BACKGROUND: This pilot study aimed to investigate how fixed orthodontic appliances simultaneously applied on the upper and lower arches affect the oral environment in the medium term. METHODS: The oral status of 30 orthodontic patients was evaluated using the number of decay-missing-filled teeth (DMFT), plaque (PI), and gingival indices (GI) before bonding of fixed orthodontic appliances (T0) and during the therapy (T1). Besides, the gingival crevicular fluid (GCF) and a dental plaque were collected. Samples were analyzed for selected Candida sp. and for 10 selected oral bacteria using mass spectroscopy and multiplex polymerase chain reaction, respectively. RESULTS: In 60% of patients, deterioration of the oral status (demonstrated by the increase in PI) was recorded (p < 0.05). Moreover, the changes in PI correlated with those of GI (p < 0.001). At the T1 time point, the mean representation of Actinomyces sp. in the total prokaryotic DNA in GCF and dental plaque of individual patients increased compared to T0 (p < 0.05). The probability of finding any of the 7 selected periodontal bacteria combined with Candida sp. was 10 times higher in patients in whom PI deteriorated between T0 and T1 (p < 0.01). CONCLUSIONS: Changes in the oral microbial diversity and an increase in PI were observed in the medium term after bonding of orthodontic appliance. Our study highlights the importance of a complex approach in this type of research as the association between clinical characteristics and combined microbial parameters is higher than when evaluated separately.
Externí cervikální invazivní resorpce (ECIR) vzniká na podkladě působení odontoklastických buněk, které resorbují dentin a cement. Nejčastěji vzniká v cervikální části zubu a ve velkém procentu případů se invazivně šíří zubními tkáněmi. Ve druhé části přehledovém článku jsou rozebrány možnosti prevence, diagnostika a terapie. Léčba, pokud je indikována, by měla vést ke kompletnímu odstranění resorptivní tkáně a rekonstrukci defektu vhodným materiálem.
External cervical invasive resorption (ECIR) is the loss of dentin and cement as a result of action of odontoclastic cells. Most commonly it begins at the cervical part of tooth and have aggressive and invasive nature. The prevention, diagnostic and treatment modalities are reviewed in this article. The treatment, if indicated, should aim to complete removal of resorptive tissue and reconstruction of resorptive defect by suitable material.

The aim of the project is to analyze possible factors in appearance of unexpected complications associated with the type of wire used and placement selection in patients in retention phase after orthodontic treatment. In this in-vitro study, deformation and stress distribution in the retainer wires as well as on the interphase of tooth and alveolar bone will be assessed by the finite element method (FEM), based on mathematical model generated from CBCT scans. Five different frequently used retainer wires and 4 different sites of retainer placement will be evaluated under the influence of symmetrical as well as asymmetrical horizontal and vertical forces simulating the anterior vector of occlusal forces and forces generated during chewing on incisors. The differences will be compared and recommendations about the selection of wire and site of placement will be concluded.
Projekt analyzuje možné příčiny vzniku nežádoucích komplikací vázané na použité dráty a jejich umístění u pacientů v retenční fázi po ortodontické léčbě. Studie je navržena jako in-vitro studie, ve které bude na základě matematického modelu z CBCT pomocí metody konečných prvků (MKP) zjišťována deformace a distribuce napětí generovaného v drátu fixního retaineru a na rozhraní zubu a alveolární kosti, a to při použití různých typů drátů (5 typů běžně užívaných drátů) a při různém umístění retaineru na povrch zubu (4 různá umístění drátu). Tyto varianty umístění retaineru a typu retainerového drátu budou zatěžovány symetrickými a stranově asymetrickými horizontálními a vertikálními silami, které simulují anteriorní komponentu okluzních sil a zátěž při ukusování na řezácích. Rozdíly budou srovnávány a bude navrženo doporučení pro typ drátu a jeho umístění na zuby.

Resorpce apexu kořene je běžným jevem u ortodonticky léčených pacientů. Autoři se ve svém sdělení zaměřují na popis vzniku tohoto jevu, jeho formy a především na faktory, které jeho vznik a charakter ovlivňují. Resorpce kořene je někdy vnímána jako závažný následek ortodontické terapie, nicméně pokud nepřesáhne určitou velikost a není klinicky významná, nemusíme ji vnímat jako faktor, který znehodnocuje výsledek ortodontické léčby.
