Nucleos(t)ide analogues entecavir (ETV) and tenofovir disoproxil fumarate (TDF) are recommended as first-line monotherapies for chronic hepatitis B (CHB). Multiple HBV genotypes/subgenotypes have been described, but their impact on treatment response remains largely elusive. We investigated the effectiveness of ETV/TDF on HBV/D-subgenotypes, D1/D2/D3/D5, studied the structural/functional differences in subgenotype-specific reverse transcriptase (RT) domains of viral polymerase, and identified novel molecules with robust inhibitory activity on various D-subgenotypes. Transfection of Huh7 cells with full-length D1/D2/D3/D5 and in vitro TDF/ETV susceptibility assays demonstrated that D1/D2 had greater susceptibility to TDF/ETV while D3/D5 exhibited poorer response. Additionally, HBV load was substantially reduced in TDF-treated CHB patients carrying D1/D2 but minimally reduced in D3/D5-infected patients. Comparison of RT sequences of D-subgenotypes led to identification of unique subgenotype-specific residues, and molecular modeling/docking/simulation studies depicted differential bindings of TDF/ETV to the active site of their respective RTs. Replacement of signature residues in D3/D5 HBV clones with corresponding amino acids seen in D1/D2 improved their susceptibility to TDF/ETV. Using high throughput virtual screening, we identified N(9)-[3-fluoro-2-(phosphonomethoxy)propyl] (FPMP) derivatives of purine bases, including N6-substituted (S)-FPMP derivative of 2,6-diaminopurine (DAP) (OB-123-VK), as potential binders of RT of different D-subgenotypes. We synthesized (S)-FPMPG prodrugs (FK-381-FEE/FK-381-SEE/FK-382) and tested their effectiveness along with OB-123-VK. Both OB-123-VK and FK-381-FEE exerted similar antiviral activities against all D-subgenotypes, although FK-381-FEE was more potent. Our study highlighted the natural variation in therapeutic response of D1/D2/D3/D5 and emphasized the need for HBV subgenotype determination before treatment. Novel molecules described here could benefit future design/discovery of pan-D-subgenotypic inhibitors. IMPORTANCE Current treatment of chronic hepatitis B relies heavily on nucleotide/nucleoside analogs in particular, tenofovir disoproxil fumarate (TDF) and entecavir (ETV) to keep HBV replication under control and prevent end-stage liver diseases. However, it was unclear whether the therapeutic effects of TDF/ETV differ among patients infected with different HBV genotypes and subgenotypes. HBV genotype D is the most widespread of all HBV genotypes and multiple D-subgenotypes have been described. We here report that different subgenotypes of HBV genotype-D exhibit variable response toward TDF and ETV and this could be attributed to naturally occurring amino acid changes in the reverse transcriptase domain of the subgenotype-specific polymerase. Further, we identified novel molecules and also synthesized prodrugs that are equally effective on different D-subgenotypes and could facilitate management of HBV/D-infected patients irrespective of D-subgenotype.

Antiretrovirová terapie je základem léčby osob infikovaných HIV (lidským virem imunodeficience). Antiretrovirotika se rozdělují do šesti skupin podle mechanismu účinku a chemické struktury. Jedna blokují funkci tří virových enzymů a jiná brání vstupu viru do buňky. U většiny pacientů se dosáhne suprese virové replikace, která je předpokladem pro návrat, resp. udržení dostatečné funkce imunitního systému. Léčba má také význam pro omezení přenosu infekce HIV na druhé osoby. Ačkoliv léčba brání klasickým zdravotním komplikacím infekce HIV vázaným na imunodeficit, nezabrání úplně aktivaci imunitního systému a tím nebezpečí non‑AIDS komplikací. Obvyklá počáteční kombinace antiretrovirotik zahrnuje dva nukleosidové, resp. jeden nukleosidový a jeden nukleotidový inhibitor reverzní transkriptázy spolu s inhibitorem integrázy. Za určitých okolností přichází v úvahu i použití např. nových nenukleosidových inhibitorů reverzní transkriptázy nebo inhibitorů proteinázy. Vedle této konvenční terapie se zvolna prosazuje používání dvojkombinačních režimů založených na spolehlivosti dolutegraviru. Tyto režimy slibují snížení toxicity, lékových interakcí a nákladů při zachování účinku. Určité teoretické překážky nejsou vyloučeny a přínos lze očekávat u vybraných skupin pacientů.
