Hypofyzární adenomy jsou nejčastější tumory selární oblasti. Nejčastějším typem adenomu jsou prolaktinomy, následují klinicky afunkční adenomy, akromegalie (STH produkující adenomy) a Cushingova choroba (ACTH produkující adenomy). Výjimečně se vyskytují tyreotropinomy (TSH produkující adenomy) a funkční gonadotropinomy. Stanovení správné diagnózy je založeno na manifestaci klinických příznaků, zhodnocení výsledků laboratorních vyšetření včetně dynamických testů a nálezů zobrazovacích vyšetření – nejčastěji magnetické rezonance mozku. Léčba hypofyzárních adenomů je často komplexní a využívá kombinace více léčebných postupů: neurochirurgické, medikamentózní a radiační léčby. Ve většině případů je metodou volby neurochirurgická operace. Výjimkou jsou prolaktinomy, u kterých je primární léčbou medikamentózní léčba dopaminergními agonisty.
Pituitary adenomas are the most common tumours of the sellar region. Prolactinomas are the most common type, followed by clinically non-functioning pituitary adenomas, acromegaly (Growth-Hormone secreting pituitary adenoma) and Cushing's disease (ACTH-secreting pituitary adenoma). Thyrotropinomas (TSH-secreting pituitary adenoma) and functional gonadotropinomas are rare. The correct diagnosis is based on the manifestation of clinical symptoms, evaluation of laboratory test results including dynamic tests, and imaging findings - most commonly Magnetic Resonance Imaging. Treatment of pituitary adenomas is often complex and involves a combination of treatment modalities: neurosurgery, medical therapy and radiotherapy. In most cases, neurosurgery is the treatment of choice. The exception is prolactinomas, for which the primary treatment is medical therapy with dopaminergic agonists.

POEMS is an acronym for a characteristic set of symptoms of a rare disease, which arises from secondary damage to various organs due to plasma cell dyscrasia. It is coined from Polyneuropathy, organomegaly, Endocrinopathy, Monoclonal protein, Skin changes. Hypogonadism, hyperprolactinemia, gynecomastia, hypothyroidism, impaired glucose metabolism, less often adrenal insufficiency or calcium-phosphate metabolism disorder belong to endocrinopathies occuring within the syndrome. endocrinologist plays an important role in multidisciplinary team that takes care of patients suffering from POEMS syndrome.
The aim of the current research was to develop a simple and rapid mass spectrometry-based assay for the determination of 15 steroid hormones in human plasma in a single run, which would be suitable for a routine practice setting. For this purpose, we designed a procedure based on the 2D-liquid chromatography-tandem mass spectrometry with a minimalistic sample pre-treatment. In our arrangement, the preparation of one sample takes only 10 min and can accommodate 40 samples per hour when tested in series. The following analytical run is 18 min long for all steroid hormones. In addition, we developed an independent analytical run for estradiol, significantly increasing the assay accuracy while taking an additional 10 min to perform an analytical run of a sample. The optimized method was applied to a set of human plasma samples, including chylous. Our results indicate the linearity of the method for all steroid hormones with squared regression coefficients R2 ≥ 0.995, within-run and between-run precision (RSD < 6.4%), and an accuracy of 92.9% to 106.2%. The absolute recovery for each analyzed steroid hormone ranged between 101.6% and 116.5%. The method detection limit for 15 steroid hormones ranged between 0.008 nmol/L (2.88 pg/mL) for aldosterone and 0.873 nmol/L (0.252 ng/mL) for DHEA. For all the analytes, the lowest calibration point relative standard deviation was less than 10.8%, indicating a good precision of the assay within the lowest concentration of interest. In conclusion, in this method article, we describe a simple, sensitive, and cost-effective 2D-LC/MS/MS method suitable for the routine analysis of a complex of steroid hormones allowing high analytical specificity and sensitivity despite minimal sample processing and short throughput times.
