INTRODUCTION: Haemophilia is a hereditary haemorrhagic disorder characterized by deficiency or dysfunction of coagulation factors. Recurrent joint and muscle bleeds lead to progressive musculoskeletal damage. Haemophilia affects patients physically but also socially and psychologically. Traumatic experiences, chronic stress and illnesses can lead to mental disorders, but many persons with haemophilia maintain a highly positive outlook. AIM: To explore qualitatively which coping mechanisms persons with haemophilia use and in what way they help them to live with their diagnosis. METHODS: We recruited five adults with haemophilia and conducted semi-structured face-to-face interviews. Transcripts were analysed using interpretative phenomenological analysis (IPA). RESULTS: Two core themes emerged from the analysis: social support as an external factor and resilience as an internal factor of coping with the disease. Persons with haemophilia usually need help with health-related complications, and this affects the social support they require. Their wider support network tends to involve family and friends but also healthcare professionals and other specialists. This network provides practical help but also functions as an important psychological protective factor. An unexpected finding was that persons with haemophilia want not only to receive support but are also keen to offer support to others. CONCLUSION: These findings can help identify persons who provide most support to people suffering from haemophilia. Haemophilic centres should include in their teams psychologists and social workers and offer individual and group therapy to their clients, group meetings for friends and families of persons with haemophilia, provide learning resources to teachers aiming to incorporate children with haemophilia in their peer group, and organize Balint groups for physicians, psychologists and other healthcare professionals.
Úvod: Hemofilie je dědičná choroba charakterizovaná nedostatkem či dysfunkcí specifických bílkovin odpovědných za srážlivost krve. Opakovaná krvácení do kloubů a svalů vedou k vážným a progresivním poškozením těchto tkání. Dospělé osoby s hemofilií čelí širokému spektru problémů, které souvisí s jejich nemocí. Při analýze rozhovorů pilotní analýzy, která se týkala sociální opory, resilience a kvality života pacientů s hemofilií, vyšly najevo důležité a problematické aspekty týkající se systému péče o tyto osoby. Cíle: Definovat problematické oblasti v systému péče o osoby s hemofilií a identifikovat problémy, které jsou spontánně reflektovány účastníky – osobami s hemofilií. Metody: Účastníky byly osoby s hemofilií. Jednalo se o pět dospělých mužů žijících v České republice. Účastníci se účastnili polostrukturovaného hloubkového rozhovoru (in-depth interview) zaměřeného na téma kvality života osob s hemofilií. Rozhovory byly tematicky analyzovány. Výsledky: Problematické oblasti v systému péče o osoby s hemofilií byly rozděleny do následujících oblastí: (1) oblast léčby hemofilie, (2) oblast informovanosti o hemofilii a (3) oblast financování léčby a zdravotních následků způsobených hemofilií. Závěr: Problematika nastavení optimálního systému péče o osoby s hemofilií je velmi komplikovaná. Předkládaná zjištění mohou přispět ke snahám zlepšovat péči o osoby s hemofilií. Informace o nedostatcích v péči z pohledu vybraného vzorku pacientů mohou sloužit i jako zpětná vazba pro pacientskou organizaci, státní instituce i členy týmů jednotlivých hemofilických center. V budoucnu by bylo vhodné rozpracovat zjištěné závěry v dalších studiích.
Introduction: Haemophilia is a hereditary disease characterised by shortage or dysfunction of specific proteins involved in blood coagulation. Repeated bleeds into joints and muscles lead to severe and progressive damage to these tissues. Adults with haemophilia face a wide range of problems associated with their condition. Analysis of interviews undertaken as part of a pilot study and focusing on social support, resilience and quality of life of haemophilia sufferers revealed some important and problematic aspects of the system of care for these people. Goals: To identify problematic areas in the care for persons with haemophilia as well as issues viewed as challenging by our respondents, persons suffering from haemophilia. Methods: Five adult men suffering from haemophilia an residing in the Czech Republic participated in our study. We conducted semi-structured in-depth interviews focusing on the quality of life of persons with haemophilia. Interviews were then analysed according to the issues that arose. Results: Problematic areas in the system of haemophilia care were assessed based on respondents’ views and divided into (1) haemophilia treatment, (2) haemophilia awareness and (3) financing of haemophilia treatment and health haemophilia-associated impairments. Conclusion: The issue of establishing an optimal system of care for persons with haemophilia is highly complex. This study’s findings could contribute towards improving care for haemophilia sufferers. Information on shortcomings of currently available care coming from patients themselves could moreover serve as feedback for patients’ organisations, state institutions and members of specialised haemophilia teams. Our findings should be developed and extended by further studies in the future.

