení může být v některých případech komplikované. Při interpretaci je nezbytné uvažovat nad interakcemi farmaceutickými, farmakodynamickými a farmakokinetickými. Je důležité správně posoudit potenciál léčiva interagovat nejen na metabolické úrovni, ale i na úrovni absorpce, distribuce a zejména eliminace. V článku jsou diskutovány základní interakce inhibitorů integrázy používaných u HIV pacientů. Na příkladu z klinické praxe je demonstrováno řešení takové interakce.
Evaluation of drug interactions and offering the most suitable solution may be complicated in some cases. Pharmaceutical, pharmacodynamic and pharmacokinetic interactions should be considered in the interpretation. It is important to properly assess the potential of drug to interact not only at the metabolic level but also at the level of absorption, distribution and especially elimination. The article discusses the basic interactions of integrase inhibitors used in HIV patients. The solution of such an interaction is shown on the example from clinical practice.

HIV-1 persists lifelong in memory cells of the immune system as latent provirus that rebounds upon treatment interruption. Therefore, the latent reservoir is the main target for an HIV cure. Here, we studied the direct link between integration site and transcription using LEDGINs and Barcoded HIV-ensembles (B-HIVE). LEDGINs are antivirals that inhibit the interaction between HIV-1 integrase and the chromatin-tethering factor LEDGF/p75. They were used as a tool to retarget integration, while the effect on HIV expression was measured with B-HIVE. B-HIVE tracks insert-specific HIV expression by tagging a unique barcode in the HIV genome. We confirmed that LEDGINs retarget integration out of gene-dense and actively transcribed regions. The distance to H3K36me3, the marker recognized by LEDGF/p75, clearly increased. LEDGIN treatment reduced viral RNA expression and increased the proportion of silent provirus. Finally, silent proviruses obtained after LEDGIN treatment were located further away from epigenetic marks associated with active transcription. Interestingly, proximity to enhancers stimulated transcription irrespective of LEDGIN treatment, while the distance to H3K36me3 only changed after treatment with LEDGINs. The fact that proximity to these markers are associated with RNA expression support the direct link between provirus integration site and viral expression.

Biktegravir (BIC) je nový inhibitor integrázy HIV 1 i HIV 2 druhé generace. V únoru 2018 byl schválen americkým Úřadem pro kontrolu potravin a léčiv (FDA) v kombinaci s emtricitabinem a tenofovir alafenamidem (BIC/F/TAF – 50/200/25 mg, Biktarvy®) pro léčbu dospělých osob s infekcí HIV 1. Podává se jedna tableta jednou denně. Jeho účinnost, bezpečnost, snášenlivost a lékové interakce byly hodnoceny ve čtyřech (GS-US-380-1489, 1490, 1844 a 1878) studiích s použitím srovnatelného komparátoru – dolutegraviru či inhibitoru proteázy u předléčených pacientů. Indikací BIC/F/TAF je infekce HIV 1 u osob dosud neléčených nebo u dříve léčených bez známek selhání před-chozího režimu a bez prokázané rezistence na některou komponentu. Bictegravir (BIC) is a novel, second generation HIV 1 and HIV 2 integrase strand transfer inhibitor. In February 2018, it has been approved by the Food and Drug Administration (FDA) in combination with emtricitabine and tenofovir alafenamide (BIC/F/TAF – 50/200/25 mg, Biktarvy®) for the treatment of adult patients with HIV 1 infection. It is administered with one tablet once daily. Its efficiency, safety, tolerability and drug interactions were elucidated in four (GS-US-380-1489, 1490, 1844 and 1878) studies using a comparator ‒ dolute-gravir, or protease inhibitor in naïve patients. The indications of BIC/F/TAF include HIV 1 infection in treatment naïve patients, or in previously treated patients with no signs of failure of the previous regimen and without a proven resistance to any component. In the period 96 weeks, no resistance to BIC was found. Other studies, including the post marketing ones, are underway.

