Syndrom GLUT-1 deficience (Glucose transporter-1 deficiency syndrome; GLUT1DS) je relativně vzácná forma léčitelné epileptické encefalopatie způsobené poruchou transportu glukózy přes hematoencefalickou bariéru do mozku, která vzniká na podkladě mutace v SLC2A1 genu. Onemocnění má velmi variabilní fenotyp. Fenotypické spektrum je nyní považováno za kontinuum, do kterého zahrnujeme klasický fenotyp, dystonii typu 9 a 18, atypické dětské absence, myoklonicko-astatickou epilepsii a paroxyzmální neepileptické poruchy hybnosti (jako je například intermitentní ataxie, choreoatetóza, dystonie, alternující hemiplegie). V klasickém případě k manifestaci dochází v kojeneckém období. Typické jsou farmakorezistentní záchvaty, mírné až těžké opoždění psychomotorického vývinu a získaná mikrocefalie. Byl definován i tzv. neklasický fenotyp, pro který je typické opoždění psychomotorického vývinu a komplexní pohybové poruchy bez epilepsie (hypotonie, spasticita, ataxie a dystonie). Výjimečně se vyskytuje adultní forma s minimálními příznaky. Při podezření na syndrom GLUT1DS je klíčové provedení lumbální punkce nalačno, kde v typickém případě nalezneme hypoglykorachii při absenci hypoglykemie v kombinaci s normální nebo nízkou hladinou laktátu. Poměr glykorachie/glykemie je typicky nižší než 0,4. Definitivním potvrzením diagnózy je průkaz mutace v SLC2A1 genu. Syndrom GLUT1DS je léčitelné onemocnění. Léčbou volby je ketogenní dieta. Časná diagnostika GLUT1DS je rozhodující, protože ketogenní dieta dramaticky zlepšuje symptomy onemocnění a dlouhodobou prognózu pacientů.
Glucose transporter-1 deficiency syndrome (GLUT1DS) is a relatively rare form of treatable epileptic encephalopathy caused by impaired glucose transport across the blood-brain barrier associated with the mutation in the SLC2A1 gene. The disorder has a very variable phenotype. The phenotypic spectrum of GLUT1DS is now known to be a continuum that includes the classic phenotype as well as dystonia 9 and 18, atypical childhood absence epilepsy, myoclonic astatic epilepsy, and paroxysmal non-epileptic abnormalities of movement such as intermittent ataxia, choreoathetosis, dystonia, and alternating hemiplegia. The classic phenotype is characterized by pharmacoresistant infantile-onset seizures, delayed neurologic development, and acquired microcephaly. Non-classical phenotype is characterised by complex movement disorder without epilepsy (hypotonia, spasticity, ataxia and dystonia). Adult form with mild symptoms rarely occurs. In suspicion of GLUT1DS, the lumbar puncture should be provided. The disease hallmark is hypoglycorrhachia in association with normoglycemia. The CSF/ blood glucose ratio is typically less than 0.4. Final confirmation of the diagnosis is the detection of the mutation in the SLC2A1 gene. GLUT1DS is treatable disorder. Ketogenic diet is the treatment of choice. Early diagnosis is crucial, because treatment with the ketogenic diet dramatically improves the symptoms and may also improve the long-term outcome of the patients.

BACKGROUND: Glucose transporter-1 (Glut-1) is a membrane glycoprotein that is, together with other glucose transporters, responsible for the regulation of glucose uptake. An increased expression of this protein seems to be a general feature of several malignant tumors that are able to reprogram their metabolism and switch from oxidative phosphorylation to aerobic glycolysis. METHODS: We performed comprehensive immunohistochemical analysis of Glut-1 expression in 336 endometrial samples, including tumors, nontumor lesions, and normal tissues. RESULTS: Expression of Glut-1 was found in 87% of endometrioid carcinomas (160/184 cases), 100% of serous carcinomas (29/29 cases), 100% of clear cell carcinomas (17/17 cases), 50% of polyps with atypical hyperplasia (8/16 cases), 12.5% of polyps with non-atypical hyperplasia (3/24 cases), 77% of hyperplasias with atypias (10/13 cases), 9% of hyperplasias without atypias (1/11 cases), 87% of secretory endometrium samples (13/15 cases), and in none of the nonsecretory endometrium samples (0/27 cases). In endometrioid carcinomas, Glut-1 was expressed in a marked geographical pattern. In nontumor lesions, its expression was more common in atypical hyperplasia and polyps with atypical hyperplasia compared with polyps with non-atypical hyperplasia and hyperplasias without atypia ( P = .00032). CONCLUSION: Our study confirms the high expression of Glut-1 not only in endometrioid carcinomas but also in other carcinomas of endometrium including clear cell and serous types. Glut-1 expression can be used as a surrogate marker in differential diagnosis between hyperplasia with and without atypia. Because of common Glut-1 expression in malignant tumors, therapeutic strategies influencing this protein or its signaling pathways can be beneficial.

