Stárnutí představuje perzistentní fyziologický děj variabilní ve své dynamice. Je procesem, který postupně a nezvratně vede k rozvoji jednoho z nejvýznamnějších geriatrických syndromů – křehkosti. Přítomnost chronických chorob či rizikových faktorů má schopnost tento rozvoj významně akcelerovat a prohlubovat tak funkční deficit. V tomto článku jsme se zaměřili na zmapování prevalence křehkosti u populace starší 50 let v České republice na základě dat projektu SHARE a identifikaci rizikových determinantů, které mohou rozvoj křehkosti negativně ovlivňovat.
Ageing is a persistent physiological process variable in its dynamics. It is a process that gradually and irreversibly leads to the development of one of the most important geriatric syndromes – frailty. The presence of chronic diseases or risk factors have the ability to significantly accelerate this development and thus exacerbate the functional deficit. In this article, we aimed to map the prevalence of frailty in the population over 50 years of age in the Czech Republic based on data from the SHARE project and to identify risk determinants that may negatively influence the development of frailty.

One of the most widely conserved hallmarks of aging is a decline in functional capabilities. Mobility loss is particularly burdensome due to its association with negative health outcomes, loss of independence and disability, and the heavy impact on quality of life. Recently, a new condition, physical frailty and sarcopenia, has been proposed to define a critical stage in the disabling cascade. Physical frailty and sarcopenia are characterized by weakness, slowness, and reduced muscle mass, yet with preserved ability to move independently. One of the strategies that have shown some benefits in combatting mobility loss and its consequences for older adults is physical activity. Here, we describe the opportunities and challenges for the development of physical activity interventions in people with physical frailty and sarcopenia. The aim of this article is to review age-related physio(patho)logical changes that impact mobility in old age and to provide recommendations and procedures in accordance with the available literature.

OBJECTIVE: The care processes directed towards institutionalized older people needs to be tailored on goals and priorities that are relevant for this specific population. The aim of the present study was (a) to describe the distribution of selected health determinants in a sample of institutionalized older adults, and (b) to investigate the impact on survival of such measures. DESIGN: Multicentre longitudinal cohort-study. SETTING: 57 nursing homes (NH) in 7EU countries (Czech Republic, England, Finland, France, Germany, Italy, The Netherlands) and 1 non-EU country (Israel). PARTICIPANTS: 3036 NH residents participating in the Services and Health for Elderly in Long TERm care (SHELTER) study. MEASUREMENTS: We described the distribution of 8 health determinants (smoking habit, alcohol use, body mass index [BMI], physical activity, social participation, family visits, vaccination, and preventive visits) and their impact on 1-year mortality. RESULTS: During the one-year follow up, 611 (20%) participants died. Overweight (HR 0.79; 95% C.I. 0.64-0.97) and obesity (HR 0.64; 95% C.I. 0.48-0.87) resulted associated with lower mortality then normal weight. Similarly, physical activity (HR 0.67; 95% C.I. 0.54-0.83), social activities (HR 0.63; 95% C.I. 0.51-0.78), influenza vaccination (HR 0.66; 95% C.I. 0.55-0.80) and pneumococcal vaccination (HR 0.76 95% C.I. 0.63-0.93) were associated with lower mortality. Conversely, underweight (HR 1.28; 95% C.I. 1.03-1.60) and frequent family visits (HR 1.75; 95% C.I. 1.27-2.42) were associated with higher mortality. CONCLUSIONS: Health determinants in older NH residents depart from those usually accounted for in younger and fitter populations. Ad hoc studies are warranted in order to describe other relevant aspects of health in frail older adults, with special attention on those institutionalized, with the ultimate goal of improving the quality of care and life.