Antiretroviral therapy is a basis for HIV (human immunodeficiency virus) treatment of positive people. Antiretroviral drugs are divided into six groups according to the mechanism of action and chemical structure. Some block the function of three viral enzymes and other block the viral entry into the cell. In most patients, a suppression of viral replication is reached that is a pre‑requisite for the return and maintenance of sufficient immune system function. The treatment is also important for reducing the risk of HIV infection transfer to other people. Although the treatment prevents classic medical complications of HIV infection associated with immunodeficiency, it does not prevent completely the activation of immune system and the risk of non‑AIDS complications. The usual initial combination of antiretroviral drugs includes two nucleoside or rather one nucleoside and one nucleotide inhibitor of reverse transcriptase together with integrase inhibitor. In certain circumstances, the use of for example the new non‑nucleoside inhibitors of reverse transcriptase or protease inhibitors is warranted. Alongside this conventional therapy, the use of dual combination regimens based on the reliability of dolutegravir is slowly asserted. The promise of the decrease of toxicity, drug interactions and cost while maintaining efficacy comes with these regimens. Certain theoretical obstacles are not out of the question and the benefit can be expected in selected groups of patients.

Infekce virem hepatitidy B (HBV) je stále celosvětově zdravotním problém s měnicí se epidemiologií v závislosti na řadě faktorů, především na vakcinační politice a migraci. Chronická infekce HBV je označení pro infekci trvající déle než 6 měsíců. Jde o dynamický proces, který odráží interakce mezi virovou replikací a hostitelskou imunitní odpovědí. Ne všichni pacienti chronicky infikovaní HBV mají chronickou hepatitidu B. Všechny osoby chronicky infikované HBV jsou ve zvýšeném riziku progrese do jaterní cirhózy a hepatocelulárního karcinomu. Dlouhodobé podávání účinných nukleosidových nebo nukleotidových inhibitorů virové DNA polymerázy – reverzní transkriptázy s vysokou genetickou bariérou proti vzniku rezistence (tenofovir, entekavir) představuje v současnosti nejúčinnější léčbu chronické hepatitidy B. U pacientů s mírně až středně pokročilou HBV lze použít k léčbě i pegylovaný interferon alfa.
Hepatitis B virus (HBV) infection remains a global public health problem with changing epidemiology due to several factors predominantlyvaccination policy and migration. Chronic hepatitis B means the duration of HBV infection for more than 6 months.It is a dynamic process reflecting the interaction between HBV replication and the host immune response and not all patientswith chronic HBV infection have chronic hepatitis B. All patients with chronic HBV infection are in increased risk of progression toliver cirrhosis and hepatocellular carcinoma. The long-term administration of potent nucleos(t)ide analogue with high barrier ofresistance (tenofovir, entecavir) represents the treatment of choice. Pegylated interferon-alpha can also be considered in mid tomoderate chronic hepatitis B patients.

Moderní antiretrovirová léčba a preventivní opatření se dnes považují za hlavní metody kontroly pandemie AIDS (acquired immune deficiency syndrome, syndrom získané imunodeficience). Od přípravků prvních generací se liší podstatně příznivějšími farmakokinetickými/farmakodynamickými vlastnostmi, vyšší rezistenční bariérou a obecně lepší tolerancí. Celodenní kombinovanou léčbu dnes v optimálním případě představuje jedna tableta podávaná jednou denně. Antiretrovirová léčba je pojištěncům v České republice hrazena v plném rozsahu. Za poslední dva roky zaregistrovala Evropská léková agentura pět antiretrovirotik, další nejméně čtyři jsou v pořadí. Racionální antiretrovirová léčba se neustále vyvíjí a zlepšuje v důsledku probíhajících klinických studií. Celosvětově existuje několik doporučených postupů léčby infekce HIV (human immunodeficiency virus, virus lidského imunodeficitu)/AIDS. Jejich inovace v České republice probíhá každé 2–3 roky, přičemž se přihlíží ke specifikům naší populace a zdravotnického systému. Základem jsou nukleos(t)idové inhibitory HIV reverzní transkriptázy spolu s inhibitory HIV integrázy nebo s potencovanými inhibitory HIV proteázy, alternativně pak nenukleosidové inhibitory HIV reverzní transkriptázy, inhibitory fúze a vstupu do CD4+ buňky. Infekce HIV u vysoce adherentních osob se díky kombinované antiretrovirové léčbě stala chronickým léčitelným, ale zatím nevyléčitelným onemocněním. Zásadně se změnila kvalita života, došlo ke snížení morbidity na oportunní infekce i k poklesu mortality. Predikovaná délka života vysoce adherentních HIV‑pozitivních osob se v současné době téměř neliší od běžné populace.
