Řada analytů krevního séra vykazuje v průběhu COVID-19 změny, dovolující predikci průběhu a závažnosti choroby Jde zejména o ALT, LD, kreatinin a eGFR, prokalcitonin, albumin, kardiální markery a lipidy. Nový přístup k harmonizaci laboratorních výsledků je transformován do podoby nové Směrnice pro in vitro diagnostika IVDR 746. Teoretickými podklady jsou normy ISO 17511:2020 a ISO 21151:2020. Cílem sdělení je zhodnotit stav harmonizace u biochemických analytů, schopných monitorovat průběh a závažnost průběhu COVID-19. Je konfrontovaný reálný stav harmonizace se stavem ideálním, který má být v duchu IVDR 746 v krátké době dosažen. Problémy nedostatku úrovně harmonizace jsou nalézány v různé míře prakticky u všech jmenovaných analytů.
During COVID-19 many serum analytes show significant concentration changes allow to make prediction on severity and development of illness. Namely results of ALT, LD, albumin, creatinine and eGFR, cardiac markers, procalcitonin, lipids. New approach to harmonization in clinical laboratories based on IVDR 746 and derived from ISO 17511:2020 and ISO 21151:2020 was assessed for these analytes. Aim of communication deals with real statement in harmonization and its problems in these analytes. WE found lot of harmonization problems in all introduced analytes.

BACKGROUND: Definition of Pseudomonas aeruginosa (Pa) microbiological status is essential for patients' inclusion in clinical trials. The aim of this study was to agree on the definitions of Pa infection status for initial infection, eradication and chronic infection to be used in clinical trials and to propose additional future study areas. METHODS: An exhaustive literature search was performed. The clinimetric properties of different definitions of Pa microbiological status were evaluated. RESULTS: Historical studies have mostly used culture-based definitions, although some have also involved complementary anti-Pa antibodies. Clinimetric analysis showed great variability in the definitions used, leading to differences in reliability, validity, responsiveness to treatment and correlation with outcome measures. Use of serology for initial Pa infection and successful Pa eradication introduced a greater level of complexity as antibody tests are not standardised. Moreover, the chronology of the immune response to Pa antigenic determinants was not completely clear. Chronic Pa infection was characterized by high levels of antibodies and good concordance between culture results and serology. CONCLUSIONS: Microbiological monitoring, regular sampling from the airways and standardization of culture methods remain essential requisites for microbiological definitions. Despite limitations, serology should be incorporated in the definitions of initial infection and eradication used in clinical trials to better classify patients at enrolment, mainly in non-expectorating children. This requires standardization of serological testing.

Cíl: Upozornit na nutnost časné diagnostiky zánětlivých revmatických příčin bolestí zad, které pro svůj vzácný výskyt bývají u pacientů s bolestmi zad snadno přehlédnuty. Vlastní text: Do skupiny zánětlivých revmatických onemocnění zad – spondyloartritid (SpA) patří autoimunitní zánětlivá onemocnění, kde zánětem je postižen axiální skelet, kořenové a periferní klouby s převahou na dolních končetinách. Nejčastější onemocnění ze skupiny SpA je ankylozující spondylitida (Bechtěrevova choroba). Diagnostika ankylozující spondylitidy byla založena na modifikovaných newyorských kritériích z roku 1984, která vyžadují splnění rentgenových průkazů poškození sakroiliakálních kloubů. Autor článku chce v této práci upozornit na to, že využití nových diagnostických metod zásadně změnilo přístup k tomuto onemocnění a podává přehled o současných klasifikačních kritériích, která umožňují zachycení onemocnění před rozvojem trvalých strukturálních změn. Závěr: Včasné stanovení diagnózy umožní i rychlé nasazení adekvátní léčby, která může významně ovlivnit další průběh onemocnění.