Apical root resorption is a common finding in orthodontically treated patients. The aim of the authors is to describe this phenomenon, different forms and especially factors influencing the etiology and type of root resorption. Root resorption is often considered to be a severe consequence of orthodontic therapy, however when it does not exceed certain limits and clinical significance, we no not have to regard it as a factor that deteriorates the outcome of orthodontic treatment.

Úvod a cíl studie: Cílem této prospektivní studie bylo zjistit, jaká je závislost míry zevní resorpce apexu kořene na velikosti aplikované ortodontické síly během intruzního pohybu zubu. Současně byla sledována závislost resorpce na rozsahu intruze. Metody: Do souboru studie bylo zahrnuto celkem 34 premolárů u 17 pacientů. Na kontralaterální zuby jednoho jedince bylo působeno malou intruzní silou – 50 cN (light force, LF) a velkou intruzní silou – 150 cN (heavy force, HF). Sledované parametry byly posuzovány v čase před experimentem (T0) a po šestiměsíční intruzi zubů (T1). Následně pak byly lineární rozměry zubu měřeny na extrahovaných premolárech in situ. Hodnotily celkem dva klinické parametry, čtyři parametry na Cone Beam CT a dva parametry na extrahovaných zubech. Rozdíly mezi skupinami byly vyhodnoceny statisticky dvouvýběrovým t–testem. Výsledky: Velikost intruze byla signifikantně větší při aktivaci velkou silou, a to téměř trojnásobná ve srovnání s aktivací malou silou (4,64 mm oproti 1,53 mm). V obou skupinách došlo ke zkrácení délky zubu vestibulárně, o 0,31 mm ve skupině LF a o 0,67 mm ve skupině HF. Rozdíl mezi oběma skupinami však nebyl signifikantní (p = 0,06). U obou skupin jsme zaznamenali signifikantní zkrácení délky zubu palatinálně o 0,49 mm a 0,75 mm, nicméně ani zde nebyl rozdíl mezi skupinami signifikantní (p = 0,37). Při porovnání změn délky kořenů v obou skupinách bylo nalezeno také signifikantní zkrácení délky vestibulárního kořene o 0,44 mm a 0,74 mm (p = 0,038 a p = 0,016). Signifikantní rozdíl mezi skupinami však nalezen nebyl (p = 0,18). Závěry: Velké intruzní síly způsobily ve srovnání s malými intruzními silami větší rozsah intruze. V průběhu aplikace intruzní síly velikosti 50 cN a 150 cN došlo k signifikantnímu zkrácení délky kořenů zubů. Nebyla však prokázána závislost apikální resorpce kořenového hrotu na velikosti aplikované intruzní síly ani na rozsahu intruzního pohybu.
Introduction, aim: The aim of this prospective study was to find a relationship between the external apical resorption and magnitude of orthodontic force during tooth intrusion. Relationship between the amount of intrusion and the extent of root resorption was investigated as well. Methods: The sample included 34 premolars in 17 patients. The light force (LF – 50 cN) and heavy intrusion force (HF – 150 cN) were applied on contra-lateral teeth. The measurements were performed at time before treatment (T0) and after six-month intrusion (T1). The measurements were registered also on the extracted premolars at the end of experiment. Two clinical parameters, four Cone Beam CT parameters and two parameters on extracted teeth were evaluated. The differences were statistically processed with two-sample t-test. Results: The extent of intrusion was significantly greater during heavy force activation; it was almost three times higher in comparison with light force (4.64 mm compared to 1.53 mm). The tooth was shortened at vestibular site in both groups (0.31 mm in LF and 0.67 mm in HF). However, the difference between both groups was not statistically significant (p = 0.06). The significantly shortened tooth was at palatal site in both groups (0.49 mm and 0.75 mm) and the difference was also not significant (p = 0.37). In comparison of root lengths, they were significantly shorter at vestibular site (0.49 mm and 0.75 mm), but the difference between groups was not significant (p = 0.18). Conclusion: Heavy forces lead to a more extensive intrusion in comparison with light forces. Significant root length shortening occurs after the intrusion with both 50 cN and 150 cN force. The relationship between the amount of intrusion force or the extent of intrusion and the apical root resorption was not confirmed.

