Úvod: Až 25 % pacientů se schizofrenií zaznamená v průběhu života některý z obsedantně kompulzivních symptomů (OCs). Přibližně 12 % pacientů naplňuje diagnostická kritéria pro obsedantně-kompulzivní poruchu (OCD). Společný výskyt schizofrenie a příznaků OCD je tedy častý. V poslední době se zájem o tuto problematiku výrazně zvýšil. Důvodem je mimo jiné zjištění, že léčba antipsychotiky druhé generace, zejména klozapinem, může vést ke vzniku nebo zhoršení OCs. Cílem naší práce bylo předložit přehled výzkumných zjištění o vztahu mezi komorbidní obsedantně kompulzivní poruchou a závažností a léčebnými výsledky schizofrenního onemocnění. Metoda: Publikace byly identifikovány v databázích Medline a Web of Science pomocí zadání klíčových slov "schizophrenia" a "obsessive-compulsive disorder". Další relevantní zdroje informací byly získány z citovaných prací u významných článků. Výsledky: OCs nebo plně rozvinutá OCD u pacientů se schizofrenií jsou spojeny s celkově závažnější psychopatologií a horšími výsledky léčby. Pacienti s touto komorbiditou vykazují výraznější neurokognitivní deficit, vyšší míru úzkosti a depresivity a zvýšené riziko suicidia. Významně oslabenější je i jejich schopnost zapojení do společenského života a profesní uplatnění. Důsledkem jsou horší subjektivní vnímání příznaků poruchy, neuspokojivější výsledky léčby a zvýšené náklady na zdravotní péči. Závěr: V klinické praxi je potřeba sledovat, zda se u pacientů se schizofrenií rozvíjí komorbidní OCs. Vznik OCs je možný kdykoliv v průběhu schizofrenního onemocnění. Včasné rozpoznání a léčba této komorbidity snižují utrpení pacienta, pozitivně ovlivňují průběh a celkový výsledek léčby.
Introduction: Up to 25 % of patients with schizophrenia suffer from obsessive-compulsive symptoms (OCs), and about 12% fulfil the diagnostic criteria of obsessive-compulsive disorder (OCD). Recently, the interest in this issue has significantly increased, probably due to the finding that second-generation antipsychotics, especially clozapine, might induce or aggravate OCs. Our study aimed to describe the research findings concerning the connection between comorbid OCs and the severity and course of schizophrenia. Method: The articles were identified by the keywords "schizophrenia comorbidity" and "obsessive compulsive disorder", using the Medline and Web of Science databases. Additional information was obtained by searching the references of relevant articles. Results: Obsessive-compulsive symptoms or fully expressed obsessive-compulsive disorder leads to more severe overall psychopathology and worse treatment outcomes in patients without this symptomatology. The comorbidity is accompanied by more severe increased neurocognitive impairment, high levels of anxiety, depression, and suicidality, larger social and vocational disability, and greater social and health service utilization. Conclusion: schizophrenia patients should be carefully monitored for OCs in clinical practice. The development of these symptoms may occur during disease, and early recognition and targeted treatment of this comorbidity reduce the patient ́s distress and positively affect the course of the disorder and overall treatment outcomes.