Current antiretroviral therapy and preventive measures are still the essential option to control the AIDS (acquired immune deficiency syndrome) pandemic. Compared with the first available drugs, current drugs have considerably more favourable pharmacokinetic/pharmacodynamic effects, a higher resistance barrier, and generally better tolerance. All‑day combination therapy today consists of only one tablet a day in optimal cases. Antiretroviral therapy is fully covered by insurance companies in the Czech Republic. During the last 2 years, there were 5 medications registered in the European Medicines Agency and at least 4 more are expected. Rational antiretroviral treatment (ART) is constantly evolving and improving as a result of continuing clinical trials. Worldwide, there are several guidelines for treatment of HIV (human immunodeficiency virus) infection/AIDS. Their innovation in the Czech Republic takes place every 2–3 years, taking into account the specificities of our population and health system. They are mainly based on nucleos(t)ide HIV reverse transcriptase inhibitors, along with HIV integrase inhibitors or boosted HIV protease inhibitors. Alternatively, non‑nucleoside HIV‑reverse transcriptase inhibitors, fusion inhibitors, and inhibitors of the entry into CD4+ cells are used. HIV infection in highly adherent people, thanks to combination antiretroviral therapy, has become a chronic, treatable but not yet curable disease. Fundamental improvement in the quality of life has been associated with reduction in morbidity caused by opportunistic infections, as well as mortality. The predicted life span of highly adherent HIV+ people at present barely differs from that of general population.

OBJECTIVES: To describe regional differences and trends in resistance testing among individuals experiencing virological failure and the prevalence of detected resistance among those individuals who had a genotypic resistance test done following virological failure. DESIGN: Multinational cohort study. METHODS: Individuals in EuroSIDA with virological failure (>1 RNA measurement >500 on ART after >6 months on ART) after 1997 were included. Adjusted odds ratios (aORs) for resistance testing following virological failure and aORs for the detection of resistance among those who had a test were calculated using logistic regression with generalized estimating equations. RESULTS: Compared to 74.2% of ART-experienced individuals in 1997, only 5.1% showed evidence of virological failure in 2012. The odds of resistance testing declined after 2004 (global P < 0.001). Resistance was detected in 77.9% of the tests, NRTI resistance being most common (70.3%), followed by NNRTI (51.6%) and protease inhibitor (46.1%) resistance. The odds of detecting resistance were lower in tests done in 1997-1998, 1999-2000 and 2009-2010, compared to those carried out in 2003-2004 (global P < 0.001). Resistance testing was less common in Eastern Europe [aOR 0.72, 95% confidence interval (CI) 0.55-0.94] compared to Southern Europe, whereas the detection of resistance given that a test was done was less common in Northern (aOR 0.29, 95% CI 0.21-0.39) and Central Eastern (aOR 0.47, 95% CI 0.29-0.76) Europe, compared to Southern Europe. CONCLUSIONS: Despite a concurrent decline in virological failure and testing, drug resistance was commonly detected. This suggests a selective approach to resistance testing. The regional differences identified indicate that policy aiming to minimize the emergence of resistance is of particular relevance in some European regions, notably in the countries in Eastern Europe.

Cíl: Srovnat chronické účinky antiretrovirotické léčby (lamivudin, stavudin, delavirdin, nelfinavir, amprenaviru a kombinace lopinaviru/ritonaviru) u březích potkanů albínů. Typ studie: Review. Název a sídlo pracoviště: Gynekologicko-porodnická klinika, Federal University of S?o Paulo (UNIFESP), Sao Paulo, SP, Brazílie. Metodika: Retrospektivní srovnávací studie se zabývala 18 skupinami po 10 březích samičkách potkanů, které byly téměř tři měsíce staré a vážily 200 gramů. Každé z nich byla denně aplikována léčba pomocí žaludeční sondy, zatímco kontrolní skupině byl aplikován 1 ml destilované vody. Studijní skupiny obdržely lamivudin (5, 15 a 45 mg/kg/den); stavudin (na 1, 3 a 9 mg/kg/den); nelfinavir (40, 120 a 360 mg/kg/den); amprenavir (na 46, 138 a 414 mg/kg/den); lopinavir/ritonavir (12.8/3.2, a 38.4/9.6 115/28.8 mg/kg/den) a delavirdin (20 a 60 mg/kg/den). To představuje jedno-, troj- a devítinásobek terapeutické dávky pro člověka, s výjimkou posledního léku, kde nebyla nejvyšší dávka podána. Byla hodnocena hmotnost samice, plodu a placenty, počet implantací a reabsorpcí, hlavní zevní malformace plodu a odumření samice nebo plodu. Pro statistické zpracování byly použity test Kruskalův-Wallisův a χ2 test. Výsledky: U všech tří dávek stavudin zvyšoval hmotnost samice (p = 0,001), zatímco lamivudin při trojnásobné a devítinásobné dávce hmotnost snižoval (p < 0,001). Amprenavir u všech dávek, a lopinavir//ritonavir u troj- a devítinásobné dávky zvyšoval úmrtnost samic (p < 0,001). Z hlediska plodů nebylo žádné z antiretrovirotik škodlivé, pokud jde o implantaci, reabsorpci, teratogenitu a úmrtnost (p > 0,05). Stavudin při všech dávkách snižoval hmotnost mláďat(p < 0,001); Nicméně lamivudin u trojnásobné, delavirdin u trojnásobné a amprenavir u trojnásobné dávky hmotnost mláďat zvyšoval (p < 0,001). Závěr: U březích samic jsme pozorovali letální toxicitu u krys, kterým byl aplikován amprenavir a ritonavir/lopinavir; hmotnost krys se měnila při užití lamivudinu a stavudinu . U plodů byly pozorovány nežádoucí účinky v souvislosti s hmotností mláďat u stavudinu, lamivudinu, amprenaviru a delavirdinu..
