Genetika se v současné kardiologii uplatňuje zejména u kardiovaskulárních onemocnění s mendelovskou dědičností. Mezi tato onemocnění patří kardiomyopatie, dědičné formy arytmických syndromů, aortální syndromy a některé vrozené vývojové vady. Společným rizikem všech zmíněných skupin onemocnění je náhlá srdeční smrt, proto je důležitou součástí i posmrtné vyšetření a péče o příbuzné a pozůstalé. Cílem klinicko-genetického vyšetření a genetického poradenství v kardiologii je provést podrobný popis onemocnění - fenotypizaci, sestavení rodokmenu, kaskádový kardiologický screening v rodině a v indikovaných případech molekulárně-genetickou diagnostiku - genotypizaci. Znalost molekulární podstaty genetických onemocnění otvírá možnosti tzv. molekulárně genetické stratifikace onemocnění, jejímž cílem je personalizovaná péče o nemocné a jejich příbuzné. Kardiogenetické vyšetření je komplexní záležitost, kdy je třeba budovat mezioborové týmy. Komunikace mezi jednotlivými odborníky i týmy v národním měřítku je zcela zásadní pro zajištění péče o pacienty s často velmi vzácnou až raritní formou dědičného onemocnění.
In contemporary cardiology, genetics is mainly applied to cardiovascular diseases with Mendelian inheritance. These diseases include cardiomyopathy, hereditary forms of arrhythmic syndromes, aortic syndromes, and some congenital developmental defects. The common risk of all mentioned groups of diseases is sudden cardiac death, which is why post-mortem examination and care for relatives and survivors are an important part. The goal of clinical genetic examination and genetic counselling in cardiology is to carry out a detailed description of the disease - phenotyping, compiling a family tree, cascade cardiology screening in the family and, in indicated cases, molecular genetic diagnosis - genotyping.

Kostní dysplázie, kterých je známo více než čtyři sta, patří mezi vzácná onemocnění. Ty nejzávažnější a letální je možné diagnostikovat prenatálně a indikovat k umělému ukončení těhotenství. Konečná diagnóza je formulována na základě pitvy a postmortálního RTG, které ve stále větší míře doplňuje genetické vyšetření. Přibližně polovinu případů vyšetřených patologem tvoří thanatoforní dysplázie a osteogenesis imperfecta; k méně častým patří achondrogeneze 2. typu a hypochondrogeneze, syndromy krátkých žeber, chondrodysplasia punctata, kampomelická dysplázie a achondroplázie. Zřídka jsou diagnostikovány kostní dysplázie s autosomálně recesivní dědičností jako perinatální letální hypofosfatázie, achondrogeneze typu 1A, diastrofická dysplázie/atelosteogeneze 2. typu nebo mukolipidóza 2. typu. V následujícím textu přinášíme přehled problematiky a praktické zkušenosti, které jsme získali během deseti let na souboru 41 případů.
We present a comprehensive review dealing with rare genetic skeletal disorders. More than 400 entities are included in the latest classification. The most severe or lethal phenotypes are identifiable in the prenatal period and the pregnancy can be terminated. Perinatal autopsy and posmortem X-rays are crucial in providing a definitive diagnosis. The number of cases confirmed by genetic testing is increasing. We report our own experience with genetic skeletal disorders based on 41 illustrative fetal and neonatal cases which we encountered over a 10-year period. Thanatophoric dysplasia and osteogenesis imperfecta represent approximately half of the cases coming to autopsy. Achondrogenesis type 2 and hypochondrogenesis, short-rib dysplasia, chondrodysplasia punctata, campomelic dysplasia and achondroplasia are less common. Skeletal dysplasias with autosomal recessive inheritance are the least frequent, e.g. perinatally lethal hypophophatasia, achondrogenesis type 1A, diastrophic dysplasia/atelosteogenesis type 2 or mucolipidosis type 2 (I cell disease).

AIMS: Recent-onset dilated cardiomyopathy (RODCM) is a disease of heterogeneous aetiology and clinical outcome. In this pilot study, we aimed to assess its genetic architecture and correlate genotype with left ventricular reverse remodelling (LVRR). PATIENTS AND METHODS: In this multi-centre prospective observational study, we enrolled 83 Moravian patients with RODCM and a history of symptoms of less than 6 months, for whole-exome sequencing (WES). All patients underwent 12-month clinical and echocardiographic follow-up. LVRR was defined as an absolute increase in left ventricular ejection fraction > 10% accompanied by a relative decrease of left ventricular end-diastolic diameter > 10% at 12 months. RESULTS: WES identified at least one disease-related variant in 45 patients (54%). LVRR occurred in 28 patients (34%), most often in carriers of isolated titin truncated variants, followed by individuals with a negative, or inconclusive WES and carriers of other disease-related variants (56% vs. 42% vs. 19%, P=0.041). CONCLUSION: A substantial proportion of RODCM cases have a monogenic or oligogenic genetic background. Carriers of non-titin disease-related variants are less likely to reach LVRR at 12- months than other individuals. Genetic testing could contribute to better prognosis prediction and individualized treatment of RODCM.