The glucose transporter-1 (Glut-1) is a cell membrane glycoprotein involved in glucose uptake. An increased expression of Glut-1 is an important cell adaptation mechanism against hypoxia. An upregulation of Glut-1 can be found in several types of malignant tumors, which are able to reprogram their metabolism from oxidative phosphorylation to aerobic glycolysis (Warburg effect). However, the data regarding melanocytic lesions is equivocal. We performed comprehensive immunohistochemical analysis of the Glut-1 expression in 225 malignant melanomas (MM) and 175 benign nevi. Only the membranous expression of Glut-1 was regarded as positive. The expression of Glut-1 (the cut-off for positivity was determined as H-score 15) was found in 69/225 malignant melanomas. The number of positive cases and the H-score of Glut-1 increased where there was a higher Breslow thickness (p < 0.00001) when comparing pT1- pT4 MM groups. All benign nevi were classified as negative. In conclusion, the membranous expression of Glut-1 is a common feature of a malignant melanoma but this type of expression is very rare in benign melanocytic nevi. Our results suggest that the membranous expression of Glut-1 can be used as a surrogate marker in the assessing of the biological nature of benign and malignant melanocytic lesions. However, despite its high specificity, the sensitivity of this marker is relatively low. Moreover, due to the fact that the increased expression of Glut-1 correlates with a shorter survival period (10-year disease free survival, recurrence free survival and metastasis free survival and MFS), it can be used as a prognostically adverse factor.

V súbore 8 ganglioneurómov bola zisťovaná prítomnosť perineurálnej diferenciácie pomocou 3 imunohistochemických perineurálnych markerov: epitelového membránového antigénu (EMA), klaudínu-1 a GLUT-1. Priemerný vek pacientov bol 42,3 rokov (rozsah 26–68 rokov), 6 tumorov bolo u žien a 2 prípady u mužov. Päť tumorov bolo v nadobličke a 3 v retroperitoneu. Morfológia tumorov bola typická, t.j. tvorená neuroidnou vretenobunkovou populáciou a zrelými gangliovými bunkami. Vretenovité bunky pozitívne na perineurálne markery klaudín-1 a GLUT-1 boli nájdené vo všetkých tumoroch. EMA-pozitívne bunky boli v 2 léziach. Bunky s perineurálnou diferenciáciou boli často usporiadané organoidne v periférii neuroidných fasciklov alebo okolo ciev. Náš nález ukazuje, že perineurálna diferenciácia je bežne prítomná v ganglioneurómoch.
Eight cases of ganglioneuroma were examined for a presence of perineural cell differentiation, using the immunohistochemical markers epithelial membrane antigen (EMA), claudin-1 and GLUT-1. The mean age of the patients was 42.3 years (range 26–68 years), six patients were females and two were males. Five tumors were located in the adrenal gland and 3 tumors in the retroperitoneum. Morphology of the tumors was typical, i.e., they were composed of neuroid spindle cell population and scattered mature appearing ganglion cells. Spindle cells positive for perineural cell markers claudin-1 and GLUT-1 were found in all lesions, at least focally. EMA+ cells were seen in 2 of 8 tumors. These perineural-type cells were often arranged in organoid fashion around the schwannoid bundles or around the vessels. Our findings indicate that perineural cell differentiation is commonly present in ganglioneuromas.

Syndróm GLUT1 deficiencie je zriedkavé neurometabolické ochorenie mozgu vznikajúce pri poruche transportu glukózy cez hematoencefalickú bariéru. Keďže mozog je takmer úplne závislý od dodávky glukózy ako hlavného zdroja energie a GLUT1 je hlavným transportérom glukózy cez hematoencefalickú bariéru, klasický typ ochorenia sa prejavuje závažnými dôsledkami neuroglykopénie: včasnou neurovývojovou encefalopatiou s epileptickými záchvatmi, získanou mikrocefáliou, kognitívnym deficitom a rozličnými perzistentnými alebo paroxyzmálnymi motorickými (často komplexnými) prejavmi, ako je ataxia, spasticita alebo dystónia. Najdôležitejším diagnostickým krokom je dôkaz hypoglykorachie (menej ako 2,2 mmol/l) v likvore a zníženého pomeru glukózy likvor/sérum (väčšinou menej než 0,4). Definitívnu diagnózu však prináša molekulárne genetické vyšetrenie SLC2A1 génu. Najúčinnejšou formou liečby je ketogénna diéta, pri ktorej sa utilizujú mastné kyseliny ako náhradný zdroj energie pre mozog. Z hľadiska prognózy je však kritickým včasné rozpoznanie ochorenia a tým včasné zavedenie ketogénnej diéty.
GLUT1-deficiency syndrome is a rare neurometabolic brain disorder caused by disruption of glucose transport across the blood-brain barrier. As brain almost completely depends on glucose supply as the main source of energy, and the GLUT-1 represents the main transporter of glucose across the blood-brain barrier, the classic form of the disease manifests by severe consequences of neuroglycopenia: early neurodevelopmental encephalopathy with seizures, acquired microcephaly, cognitive deficit and various persistent or paroxysmal motor (often complex in nature) features such as ataxia, spasticity or dystonia. The evidence of low glucose cerebrospinal fluid level (lower than 2.2 mmol/l) and the low CSF/serum glucose ratio (mostly lower than 0.4) are the most important diagnostic clues. Definite diagnosis, however, lies on the molecular-genetic testing of the SLC2A1 gene. The most efficacious treatment strategy is the ketogenic diet, in which the body utilizes fatty acids as an alternative source of energy for the brain. However, early recognition of the disease and early introduction of ketogenic diet is crucial for better prognosis.