To understand the persistence of latently HIV-1 infected cells in virally suppressed infected patients, a number of in vitro models of HIV latency have been developed. In an attempt to mimic the in vivo situation as closely as possible, several models use primary cells and replication-competent viruses in combination with antiretroviral compounds to prevent ongoing replication. Latency is subsequently measured by HIV RNA and/or protein production after cellular activation. To discriminate between pre- and post-integration latency, integrase inhibitors are routinely used, preventing novel integrations upon cellular activation. Here, we show that this choice of antiretrovirals may still cause a bias of pre-integration latency in these models, as unintegrated HIV DNA can form and directly contribute to the levels of HIV RNA and protein production. We further show that the addition of reverse transcriptase inhibitors effectively suppresses the levels of episomal HIV DNA (as measured by 2-LTR circles) and decreases the levels of HIV transcription. Consequently, we show that latency levels described in models that only use integrase inhibitors may be overestimated. The inclusion of additional control conditions, such as 2-LTR quantification and the addition of reverse transcriptase inhibitors, is crucial to fully elucidate the actual levels of post-integration latency.

Moderní antiretrovirová léčba a preventivní opatření se dnes považují za hlavní metody kontroly pandemie AIDS (acquired immune deficiency syndrome, syndrom získané imunodeficience). Od přípravků prvních generací se liší podstatně příznivějšími farmakokinetickými/farmakodynamickými vlastnostmi, vyšší rezistenční bariérou a obecně lepší tolerancí. Celodenní kombinovanou léčbu dnes v optimálním případě představuje jedna tableta podávaná jednou denně. Antiretrovirová léčba je pojištěncům v České republice hrazena v plném rozsahu. Za poslední dva roky zaregistrovala Evropská léková agentura pět antiretrovirotik, další nejméně čtyři jsou v pořadí. Racionální antiretrovirová léčba se neustále vyvíjí a zlepšuje v důsledku probíhajících klinických studií. Celosvětově existuje několik doporučených postupů léčby infekce HIV (human immunodeficiency virus, virus lidského imunodeficitu)/AIDS. Jejich inovace v České republice probíhá každé 2–3 roky, přičemž se přihlíží ke specifikům naší populace a zdravotnického systému. Základem jsou nukleos(t)idové inhibitory HIV reverzní transkriptázy spolu s inhibitory HIV integrázy nebo s potencovanými inhibitory HIV proteázy, alternativně pak nenukleosidové inhibitory HIV reverzní transkriptázy, inhibitory fúze a vstupu do CD4+ buňky. Infekce HIV u vysoce adherentních osob se díky kombinované antiretrovirové léčbě stala chronickým léčitelným, ale zatím nevyléčitelným onemocněním. Zásadně se změnila kvalita života, došlo ke snížení morbidity na oportunní infekce i k poklesu mortality. Predikovaná délka života vysoce adherentních HIV‑pozitivních osob se v současné době téměř neliší od běžné populace.
Current antiretroviral therapy and preventive measures are still the essential option to control the AIDS (acquired immune deficiency syndrome) pandemic. Compared with the first available drugs, current drugs have considerably more favourable pharmacokinetic/pharmacodynamic effects, a higher resistance barrier, and generally better tolerance. All‑day combination therapy today consists of only one tablet a day in optimal cases. Antiretroviral therapy is fully covered by insurance companies in the Czech Republic. During the last 2 years, there were 5 medications registered in the European Medicines Agency and at least 4 more are expected. Rational antiretroviral treatment (ART) is constantly evolving and improving as a result of continuing clinical trials. Worldwide, there are several guidelines for treatment of HIV (human immunodeficiency virus) infection/AIDS. Their innovation in the Czech Republic takes place every 2–3 years, taking into account the specificities of our population and health system. They are mainly based on nucleos(t)ide HIV reverse transcriptase inhibitors, along with HIV integrase inhibitors or boosted HIV protease inhibitors. Alternatively, non‑nucleoside HIV‑reverse transcriptase inhibitors, fusion inhibitors, and inhibitors of the entry into CD4+ cells are used. HIV infection in highly adherent people, thanks to combination antiretroviral therapy, has become a chronic, treatable but not yet curable disease. Fundamental improvement in the quality of life has been associated with reduction in morbidity caused by opportunistic infections, as well as mortality. The predicted life span of highly adherent HIV+ people at present barely differs from that of general population.