Pseudodemence, jako jeden ze subtypů tranzientních či reverzibilních demencí, je stav charakterizovaný přechodným významným snížením poznávacích schopností. Přehledový článek shrnuje dostupné zdroje zabývající se psychopatologií, etiologií a etiopatogenezí pseudodemence. Klinický význam pseudodemence spočívá v jejím potenciálu vytvářet "dojem hlubšího kognitivního deficitu", než je skutečný stav výkonových kognitivních funkcí. Pacienti nemají potíže s vybavitelností jednotlivých poznatků při cíleném dotazování, ale selhávají v jejich systemizaci a propojování do souvislostí, a to vede ke snížené kognitivní variabilitě nebo k narušení schopnosti tvorby úsudku. V literatuře se objevuje pojem pseudodemence především u depresivní poruchy a u disociativních poruch. Podobnost příznaků demence a pseudodemence někdy vede ke stanovení nesprávné diagnózy a tím podávání léků, které u deprese neúčinkují. Zatímco u demence jsou na místě kognitiva, u depresivní pseudodemence je hlavním přístupem podávání antidepresiv, elektrokonvulze nebo jejich kombinace. Samotná administrace účinné léčby deprese antidepresivy tedy může významně pomoci při rozlišení mezi pseudodemencí a demencí. U disociativní amnézie dominuje ztráta paměti obvykle na důležité nedávné události. Jde o poruchu epizodické paměti, kde chybí schopnost vybavit si vzpomínky na konkrétní události, nikoli na získané dovednosti či znalosti. Počátek i ukončení bývají náhlé. Končí zpravidla plným uzdravením. U některých pacientů však nedochází k rozpomenutí ani po letech. Ganserův syndrom je charakterizován regresivním chováním a "přibližnými odpověďmi". Objevuje se především u vězňů ve vyšetřovací vazbě. Poruchy paměti jsou obvykle doprovázeny dalšími disociativními příznaky. Prognosticky lze hodnotit syndrom pseudodemence ve dvou alternativách - jako prodrom typické demence a jako sekundární důsledek jiné psychopatologie nebo jiného patofyziologického procesu.
Pseudodementia, as one of the subtypes of transient or reversible dementias, is a condition characterized by a significant transient reduction in cognition. The review article summarizes the available resources dealing with the psychopathology, etiology and etiopathogenesis of pseudodementia. The clinical significance of pseudodementia lies in its potential to create an "impression of a deeper cognitive deficit" than the actual state of cognitive performance functions. Patients do not have difficulty equipping individual knowledge in targeted questioning but fail to systematize it and connect it in context, leading to reduced cognitive variability or impaired judgment. In the literature, the concept of pseudodementia appears mainly in depressive disorders and dissociative disorders. The similarity of dementia and pseudodementia symptoms sometimes leads to misdiagnosis and thus the administration of drugs that do not work in depression. While cognition is in place in dementia, antidepressants, electroconvulsive therapy, or a combination thereof are the primary approach in depressive pseudodementia. Thus, effective antidepressant treatment for depression alone can significantly help distinguish between pseudodementia and dementia. In dissociative amnesia, memory loss usually dominates critical recent events. It is an episodic memory disorder that cannot recall specific events, not acquired skills or knowledge. The beginning and the end are sudden. It usually ends in full recovery. However, some patients do not remember after years. Ganser syndrome is characterized by regressive behaviour and "approximate responses." It occurs mainly in prisoners in pre-trial detention. Other dissociative symptoms usually accompany memory disorders. Pseudodementia syndrome can be evaluated prognostically in two alternatives - as a prodrome of typical dementia and a secondary consequence of another psychopathology or other pathophysiological process.
Mobbing je opakující se neadekvátní chování skupiny proti jednotlivci. Toto chování může nabývat forem verbální agrese, jako jsou urážky, hrozby, jednostranné pomluvy, obtěžování nebo vylučování ze společnosti, a někdy také fyzické agrese. Pro mobbing je charakteristický nepoměr sil aktérů, přičemž oběť je nejčastěji ve výrazně slabší pozici. Často se jedná o podřízeného, ale výjimkou není ani šikana nadřízeného ze strany podřízených osob. V neformální hierarchii skupiny se šikanované osoby zpravidla nachází na okraji, bývají tzv. "černými ovcemi". Podstatou mobbingu je znehodnocení jedince po stránce psychické i fyzické. Toto chování se pak může odrážet na spokojenosti v zaměstnání a být spojeno se syndromem vyhoření. Dalšími důsledky jsou horší tělesné i duševní zdraví oběti, včetně vyšší incidence kardiovaskulárních onemocnění a rizik rozvoje psychických poruch, především adaptační poruchy, poruch nálady nebo dalších úzkostných poruch. Byla také pozorována vyšší incidence úzkostných poruch a poruch nálady v rané dospělosti u osob šikanovaných v mladším věku. Naopak z hlediska pachatele může být mobbing spojen se specifickými osobnostními charakteristikami, diagnózou poruchy osobnosti, impulzivitou a sebevědomým vystupováním a nižší schopností sebereflexe a decentralizace.