Objective: To compare the chronic effects of antiretrovirals (lamivudine, stavudine, delavirdine, nelfinavir, amprenavir and an association of lopinavir/ritonavir) on albino pregnant rats. Design: Review. Setting: Department of Obstetrics, Federal University of S?o Paulo (UNIFESP), S?o Paulo, SP, Brazil. Methods: This was a comparative retrospective study formed by 18 groups of 10 pregnant rats each, which were nearly three months of age and weighed 200 g. All of them were medicated every day using a stomach probe, while the control group was given 1 mL of distilled water. The study groups received lamivudine (at 5, 15 and 45 mg/kg/day); stavudine (at 1, 3 and 9 mg/kg/day); nelfinavir (at 40, 120 and 360 mg/kg/day); amprenavir (at 46, 138 and 414 mg/kg/day); lopinavir/ritonavir (at 12.8/3.2, 38.4/9.6 and 115/28.8 mg/kg/day) and delavirdine (at 20 and 60 mg/kg/day). These represented 1, 3 and 9 times the human therapeutic dose, except for the last drug, for which the 9-times dose was not used. Maternal, litter and placental weights, implantation and reabsorption numbers, major external fetal malformations and fetal and maternal deaths were evaluated. The Kruskal-Wallis test was used to compare quantitative variables and the chi-square test was used to compare qualitative variables. Results: At all three doses, stavudine increased the maternal weight (p=0.001), while lamivudine at 3- and 9-times doses reduced it (p<0.001). Amprenavir at all of the doses, and lopinavir/ritonavir at 3- and 9-times doses, caused higher rates of maternal death (p<0.001). Regarding the fetuses, none of the antiretroviral drugs studied were harmful with regard to implantation, reabsorption, teratogenity and mortality (p>0.05). Stavudine at all doses reduced the litter weights (p<0.001); however, lamivudine at the usual and 3-times doses, delavirdine at 3-times dose, and amprenavir at 3-times dose increased the litter weight (p<0.001). Conclusion: In the maternal compartment, we observed lethal toxicity in the pregnant rats that received amprenavir and ritonavir/lopinavir; and maternal weight change with lamivudine and stavudine. In the fetal compartment, adverse effects were observed in relation to litter weight from stavudine, lamivudine, delavirdine and amprenavir. Od druhé poloviny devadesátých let minulého století je hlavní léčebnou strategií HIV infekce tzv. kombinovaná antiretrovirová terapie. Jednou z tradičně používaných lékových skupin jsou inhibitory reverzní transkriptázy. Etravirin je zástupcem druhé generace nenukleosidových inhibitorů reverzní transkriptázy (NNRTI). Vzhledem k nutnosti kombinovat léky z různých terapeutických skupin, je každý nový léčebný přípravek možnou alternativou v případě nesnášenlivosti nebo neúčinnosti předchozí terapie. Etravirin je obecně charakterizován dobrou účinností, menším rizikem vzniku nežádoucích účinků a vyšší odolností proti vzniku rezistence v porovnání se zástupci první generace NNRTI.
Since the mid-nineties of the last century, combination antiretroviral therapy has been the main treatment strategy for HIV. Reverse transcriptase inhibitors are among the traditionally used drug classes. Etravirine is a second-generation non-nucleoside reverse transcriptase inhibitor (NNRTI). Due to the need to combine drugs from different classes, each newly approved medicinal product is an option for patients with resistance or intolerance to previous therapy. Etravirine is generally characterized by high efficacy, a low risk of side effects and a higher resistance barrier in comparison with first-generation NNRTIs.