Cíl: Cílem retrospektivní studie bylo zhodnotit výskyt apikálních resorpcí kořenů zubů po léčbě pevným ortodontickým aparátem a zjistit, zda závisí na pohlaví, věku, Angleově třídě, délce a typu ortodontické léčby. Materiál a metodika: Do retrospektivní studie bylo zařazeno 150 náhodně vybraných pacientů (75 mužů a 75 žen) v retenční fázi po léčbě pevným ortodontickým aparátem. Měření byla prováděna na ortopantomogramech a bočních kefalometrických snímcích po prosvícení na negatoskopu, zhotovených před léčbou a po léčbě. Pro hodnocení byla použita standardizovaná metodika podle autorů Linge a Linge. Výsledky: Apikální resorpci jsme nezaznamenaly u 32 % pacientů. Pokud se resorpce vyskytovaly, šlo většinou (38,7 % pacientů) o resorpce typu nepravidelného povrchu kořene. Závažné resorpce přesahující třetinu délky kořene jsme registrovaly u 2 (1,3 %) pacientů (u 1 dívky a 1 muže). Nezjistily jsme statisticky významnou závislost výskytu apikálních resorpcí na pohlaví, Angleově třídě a věku pacienta při zahájení léčby pevným ortodontickým aparátem. Apikální resorpce se však významně častěji vyskytovaly u pacientů s extrakční ortodontickou léčbou (p = 0,012). Resorpce se vyskytovaly méně často u pacientů, u kterých byla délka léčby kratší než 22 měsíců (p = 0,028). Statisticky významné rozdíly byly patrné zejména při porovnání skupin pacientů s celkovou délkou léčby kratší než 22 a delší než 34 měsíců (p = 0, 003). Závěr: Apikální resorpce kořenů horních stálých řezáků se vyskytovaly u 68 % pacientů léčených pevným ortodontickým aparátem. Extrakční ortodontickou léčbu a narůstající celkovou délku léčby je možné považovat za rizikové faktory pro vznik apikálních resorpcí.
Aim: The aim of the present study was to assess the incidence of apical root resorption in patients treated with fixed orthodontic appliances and find out whether the frequency of their occurrence depends on sex, patient's age, Angle class malocclusion, total length and type of orthodontic treatment. Material and methods: The retrospective study comprised 150 randomly selected patients (75 males and 75 females) who had completed treatment with fixed orthodontic appliances. The measurements were performed on panoramic radiographs and lateral cephalograms taken before and after treatment with the fixed orthodontic appliance. Classification after Linge and Linge was used for the assessment of the apical root resorption. Results: No signs of apical root resorption were recorded in 32% patients and the irregular root contour in 38.7% patients. The apical root resorption exceeding one-third of the original root length was registered in 2 (1.3%) patients (1 female and 1 male). No significant differences in the incidence of apical root resorption were found in the sex, Angle class malocclusion and patient´s age. The incidence of apical root resorption was significantly higher in patients with extractions (p = 0.012). The root resorption was significantly less frequent in patients with total length of treatment shorter than 22 months (p = 0.028). Significant differences were observed between the groups of patients with length of treatment shorter than 22 months and longer than 34 months (p = 0.003). Conclusions: The signs of apical root resorption of upper permanent incisors were observed in 68 % patients after fixed orthodontic treatment. It is possible consider the extraction orthodontic treatment and longer length of treatment as risk factors for apical root resorption.