Summary: The group of inflammatory rheumatic diseases of the back (spondyloarthritides /SpA/) include autoimmune inflammatory diseases wherein the inflammation affects the axial skeleton, root joints, and peripheral joints, predominantly in the lower limbs. The most common disease of the SpA group is ankylosing spondylitis (Bechterew’s disease). The diagnosis of ankylosing spondylitis has been based on the 1984 modified New York criteria which require radiographic confirmation of injury to the sacroiliac joints. The author aims to highlight in the article that the use of novel diagnostic methods has fundamentally changed the approach to this disease, and presents an overview of the current classification criteria that allow for detection of the disease prior to developing permanent structural changes. Conclusion: Early diagnosis allows for a rapid commencement of adequate treatment that can significantly affect the further course of the disease.

OBJECTIVES: To develop recommendations for investigation and monitoring of children with Raynaud's syndrome, based on paediatric evidence collated by a systematic review. METHODS: A systematic review was undertaken to establish the paediatric evidence for assessment and monitoring of Raynaud's syndrome. An expert panel including members of the Paediatric Rheumatology European Society (PRES) Scleroderma Working Group, were invited to a consensus meeting where recommendations were developed based on evidence graded by the systematic review and where evidence was lacking, consensus opinion. A nominal technique was used where 75% consensus was taken as agreement. RESULTS: The expert panel recommended testing anti-nuclear antibody (ANA), more specific antibodies associated with connective tissue disease and nail-fold capillaroscopy in all children presenting with Raynaud's syndrome as data suggests these can be risk factors for evolution into a connective tissue disease. The frequency of follow-up recommended depends on presence of these risk factors with the aim to detect evolving connective tissue disease early in high risk individuals. Those with no abnormalities on capillaroscopy and negative autoantibodies were deemed low risk of progression, whereas those with ANA positivity, specific autoantibodies and/or nailfold capillary changes were deemed high risk and more frequent follow-up was recommended. CONCLUSIONS: Recommendations, primarily based on consensus opinion, were agreed regarding investigation and monitoring of children who present with Raynaud's syndrome. Further prospective studies are needed to better define the risk factors for progression to connective tissue disease.

The most frequent cause of malabsorption syndrome (MAS) in developed countries is the celiac disease (CD). There are two possible approaches for exact diagnostic of this disorder: in case of full blown clinical symptoms the golden standard is duodenal biopsy. When it shows classical mucosa lesions (Marsh type 3 mucosa atrophy of various grades), the diagnosis of CD is confirmed. However, if intraepithelial lymphocyte infiltration (IEL) and/or hyperplastic crypts can be seen in the absence of mucosa atrophy (Marsh type 1 and/or 2 lesions) only, it is reasonable to examine the serologic markers. The most frequent enzyme linked immunosorbent assay (ELISA) tests are: detection of tissue transglutaminase 2 antibody (anti-tTGase 2, especially of IgA class) and the deamidated gliadin peptide antibody (anti-DGP) assay. A simpler serological test, useful for screening, is the detection of anti-endomysium antibody (AEmA) by indirect fluorescent antibody (IFA) in monkey intestine or esophagus sections. This is reasonable to combine with the microdot test for anti-gliadin antibody (AGA), especially by using the endomysium/gliadin mosaic kit (Euroimmun). Taken together, at least three serological markers (anti-tTGase 2, anti-DGP and AGA) are highly specific for CD; their detection first (i.e. prior to duodenal biopsy) is recommended in children as well as in adults showing atypical, silent and or latent forms of CD. In both of them, serology represents a reliable non-invasive approach, which allows precise diagnosis with a probability of nearly 90 %. Another advantage of the non-invasive serological technic is that it can be easily repeated when following the outcome of gluten-free diet (GFD). Nevertheless, duodenal biopsy should be performed in any case, if the serology does not correlate with clinical symptoms. In such patients as well as in those revealing positive serology but negative and/or non-specific histology, immunogenetic typing should be performed in order to assess the expression of DQ2 [DQA*0501-DQB*0201] or DQ8 [DQA*0301-DQB1*0302] haplotypes of the human leukocyte antigen (HLA) class II region critical for the development of CD.
