BACKGROUND & AIMS: After a first Helicobacter pylori eradication attempt, approximately 20% of patients will remain infected. The aim of the current study was to assess the effectiveness and safety of second-line empiric treatment in Europe. METHODS: This international, multicenter, prospective, non-interventional registry aimed to evaluate the decisions and outcomes of H pylori management by European gastroenterologists. All infected adult cases with a previous eradication treatment attempt were registered with the Spanish Association of Gastroenterology-Research Electronic Data Capture until February 2021. Patients allergic to penicillin and those who received susceptibility-guided therapy were excluded. Data monitoring was performed to ensure data quality. RESULTS: Overall, 5055 patients received empiric second-line treatment. Triple therapy with amoxicillin and levofloxacin was prescribed most commonly (33%). The overall effectiveness was 82% by modified intention-to-treat analysis and 83% in the per-protocol population. After failure of first-line clarithromycin-containing treatment, optimal eradication (>90%) was obtained with moxifloxacin-containing triple therapy or levofloxacin-containing quadruple therapy (with bismuth). In patients receiving triple therapy containing levofloxacin or moxifloxacin, and levofloxacin-bismuth quadruple treatment, cure rates were optimized with 14-day regimens using high doses of proton pump inhibitors. However, 3-in-1 single capsule or levofloxacin-bismuth quadruple therapy produced reliable eradication rates regardless of proton pump inhibitor dose, duration of therapy, or previous first-line treatment. The overall incidence of adverse events was 28%, and most (85%) were mild. Three patients developed serious adverse events (0.3%) requiring hospitalization. CONCLUSIONS: Empiric second-line regimens including 14-day quinolone triple therapies, 14-day levofloxacin-bismuth quadruple therapy, 14-day tetracycline-bismuth classic quadruple therapy, and 10-day bismuth quadruple therapy (as a single capsule) provided optimal effectiveness. Noví parciální agonisté dopaminu slibují v terapii schizofrenie vyváženost mezi dobrou účinností a příznivým spektrem nežádoucích účinků. Jsou vhodnými léky k řešení metabolického syndromu způsobeného jinými druhy antipsychotik. Převedli jsme úspěšně pacienta z terapie 210 mg depotního olanzapinu na 4 mg brexpiprazolu. Brexpiprazol vykazuje velmi nízké hodnoty přerušení léčby z důvodu nežádoucích účinků, a je proto možnou alternativou v takových případech, kdy si nemůžeme vybrat mezi ostatními antipsychotiky, která máme k dispozici v injekcích.
New partial dopamine agonists promise a balance between good efficacy and a favorable spectrum of side effects in the treatment of schizophrenia. They are suitable drugs to solve the metabolic syndrome caused by other types of antipsychotics. We successfully transferred a patient from 210 mg depot olanzapine to 4 mg brexpiprazole. Brexpiprazole has very low discontinuation rates due to side effects and is therefore a possible alternative in cases where we cannot choose from other antipsychotics available for injection.

Brexpiprazol je nové antipsychotikum ze skupiny parciálních receptorových agonistů dopaminu (DRPA). Prezentujeme sedm případových studií, které jsou příkladem vhodného využití brexpiprazolu v komplexní léčbě schizofrenie. Kromě účinku na pozitivní a negativní příznaky je nejenom lékaři, ale i pacienty oceňován při léčbě brexpiprazolem efekt na redukci hmotnosti, absence sedace, akatizie či neuroleptické dysforie, sociální oživení a návrat ke studijní či pracovní činnosti. Kazuistiky dokládají, že brexpiprazol není zaměnitelný s jinými antipsychotiky a znamená obohacení léčby, kterou lze dosáhnout funkční remise pacientů a podstatného zlepšení kvality života.
Brexpiprazole is a new antipsychotic that belongs to dopamine receptor partial agonists (DRPA). We present seven case studies that represent prosperous use of brexpiprazole in complex treatment of schizophrenia. Not only psychiatrists but patients appreciate weight loss, absence of sedation, akathisia, neuroleptic dysphoria and return to society, study or working activities together with improvement of positive and negative symptoms with brexpiprazole treatment. The case studies documented that brexpiprazole is not exchangeable with other antipsychotics. Brexpiprazole meants enrichment of treatment, because patients might reach the functional remision of schizophrenia and substantial quality of life improvement.

BACKGROUND: For most of the >2000 CFTR gene variants reported, neither the associated disease liability nor the underlying basic defect are known, and yet these are essential for disease prognosis and CFTR-based therapeutics. Here we aimed to characterize two ultra-rare mutations - 1717-2A > G (c.1585-2A > G) and S955P (p.Ser955Pro) - as case studies for personalized medicine. METHODS: Patient-derived rectal biopsies and intestinal organoids from two individuals with each of these mutations and F508del (p.Phe508del) in the other allele were used to assess CFTR function, response to modulators and RNA splicing pattern. In parallel, we used cellular models to further characterize S955P independently of F508del and to assess its response to CFTR modulators. RESULTS: Results in both rectal biopsies and intestinal organoids from both patients evidence residual CFTR function. Further characterization shows that 1717-2A > G leads to alternative splicing generating <1% normal CFTR mRNA and that S955P affects CFTR gating. Finally, studies in organoids predict that both patients are responders to VX-770 alone and even more to VX-770 combined with VX-809 or VX-661, although to different levels. CONCLUSION: This study demonstrates the high potential of personalized medicine through theranostics to extend the label of approved drugs to patients with rare mutations.

BACKGROUND: We have recently shown that human epididymis protein 4 (HE4) levels correlate with the severity of cystic fibrosis (CF) lung disease. However, there are no data on how HE4 levels alter in patients receiving CFTR modulating therapy. METHODS: In this retrospective clinical study, 3 independent CF patient cohorts (US-American: 29, Australian: 12 and Irish: 19 cases) were enrolled carrying at least one Class III CFTR CF-causing mutation (p.Gly551Asp) and being treated with CFTR potentiator ivacaftor. Plasma HE4 was measured by immunoassay before treatment (baseline) and 1-6 months after commencement of ivacaftor, and were correlated with FEV1 (% predicted), sweat chloride, C-reactive protein (CRP) and body mass index (BMI). RESULTS: After 1 month of therapy, HE4 levels were significantly lower than at baseline and remained decreased up to 6 months. A significant inverse correlation between absolute and delta values of HE4 and FEV1 (r = -0.5376; P < .001 and r = -0.3285; P < .001), was retrospectively observed in pooled groups, including an independent association of HE4 with FEV1 by multiple regression analysis (β = -0.57, P = .019). Substantial area under the receiver operating characteristic curve (ROC-AUC) value was determined for HE4 when 7% mean change of FEV1 (0.722 [95% CI 0.581-0.863]; P = .029) were used as classifier, especially in the first 2 months of treatment (0.806 [95% CI 0.665-0.947]; P < .001). CONCLUSIONS: This study shows that plasma HE4 levels inversely correlate with lung function improvement in CF patients receiving ivacaftor. Overall, this potential biomarker may be of value for routine clinical and laboratory follow-up of CFTR modulating therapy.