Mycoplasma pneumoniae (M. pneumoniae) je extracelulárny patogén prenikajúci ciliárnou vrstvou dýchacej sliznice a pevne adherujúci na povrch epitelových buniek. Od ostatných baktérií sa odlišuje tým, že netvorí peptidoglykán a pevnú bunkovú stenu. Tento tesný kontakt buniek a mykoplaziem umožňuje fúziu membrán a výmenu antigénov, čím sa vysvetľujú mnohé extrapulmonálne komplikácie infekcie na základe autoimunitných reakcií , ako napr. myokarditída, perikarditída, encefalitída, periférna neuropatia, hemolytická anémia, koagulopatie, erythema nodosum, hepatocelulárny ikterus, akútna pankreatitída, akútna glomerulonefritída a iné. Diagnózu môžeme s vysokou pravdepodobnosťou stanoviť sérologickým vyšetrením titra protilátok imunoglobulínu M (IgM), kde za vysoko signifikantné sa považuje titer 1:32. Pozitivita IgM protilátok sa dokáže iba u 50–70 % pacientov po 7–10 dňoch od prepuknutia ochorenia, negatívny výsledok však možnú prebiehajúcu infekciu nevylučuje.
Mycoplasma pneumoniae (M. pneumoniae) is an extracellular pathogen that penetrates the ciliated layer of the respiratory mucus and firmly adheres to the surface of the epithelial cells. It differs from other bacteria by not producing peptidoglycan and thereby not forming a firm cell wall. This close contact of cells and mycoplasmas allows for membrane fusion and the exchange of antigens, which explains many extrapulmonary complications from autoimmune reactions (Such as myocarditis, pericarditis, encephalitis, peripheral neuropathy, haemolytic anaemia, Stevens-Johnson syndrome, mucositis, epidermal necrolysis, myelitis, coagulopathy, erythema multiforme, macular rash, erythema nodosum, hepatocellular jaundice, acute pancreatitis, acute glomerulonephritis, renal failure, cholestatic hepatitis, even coma and others). M. pneumoniae is a common cause of upper respiratory tract infections, but it can take the course of atypical bronchopneumonia. Diagnosis can be confirmed with high probability by serological examination of antibody titre of immunoglobulin M (IgM), where an IgM antibody titre of 1:32 is considered highly significant. However, positivity is manifested in only 50–70 % of patients 7–10 days after the outbreak of disease. A negative result, however, does not exclude a possible ongoing infection.

Solitárny benígny ulkus slepého čreva je neobvyklým nálezom bez charakteristického klinického či rádiologického prejavu. Vzhľadom na svoju zriedkavosť a klinický prejav často vzbudzujúci podozrenie na prítomnosť závažných ochorení ako sú malignity, obraz akútneho brucha či krvácanie zo spodnej časti tráviaceho traktu, je tento nález málokedy diagnostikovaný predoperačne. Definitívna diagnóza sa takmer vždy stanoví až patologickým vyšetrením. V tejto správe opisujeme klinický, rádiologický a histologický nález u pacientky s ťažkosťami spôsobenými solitárnym benígnym ulkusom céka.
Problematika farmakotepie počas tehotenstva je veľmi náročná vzhľadom nato, že každý liek je potencionálne teratogénny. Aj keď sa na celkovom počte vrodených poškodení lieky podieľajú len asi 1 ‰, žiada sa, aby sme sa ich použitiu vyhli všade tam, kde to je len možné. K používaniu farmák počas tehotenstva by nás mal viesť stav pacientky, uvážlivosť a prevyšujúci benefit z ich použitia ako možné riziko.
The issue of the pharmacotherapy during pregnancy is very difficult, while every drug is potentially teratogenic. Even if medicaments contribute to inborn defects only in 1‰ of cases, it is highly advisable to avoid their use in all possible cases. Condition of the patient, cautiousness and the preponderant benefit against the possible risk are determinant factors in using medicaments during pregnancy.