BACKGROUND: Delayed hypopituitarism is the most common complication after stereotactic radiosurgery (SRS) for pituitary adenomas. OBJECTIVE: To investigate the relationship between neuroanatomic structure distances from the radiation target and anterior pituitary function preservation after SRS through multicenter study. METHODS: We retrospectively reviewed the International Radiosurgery Research Foundation database from January 2002 to December 2021 for adult patients undergoing SRS for pituitary adenomas with >6 months of follow-up. Distances between centers or edges of hypothalamic-pituitary axis structures and SRS target volumes were measured using MRI. The primary outcome was anterior pituitary function preservation. Predictors were analyzed using multivariable logistic regression and area under the receiver operating curve (AUROC) curve analyses. RESULTS: Four hundred eighty-seven patients were categorized by preservation (n = 384) and no preservation (n = 103) of anterior pituitary function. The mean margin dose was 19.1(6.2) Gy. Larger distance from the center of the stalk to the tumor margin isodose was a positive predictor (adjusted odds ratio [aOR] = 1.162 [1.046-1.291], P = .005), while pre-SRS hypopituitarism (aOR = 0.646 [0.405-1.031], P = .067) and larger treatment volume (aOR = 0.965 [0.929-1.002], P = .061) were near negative predictors of the primary outcome. An interaction between the treatment volume and center stalk to margin isodose distance was found (aOR = 0.980 [0.961-0.999], P = .045). Center stalk to margin isodose distance had an AUROC of 0.620 (0.557-0.693), at 3.95-mm distance. For patients with treatment volumes of <2.34 mL, center stalk to margin isodose distance had an AUROC of 0.719 (0.614-0.823), at 2.95-mm distance. CONCLUSION: Achieving a distance between the center of the pituitary stalk and the tumor margin isodose ≥3.95 mm predicted anterior pituitary function preservation. For smaller treatment volumes <2.34 mL, the optimal distance was ≥2.95 mm. This may be modifiable during trans-sphenoidal resection to preserve pituitary function.

Gynekomastie (GM) je benigní proliferace prsní žlázy u mužů, která nastává nejčastěji v důsledku nerovnováhy mezi proliferačními účinky estrogenů a inhibičními vlivy androgenů. GM se vyskytuje s vysokou prevalencí, a sice u 50–60 % adolescentů a až u 70 % mužů nad 50 let. Příčina GM je nalezena zhruba u 40–50 % dospělých mužů. GM může být unilaterální či bilaterální. Klinicky je diagnostikována jako symetricky hmatná měkká tkáň v oblasti areoly. Od GM je nutné odlišit karcinom prsu a pseudogynekomastii. Léčba je závislá na vyvolávající příčině a době trvání gynekomastie.
Gynecomastia (GM) is a benign proliferation of the glandular tissue of the male breast, and it is caused by an imbalance between a proliferative effect of estrogen and inhibitory effect of androgen. GM is high prevalent disorder. It occurs in 50–60 % of adolescents and up to 70 % of males over 50 years. The cause of GM is found in 40–50 % of adult males. GM is unilateral or bilateral. It is clinically characterized by the presence of a tender mass extending concentrically from the nipple. Breast cancer and pseudogynecomastia must be distinguished. Treatment depends on the underlying cause and duration of gynecomastia.
Diferenciální diagnostika příčin centrálního diabetu insipidu je poměrně široká a spektrum možných příčin zahrnuje stavy po operacích nádoru hypofýzy, kraniocerebrální úrazy, některé selární expanze (kraniofaryngeom) a infiltrativní procesy. Prezentujeme případ třicetileté ženy s diabetem insipidem vzniklým na podkladě ANCA asociované vaskulitidy.
Differential diagnosis of central diabetes insipidus involves broad spectrum of origin including surgery for pituitary tumor, brain injury, some specific tumors in sellar region e.g., craniopharyngioma and infiltrative disorders. Here we present a case of 30year old woman with diabetes insipidus caused by ANCA associated vasculitis.
