BACKGROUND/AIM: To evaluate the effectiveness of curative (chemo)radiotherapy in patients with nasopharyngeal carcinoma and to identify prognostic factors influencing treatment outcomes. PATIENTS AND METHODS: We conducted a retrospective study of 73 consecutive patients, treated with definitive (chemo)radiotherapy from 2002 to 2019 (median stage III/IV 78%). The median total dose of radiotherapy achieved was 70 Gy. Concomitant chemotherapy was given to 82% of patients. RESULTS: The five- and ten-year locoregional controls were 73% and 72%, respectively; the five- and ten-year distant controls were 93% and 93%, respectively. The five- and ten-year overall survival rates were 46% and 34%, respectively. A multivariate analysis identified age, smoking, and the initial response to treatment as the strongest prognostic factors in predicting survival. CONCLUSION: Smoking ≤5 years before starting curative (chemo)radiotherapy for nasopharyngeal carcinoma was shown to be an independent negative prognostic factor for overall survival with a four-fold higher risk of death compared to non-smokers.
This study aimes to evaluate platelet and leucocyte indicators, such as the mean platelet volume, platelet distribution width, plateletcrit, white blood cell count, neutrophil to lymphocyte ratio in nasopharyngeal cancer patients and also to evaluate the relationship between these indicators and nasopharyngeal cancer with distant metastasis. The medical records of 118 patients diagnosed with nasopharyngeal cancer in our hospital between January 2006 and August 2015 were reviewed. The nasopharyngeal cancer group was further sub grouped according to the presence or absence of distant metastasis and TNM (tumour - T, node - N, metastasis - M) classification. A control group consisted of 120 healthy patients. The platelet and leucocyte values at the time of the initial diagnosis were recorded. Neutrophil to lymphocyte ratio and platelet distribution width values were significantly higher in the nasopharyngeal cancer group. But only platelet distribution width values were significantly higher in the nasopharyngeal cancer group with distant metastasis compared to the nasopharyngeal cancer group without distant metastasis. Neutrophil to lymphocyte ratio and platelet distribution width values may increase in nasopharyngeal cancer. But only the platelet distribution width values may give us an idea about the distant metastasis in nasopharyngeal cancer.

OBJECTIVES: Access to the pterygopalatine fossa is very difficult due to its complex anatomy. Therefore, an open approach is traditionally used, but morbidity is unavoidable. To overcome this problem, an endoscopic endonasal approach was developed as a minimally invasive procedure. The surgical aim of the present study was to evaluate the utility of the endoscopic endonasal approach for the management of both benign and malignant tumors of the pterygopalatine fossa. METHOD: We report our experience with the endoscopic endonasal approach for the management of both benign and malignant tumors and summarize recent recommendations. A total of 13 patients underwent surgery via the endoscopic endonasal approach for pterygopalatine fossa masses from 2014 to 2016. This case group consisted of 12 benign tumors (10 juvenile nasopharyngeal angiofibromas and two schwannomas) and one malignant tumor. RESULTS: No recurrent tumor developed during the follow-up period. One residual tumor (juvenile nasopharyngeal angiofibroma) that remained in the cavernous sinus was stable. There were no significant complications. Typical sequelae included hypesthesia of the maxillary nerve, trismus, and dry eye syndrome. CONCLUSION: The low frequency of complications together with the high efficacy of resection support the use of the endoscopic endonasal approach as a feasible, safe, and beneficial technique for the management of masses in the pterygopalatine fossa.

Za 15leté období od roku 1999-2013 bylo na Klinice dětské ORL vyšetřeno a následně operováno 11 chlapců pro juvenilní nazofaryngeální angiofibrom. Věkové rozpětí bylo 13-18 let. Primární léčba byla chirurgická, která následovala po provedené angiografii s embolizací přívodných cév tumoru. U 9 pacientů byl tumor odstraněn endoskopickou operací, u 2 pacientů pak zevním přístupem, laterální rinotomií. Reziduum bylo zjištěno u 1 pacienta při kontrolní MR. V pooperačním období byli pacienti pravidelně endoskopicky sledováni, po roce byla provedena radiologická kontrola, CT nebo MR. Celkové průměrná délka sledování byla 3,2 roky.
Eleven boys have been examined for juvenile nasopharyngeal angiofibroma and subsequently operated on at the Child ORL Clinic in the 15-year period from 1999 till 2013. The age span was 13-18 years. Surgery was the primary treatment, which followed after angiography and embolization of supplying tumor vessels. The tumor was removed in nine patients by endoscopic operation, and in two patients an external approach for lateral rhinotomy was used. A residuum was detected in one patient during a control MR. The patients were regularly followed up in the postoperative period and a radiological examination, CT or MR was performed one year after the surgery. The overall period for which the patients were followed lasted 3.2 years on the average.

BACKGROUND AND PURPOSE: To present the feasibility and results of accelerated radiotherapy with concomitant boost technique (69.5 Gy/5 weeks) in the treatment of locally advanced head and neck cancer. PATIENTS AND METHODS: A total of 65 patients were treated between June 2006 and August 2009. The distribution of clinical stages was as follows: II 11%, III 23%, IV 61%, and not defined 5%. RESULTS: The median follow-up was 30.5 months. The treatment plan was completed in 94% of patients. Patients were treated using the conformal or intensity-modulated radiotherapy (IMRT) technique. The median overall treatment time was 37 days (13-45 days). The mean radiotherapy dose was 68.4 Gy (16-74 Gy). Overall survival was 69% after 2 years. Disease-free survival was 62% after 2 years. Acute toxicity ≥ grade 3(RTOG scale) included mucositis (grade 3: 42.6%), pharynx (grade 3: 42.3%), skin (grade 3: 9.5%), larynx (grade 3: 4%), while late toxicity affected skin (grade 3: 6.25%) and salivary glands (grade 3: 3.7%). CONCLUSION: Accelerated radiotherapy with concomitant boost technique is feasible in patients with locally advanced head and neck cancer, has an acceptable toxicity profile, and yields promising treatment results.

Small cell neuroendocrine carcinoma, also known as oat cell carcinoma is a frequently encountered bronchogenic neoplasm. Neuroendocrine carcinomas of the head and neck are rare aggressive tumors and pose a diagnostic and management challenge. Differentiation from conventional nasopharyngeal carcinoma is difficult due to morphological characteristics and small biopsy specimen. Distinguishing is very important because of difference in natural history ,prognosis and treatment among these neoplasms. In this article we present an unusual case of 51 year old man with small cell neuroendocrine carcinoma of nasopharynx, who was treated with radiotherapy and achieved complete tumor remission. Subsequently he developed local recurrence and liver metastasis and succumbed to the disease.

Epipharyngeal angiofibroma appears as the most difficult lesion in the structure of benign tumors of ear nose throat organs. Very low occurrence of frequency of this pathology in females makes the interesting the case of surgical treatment of “atypical angiofibroma” diagnosed in a young girl.