Mobbing is the recurring inadequate behavior of a group against an individual. This behavior can take the form of verbal aggression such as insults, threats, unilateral slander, harassment or exclusion from society, and sometimes physical aggression. Mobbing is characterized by a disproportionate force of actors, with the victim most often in a significantly weaker position. It is often a subordinate, but the bullying of a superior by subordinates is no exception. In the informal hierarchy of the group, bullied people are usually on the margins, they tend to be so-called "black sheep". The essence of mobbing is the mental and physical degradation of an individual. This behavior can then affect job satisfaction and be associated with burnout. Other consequences are the victim's poorer physical and mental health, including a higher incidence of cardiovascular disease and the risk of developing mental disorders, especially adaptation disorders, mood disorders or other anxiety disorders. A higher incidence of anxiety and mood disorders in early adulthood has also been observed in those bullied at a younger age. Conversely, from the offender's point of view, mobbing may be associated with specific personality characteristics, diagnosis of personality disorder, impulsivity and self-confidence, and a lower ability to self-reflect and decentre.

Porucha s bludy se zpravidla vyznačuje jediným bludným přesvědčením nebo systémem několika vzájemně propojených bludů. Bludná přesvědčení se mohou vytvořit na podkladu reálné, obvykle zraňující, ponižující či stresující události. Rozeznáváme několik podtypů: persekuční, žárlivecký, erotomanický, somatický, grandiózní, smíšený a nespecifický. Bludy mohou přetrvávat dlouhodobě, mnohdy mnoho let. Terapie pacientů s touto poruchou bývá obvykle náročná, protože se zpravidla nepovažují za nemocné a nemají sklony vyhledávat léčbu. Úspěch léčby výrazně závisí na kvalitě terapeutického vztahu. Jedním z terapeutických cílů je motivovat pacienta k užívání antipsychotik, která jsou léčbou volby. U bludů somatického podtypu může pomoci léčba antidepresivy. Kognitivně behaviorální terapie vyvinula strategie pro léčbu chronických bludů. Specifickou strategií pro změnu bludného přesvědčení je periferní dotazování.
Delusional disorder usually presents by a single delusional belief that gradually arises in time or by a system of several interconnected delusions. Delusions can develop from a misrepresentation of real, usually hurtful, degrading, or stressful events. There are several subtypes: persecutory, jealous, erotomaniac, somatic, grandiose, mixed, and non-specific. Delusions can persist for a long time, sometimes for many years. The treatment is usually challenging, as these patients generally do not consider themselves ill and tend to not seek treatment. The therapeutic success heavily relies on a safe and stable therapeutic relationship. One of the psychotherapeutic goals is to motivate the patient to use antipsychotics which are the treatment of choice. Antidepressants may then alleviate symptoms of somatic delusions. Cognitive behavioural therapy has developed strategies for the treatment of chronic delusions. The most specific strategy for changing delusional beliefs is peripheral questioning.

Manželské neshody mohou být spojené s úzkostí u jednoho nebo obou partnerů. Nelze však přesně určit, jak se manželství podílí na vzniku a udržování těchto příznaků. Důležitost porozumění partnerským interakcím u pacientů s panickou poruchou nebo agorafobií je podporovaná řadou studií. Výsledky uvedené v tomto přehledu obecně naznačují, že partnerský vztah může ovlivňovat závažnost panické poruchy a agorafobie a naopak. Souvislost mezi panickou poruchou a vztahem mezi partnery je obousměrná: psychologické problémy nepříznivě ovlivňují vztahy a postoje partnera vůči pacientovi a významně ovlivňují jeho úzkost. Často je obtížné určit, co je příčinou, jaký je důsledek a kolik úzkosti se překrývá s partnerskými problémy. Zdá se také, že nedostatečná partnerská podpora souvisí se závažností panické poruchy a agorafobie, zatímco pozitivní sociální podpora může být pro pacienty prospěšná. Tyto výsledky zdůrazňují důležitost partnerů při pomoci lidem zotavit se z poruchy. Zdá se, že emoční vazba pacienta koreluje s výsledky psychoterapie. Intervence, které vedou k úpravám ve vztahu (např. odstranění vyhýbání se komunikaci), zlepšují výsledek léčby úzkostné poruchy. Integrace partnerů do terapie, jakož i poskytnutí nástrojů (např. psychoedukace, psychoterapie pro partnera) k tomu, jak se vypořádat s příznaky poruchy, by mohla být prospěšná jak pro pacienta, tak pro partnera. Pro posouzení úrovně podpory by však bylo zapotřebí více studií zaměřených na to, který typ partnerské podpory je užitečný, a který naopak problémy udržuje.