BACKGROUND: The study analyzed the impact of first-trimester screening on the spectrum of congenital heart defects (CHDs) later in pregnancy and on the outcome of fetuses and children born alive with a CHD. METHODS: The spectrum of CHDs, associated comorbidities, and outcome of fetuses, either diagnosed with a CHD in the first trimester (Group I, 127 fetuses) or only in the second-trimester screening (Group II, 344 fetuses), were analyzed retrospectively between 2007 and 2013. Second-trimester fetuses diagnosed with a CHD between 2007 and 2013 were also compared with Group III (532 fetuses diagnosed with a CHD in the second trimester from 1996 to 2001, the period before first-trimester screening was introduced). RESULTS: The spectrum of CHDs diagnosed in the first and second trimesters in the same time period differed significantly, with a greater number of comorbidities (P<0.0001), CHDs with univentricular outcome (P<0.0001), intrauterine deaths (P=0.01), and terminations of pregnancy (P<0.0001) in Group I compared with Group II. In Group III, significantly more cases of CHDs with univentricular outcome (P<0.0001), intrauterine demise (P=0.036), and early termination (P<0.0001) were identified compared with fetuses diagnosed with CHDs in the second trimester between 2007 and 2013. The spectrum of CHDs seen in the second-trimester groups differed after first-trimester screening was implemented. CONCLUSIONS: First-trimester screening had a significant impact on the spectrum of CHDs and the outcomes of pregnancies with CHDs diagnosed in the second trimester. Early detection of severe forms of CHDs and significant comorbidities resulted in an increased pregnancy termination rate in the first trimester.

Prezentujeme kasuistiku rodiny s výskytem arytmogenní kardiomyopatie postihující dominantně levou komoru. Šestačtyřicetiletý muž byl přijat na urgentní příjem pro kolapsový stav při sportovní aktivitě se zástavou dechu i oběhu. Na úvodním EKG byla zjištěna fibrilace komor a v rodině pacienta došlo k několika náhlým úmrtím v nízkém věku. Nebyla prokázána významná stenóza na koronárních tepnách a na podkladě echokardiografického a EKG nálezu byla indikována magnetická rezonance srdce. Nález na magnetické rezonanci srdce odpovídal méně časté formě arytmogenní kardiomyopatie s dominujícím postižením levé komory. Nejbližší příbuzní pacienta byli následně podrobně vyšetřeni na naší klinice a na magnetické rezonanci srdce pacientova mladšího bratra byl popsán podobný nález. Oběma bratrům byl implantován implantabilní kardioverter-defibrilátor a celá rodina zůstává kardiologicky sledována na naší klinice.
A case of a family suffering from arrhythmogenic cardiomyopathy affecting dominantly the left ventricle of the heart was diagnosed in our hospital. A forty-six-year-old man was admitted to the emergency room because of a collapse with both respiratory and circulatory arrest during a sport activity. Fibrillation of the ventricles was initially registered upon electrocardiography, and there had been several sudden deaths at a young age in the patient’s family. There was no significant stenosis of his coronary arteries and because of the electrocardiographic and echocardiographic findings, cardiac magnetic resonance was indicated. The cardiac magnetic resonance finding on the patient’s heart probably corresponds to a less frequent type of arrhythmogenic cardiomyopathy with dominant left ventricular involvement. The patient’s first-degree relatives underwent comprehensive examinations at our cardiology department, and a similar cardiac magnetic resonance finding concerning the heart was made in the younger brother of the proband. Both brothers had an implantable cardioverter-defibrillator implanted and the whole family remains subject to regular follow-ups at our department.

Hypertrofická kardiomyopatie (HCM) je nejčastější geneticky podmíněné srdeční onemocnění s prevalencí 1 : 500. Ve familiárních případech je dědičnost autozomálně dominantní s neúplnou penetrancí a různou expresivitou, může ovšem vznikat i de novo mutacemi. Nesmírná heterogenita projevů i prognózy HCM případ od případu značně komplikuje klinický management, a proto toto onemocnění stále často přináší mnoho otázek nejen pro lékaře prvního kontaktu, ale i pro kardiology. Stanovení rizika náhlé srdeční smrti u pacientů s HCM je integrální součástí klinického managementu a doporučení pro stratifikaci se neustále vyvíjí. V této kazuistice prezentujeme případ familiární HCM u dvou bratrů nesoucích stejnou mutaci s velmi rozdílným klinickým nálezem a průběhem.
Hypertrophic cardiomyopathy (HCM) is the most common genetic cardiac disease with a prevalence of 1 : 500. In familial cases the inheritance is autosomal dominant with non‑complete penetrance and variable expression; however, it can also be caused by de novo mutations. The heterogeneity of both its presentation and prognosis from case to case largely complicates clinical management, and therefore the disease often represents a dilemma for primary care clinicians as well as cardiologists. An estimation of sudden cardiac death risk is an integral part of clinical management and the stratification guidelines are continuously developing. We report on a case of familial HCM in two brothers with the same gene mutation, with very different clinical presentations and consequences.
