Systémová sklerodermie (SSc) je vzácné a velmi závažné revmatické onemocnění, pro které dosud neexistuje účinná univerzální léčba. Postižení orgánů výrazně zvyšuje úmrtnost těchto nemocných, nicméně i samotné postižení kůže a muskuloskeletálního systému vede k vyšší morbiditě, ztrátě funkce a snížení kvality života. Údaje z literatury stejně jako naše dosavadní zkušenosti naznačují, že intervence týkající se fyzické aktivity tvoří neodmyslitelnou a nedílnou součást léčby pacientů se SSc. Většina existujících studií, jež se zabývaly nefarmakologickou léčbou pacientů se SSc, je však limitována jejich nízkou metodologickou kvalitou, nedostatečným počtem pacientů, absencí kontrolní skupiny či nedostatečnou dobou sledování (tzv. „follow-up“) či jen krátkodobou intervencí. Účelem tohoto přehledového článku je představit významné práce, jež se zabývaly bezpečností a přínosem různých nefarmakologických přístupů, které se dostávají do popředí zájmu v oblasti klinického výzkumu u SSc. Systemic sclerosis (SSc) is a rare and very severe rheumatic disease for which no effective universal therapy exists. Organ manifestations significantly increase the mortality of these patients, nevertheless, the involvement of the skin and the musculoskeletal system also leads to higher morbidity, loss of function, and decreased quality of life. Literature data, as well as our experience, suggest that physical activity interventions are an inherent and integral part of the treatment of SSc patients. However, most existing studies dealing with non-pharmacological treatment of SSc patients are limited by their suboptimal methodological quality, insufficient number of patients, absence of a control group or lack of follow-up or a short duration of intervention. The purpose of this review article is to present important publications that deal with the safety and benefits of various non-pharmacological approaches that are at the forefront of clinical research interest in SSc. AIM: Using functional scales and face video analysis, changes in central facial paresis are monitored in patients with stroke after orofacial therapy and correlations between changes in mimicry, mental function and overall quality of life of patients after stroke are made. MATERIALS AND METHODS: A prospective randomized study of patients after stroke with facial paresis. The functional status of the experimental group of 50 cases with orofacial regulation therapy and 49 control cases without mimicry therapy is observed after four weeks of rehabilitation. RESULTS: Changes in mimicry functions evaluated by the House-Brackmann Grading System (HBGS) clinical range and using 2D video analysis of the distance between the paretic corner of the mouth and earlobe at rest and during smiling were statistically better in the experimental group than in controls. Changes in mental function - depression using Beck Depression Inventory and changes in the quality of life using Bartle index and modified Rankin score (scale) were significantly greater in the experimental group. There was a very close relationship between the changes in mimicry, mental state and overall quality of life according to the Spearman correlative coefficient. CONCLUSION: Orofacial rehabilitation therapy for patients with paresis after stroke has a significant influence on the adjustment of mimicry, mental functions and overall quality of life after 4 weeks of treatment.

Poruchy funkcí při centrální obrně lícního nervu lze ovlivnit pomocí cílených rehabilitačně-logopedicko-ošetřovatelských postupů majících kladný efekt nejen na mimiku, ale i na řeč, případně na úpravu dysfagie a celkový psycho-somaticko-sociální stav nemocného. Cílem článku je uvést přehled orofaciálních rehabilitačních postupů, které je možno použít při léčbě následků centrálních paréz lícního nervu.
Functional disorders incentral paresis of facial nerve may be influenced by pointed rehabilitation logopedic-nursing procedures which favorably influence facial expression as well as speech or to adjustment of dysphagia and general psychosomatic social state of the patient. The purpose of the paper was to present a survey of orofacial rehabilitation procedures, which can be used in therapy of consequences of central pareses of facial nerve.
U pacientů po cévní mozkové příhodě je paréza n. facialis jedním ze základních symptomů. Funkčně se paréza n. facialis projevuje od mírných, nenápadných stadií, až po těžká znetvořující stadia, která se vyznačují poruchou mimiky a ve svém důsledku i poruchou ostatních orofaciálních funkcí, jako je porucha řeči-dysartrie, event. porucha příjmu potravy. Jak významný vliv má centrální paréza n. facialis u pacientů po CMP na orofaciální funkce je prezentováno formou studie případů a kontrol.
In patients having undergone a brain vascular event n. facialis paresis belongs to basic symptoms. The functional aspects of n. facialis paresis include mild inapparent stages up to severe defacing stages which are characterized by mimic disorders and consequently defects of other orofacial functions such as speech disorder – dysarthria or even disorder of food intake. The various consequences of central n. facialis paresis in patients after CMP on orofacial functions are presented in a case-control study.

We describe a case of slowly developing Foix-Chavany-Marie syndrome associated with distinctive bilateral cortical atrophy of frontal lobes in a 59-year old woman. The neurophysiological mechanism and differential diagnosis concerning the unusually slow progression are discussed.

The care for mentally ill patients with combined impairment deals with orofacial dysfunctions of the mentally ill, especially children suffering from Down syndrome and cerebral palsy. Objective of the study was to assess urine fluoride excretion in disabled patients with orofacial dysfunctions in relation to prevention of dental caries. The urine fluoride concentrations in disabled people were analysed to assess their fluoride exposure and possibly preventive contribution of fluoride intake. The patients for the study were recruited from the clients of three day-stay establishments from two regions in the Czech Republic. All the clients from the selected establishments were examined except for a small number of clients, whose statutory representatives (usually parents) did not agree with oral examination. Signed informed consents were obtained from the statutory representatives of all the examined patients. Totally 95 mentally disabled patients were examined with the mean age of 11.8 years (ranging from 6 months to 28 years) for orofacial dysfunctions using the Castillo Morales concept. Fluoride concentrations were measured by fluoride-selective electrode. The urine density was measured by means of hydrostatic weighing. The above-mentioned analytical methods were chosen since urine density reflects the overall liquids intake by the person and the fluoride excretion by the urine reflects its intake from all environmental sources. Atypical swallowing act was found in 98% of the cohort. Mean urine density was 1.038 g/ml ranging from 1.002 to 1.069 (77 samples). The mean urine fluoride concentration was 0.816 mg/l ranging from 0.045 to 3.225 (82 samples). Orofacial dysfunctions decrease the quality of life. Among other aspects, they influence liquids intake due to an impaired swallowing act. Moreover, patients are often dependent on the assistance of the other persons.