Úvod: Demineralizace skloviny projevující se jako bílé skvrny a vznikající v důsledku léčby pevným ortodontickým aparátem jsou významným klinickým problémem. Cílem prezentované práce bylo zhodnotit přítomnost těchto skvrn u pacientů léčených pevným ortodontickým aparátem a zjistit, zda četnost jejich výskytu závisí na pohlaví, věku pacienta a délce léčby. Materiál a metodika: Do retrospektivní studie bylo zařazeno 106 náhodně vybraných pacientů (35 mužů a 71 žen), kteří absolvovali léčbu pevným ortodontickým aparátem. Přítomnost bílých skvrn byla hodnocena před nasazením a těsně po sejmutí pevného ortodontického aparátu. Pro vizuální posouzení závažnosti demineralizací skloviny byla použita klasifikace podle Gorelicka. Výsledky: Známky demineralizace skloviny po sejmutí pevného ortodontického aparátu jsme zaznamenali u 44 (41,5 %) pacientů, resp. u 23,9 % zubů. Muži byli postiženi stejně často jako ženy (p > 0,05), ale počet zubů, u nichž se vyskytla bílá skvrna, byl u mužů významně vyšší než u žen (p < 0,05). Nejvyšší výskyt demineralizací jsme zjistili na prvních molárech a horních řezácích a nejnižší výskyt na dolních řezácích. Prokázali jsme statisticky významný rozdíl ve výskytu demineralizací skloviny v závislosti na věku pacientů při zahájení léčby pevným aparátem (p < 0,01). Závislost na celkové délce léčby však nebyla nalezena (p > 0,05). Závěr: Výskyt demineralizací skloviny u pacientů léčených pevným ortodontickým aparátem je vysoký, zejména u mladších pacientů. Při zahájení léčby pevným aparátem je proto zapotřebí pečlivě zvažovat věk pacienta. Komplexní péče o demineralizace skloviny zahrnuje kromě preventivních opatření metody podporující remineralizaci existujících lézí.
Introduction: Enamel demineralization manifesting itself by white spot lesions and occurring as a result of treatment with fixed orthodontic appliances is a significant clinical problem. The aim of the present study was to assess the incidence of white spot lesions in patients treated with fixed orthodontic appliances and find out whether the frequency of their occurrence depends on sex, patient's age and treatment duration. Material and methods: The retrospective study comprised 106 randomly selected patients (35 males and 71 females) who had completed treatment with fixed orthodontic appliances. The presence of white spot lesions was assessed before placement and just after removal the fixed orthodontic appliance. Classification after Gorelick was used for the visual assessment of the severity of enamel demineralization. Results: Signs of enamel demineralization after the removal of fixed orthodontic appliances were recorded in 44 (41.5%) patients, i. e. 23.9% of teeth. The frequency of occurrence of enamel demineralization was the same in males as in females (p > 0.05) but the number of teeth with white spot lesions in males was significantly higher than that in females (p < 0.05). The highest incidence of demineralization was found in the first molars and maxillary incisors, the lowest in the mandibular incisors. The statistically significant difference in the incidence of enamel demineralization depending on the patients' age at the start of treatment with fixed appliances was proven (p < 0.01). However, dependence on the total treatment duration was not found (p > 0.05). Conclusion: The enamel demineralization incidence in patients treated with the fixed orthodontic appliances was high, particularly in younger patients. Therefore, patient's age should be considered carefully before fixed appliance orthodontic treatment. Besides preventive measures, complex enamel demineralization care includes methods for remineralization of existing lesions.

Aim. This study evaluated the behavior of oral epithelial cells in contact with orthodontic appliances. Methods. Oral epithelial cells of clinically normal buccal mucosa were obtained by liquid-based exfoliative cytology from 22 orthodontic patients. The following regions were evaluated: a) oral mucosa exposed to friction from orthodontic brackets; b) oral mucosa exposed to friction from the tube on the orthodontic band, and c) oral mucosa not exposed to friction (control area). Nuclear (NA) and cytoplasmic (CA) areas and NA/CA ratio were assessed by an image analysis system. Cell morphology and cellularity were also analyzed by Papanicolaou technique. Results. The NA of the cells in contact with orthodontic brackets and bands were smaller than the control area. Cells in contact with the brackets showed the greatest reduction in CA in comparison with the cells subjected to friction from the tube, and the control group. Smears classified as type I predominated in all regions analyzed, although type II were predominant, together with superficial epithelial cells, mainly in the oral mucosa in contact with the band tube. Conclusion. Preclinical alterations in the epithelial cells of oral mucosa, caused by orthodontic appliances, are reduction in NA, increase in cell keratinization and inflammatory features, especially in the band tube area.