OBJECTIVES: The objective of this study was to define the natural genotypic variation of the HIV-1 integrase gene across Europe for epidemiological surveillance of integrase strand-transfer inhibitor (InSTI) resistance. METHODS: This was a multicentre, cross-sectional study within the European SPREAD HIV resistance surveillance programme. A representative set of 300 samples was selected from 1950 naive HIV-positive subjects newly diagnosed in 2006-07. The prevalence of InSTI resistance was evaluated using quality-controlled baseline population sequencing of integrase. Signature raltegravir, elvitegravir and dolutegravir resistance mutations were defined according to the IAS-USA 2014 list. In addition, all integrase substitutions relative to HXB2 were identified, including those with a Stanford HIVdb score ≥ 10 to at least one InSTI. To rule out circulation of minority InSTI-resistant HIV, 65 samples were selected for 454 integrase sequencing. RESULTS: For the population sequencing analysis, 278 samples were retrieved and successfully analysed. No signature resistance mutations to any of the InSTIs were detected. Eleven (4%) subjects had mutations at resistance-associated positions with an HIVdb score ≥ 10. Of the 56 samples successfully analysed with 454 sequencing, no InSTI signature mutations were detected, whereas integrase substitutions with an HIVdb score ≥ 10 were found in 8 (14.3%) individuals. CONCLUSIONS: No signature InSTI-resistant variants were circulating in Europe before the introduction of InSTIs. However, polymorphisms contributing to InSTI resistance were not rare. As InSTI use becomes more widespread, continuous surveillance of primary InSTI resistance is warranted. These data will be key to modelling the kinetics of InSTI resistance transmission in Europe in the coming years.

Heterocyclic substances perform a very unique role in drug design and discovery. This article provides the primary objectives of the analysis within pyrimidine centered new heterocyclic elements chronologically from their finding focusing on one of the essential enzyme of HIV virus particle that is integrase upon suppressing its strand transfer function. The class of compounds reviewed here includes bicyclic pyrimidines, dihydroxypyrimidines, pyrimidine-2,4-dinones, N-methylpyrimidones, pyranopyrimidine, pyridine-quinoline conjugates, pyrimidine-2-carboxamides, N-3 hydroxylated pyrimidine-2,4-diones as well as their various substituted analogues. Such initiatives released an effective drug Raltegravir as a first FDA approved anti-HIV integrase inhibitor as well as several of its derivatives along with other pyrimidones is under clinical or preclinical growth. Some of the provided scaffolds indicated dual anti-HIV efficacies against HIV reverse transcriptase and integrase enzymes at both cites as 3'-processing and strand transfer, while several scaffolds exhibited potency against Raltegravir resistant HIV mutant strains determining themselves a potent class of compounds having appealing upcoming implementations. Connections of the new compounds' molecular structure and HIV viral target has been overviewed to be able to accomplish further growth of promising anti-HIV agents in future drug discovery process.

Heterocyclic compounds execute a very important role in drug design and discovery. This article provides the basic milestones of the research for pyrroloaryl and pyrroloheteroaryl based components targeting HIV viral replication cycle. Anti-HIV activity is elaborated for several classes of pyrrolo-compounds as pyrrolopyridines, pyrrolopyrimidines, pyrrolopyridazines, pyrrolobenzodiazepinones, pyrrolobenzothiazepines, pyrrolobenzoxazepinones, pyrrolophenanthridines, pyrroloquinoxalines, pyrrolotriazines, pyrroloquinolines, pyrrolopyrazinones, pyrrolothiatriazines, arylthiopyrroles and pyrrolopyrazolones targeting two essential HIV enzymes, reverse transcriptase and integrase as well as attachment/fusion of HIV virons to the host CD-4 cell. Such attempts were resulted in a discovery of highly potent anti-HIV agents suitable for clinical trials, for example, BMS-378806, BMS-585248, BMS-626529, BMS-663068, BMS-488043 and BMS-663749, etc. as anti-HIV attachment agents, triciribine, QX432, BI-1 and BI-2 as HIV RT inhibitors which are in preclinical or clinical development. Mechanism of action of compounds presented in this article towards the suppression of HIV attachment/fusion as well as against the activities of HIV enzymes reverse transcriptase and integrase has been discussed. Relationships of new compounds' molecular framework and HIV viral target has been overviewed in order to facilitate further construction of promising anti-HIV agents in future drug discovery process.
