Jedna blokují funkci tří virových enzymů a jiná brání vstupu viru do buňky. Ačkoliv léčba brání klasickým zdravotním komplikacím infekce HIV vázaným na imunodeficit, nezabrání úplně aktivaci imunitního systému a tím nebezpečí non‑AIDS komplikací. Obvyklá počáteční kombinace antiretrovirotik zahrnuje dva nukleosidové, resp. jeden nukleosidový a jeden nukleotidový inhibitor reverzní transkriptázy spolu s inhibitorem integrázy. Určité teoretické překážky nejsou vyloučeny a přínos lze očekávat u vybraných skupin pacientů.
Antiretroviral therapy is a basis for HIV (human immunodeficiency virus) treatment of positive people. Antiretroviral drugs are divided into six groups according to the mechanism of action and chemical structure. Some block the function of three viral enzymes and other block the viral entry into the cell. In most patients, a suppression of viral replication is reached that is a pre‑requisite for the return and maintenance of sufficient immune system function. The treatment is also important for reducing the risk of HIV infection transfer to other people. Although the treatment prevents classic medical complications of HIV infection associated with immunodeficiency, it does not prevent completely the activation of immune system and the risk of non‑AIDS complications. The usual initial combination of antiretroviral drugs includes two nucleoside or rather one nucleoside and one nucleotide inhibitor of reverse transcriptase together with integrase inhibitor. In certain circumstances, the use of for example the new non‑nucleoside inhibitors of reverse transcriptase or protease inhibitors is warranted. Alongside this conventional therapy, the use of dual combination regimens based on the reliability of dolutegravir is slowly asserted. The promise of the decrease of toxicity, drug interactions and cost while maintaining efficacy comes with these regimens. Certain theoretical obstacles are not out of the question and the benefit can be expected in selected groups of patients.

Bictegravir (BIC) is a novel, second generation HIV 1 and HIV 2 integrase strand transfer inhibitor. In February 2018, it has been approved by the Food and Drug Administration (FDA) in combination with emtricitabine and tenofovir alafenamide (BIC/F/TAF – 50/200/25 mg, Biktarvy®) for the treatment of adult patients with HIV 1 infection. It is administered with one tablet once daily. Its efficiency, safety, tolerability and drug interactions were elucidated in four (GS-US-380-1489, 1490, 1844 and 1878) studies using a comparator ‒ dolute-gravir, or protease inhibitor in naïve patients. The indications of BIC/F/TAF include HIV 1 infection in treatment naïve patients, or in previously treated patients with no signs of failure of the previous regimen and without a proven resistance to any component. In the period 96 weeks, no resistance to BIC was found. Other studies, including the post marketing ones, are underway.

OBJECTIVES: The purpose of this study was to investigate dolutegravir (DTG) use among women and exposure to DTG during pregnancy in real world in Central and Eastern Europe and neighboring countries. MATERIAL AND METHODS: Centres from 20 countries included in the Euroguidelines in Central and Eastern Europe (ECEE) Network and Finland were asked to complete an on-line questionnaire. RESULTS: Seven centres from Czech Republic, Finland, Greece, Poland, Slovakia, and Turkey provided detailed information. DTG exposure was reported in 415 women, of which 26 were during pregnancy. Of those, 22 were on DTG at the time of conception and 4 had started DTG during pregnancy. Few women had conventional risk factors. The data on folic acid usage was unknown for eight women; 14 were using and four were not using folic acid. Four pregnancies were ongoing at the time of the study and of those with an outcome, 77.3% resulted with term, 13.6% preterm delivery, 4.5% spontaneous and 4.5% medical abortion. CONCLUSIONS: The DTG signal report indicates the importance of safety research for drug use in pregnancy and highlights the urgent need for systematic surveillance of pregnancy outcomes and neonatal surveillance. Countries with low- or moderate HIV prevalence should be included in studies reviewing pregnancy outcomes and in any surveillance system to ensure the accuracy of drug safety revision.

Current antiretroviral therapy and preventive measures are still the essential option to control the AIDS (acquired immune deficiency syndrome) pandemic. Compared with the first available drugs, current drugs have considerably more favourable pharmacokinetic/pharmacodynamic effects, a higher resistance barrier, and generally better tolerance. All‑day combination therapy today consists of only one tablet a day in optimal cases. Antiretroviral therapy is fully covered by insurance companies in the Czech Republic. During the last 2 years, there were 5 medications registered in the European Medicines Agency and at least 4 more are expected. Rational antiretroviral treatment (ART) is constantly evolving and improving as a result of continuing clinical trials. Worldwide, there are several guidelines for treatment of HIV (human immunodeficiency virus) infection/AIDS. Their innovation in the Czech Republic takes place every 2–3 years, taking into account the specificities of our population and health system. They are mainly based on nucleos(t)ide HIV reverse transcriptase inhibitors, along with HIV integrase inhibitors or boosted HIV protease inhibitors. Alternatively, non‑nucleoside HIV‑reverse transcriptase inhibitors, fusion inhibitors, and inhibitors of the entry into CD4+ cells are used. HIV infection in highly adherent people, thanks to combination antiretroviral therapy, has become a chronic, treatable but not yet curable disease. Fundamental improvement in the quality of life has been associated with reduction in morbidity caused by opportunistic infections, as well as mortality. The predicted life span of highly adherent HIV+ people at present barely differs from that of general population.

OBJECTIVES: The objective of this study was to define the natural genotypic variation of the HIV-1 integrase gene across Europe for epidemiological surveillance of integrase strand-transfer inhibitor (InSTI) resistance. METHODS: This was a multicentre, cross-sectional study within the European SPREAD HIV resistance surveillance programme. A representative set of 300 samples was selected from 1950 naive HIV-positive subjects newly diagnosed in 2006-07. The prevalence of InSTI resistance was evaluated using quality-controlled baseline population sequencing of integrase. Signature raltegravir, elvitegravir and dolutegravir resistance mutations were defined according to the IAS-USA 2014 list. In addition, all integrase substitutions relative to HXB2 were identified, including those with a Stanford HIVdb score ≥ 10 to at least one InSTI. To rule out circulation of minority InSTI-resistant HIV, 65 samples were selected for 454 integrase sequencing. RESULTS: For the population sequencing analysis, 278 samples were retrieved and successfully analysed. No signature resistance mutations to any of the InSTIs were detected. Eleven (4%) subjects had mutations at resistance-associated positions with an HIVdb score ≥ 10. Of the 56 samples successfully analysed with 454 sequencing, no InSTI signature mutations were detected, whereas integrase substitutions with an HIVdb score ≥ 10 were found in 8 (14.3%) individuals. CONCLUSIONS: No signature InSTI-resistant variants were circulating in Europe before the introduction of InSTIs. However, polymorphisms contributing to InSTI resistance were not rare. As InSTI use becomes more widespread, continuous surveillance of primary InSTI resistance is warranted. These data will be key to modelling the kinetics of InSTI resistance transmission in Europe in the coming years.
