Background: A new silver-based dressing has been designed to reduce surgical sited infections. Patients and Methods: A prospective multicenter observational study was conducted from January 2020 to October 2021. Patients with and without silver-based dressing after surgical incision were observed and their data analyzed. The study aimed to assess the incidence of incisional surgical site infection and primary healing after general surgery procedures. Results: Overall, 218 patients with silver-based (n = 109) and conventional silver-free dressing (n = 109) were analyzed. Surgical site infection (SSI) and primary incision healing were reported in 10 (9.2%) versus 21 (19.3%) (p = 0.037) and in 95 (87.2%) versus 86 (78.9%) (p = 0.107) patients treated with and without silver-based dressing, respectively. Conclusions: Silver-based dressing demonstrated a lower incidence of incisional SSI and improved primary healing in comparison with patients in whom conventional non-silver-based dressing has been used.
Older people often suffer from different types of ulcers, with the most prevalent being chronic leg ulcers (CLUs) and diabetic foot ulcers. There are major issues in the current medical approach because these ulcers are hard to heal, and, in the case of CLUs, very painful and with a high frequency of relapse. Older people complain of pain more frequently than young people, frequently due to a combination of painful chronic wounds with other comorbidities (e.g. arthritis, peripheral arterial disease, vertebrogenic pain syndrome). However, chronic pain appears to be managed better by older people because the pain sensitivity is downregulated and the pain threshold is higher in older people. Pain management of chronic wounds is often insufficient, especially in older individuals. It is highly important to use non-traumatic wound dressings and pay attention to patients' feelings and fears because pain in chronic ulcers can impair wound healing. Key factors include good preparation for dressing change and adequate analgesia, ideally a combination of topical and oral agents.

Wound dressings with silver have been shown to be cytotoxic in vitro. However, the extrapolation of this cytotoxicity to clinical settings is unclear. We applied dressings with various forms of silver on porcine skin ex vivo and investigated silver penetration and DNA damage. We assessed antimicrobial efficacy, cytotoxicity to skin cells, and immune response induced by the dressings. All dressings elevated the DNA damage marker γ-H2AX and the expression of stress-related genes in explanted skin relative to control. This corresponded with the amount of silver in the skin. The dressings reduced viability, induced oxidative stress and DNA damage in skin cells, and induced the production of pro-inflammatory IL-6 by monocytes. The oxidative burst and viability of activated neutrophils decreased. The amount of silver released into the culture medium varied among the dressings and correlated with in vitro toxicity. However, antimicrobial efficiencies did not correlate strongly with the amount of silver released from the dressings. Antimicrobial efficiency and toxicity are driven by the form of silver and the construction of dressings and not only by the silver concentration. The damaging effects of silver dressings in ex vivo skin highlight the importance of thorough in vivo investigation of silver dressing toxicity.

Chronické rány představují heterogenní skupinu ran, u kterých fyziologicky probíhající proces hojení je narušen, neprobíhá v daném sledu a časovém rámci, formuje se méněcenná tkáň, případně se tvoří nezhojitelný chronický defekt. Každá chronická rána má svoji etiopatogenezi, klinický obraz a zpravidla je provázena subjektivními symptomy. Nejčastěji pacienty uváděným steskem je bolest, která výrazně snižuje kvalitu života nemocného a současně má nepříznivý dopad i na osoby nejbližší. Bolest je třeba u každého jedince individuálně posoudit, v žádném případě ji nelze přehlížet. Dlouho trvající bolest chronické rány nemocného vyčerpává, vyvolává stresovou situaci, zasahuje nepříznivě do imunitního systému organismu, což má negativní dopad na hojení. V rámci komplexního přístupu k nemocnému je třeba určit příčinu bolesti, která může být podmíněna etiologií onemocnění, ztrátou tkáně, přidruženou infekcí, zánětlivými změnami v okolí, ale může mít souvislost s nevhodným způsobem ošetření či výběrem krytí. Léčba chronických ran, kterou se donedávna zabývali jen někteří specialisté, se stala v dnešní době záležitostí multidisciplinární a multiprofesní. V mnoha státních i soukromých zdravotnických zařízeních vznikla Centra léčby chronických ran, stejně jako Centra léčby bolesti.
The chronic wounds are the heterogenous group of wounds, in which is the physiological processs healing delay, wounds repair does not take place in a given sequence and timeframe, healing is slow, it is shaped inferior tissue, eventually is created nonhealing wound. Each wound has a specific etiopathogenesis, clinical picture and is usually accompanied with subjective symptomps.The most patients report the significant pain, which worsens their quality of life and has adverse effect on the closest person. The pain needs evaluaten for each individual, in any case it can not be ignored. Long-lasting pain of chronic wounds patient exhaust, inducesstresssituation, adversely affectsthe immune system of the organism, which has a negative impact on healing. In the holistic approach to the patient, it is necessary to determine the cause of illness, that may be subject to the etiology of disease, tissue loss, associated infections, inflamatory changes in the neighborhood, but may be related to inapropriate treatment method or by selecting of wound dressing. Treatment of chronic wounds, which recently dealth with only some specialists, has become nowadays a matter multidisciplinary. In many state and private facilities, are „Pain management center“ and „Wound healing center“.

Léčba chronických ran zaznamenala v posledních desetiletích významný pokrok a standardně se začaly používat nové terapeutické postupy i materiály. Orientace v dané oblasti je pro lékaře i sestry náročná a vyžaduje užší specializaci. Jako jedna z možných cest se ukazuje vyčlenění lékařů i sester, kteří se touto problematikou budou na pracovišti zabývat a budou mít prostor ne jenom pro realizaci, ale i pro další vzdělávaní. Autoři prezentují aktuální poznatky a své zkušenosti z nově zřízené ambulance pro léčbu chronických ran na Chirurgické klinice LF UK a FN v Hradci Králové, jakož i zkušenosti z kurzů, které pořádají pod záštitou Aesculap Akademie v Hradci Králové.
The process of healing chronic wounds made a great progress in recent years. Today, we have access to new materials, methods and wound management techniques. It is important for doctors and nurses to be oriented in wound healing practices. It is necessary to have practitioners from both professions specializing in this specific field and to create a special group for chronic wound healing in each hospital. The authors are presenting their experiences and knowledge from a new ward for chronic wounds started within the Surgical clinic in the Hradec Kralove University Hospital as well as their experiences from courses arranged by them under the aegis of Aesculap Academy in Hradec Králové.