Background: Influenza and pneumococcal vaccines have been proved to be effective and safe in preventing and controlling infection among elderly, reducing morbidity and mortality. However, some evidences raised health concerns related to these vaccinations. This study aims to identify prevalence and outcomes related to influenza and pneumococcal vaccinations in a large European population of frail old people living in nursing homes (NHs). Methods: We conducted a survival analysis of NH residents participating to the Services and Health for Elderly in Long-TERm project, a prospective cohort study collecting information on residents admitted to 57 NH in eight countries (Czech Republic, England, Finland, France, Germany, Italy, The Netherlands and Israel). Clinical and demographical data were collected using the international resident assessement instrument for long-term care facilities. Incident mortality was recorded during 1-year follow-up. A shared-frailty Cox regression model was used to assess the impact of vaccination status on mortality. Results: Mean age of 3510 participants was 84.6 years (SD = 7.7). In total, 81.7 and 27.0% received influenza and pneumococcal vaccination, respectively. Overall, 727 (20.7%) residents died during the follow-up period. After adjusting for potential confounders, which included age, sex, number of diseases, depression, cognitive and functional status, influenza (HR = 0.80; 95% CI 0.66-0.97) and the combination of influenza and pneumococcal vaccination (HR = 0.72; 95% CI 0.57-0.91), but not pneumococcal vaccination alone (HR = 0.52; 95% CI 0.25-1.06), were associated with a statistically significant reduction in mortality in respect of no vaccinations. Conclusion: In a population of older adult living in NH influenza and the combination of influenza and pneumococcal vaccination were associated with a reduction in all-cause mortality respect to no vaccination.

AIM: Frail older people typically suffer several chronic diseases, receive multiple medications and are more likely to be institutionalized in residential aged care facilities. In such patients, optimizing prescribing and avoiding use of high-risk medications might prevent adverse events. The present study aimed to develop a pragmatic, easily applied algorithm for medication review to help clinicians identify and discontinue potentially inappropriate high-risk medications. METHODS: The literature was searched for robust evidence of the association of adverse effects related to potentially inappropriate medications in older patients to identify high-risk medications. Prior research into the cessation of potentially inappropriate medications in older patients in different settings was synthesized into a four-step algorithm for incorporation into clinical assessment protocols for patients, particularly those in residential aged care facilities. RESULTS: The algorithm comprises several steps leading to individualized prescribing recommendations: (i) identify a high-risk medication; (ii) ascertain the current indications for the medication and assess their validity; (iii) assess if the drug is providing ongoing symptomatic benefit; and (iv) consider withdrawing, altering or continuing medications. Decision support resources were developed to complement the algorithm in ensuring a systematic and patient-centered approach to medication discontinuation. These include a comprehensive list of high-risk medications and the reasons for inappropriateness, lists of alternative treatments, and suggested medication withdrawal protocols. CONCLUSIONS: The algorithm captures a range of different clinical scenarios in relation to potentially inappropriate medications, and offers an evidence-based approach to identifying and, if appropriate, discontinuing such medications. Studies are required to evaluate algorithm effects on prescribing decisions and patient outcomes. Geriatr Gerontol Int 2016; 16: 1002-1013.

Neexistuje jedna obecná defi nice seniorské křehkosti. Někteří autoři ji popisují jako symptom, jiní jako komplexní syndrom, nebo jako nahromadění funkčních defi citů u seniora. Dle odborných publikací se křehkost vyskytuje až u 25 procent seniorů (Holmerová, 2007, s. 100). Rozvoj seniorské křehkosti může být mimo jiné podmíněn multimorbiditou, nežádoucími účinky léků, pohybovou inaktivitou, malnutricí, osamělostí či depresí (Kalvach, 2008, s. 122). Cílem práce bylo vyhledání a analýza odborných zahraničních článků týkající se křehkosti seniorů a následně získané poznatky využít v diagnostice NANDA International. Pro získání relevantních dat byly využity elektronické databáze Pubmed, Medline, ProQuest a Springer a vyhledávač Google. Byla využita specifi cká vyhledávací strategie. Na základě diagnostického algoritmu dle NANDA International byly identifi kovány diagnostické prvky a stanoveny ošetřovatelské diagnózy. Ošetřovatelská diagnostika dle NANDA International se jeví jako vhodná při poskytování ošetřovatelské péče u křehkých seniorů.
There is no general definition of frailty in the elderly. Some authors describe it as a symptom, others as a complex syndrome, or as an accumulation of functional defi cits in older adults. According to professional publications, frailty occurs in up to 25 percent of the elderly (Holmerová, 2007, p. 100). Developing of frailty in older adults may be subject to multimorbidity, undesired eff ects of medication, physical inactivity, malnutrition, loneliness or depression (Kalvach, 2008, p. 122). The aim of the article is to search for and analyse foreign scientifi c articles related to frailty in the elderly and to use gained knowledge in NANDA International diagnostics. To obtain the relevant data, electronic databases Pubmed, Medline, ProQuest, Springer and Google were used. Published articles concerning frailty in the elderly were found by a specifi c search strategy. Diagnostic elements were identifi ed and nursing diagnoses were determined according to the diagnostic algorithm by NANDA International. Nursing diagnostics of NANDA International seems appropriate in providing nursing care to frail older adults.