Marital disagreements can be associated with anxiety in one or both partners. However, it is not possible to determine exactly how marriage contributes to the onset and maintenance of these symptoms. The importance of understanding partner interactions in patients with panic disorder or agoraphobia is supported by a number of studies. The results presented in this review generally suggest that a relationship may affect the severity of panic disorder and agoraphobia and vice versa. The relationship between panic disorder and the relationship between partners is bi-directional: psychological problems adversely affect the relationships and attitudes of the partner towards the patient and significantly affect his anxiety. It is often difficult to determine what is the cause, what is the consequence, and how much anxiety overlaps with the partner's problems. Insufficient partner support also seems to be related to the severity of panic disorder and agoraphobia, while positive social support can be beneficial for patients. These results emphasize the importance of partners in helping people recover from the disorder. The patient's emotional attachment seems to correlate with the results of psychotherapy. Interventions that lead to relationship adjustments (e. g. communication avoidance) improve the outcome of anxiety disorder treatment. Conversely, if the relationship does not develop more adaptive patterns, the results of psychotherapy are worse. Living with someone who suffers from a psychological problem can be challenging for relatives because they may not know how to optimally support or respond to the patient's behavior without supporting the maintenance of the disorder. Integrating partners into therapy as well as providing tools (psychoeducation, partner psychotherapy) to deal with the symptoms of the disorder could benefit both the patient and the partner. However, in order to assess the level of support, more studies would be needed to determine which type of partnership support is useful and which in turn maintains the problems.

Historicky byla hraniční porucha osobnosti (HPO) vnímána jako celoživotní a vysoce invalidizující porucha. Výzkumy provedené v posledních dvou desetiletích však toto chápání HPO vyvracejí. HPO je nejčastěji diagnostikována v časné dospělosti a průběh poruchy je obecně podobný, s klesající symptomatologií v průběhu času. Cílem přezkumu je poskytnout aktuální informace o konceptu zotavení u pacientů s HPO a možnostech, jak toho dosáhnout. Počítačové vyhledávání v databázi bylo provedeno v PubMed a Web of Science za použití různých kombinací klíčových slov relevantních pro dané téma. Hledání bylo filtrováno na období mezi lednem 1990 a prosincem 2019. Výsledky byly přezkoumány a diskutovány ve čtyřech různých kategoriích souvisejících s cílem této studie: 1) Proč je smysluplné zvážit koncept zotavení v HPO, 2) Zotavení spojené se zmírněním příznaků v průběhu času, 3) Účinná léčba HPO a zotavení, 4) Převzetí odpovědnosti pacienta za zotavení. Po přezkoumání současné literatury bychom mohli navrhnout závěr, že uzdravení je nejen snížení příznaků, ale také budování pozitivní osobní identity a účelného života, lze považovat za dosažitelný cíl léčby. Nezbytným požadavkem je však vlastní zapojení pacienta do převzetí odpovědnosti za terapii.
Historically, borderline personality disorder (BPD) has been viewed as alifelong and highly disabling disorder. However, investigations throughout the past two decades have confronted these postulations. BPD is most commonly diagnosed during early adulthood, and the course of the disorder is generally similar, with decreasing symptomatology over time. The review aims to deliver up-to-date information on the concept of recovery in BPD patients, and possibilities to reach it. Acomputerized database search was conducted in PubMed and Web of Science, using various combinations of keywords relevant to the topic. The search was filtered for the time period between January 1990 and December 2019 The results were reviewed and discussed in four distinct categories relevant to the aim of this study: 1) Why it is meaningful to consider the concept of recovery in BPD, 2) Recovery connected to alleviation of symptoms during the course of time, 3) Effective treatment of the BPD and recovery, 4) Patient'staking responsibility for the recovery. After reviewing the current literature, we could propose aconclusion, that the recovery, including not only reduction of symptoms, but also building apositive personal identity and purpose of life, could be viewed as areachable treatment objective. However, the necessary requirement is patient'sown involvement in taking responsibility for the therapy. Results: Complete remission or complete disappearance of the symptoms of borderline personality disorder usually does not occur soon after the start of treatment, so we consider complete recovery as adistant goal. Patients with BPD do not have apoor prognosis if they seek treatment and are actively involved. Patients with BPD are open to treatment and can be treated well, but the condition is that they follow their treatment program and monitor their progress and learn how to cope effectively with their problems. Conclusion: Recovery is an achievable goal in the treatment of individuals with BPD. Stable recovery is an option that requires long-term maintenance of adaptive patterns created in treatment and awillingness to seek help in the event of acrisis. Aprerequisite for recovery is the patient taking responsibility for his or her health condition. Psychotherapy and psychosocial rehabilitation and, in addition, pharmacotherapy help people with borderline personality disorder increase their quality of life and can significantly help on their personal path to recovery.
Úvod: Citová vazba vzniká v raném dětství mezi dítětem a primární pečující osobou a zásadním způsobem ovlivňuje sebehodnocení dítěte a jeho vnímání světa jako bezpečného místa. Cílem této přehledové studie je představit současný stav poznání týkající se vztahu mezi vazbou a obsedantně kompulzivní poruchou (OCD), průběhem poruchy a účinností léčby.
Introduction: Emotional attachment arises between the child and the primary caregiver in early childhood and fundamentally affects the child's self-esteem and their perception of the world as a safe place. The aim of this review is to present the current state of knowledge about the relationship between attachment and obsessive-compulsive disorder (OCD), the course of the disorder and the effectiveness of the treatment. Method: A literature search was performed using PubMed, Web of Science, Google Scholar, and ScienceDirect databases using key search terms. Scientific texts were extracted for the period from January 1990 to the end of March 2020. Results: An insecure attachment reduces a personal sense of self-worth and can lead to perfectionism and compulsive behavior, which becomes a means of trying to secure and stabilize the value of self-concept and control the surrounding events. Both characteristics are common in patients with OCD. Uncertain attachment is further associated with maladaptive cognitive processes (such as excessive sense of responsibility, perfectionism, and mind control), which are also associated with OCD. Of the two dimensions that define uncertain adulthood attachment (anxiety and avoidance), attachment anxiety is more closely related to OCD. No research has been described in the articles we found to prove a direct link between the attachment and the severity of OCD. Insecure emotional attachment and OCD is complicated by frequent comorbid depression. Conclusions: Anxious emotional attachment is common in patients with OCD and interconnects with basic OCD symptomatology. From this perspective, strategies that promote feelings of safety, acceptance, and appreciation within a therapeutic relationship may be important in the treatment of OCD.
Rozpoznání poruch nálady a jejich léčba se v posledních letech značně zlepšuje. V rámci veřejného zdraví a problematiky rozvoje depresivní poruchy je důležité výzkumně identifikovat rizikové faktory, které se podílejí na jejím vzniku. Jedním z rizikových faktorů je manželský nesoulad, avšak hlubší pochopení konkrétních činitelů v procesu vztahové problematiky, rozvoje a udržování depresivní poruchy je dosud málo známé. Pro zlepšení léčebných výstupů je zapotřebí dalšího výzkumu interpersonálních faktorů podílejících se na nástupu a průběhu depresivní poruchy.
Recognition of mood disorders and their treatment has improved considerably in recent years. In terms of public health and the development of depressive disorder, it is important to explore the risk factors involved in its development. One of the risk factors is marital discrepancy, however deeper understanding of specific factors in the process of relationship issues, development and maintenance of depressive disorder is still little known. Research into interpersonal factors affecting the onset and course of depression is therefore an important topic in research efforts.
